임플란트 환자의 보철물에 대한 인식 및 관리 실태에 관한 연구 (A Study on subjective awareness and management status of Dental Implant Patients)

  • Purpose: The purpose of this study is to provide basic data for the improvement of dental care service for patient care. Methods: The survey period was from December 10, 2018 to June 10, 2019. The subjects were 180 patients who were treated with dental implant in 20 dental hospitals and clinics in Daejeon and Gyeonggi province. A total of 145 questionnaires were analyzed except for 35 copies which were unreliable or difficult to analyze. Results: The most important care method for prolonging the life time of dental implant prosthesis was the highest rate of correct brushing (39.4%), and a majority in the subjects used oral hygiene products (91.0%). With regard to discomfort after dental implant prosthesis, food particles between the teeth showed the highest response (49.7%), and discomfort with gum infections and bleeding and reduced mastication increased significantly with age (p<0.05), whereas feeling of irritation decreased significantly with age (p<0.05). Conclusion: This study is meaningful in that it approached the perception and discomfort of prosthesis from the patients' point of view. It needs to establish the basic data to improve dental treatment service for patient care.

『준생팔전(遵生八牋)』의 '죽미류(粥穈類)'에 관한 연구(硏究) - 『노노항언(老老恒言)』과 관련하여 - (A Study on Jukmiryu(粥穈類) in Junsaengpaljeon(遵生八牋) - with regard to Nonohangeon(老老恒言))

  • Objectives : In this study, I want to work on efficacious porridges through inquiry into the Jukmiryu(粥穈類) based on the classical literature. It is intended to serve as a basic study to explore the modern-day application of dishes that are good as patient meals or healthy foods for elderly people. Methods : I have taken the original text of Jukmiryu(粥穈類) included in Junsaengpaljeon(遵生八牋) and translated it. Further, I have compared it to those porridges treated in Nonohangeon (老老恒言), a book in geriatrics from Ming Dynasty. Results : Nonohangeon(老老恒言) describes sources and effects of 100 porridges. With recipes too simplified, however, it doesn't provide any practical help. In contrast, Junsaengpaljeon(遵生八牋) presents 40 porridges together with their effects and recipes. It is practically more beneficial, as it provides recipes more specific than Nonohangeon(老老恒言). Conclusions : Jukmiryu(粥穈類) in Junsaengpaljeon(遵生八牋) set great store on the pharmacological effects of food. As it provides detailed description of porridge recipes and ingredients as well as their effects, readers can easily apply them to symptoms. Since individuals may choose what would be appropriate for their own symptoms, it could be helpful in not only providing patient meals and foods for elderly people but also improving dietary practice.

  • Essure (Bayer) received approval from the U.S. Food and Drugs Administration as a permanent non-hormonal contraceptive implant in November 2002. While the use of Essure in the management of hydrosalpinx prior to in vitro fertilization (IVF) remains off-label, it has been used specifically for this purpose since at least 2007. Although most published reports on Essure placement before IVF have been reassuring, clinical experience remains limited, and no randomized studies have demonstrated the safety or efficacy of Essure in this context. In fact, no published guidelines deal with patient selection or counseling regarding the Essure procedure specifically in the context of IVF. Although Essure is an irreversible birth control option, some patients request the surgical removal of the implants for various reasons. While these patients could eventually undergo hysterectomy, at present no standardized technique exists for simple Essure removal with conservation of the uterus. This article emphasizes new aspects of the Essure procedure, as we describe the first known association between the placement of Essure implants and the subsequent development of fluid within the uterine cavity, which resolved after the surgical removal of both devices.

Intestinal Hypoganglionosis Leading to Intestinal Failure and the Compassionate Use of OmegavenTM

  • Intestinal hypoganglionosis is a rare innervation disorder that provides numerous nutritional, medical and surgical challenges. In this case report, we present a case of a newborn with intestinal hypoganglionosis leading to intestinal failure and intestinal failure-associated liver disease who responded to $Omegaven^{TM}$, a fat emulsion comprised of omega-3 fatty acids. $Omegaven^{TM}$ has been shown to be beneficial in the management of cholestatic liver injury. Clinical success with $Omegaven^{TM}$ was seen in this patient with a clear decrease in aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase and complete resolution of cholestasis with a direct bilirubin of zero within two weeks of initiation of $Omegaven^{TM}$. No current guidelines for the diagnosis and management of hypoganglionosis are available. We recommend a multidisciplinary approach and the use of novel therapies such as fat emulsions composed of omega-3 fatty acids for improved patient outcomes. Appropriate compassionate use protocols should be obtained from the Food and Drug Administration prior to initiation of $Omegaven^{TM}$.

  • 급성설사는 발병의 시작이 급하고, 바이러스 감염에 의한 경우가 대부분이며 3주 이내로 지속되는 경우를 말한다. 이런 경우 발열, 오한, 식욕부진, 그리고 구토 등을 동반할 수 있는데, 대부분의 경우는 기본적인 처치로 호전되나, 탈수나 패혈증 및 기타 합병증으로 발전할 수 있으므로 주의해야 한다. 본 증례에서는 음식상으로 인한 급성설사 환자를 사상의학적 관점에서 치료하여 임상증상의 호전을 경험하였다. 75세 남환으로 복통을 동반한 수양성 설사(내원전 24시간동안 10여회), 오한, 전신무력감으로 내원하였는데. 이를 소양인 신한복통망음증으로 분류하고 표음강기의 치법으로 활석고삼탕을 투여한 결과 전신증 상이 호전되어 이를 보고하는 바이다.

  • 상하악 풀마우스 보철을 만들어주어야 할때 정확한 진단을 하고 계획을 잘 세워서 보철물을 디자인해야 한다. 특히 교합고경이 무너지고 잘못된 교합평면으로 인한 환자의 경우에는 예전의 교합고경으로 회복시켜주어야 하고 올바른 교합평면을 만들어 주어야 한다. 또한 적절한 재료를 사용하여 환자가 음식을 잘 저작하고 무리한 힘이 가해지지 않도록 해야한다. 임플란트의 위치가 부적절한 위치에 있을때는 보철물을 디자인할때 대합치와의 관계와 교합등을 고려하여 결정한다. 그리고 임플란트 주변의 청소성을 위한 디자인을 해주어야 한다.

고유수용성신경근촉진법을 이용한 목근육 강화운동이 삼킴 장애환자에게 미치는 영향: 단일사례연구 (The Effect of Neck Strengthening Exercise Using Proprioceptive Neuromuscular Facilitation on Swallow ability of Patient with Dysphagia: A Single Case Study)

  • Purpose: The current study seeks to identify the effect of neck muscle strengthening exercise using proprioceptive neuromuscular facilitation (PNF) on the swallowing ability of patients diagnosed with dysphagia due to stroke. Methods: As a single case study, the current research conducted neck muscle strengthening exercise using PNF on the patient with dysphagia for 40 minutes, four times per week for a six-week period. At the same time, typical rehabilitation therapy for dysphagia was provided. This type of therapy included food-swallowing and the relation of the muscles surrounding the neck. The functional dysphagia scale and the penetration-aspiration scale were used to assess swallowing ability. Results: After the therapy, the functional dysphagia scale and the penetration-aspiration scale decreased by 18 points and 3 points, respectively, which proves the effectiveness of this type of therapy for dysphagia. Conclusion: The results of the current study indicate that neck muscle strengthening exercise using PNF reduces penetration-aspiration in patients with dysphagia, and that PNF can be clinically utilized to improve the swallowing ability of dysphagic patients.

Trichinosis Imitating an Inflammatory Systematic Disease

  • Trichinellosis (trichinosis) is a parasitic infection caused by nematodes of the genus Trichinella. Pigs are the most common source of human infection. We describe a case of a 47-year-old woman presented with a wide range of intermittent symptoms including prolonged fever, dry cough, diarrhea, rash, myalgias and arthralgias. The patient was attended by physicians with various medical specialties such as dermatologists, rheumatologists and allergiologists, but they did not establish a certain diagnosis because of the gradual onset of symptoms, raising the suspicion of a systematic disease. After extensive work up, the diagnosis of trichinosis was established with femoral muscle biopsy compatible with inflammatory myopathy of parasitic etiology with trichinosis to be the predominant diagnosis. Despite the significant delay of diagnosis for almost three months, patient was treated successfully with no further complications. Trichinellosis is a food-borne treatable infection. Preventive measures include community education especially in zones where parasite prevalence is increased, improvement of farming and cooking techniques.

당뇨식 잔반 감량을 위한 영양서비스 개선 활동의 효과 (Effects of Nutrition Service Improvement Activities for Reducing Plate Waste of the Diabetic Mellitus Diet in a General Hospital)

  • Hospital malnutrition could be caused by not completing the food served in the hospital. This has been a big problem since it delays the recovery of the patient and extends the length of hospital stay. The purpose of the study was to reduce the plate waste for the DM diet by performing several nutrition service improvement activities. The study was performed in a general hospital with 900 beds. A questionnaire survey was taken by 39 DM patients to obtain their aspect of the hospital foodservice systems and the quality of the meals at the beginning of the study. The amounts of foods served in the hospital kitchen and returned were measured by weights. After the improvement activities, the measurement of the plate waste was performed again for comparison. The average percentage of plate waste for the DM diet was 23.2%. The survey showed no difference by sex, age or duration of admission in plate waste. However, this food wastage percentage showed differences between the patients having a chance to get information about the diet therapy (12.21%) and not having one (26.06%) (p < 0.05). Using a five-point Likert-type scale, the quality of food by its taste was 2.49 (1: very poor, 5: excellent), the temperature score was 3.56 (1: very poor, 5: excellent), and the amount of food served score was 2.95 (1: very poor, 5: excellent), and the preference score was 3.13 (1: very dislike, 5: very like). Nutritional care improving activities were performed by adjusting seasonings, developing new menus, and standardizing cooking methods in order to increase the satisfaction of meal quality. The dietitian's inpatients care protocol was adjusted to expand the nutritional counseling chance for the DM patients. After the improvement activities, the average plate waste was reduced to 14.6%, and the satisfaction of food taste and preference increased to 3.21 (p < 0.001), and 3.36 (p < 0.05) correspondingly. The result shows that, for therapeutic diet patients, food intake could be increased by improving the food service satisfaction by controlling the meal quality and clinical nutritional service activities.

대한민국에서 복어독에 의한 식중독 발생 특성 (Characteristics of Puffer Fish Poisoning Outbreaks in Korea (1991-2002))

  • 우리나라에서 1991년부터 2002년 사이의 복어독 중독 발생상황에 대하여 신문에 보도된 내용을 검색하여 정리하고 특성을 분석하였다. 조사기간 중 우리나라에서는 총 32건의 복어독 중독사고가 발생하여, 111명이 중독되었고 그 중 30명이 사망하여 27.0%의 치사율을 나타내었으며, 연도에 따라 중독건수, 환자수 및 사망자수의 변동이 심하였다. 사망자는 29세 이상의 남성이 대부분을 차지하였다. 식중독 발생건별 환자수를 보면 4인 이하의 소규모 중독사건이 전체 발생건수의 75.0%인 24건을 차지하였고, 식중독 통계 집계기준인 5인 이상의 집단 식중독은 8건에 지나지 않았다. 우리나라에서 복어독 중독 발생건수의 59.4%(19/32)가 11월부터 익년 1월 사이에 발생하였고, 이 시기의 환자수 및 사망자수 또한 각각 65.8%(73/111), 66.7%(20/30)로 나타나 복어중독은 겨울철에 집중되었다. 지역적으로는 발생 건수의 75.0%(24/32)가 부산, 경남, 전남, 제주 등 남해안 지역에서 집중적으로 발생하였다. 섭취장소별로는 총 32건 중 18건(56.3%)이 선박에서 발생하였고, 가정에서 8건(25.0%)이 발생하였으며, 음식점에서 발생한 것은 5건(15.6%)이었다. 복어독 중독의 가장 중요한 원인 음식물은 국(탕 포함)으로 발생건수의 68.8%(22/32), 환자수의 64.0%(71/111), 사망자의 66.7%(20/30)를 차지하였고, 그 외 찜, 회, 내장탕, 알 등도 원인이 된 경우가 있었다.
