Family Factors Influencing Korean Mothers' Postpartum Depression

  • The purpose of the study was to examine the effects of family related factors (mothers' self-esteem, mothers' parenting stress, mothers' marital satisfaction, fathers' parenting involvement, and social support) on mothers' postpartum depression. The subjects were 797 households that were extracted from the $1^{st}$ wave of the Panel Study of Korean Children (PSKC). The study variables were measured using the survey questionnaires, and analyses of Pearson's correlation and multiple regression were conducted. Results showed that family related factors significantly predicted mothers' postpartum depression. Moreover, the most significant predictor was mothers' parenting stress, followed by marital satisfaction, self-esteem, fathers' parenting involvement, and social support. Study findings indicate that mothers' postpartum depression is attributed to not only personal but also family related factors. Our results suggest importance of parent education and family support along with social support.

유아기 자녀를 둔 결혼이주여성의 양육행위 유형별 모성이데올로기 및 양육스트레스 (Motherhood Ideology and Parenting Stress according to Parenting Behavior Patterns of Married Immigrant Women with Young Children)

  • Purpose: This study aims to provide base data for designing education and counseling programs for child-raising by identifying the types, characteristics and predictors of parenting behaviors of married immigrant women. Methods: We used a self-report questionnaire to survey 126 immigrant mothers of young children, who agreed to participate, and who could speak Korean, Vietnamese, Chinese, Filipino, or English, at two children's hospitals and two multicultural support centers. Statistical analysis was conducted using descriptive analysis, K-means clustering, ${\chi}^2$ test, Fisher's exact test, one-way ANOVA, $Sch{\acute{e}}ffe^{\prime}s$ test, and multinominal logistic regression. Results: We identified three clusters of parenting behaviors: 'affectionate acceptance group' (38.9%), 'active engaging group' (26.2%), and 'passive parenting group' (34.9%). Passive parenting and affectionate acceptance groups were distinguished by the conversation time between couples (p=.028, OR=5.52), ideology of motherhood (p=.032, OR=4.33), and parenting stress between parent and child (p=.049, OR=0.22). Passive parenting was distinguished from active engaging group by support from spouses for participating in multicultural support centers or relevant programs (p=.011, OR=2.37), and ideology of motherhood (p=.001, OR=16.65). Ideology of motherhood was also the distinguishing factor between affectionate acceptance and active engaging groups (p=.041, OR=3.85). Conclusion: Since immigrant women's parenting type depends on their ideology of motherhood, parenting stress, and spousal relationships in terms of communication and support to help their child-raising and socio-cultural adaptation, it is necessary to provide them with systematic education and support, as well as interventions across personal, family, and community levels.

암 환아 가족의 질병에 대한 불확실성과 대처양상에 관한 연구 (A Correlational Study on Uncertainty and Coping in Families of children with Cancer)

  • For most parents their child's illness and hospitalization is strssful. Internal stress and over anxiousness leads to uncertainty. Parents have a variety of ways of coping when faced with such stress factors, especialy when the child has cancer. The purpose of this study was to examine the relationship between uncertainty in illness and mastery and coping styles in parents of pediatric cancer patients, and further to identify differences between three groups of subjects. A cross-sectional descriptive correlational design was used in this study to investigate the relationship among the three groups. The sample was composed of 59 parents of children with cancer in pediatric wards in three hospitals in Seoul : 21 in Group 4 with a child newly diagnosed with cancer : 25 in Group B, with a child under treatment and rehospitalized with a relapse : and 13 in Group C, with a child with a terminal cancer, A formalized questionnaire which included demegrapic information and consisted of 75 items was used in this study ; The Parent Perception of Uncertainty Scale(PPUS) adapted form Mishel’s Uncertainty in Illness Scale, Pearlin & Schooler's Mastery Scale(MS) and Folkman & Lazarus’s Ways of Coping Checklist(WCC) were used to measure the variables : uncertainty in illness, mastery, and coping styles. A pretest was conducted on the questionnaire items for reliability. The results gathered were analyzed using SPSS /PC/sup +/. Data analysis included descriptive correltional statistics such as ANOVA, Pearson Correlation Coefficient, amd chi square test. The conclusions of this analysis are summarized as follows : 1. A higher level of uncertainty was seen among parents of children with terminal cancer. 2. The first hypothesis that uncertainty in illness would differ among the three groups was supported(F=4.182, P=.020). The second and third hypotheses that mastery and coping styles would differ among the three groups were not supported. There was a correlation between uncertainty, mastery, and coping styles which was positive, also there was a positive correlation between mastery and coping style(r=.3744, P<.001) but a negative correlation between uncertainty md mastery(r=-.4749, P<.01). From the above results, it can be concluded that prediciting and controlling uncertainty in illness by considering coping styles and mastery are necessary for improved, efficient nursing interventions.

Transfer anxiety in parents of children transferred from pediatric intensive care units to general wards in South Korea: a hybrid concept analysis

  • Purpose: This study aimed to analyze the concept of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Methods: The hybrid model by Schwarz-Barcott and Kim was used to analyze the characteristics of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Results: Transfer anxiety was defined by the following attributes: 1) stress concerning the adaptation process, 2) concern about the child's condition worsening due to the parent's caregiving, and 3) involuntary changes in daily life due to the treatment. Transfer anxiety has the following antecedents: 1) uncertainty; 2) a lack of knowledge about the illness, medical devices, and caregiving; and 3) a lack of social support. It resulted in 1) caregiver burden, 2) a decrease in the capacity for coping with caregiving, 3) delays in the child's physical and psychological recovery, and 4) decreased quality of life. Conclusion: It is necessary to develop an assessment scale that considers the attributes of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Furthermore, an effective nursing intervention should be developed to reduce transfer anxiety.

암 환아 부모의 경험에 대한 질적 연구 (The Experience of Parents Whose Child is Dying with Cancer)

  • The purpose of this research was to understand the structure of the lived experience of parents of a child terminally ill with cancer The research question was “What is the structure of the experience of parents of a child terminally ill with cancer\ulcorner” The sample consisted of 17 parents of children admitted to the cancer units of two university hospitals in Seoul. The unstructured interviews were carried out from October 10, 1991 through January 10, 1992. They were audio-recorded and analysed using Van Kaam's method. Parents ascribed the cause of the cancer to the mother's emotional imbalance during pregnancy, the mother's stress, failure to observe religious rites, food, the parent's sin, misfortune and pollution. The theme clusters were tension, fear and depression experienced during pregnancy, stress that children suffer from abusive parents, failure to observe religious activites, bad luck, and sins committed during a previous life. When the child suffered a recurrence of cancer, the parents experienced negative emotions, nervousness, sorrow. depression and death. The theme clusters were feelings of despair, helplessness, regret, guilt, insecurity, emptyness and apathy. The long struggle with cancer resulted in the loss of economic security, loss of psychological and physical well being, and social withdrawal. The theme clusters were the economic burden of medical cost, giving up treatment, debt, limited medical insurance coverage and blood transfusion. The loss of psychological well being included stress, lack of support systems, inability to carry out responsibilities, lack of trust of the medical ten family breakdown, inappropriate expression of emotion and not disclosing the diagnosis to the child. Physically the parents suffered fatigue, insomnia, loss of appetite, loss of weight, dizzness, headache, psychosomatic symptoms, and increased consumption of liquor and cigarettes. Social withdrawal was manifested by taking time off from work to look after the child, decrease of outside social activities and feelings of isolation. Influences on family life were spousal conflicts, negative response of siblings, separation of the family members and economic hardship. The theme clusters were blaming a spouse for the cause of the illness and disagreements, maladjustment, lonliness, hostility and depression of siblings. The high price of medical care over the long period was a major factor influencing the life of the family. Positive experiences during the child's long illness were the strengthening of support systems and religious beliefs and financial help from social organizations. The support of one's spouse primarily helped to overcome the stress of the long illness. In addition, support was received from parents of other children with cancer and from nurses and religious leaders. The nurse, by providing empathetic support, should be a person with whom parents can express their feelings and share their experiences.

어머니가 지각하는 사회적 지지와 양육스트레스, 자아인식 및 양육행동 간의 구조적 관계 (The Structural Relationship among Social Support, Parenting Stress, Self-perception and Parenting Behavior Perceived by Mother)

  • The purpose of this study was to explore structural relationships among social support, parenting stress, self-perception and parenting behavior perceived by mother and provide preliminary data useful for desirable parenting behavior. For this purpose, the data of fourth wave Panel Study on Korean Children(PSKC) including social support, parenting stress, self-perception and parenting behavior measured by 1,746 mothers with 3-year-old children were analyzed. We identified structural relationships among the variables using SPSS 18.0 and AMOS 18.0 applying structural equation modeling. Measurement model and structure model had favorable goodness of fit and the results of structure models on each path were as follows. First, parenting behavior had positive correlations with social support and self-perception but there was a negative correlation between parenting behavior and parenting stress. Second, the relationship between social support and self-perception was mediated by parenting stress and parenting stress and self-perception mediated the relationship between social support and parenting relationship. In conclusion it is required to raise awareness about the importance of development of various parent education programs and parenting behavior.

주의력결핍과잉행동장애 아동 어머니의 우울감, 양육 스트레스 및 양육 관련 태도가 양육 행동에 미치는 영향 (THE EFFECT OF ADHD CHILD MOTHER'S DEPRESSIVE MOOD, PARENTING STRESS, AND PARENTING RELATED ATTITUDE ON PARENTING BEHAVIOR)

  • 요 약:본 연구에서는 어머니의 우울감과 양육 스트레스, 양육 효능감 및 역기능적 사고 등 양육 관련 태도가 부모의 양육 행동에 미치는 영향을 살펴보았으며, 특히 주의력결핍과잉행동장애 아동 어머니를 중심으로 살펴보았다. 방 법:대상은 DSM-Ⅳ 진단 준거에 의한 정신과 전문의의 소견과 심리학적 진단 평가 하에 주의력결핍과잉 행동장애로 진단된 아동 31명의 어머니였고 연령 범위는 28세에서 44세였다. 비교 집단으로는 동일 연령대의 정상 아동 51명의 어머니를 대상으로 하였으며, 연령 범위는 32세에서 45세였다. 주의력결핍과잉행동장애 아동의 연령 범위는 6세에서 11세까지(남자 29명, 여자 2명)였고, 비교 집단은 선별 검사로 진단적 평가 척도(DRS)의 부모용과 교사용을 사용하여 주의력결핍과잉행동장애가 아닌 것으로 밝혀진 6세에서 11세까지의 아동(남자 21명, 여자 30명)이었다. 모든 아동의 어머니에게 우울감 및 양육 관련 태도를 측정하기 위해 부모용 질문지 배터리를 실시하였으며 Student's t-test와 상관 분석, 단계적, 위계적 회귀 분석을 실시하였다. 결 과:상관분석에서 강압적 양육 행동이 주의력결핍과잉행동장애 아동 부모에게서 높게 나타났고, 양육 관련 태도도 더욱 부정적인 결과를 보였으며 우울 역시 더 높게 보고되었다. 회귀 분석 결과, 어머니의 양육 스트레스, 역기능적 사고와 양육 효능감 그리고 어머니의 우울감은 강압적 양육행동의 50%를 유의미하게 예측하는 변인임이 밝혀졌는데, 어머니의 우울감이 강압적 양육 행동의 29%를 유의미하게 설명하고 있어, 우울감을 느끼는 경우 더욱 강압적 양육행동을 보일 수 있음이 시사되었다. 따라서 주의력결핍과잉행동장애 아동 부모의 경우, 아동의 증상이 양육 스트레스를 야기하고, 높은 역기능적 사고와 낮은 양육 효능감으로 더욱 우울감이 심화될 가능성이 높고 결국에는 아동에게 강압적 양육 행동을 나타내는 악순환이 나타나는 것으로 생각된다. 결 론:주의력결핍과잉행동장애 아동의 어머니의 경우, 높은 양육 스트레스와 낮은 양육 효능감 및 아동과의 관계에서의 역기능적 사고의 영향 뿐 아니라 부모 자신의 우울감이 강압적인 양육 행동에 영향을 미침을 시사한다. 따라서 부모의 강압적인 양육행동을 변화시키기 위해서는 주의력결핍과잉행동장애 아동 부모의 양육 스트레스, 아동에 대한 역기능적 사고 및 양육 효능감을 다루어야 할 뿐 아니라, 부모의 우울감을 치료 시 고려해야 할 것이다.

COVID-19의 유행으로 인한 스트레스가 대학생의 삶의 질에 미치는 영향: 가족건강성의 조절효과 (The Effect of Stress Caused by the Spread of COVID-19 on the Quality of Life in University Students: The Moderating Effect of Family Health)

  • Objectives: As COVID-19 drags on, university students experience more stress and feel more anxious about their studies and career plans. Against the backdrop, this study aims to analyze the moderating effect of family health on the relationship between the spread of COVID-19 and the quality of life in university students. Methods: In order to achieve the purpose of this study, the results of an online survey performed with 216 university students in cities K and D were analyzed using frequency analysis, factor analysis, correlation analysis, regression analysis, and moderating effect analysis. Results: It was found that COVID-19 stress had a significant negative effect on the quality of life in university students, while family health showed a significant positive effect. In addition, the moderating effect of family health was statistically significant on the relationship between COVID-19 stress and the quality of life in university students. Conclusions: This study found that family health plays an essential role as a safety net in reducing stress and improve the quality of life in university students amid the social crisis of the COVID-19 pandemic. In this regard, it is required to develop various programs for communication with parents, including parent-child communication techniques.

학령 초기 자녀를 둔 어머니들의 부모됨 경험에 대한 질적 연구 (A Qualitative Study on Experiences of Parenthood Among Mothers of Early School-Age Children)

  • Objective: The purpose of this study was to gain an understanding of parenthood of mothers of early school-age children, with emphasis on cognitive, behavioral, and emotional aspects of parenthood. Methods: Nineteen mothers whose children were 1st and 2nd grade elementary school children participated in this study. Data were collected through individual in-depth interviews. During the interview, mothers were asked to speak freely about their experiences as parents of early-school age children, including parenting beliefs, parenting behaviors, and emotional experiences related to parenting. These interviews were transcribed verbatim and analyzed in a thematic way using MAXQDA. Results: Using qualitative methods, five major themes were emerged: (1) Recognizing the role of parents as a supporter of growth; (2) recognizing parenthood of the present generation that imposes expanded roles to mothers; (3) co-parenting of early school-age children and marital relationship; (4) co-existence of different parenting styles based on specific mother-child interaction situations; (5) experiencing a wide spectrum of emotions including anxiety, guilt, and depression, as well as happiness. Conclusion: This study revealed the unique cognitive, behavioral, and emotional aspects of parenthood experienced by mothers with early school-age children. Results of this study can fill a gap in our understanding of parenthood of mothers when their children entered into the new developmental stage of middle childhood. The results could also be used as a basis for developing parent education and family relationship programs.

한의원 내원 환아 및 보호자의 정신건강 특성 연구 (Psychological Characteristics of Infant Outpatients and their Parents in Korean Traditional Clinic)

  • Objectives: Since Korean traditional medicine has an integrative perspective by its nature, the patients' psychological and physical problems in a Korean traditional clinic may be resolved through a more integrative approach. The purpose of this study was to describe the latent characteristics of psychological health of child outpatients and their parents visiting Korean traditional clinic with physical complaints and was to propose an alternative curriculum to meet their needs based on their psychological and physical aspects. Methods: Subjects studied consisted of 100 outpatients (59 boys, 41 girls, mean age 55 months, age range 18 to 83 months) and their parents. Behavior characteristics for children were measured by Korean version of Child Behavior Checklist for Ages 1.5-5 (CBCL1.5-5) while parenting stress, depression, and anxiety for parent were measured by Korean-Parenting Stress Index-Short Form (K-PSI-SF), Center for Epidemiologic Studies Depression Scale (CES-D) and State-Trait Anxiety Inventory (STAI), respectively. The data of subjects were compared to those of standardized groups and then were compared according to child's gender within sample. Results: Child participants were reported to have significantly higher scores of all CBCL 1.5-5 subscales than the standardized group. When compared, no differences of K-PSI-SF, CES-D, and STAI was found between parents of participants and the standardized groups. However, parents of child participants with at-risk psychological problems showed significantly higher scores of all parenting stress subscales than those of all child participants. In addition, the mothers of boy participants demonstrated significantly higher anxiety, depression and parenting stress than those of girl participants irrespective of psychological severity. Conclusions: We discussed the implication of these results in clinical situation and make recommendations for curriculum of psychiatry and pediatrics with the aim of improving proper diagnosis, consulting and treatment.
