정신분열병 환자 가족의 단기 가족교육 후 대처전략 및 스트레스 변화에 대한 연구 (Changes of Stress and Coping Strategy in Families of Schizophrenic Patients after Brief Program of Family Education)

  • 연구목적 : 본 연구는 정신분열병 환자의 가족이 겪는 각종 스트레스(불안, 우울, 거부감, 걱정 등)와 함께 가족의 대처 전략의 정도를 평가하고 단기가족교육 후 정신분열병에 대한 지식과 스트레스, 대처전략의 변화 정도를 대조군과 비교해 단기가족교육의 효과를 평가 하고자 하였다. 방법 : DSM-IV 진단기준에 따라서 분류된 정신분열병 환자의 가족 중에서 가족교육을 받은 24 명을 교육군으로 하고 동 질환으로 본과에 입원한 환자의 가족 중에서 22명을 선정하여 대조군으로 하였다. 교육군에게는 가족교육 전후에, 대조군에게는 환자 입원 시와 4주 이후에 두 번 설문지를 작성하도록 하였다. 사용된 설문지는 Family Coping Questionnaire(FCQ), Patient Rejection Scale(PRS), 걱정설문지, 지식설문지, Beck Depression Inventory(BDI), State-Trait Anxiety Inventory(STAI) 등이다. 결과 : 1) 교육군에서 걱정설문지, BDI, STAI-S는 사후평가 점수가 사전평가 점수보다 유의하게 낮았고 FCQ의 사회적 관심항목과 지식설문지는 사후평가 점수가 사전평가 점수보다 유의하게 높았다. FCQ요인에서는 회피와 관련된 사회적 관심의 전략이 사후평가 점수가 사전평가 점수보다 유의하게 높았다. 반면에 대조군에서는 모든 설문지에서 사전평가 점수와 사후평가 점수 사이에 유의한 차이가 보이지 않았다. 2) 교육군에서 걱정설문지의 점수 차와 STAI-S의 점수 차 사이, 걱정설문지의 점수 차와 STAI-T의 점수 차 사이에 유의한 정적 상관관계가 있었고 FCQ의 긍정적 상호작용항목의 점수 차와 PRS의 점수 차 사이, 지식설문지의 점수 차와 STAI-S의 점수 차 사이에는 유의한 부적 상관관계가 있었다. 반면에 대조군에서는 FCQ의 정보항목의 점수 차와 PRS의 점수 차 사이, 긍정적 상호작용항목의 점수 차와 PRS의 점수 차 사이에만 유의한 부적 상관관계가 있었다. 3) 교육군에서는 걱정설문지의 점수 차와 환자의 나이 사이에 유의한 정적 상관관계가 있었고 지식설문지의 점수 차와 환자의 유병기간 사이에 유의한 부적 상관관계가 있었지만 대조군에서는 유의한 상관관계가 존재하지 않았다. 결론 : 단기가족교육 후에 가족들의 지식이 증가하고 걱정과 불안감이 감소하였으며 환자에게서 벗어나 사회적 관심이 높아지게 되었다. 한편, 교육군에서는 지식의 증가에 따라 불안감이 감소했지만 대조군에서는 상관 관계가 없었다. 또한 교육군과 대조군 모두에서 지식의 변화와는 상관없이 거부감이 적어질수록 긍정적 의사소통의 대처전략을 많이 사용했고 대조군에서 거부감의 영향을 더 많이 받았다.

한방근무 간호사의 업무분석 (An Analysis on Nursing Activity in Oriental Hospital)

  • The need of oriental nursing role is recently increasing. The implication of this trend is enormous not only for the need of independent of independent oriental nursing intervention. Hence, this study was designed to describe the characteristics of oriental nursing activity and identify the meaning of nursing activity in oriental hospital. A total of 24 nurses were selected by convenience sampling from eight units at one oriental hospital. A total of 168 data were collected using minute-by-minute recording during nurse s work shift. The data were analyzed using nova and scheffe method as post-hoc test The results of this study are as follows; 1. the category of nursing activity in oriental hospital 74.4% of nursing activity was the patient-oriented nursing care. The nursing activity included others (18.28%), the function-oriented nursing care (7.07%), and staff-oriented care (0.2%). 29.1 % of the patient-oriented nursing care was the direct care, and 24. 5 % of the patient-oriented nursing care was the indirect care. There was a lack of patient education and the communication patient. 2. Nursing activity in oriental hospital by characteristics 1) Nursing activity by unit For patient-oriented nursing care, there was a statistically significant difference among 8 units. In the critical care unit, the patient-oriented nursing care was mostly performed. For function-oriented nursing care, there was statistically significant difference among 8 units. In the physical therapy unit, the function-oriented nursing care was mostly performed. 2) Nursing activity in oriental hospital For all kinds of nursing activity, there was a statistically significant difference between shifts. In night shift patient-oriented nursing care, function-oriented nursing care, and other kinds of nursing care was mostly performed. Patient-oriented nursing care included indirect care, treatment set management, and the breaktime. In day shift, staff-oriented nursing care was performed. 3) Nursing activity in oriental hospital For all kinds of nursing activity, there was not a statistically significant difference by day. The day has not impact on nursing activity. 3. the meaning of activity in oriental hospital The results of this study show that the patient-oriented care was vital check, input! output check, medication, bedsore prevention, nasogastric feeding, oral care, catheterization care, perinatal care, nursing care associated with acupuncture and moxacautery, observation (fever, sweating), heat and cold application communication with patent and family, and patient room management In conclusion, two issues associated with the findings of the research appeated to be involved in the difficulty of nursing activity in hospital. The first issue was the nursing care in oriental hospital provided by nurses who have a limited understading of the principles of oriental medicine. The second issue was the deficiency of systematic guideling for oriental nursing activity. The findings suggest the need to develop the systematic guideline for oriental nursing activity. The need of continuing education for nurses who work in the oriental hospital, and the need of nursing education including oriental nursing are critical.

입원환자의 환자안전인식과 환자안전활동 수행 정도 (Awareness of Patient Safety and Performance of Patient Safety Activities among Hospitalized Patients)

  • 본 연구는 입원환자를 대상으로 환자안전인식과 환자안전활동 수행 간의 관계를 확인하고 환자안전인식과 환자안전활동 수행을 증진시키기 위한 중재 프로그램을 개발하는데 기초자료로 제공하고자 수행하였다. 본 연구의 대상자는 D광역시 일 상급종합병원에 입원한 성인 환자 103명이었다. 자료수집은 2021년 2월 22일부터 3월 12일까지 이루어졌고, 환자안전인식과 환자안전활동 수행을 측정하기 위한 구조화된 설문지를 사용하였다. 입원환자의 환자안전인식은 5점 만점에 4.22±0.52점, 환자안전활동 수행은 4점 만점에 3.35±0.48점이었다. 환자안전인식은 환자안전교육 경험과 유의한 차이가 있었고(t=4.85, p<.001), 환자안전활동 수행은 결혼상태(t=2.75, p=.007), 환자안전교육 경험(t=3.88, p<.001)에 따라 유의한 차이가 있었다. 환자안전인식과 환자안전활동 수행은 유의한 상관관계가 있었다(r=.59, p<.001). 본 연구의 결과를 통해 임상에서 적용 가능한 입원환자의 환자안전인식과 환자안전활동 수행 증진을 위한 프로그램을 개발하고 시스템적으로 적용함으로써, 환자안전문화 향상을 도모할 필요가 있다.

리플릿 교육 후 MRI 환자의 이해도 및 만족도 비교분석 (Comparative Analysis of Understanding and Satisfaction of MRI Patients after Leaflet Training)

  • 최근 OECD 가입국 중에 MRI 보급률이 평균보다 높게 보급되고 있고, MRI 검사의 횟수도 증가하고 있다. MRI 검사는 다른 의료장비에 비해 검사시간이 길며, 환자의 움직임이 최소화되어야 한다. 그러므로 환자의 불편이 항상 뒤따른다. 이러한 불편한 환경에서 검사 시 정확한 검사가 어려우며 진단적 가치의 영상획득에는 어려움이 따른다. 기존 MRI 검사 시 MRI의 위험성과 협조를 구하기 위해 글로 되어있는 안내문을 제공 하였으나, 전문용어와 이해하기 어려운 문장들로 구성이 되어있어 이해도가 낮아졌다. 본 연구는 이러한 문제점을 보완하고 MRI 검사의 이해도를 높여 진단적 가치가 있는 영상획득에 도움이 되고자 하였다. 그리고 환자의 이해도와 만족도를 평가하여 리플릿 교육의 우수성을 평가하고자 한다. 본 연구의 결과에서 나타났듯이 리플릿 교육 전보다 교육 후의 이해도와 만족도의 점수는 각각 4.44±0.55 점, 4.85±0.37 점으로 높았으며, 학력에 따라 이해도의 차이는 유의함을 알 수 있었다. 그러나 MRI 검사 횟수는 차이가 없었다. 향후 MRI 검사에서 다양하고 이해하기 쉬운 리플릿 교육이 널리 보급되어 환자의 적극적인 검사 협조로 보다 질 좋은 영상을 획득할 수 있기를 기대한다.

의학교육 평가인증의 국제적 동향 (Current Trend of Accreditation within Medical Education)

  • Currently, accreditation within medical education is a priority on the agenda for many countries worldwide. The World Federation for Medical Education's (WFME) launch of its first trilogy of standards in 2003 was a seminal event in promoting accreditation within basic medical education (BME) globally. Parallel to that, WFME also actively spearheaded a project to recognize the accrediting agencies within individual countries. The introduction of competency-based medical education (CBME) with the two key concepts of "entrusted professional activity" and milestones has enabled researchers to identify the relationship between patient outcomes and medical education. Recent data driven by CBME has been used for the continuous quality improvement of trainees and training programmes as well. The goal of accreditation has shifted from the single purpose of quality assurance to balancing quality assurance and quality improvement. Even though there are a plethora of types of postgraduate medical education (PGME), it may be possible to accredit resident programmes on a global scale by adopting the concept of CBME. In addition, the alignment of the accreditation for BME and PGME, which center on competency, will be achievable. This argument may extend the possibility of measuring the outcomes of the accreditation itself against patient outcomes as well. Therefore, evidence of the advantages of costly and labor-consuming accreditation processes will be available in the near future and quality improvement will be the driving force of the accreditation process.

구조화된 환자교육이 뇌졸중 환자의 조기재활에 관한 지식과 활동수행에 미치는 영향 (The Effect of the Structured Education on the Early Rehabilitation Knowledge and Activity Performance of the C.V.A. Patients)

  • This study has been attempted to set up the strategies of the nursing which can promote the activity performance for early rehabilitation for the patients by examining the effect of the structured patient education on the early rehabilitation knowledge and activity performance of the C.V.A patients. The study method has been done by investigating the experiment group and control group in advance through the question papers and interview and observation on 65 patients who had been hospitalized at oriental medicine hospital of K Medical Center from July 1st 1995 to the end of Sep, 1995. The analysis of the collected material had been done for the homogeneity test in which general characters of experiment group and control group had been tested by X²and the homogeneity test of ADL by t-test. To test the hypothesis the t-test had been given for the difference of the early rehabilitation knowledge and activity performance between the two groups and the correlation between early rehabilitation knowledge and activity performance had been tested by Pearson's Correlation Coefficient. The result of the test of the hypothesis is as the below. 1 The 1st hypothesis “The experiment group which had received the structured education should be higher in the early rehabilitation knowledge than the control group” was supported(t=4.45. p=.000). 2. The 2nd hypothesis “The experiment group which received the structured education should be higher in the early rehabilitation activity performance than the control group”was supported(t=2.11, p=.036). 3. The 3rd hypothesis “The higher the early rehabilitation knowledge of the patient the higher the activity performance degree” was rejected (r=.1546, p=.219). In conclusion, the patients who received the structured education showed the increase in the degree of early rehabilitation knowledge and activity performance, so it has been judged that education has been prerequisite in increasing the knowledge and activity performance of early rehabilitation.

환자시뮬레이터활용교육에서의 자기주도적 학습능력과 집단효능감의 변화 (Effect of Human Patient Simulator-based Education on Self-directed Learning and Collective Efficacy)

  • 본 연구는 환자시뮬레이터활용교육에 따른 간호대생의 자기주도적 학습능력과 집단효능감 변화를 파악하고자 시도한 단일군 전후 실험연구이다. 대상자는 환자시뮬레이터활용교육을 받은 적이 없는 2학년 전체 학생 중 통합실습과목에 등록하고 연구에 동의한 92명이었다. 환자시뮬레이터활용교육은 팀활동학습, 술기훈련, 환자시뮬레이터를 이용한 팀시뮬레이션, 디브리핑으로 구성되어 12주간 운영되었다. 사전 자료수집은 2010년 8월 23일에서 27일까지 이루어졌고 사후 자료수집은 12주간의 환자시뮬레이터수업을 수행한 후 2010년 11월 29일에서 12월 3일 까지 시행되었다. 환자시뮬레이터활용교육 전 후로 자기주도학습능력의 증가(t=4.24, p=0.000)는 통계적으로 유의미하였으나 집단효능감의 증가는 유의한 효과를 나타내지 못하였다. 이러한 결과에 근거하여 환자시뮬레이터활용교육은 간호대생의 자기주도적 학습능력 향상에 영향을 미치는 교육전략임을 알 수 있다.

Palliative Care Education in Gynecologic Oncology: a Survey of Gynecologic Oncologists and Gynecologic Oncology Fellows in Thailand

  • Background: The main purpose of this study was to survey the education and training of certified gynecologic oncologists and fellows in Thailand. A secondary objective was to study the problems in fellowship training regarding palliative care for gynecologic cancer patients. Materials and Methods: A descriptive study was conducted by sending a questionnaire regarding palliative care education to all certified gynecologic oncologists and gynecologic oncology fellows in Thailand. The contents of the survey included fellowship training experience, caring for the dying, patient preparation, attitudes and respondent characteristics. Statistics were analyzed by percentage, mean and standard deviation and chi-square. Results: One hundred seventy completed questionnaires were returned; the response rate was 66%. Most certified gynecologic oncologists and fellows in gynecologic oncology have a positive attitude towards palliative care education, and agree that "psychological distress can result in severe physical suffering". It was found that the curriculum of gynecologic oncology fellowship training equally emphasizes three aspects, namely managing post-operative complications, managing a patient at the end of life and managing a patient with gynecologic oncology. As for experiential training during the fellowship of gynecologic oncology, education regarding breaking bad news, discussion about goals of care and procedures for symptoms control were mostly on-the-job training without explicit teaching. In addition, only 42.9 % of respondents were explicitly taught the coping skill for managing their own stress when caring for palliative patients during fellowship training. Most of respondents rated their clinical competency for palliative care in the "moderately well prepared" level, and the lowest score of the competency was the issue of spiritual care. Conclusions: Almost all certified gynecologic oncologists and fellows in gynecologic oncology have a positive attitude towards learning and teaching in palliative care. In this study, some issues were identified for improving palliative care education such as proper training under the supervision of a mentor, teaching how to deal with work stress, competency in spiritual care and attitudes on responsibility for bereavement care.

설명간호사의 현황과 법적 지위에 관한 고찰 (A Study on the Nurse in Charge of Education'S Current Status and Legal Status)

  • Recently several hospitals have established a new nursing position so called, 'the nurse in charge of education (NCE).' The job description of NCE is to give a detail explanation on examination or test introduced to patients or a guide for those who are not familiar to hospital facilities at the out-patients level. The motivation of NCE position is the quality improvement in patient education on the sophisticated procedures or the follow up care for medical purposes to improve their compliances, as well in delivering services for outpatients or visitors in more informative and efficient way. The application of NCEA has been turned out positive and effective in patient satisfaction and unit management. However, special attention is brought to the scope and depth of the contents of education delivered by NCE which might overlap with the duty of physicians by the Medical Act. It is needed to clarify the role and job description of NCE in the context of Medical Act. The engagement of NCE to the Advanced Nursing Practitioner (ANP) is one of the possible solutions for a duty charge on patient education since ANP is a legislatively official position with higher license of RN at master level. Further discussion is needed to elaborate and arrange the details on the scope and content of patent education among health science professionals including RN, ANP and physicians.

고성능 인체 환자 모형을 활용한 저혈당 시나리오 기반의 시뮬레이션 교육이 간호대학생의 당뇨병 지식과 학업적 자기효능감 습득과 지속성에 미치는 효과 (Effect of a Simulated Education-based Hypoglycemia Scenario Using a High-fidelity Simulator on Acquisition and Retention of Diabetes Knowledge and Academic Self-efficacy in Nursing Students)

  • Purpose: This study evaluated the acquisition and retention effects on diabetes knowledge and academic self-efficacy of patient simulation-based hypoglycemia scenario using a high-fidelity simulator. Methods: A nonequivalent control group pretest/posttest repeated measures quasi-experimental design determined if nursing simulation using a high-fidelity simulator helps students acquire and retain diabetes knowledge and academic self-efficacy. A total of 101 nursing students from two universities participated in this study, comprising the experimental and control groups. The experimental group (n=52) at a university in S city comprised participants in an adult nursing class, nursing practicum, and simulation education. The control group (n=49) at a university in C city participated in an adult nursing class and nursing practicum without simulation education. Results: The experimental group had statistically significant group-time interaction effects on diabetes knowledge (p=.044) and self-confidence (p<.001) of academic self-efficacy compared to the control group. Conclusion: The results indicated that the patient simulation-based hypoglycemia scenario using a high-fidelity simulator applied to nursing education is useful in encouraging nursing students to engage in strategies with acquisition and retention effects, enhancing their diabetes knowledge and self-confidence of academic self-efficacy. Therefore, educators should use simulation programs effectively with nursing classes and practicum to enhance the effects of nursing competencies.
