The Effect of Manual Lymphatic Drainage on the Stress and Pain in Patient with Postoperative Breast Cancer

  • Objective: This study was at investigated the effects of manual lymphatic drainage (MLD) on stress and pain in patients with postoperative breast cancer. Design: A randomized controlled trial. Methods: A total of twenty-two patients with postoperative breast cancer voluntarily participated in the study. Subjects were randomly assigned to the MLD groups (n=12) and rest groups (n=12). The intervention was conducted in each group for twenty minutes a day, five times a week, for four weeks. Before and after the intervention, the participants measured sympathetic nerve, parasympathetic nerve, and pain by using a sphygmograph and short-form McGill pain questionnaire.An independent t-test was used to analyze pretest and posttest changes between the groups, a paired t-test was used to analyze pre-posttests within each group. Results: After analyzing, the MLD group has been shown a significant decrease in the sympathetic nervous system (p<0.05), a significant increase in the parasympathetic nervous system (p<0.05), and a significant increase in pain (p<0.05). However, the rest group was no significant difference between pre and post. Conclusions: The results of this study confirmed that MLD techniques are an effective method in reducing stress and pain in patients with postoperativebreast cancer. And it is thought that can be used as basic data and to develop programs on stress and pain management reduction for patients with breast cancer.

Effects of Thoracic Mobility Exercise Program on Pain, Proprioception, and Static Balance Ability in Patients with non-Specific Chronic Low Back Pain

  • Objective: The objective of this study was to investigate the effect of the thoracic mobility exercise program on pain, proprioception, and static balance ability in patients with non-specific chronic low back pain. Design: Randomized controlled trial design. Methods: Thirty patients with non-specific chronic low back pain participated in this study. The participants were randomized into the thoracic mobility exercise group (n=15) and the lumbar stabilization exercise group (n=15). Both groups received traditional physical therapy for 30 minutes per session. In addition, the thoracic mobility exercise group and the lumbar stabilization exercise group each exercise 3 times a week for 6 weeks. All participants were measured visual analog scale, proprioception test, and static balance ability before and after the intervention. Results: After 6 weeks of interventions, the thoracic mobility exercise group showed greater improvement in visual analog scale, proprioception test, and static balance ability than the LSE group (p<0.05). Further, the thoracic mobility exercise group had significant Enhancements in all measured variables compared to the baselinetest (p<0.05). However, the lumbar stabilization exercise group had significant improvement only visual analog scale, and static balance ability compared to the baselinetest (p<0.05). Conclusions: Our investigation demonstrates that the thoracic mobility exercise is an effective intervention method for improving pain, proprioception, and static balance ability in patients with non-specific chronic low back pain.

만성 봉우리 밑 충돌증후군을 위한 물리치료적 접근법 : 편심성 훈련과 일반적 운동의 효과 비교 (Physiotherapy Approaches for Chronic Subacromial Impingement Syndrome : Comparison of Effects of Eccentric Training and General Exercise)

  • Purpose : This study was to verify the effect of eccentric training and suggest a specific application method by comparing the effects of acromohumeral distance (AHD), supraspinatus tendon thicknees (STT), pain intensity and functional performance after MWM and eccentric training (MWM-ET) or general exercise (MWM-GE) in chronic subacromial impingement syndrome (SAIS) patients. Methods : A total of 55 participants were randomly assigned to each group, and according to the intervention method, "MWM-ET group (n=28)" vs. "MWM-GE group (n=27)" was divided into two groups. AHD, STT, pain intensity, and functional performance were measured before intervention, and both groups were re-measured 3 times a week after 6 weeks of intervention in the same way. Results : The AHD was significantly increased in MWM-ET group compared to MWM-GE group. No significant difference was observed between the groups in the STT, but Pain intensity was significantly lower in MWM-ET group than in MWM-GE group, and functional performance was significantly increased in MWM-ET group compared to MWM-GE group. Conclusion : As a result of MWM-ET intervention that further increases AHD compared to MGE, it can be clinically presented as a more effective intervention method for faster recovery from injury due to pain reduction and smooth return to daily life due to improved functional performance.

먼쪽 노뼈 골절환자에게 적용한 칼텐본-에반스 관절가동술의 적용 횟수가 노자관절의 운동성, 통증, 기능과 악력에 미치는 영향 (The Effects of Kaltenborn-Evjenth Joint Mobilization of Application Count on Joint Mobility, Pain, Functions and Grip Strength in Patients with Distal Radius Fracture)

  • Purpose : The purpose of this study was to investigate the effects of Kaltenborn-Evjenth (KE) joint mobilization of the distal radioulnar joints (RUJ) and proximal RUJ in distal radius fractures (DRFs) on range of motion (ROM), grip strength (GS), and patient-written wrist evaluation (PRWE) in each group once, thrice, or fivefold. Methods : Forty-two subjects participated in this study. We divided the subjects with DRFs into groups applying KE concepts RUJ mobilization once, thrice, and fivefold. The patients' ROM and GS were measured using a joint goniometer and dynamometer, respectively. Pain and function were also assessed using a PRWE. In the statistical analysis, all data were tested for normality using the Shapiro-Wilk test, and paired t-tests were performed for within-group before-and-after comparisons of each intervention. One-way analysis of variance was used for between-group comparisons of differences. All statistical significance levels were set at α=.05. Results : There were significant differences in the ROM in all three groups before and after the intervention (p<.05), but there were no significant differences between the groups. There were significant differences in the GS in the three groups before and after the intervention (p<.05), but there were no significant differences between the groups. In the pain part of the PRWE, all three groups had significant differences before and after intervention (p<.05), but there was no significant difference between the groups. In the functional part of the PRWE, there were significant differences in the three groups before and after intervention (p<.05), but no significant difference occurred between the groups. Conclusion : Based on the aforementioned results, there were no significant between-group differences in ROM, GS, and PRWE (pain and function) after the application of the K-E joint mobilization to DRFs once, thrice, and fivefold. Nevertheless, there were significant within-group differences in all the above.

흉추 도수교정이 둥근어깨자세를 가진 30대 성인남녀의 통증, 관절가동범위, 근활성도에 미치는 영향 (The Effects of Thoracic Spine Thrust Manipulation on Shoulder Pain, Range of Motion and Muscle Activity in 30′s Adults with Rounded Shoulder Posture)

  • Purpose: This study was aimed to determine the effects of thoracic spine thrust manipulation on muscle activities of the scapular upward rotators and middle deltoid, active range of motion (AROM), shoulder pain, and rounded shoulder posture in young adults with rounded shoulder. Methods: The subjects were 30 young adults (14 males, 16 females) with rounded shoulder. Thirty subjects were randomly assigned to an experimental (manipulation) and control (placebo) groups of fifteen subjects respectively. The manipulation group received the manipulation (high velocity, low amplitude), which was performed by a physical therapist with the subject in the supine position and with the arms crossed over the chest and hands passed over the shoulders. For the sham group, the same procedure was performed, with the exception that the high-velocity thrust was not applied. Measurements were taken before and after the intervention. Muscle activity of upper and lower trapezius, serratus anterior, middle deltoid was measured using surface electromyography. Visual analog scale (VAS) was used for shoulder pain. Goniometry was used for shoulder abduction active range of motion (AROM). Straight edge was used for supine rounded shoulder posture (RSP) distance. Results: The muscle activity of the upper trapezius, lower trapezius and middle deltoid muscle increased significantly after the intervention (p<.05). However, no significant difference was observed in serratus anterior muscle (p>.05). The VAS was significantly decreased and AROM significantly increased after the intervention (p<.05). The distance of RSP were not significant (p>.05). The control group showed no differences before and after the intervention (p>.05). Conclusion: The results of this study suggest that thoracic spine thrust manipulation can be an effective component of treatment plan to improve pain and function.

녹음된 목소리를 통한 가족중재가 인공호흡기 이탈 과정 환자에게 미치는 효과 (Effect of an Intervention Using Voice Recording of a Family Member on Patients Undergoing Mechanical Ventilator Weaning Process)

  • Purpose: This study aimed to determine the impact of an intervention using voice recording of family members on pain, anxiety, and agitation in patients undergoing weaning from mechanical ventilation. Methods: A randomized control pre-post experimental design was implemented to 53 participants, with 27 and 26 participants in the experimental and control groups, respectively. A 70-second voice recording of a family member, repeated three times at 10-minute intervals was used as an intervention for the experimental group. Meanwhile, participants in the control group used headset for 30 minutes. Structured instruments were utilized to measure pain, anxiety, agitation, and the weaning process. Wilcoxon Signed Ranks test and the Mann-Whitney U test, or χ2 test, were used for data analysis. Results: The experimental group exhibited significant decrease in pain (Z = - 3.53, p < .001), anxiety (t = 5.45, p < .001), and agitation (Z = - 2.99, p = .003) scores compared with those of the control group. However, there was no significant difference between groups in the weaning process' simplification (χ2 = 0.63, p = .727). Conclusion: Intervention using family members' voice recording effectively reduces pain, anxiety, and agitation in patients undergoing weaning from mechanical ventilation. This can be actively utilized to provide a more comfortable process for patients.

VDT 증후군 환자에 물리치료와 인간공학적 개입: 사례 연구 (Physical Therapy and Ergonomic Interventions in Patients with VDT Syndrome: 4 Cases Study)

  • Purpose: Among computer users, the awkward posture and workstation setups of workers contribute to work-related upper limb musculoskeletal disorders. The purpose of this study was to evaluate the efficacy of a traditional physical therapy and ergonomic intervention by physical therapists in 4 computer users. Methods: After checking Visual Analogue Scores (VAS), four subjects who were treated by physical therapy for neck and shoulder problems related to VDT syndrome were enrolled in the study. All subjects spent at least 40 hours per week at a computer workstation. All subjects had pain and ergonomic states evaluated using methods such as VAS, Neck Disability Index (NDI), Workstyle short form, Rapid Upper Limb Assessment (RULA), and OSHA VDT checklist (Occupational Safety and Health Administration video display terminal) before a physical therapy + ergonomic intervention. Participants were re-evaluated 1 month later. Results: Participants showed more improvement of their neck pain after being treated with a physical therapy plus ergonomic intervention than when treatment consisted only of physical therapy. Improvements in RULA, Workstyle short form, and OSHA VDT checklist also were achieved. Conclusion: This case study suggests the importance of examining the work habits and work-related postures of subjects who complain of neck and shoulder pain that is exacerbated by computer use. Personalized ergonomic interventions and physical therapy can lead to improvement of patients with VDT syndrome.

아로마테라피 중재를 적용한 간호학 논문 분석 (An Analysis of Aromatherapy Intervention Studies in Nursing)

  • Purpose: This study aimed to analyze the effects and intervention methods of aromatherapy studies in nursing. Methods: A systematic literature review was conducted. Nursing literature databases were searched to identify intervention studies published in Korea between 2000 and 2011. Finally, 53 articles were included in the review. Results: The most preferred application method of aromatherapy was massage. The major dependent variables were anxiety, depression, pain, and sleep disturbance. 76.5% of the studies reported positive effects on anxiety. 93.8% of the studies reported positive effects on depression. 86.7% of the studies reported positive effects on pain. 92.3% of the studies reported positive effects on sleep disturbance. Psychometric measurements were mainly used in the studies. Only 33.9% of the studies adopted physical or physiological measurements. The most prevalent physical or physiological instrument was vital signs. Conclusion: Aromatherapy had positive effects on anxiety, depression, pain, and sleep disturbance measuring when using psychometric instruments. However, there was no sufficient evidence measuring when using physical or physiological instruments. These need to be studied using well-designed, randomized, controlled trials, and should encourage the use of both psychometric and physiological measures for aromatherapy intervention in nursing studies.

  • Hyaluronic acid (HA) has long been studied in diverse applications. It is a naturally occurring linear polysaccharide in a family of unbranched glycosaminoglycans, which consists of repeating di-saccharide units of N-acetyl-D-glucosamine and D-glucuronic acid. It is almost ubiquitous in humans and other vertebrates, where it participates in many key processes, including cell signaling, tissue regeneration, wound healing, morphogenesis, matrix organization, and pathobiology. HA is biocompatible, biodegradable, muco-adhesive, hygroscopic, and viscoelastic. These unique physico-chemical properties have been exploited for several medicinal purposes, including recent uses in the adjuvant treatment for chronic inflammatory disease and to reduce pain and accelerate healing after third molar intervention. This review focuses on the post-operative effect of HA after third molar intervention along with its various physio-chemical, biochemical, and pharmaco-therapeutic uses.

음악요법이 말기암 환자의 통증과 우울에 미치는 효과 (Effects of Music Therapy on Pain and Depression in Terminal Cancer Patients)

  • Purpose: The purpose of this study was to examine the effect of music therapy on pain, depression in terminally-ill patients. Methods: The subjects for this study were collected from 44 patients who were admitted in F hospital, located in D city from July 1 to July 31 in 2008. The subjects were divided into two groups: 23 experimental group members and 21 control members. While the control group was not received any additional intervention, the experimental group was received music therapy intervention. Data analysis were performed by the chi-square test, t-test and ANCOVA with SPSS/WIN 12.0 Program. Results: The 1st hypothesis is, "the experimental group who received music therapy would have less pain score than the control group who did not receive music therapy", was rejected (F=2.33, p=.14). The 2nd hypothesis is, "the experimental group who received music therapy would have less depression score than the control group who did not receive music therapy", was supported (F=11.616, p=.001). These results demonstrated that music therapy to terminally patients was an effective method in decreasing depression. Conclusion: Based on the results of this study, Music therapy can be utilized as an effective nursing intervention for the terminally ill.
