Outcome of Concomitant Cox Maze Procedure with Narrow Mazes and Left Atrial Volume Reduction

  • Background: To improve sinus rhythm conversion, the Cox maze III procedure with narrow mazes (width: ${\leq}3.0cm$) was performed in combination with left atrial volume reduction. Methods: From October 2007 to April 2013, 87 patients with atrial fibrillation (paroxysmal in 3, persistent in 14, and permanent in 70) underwent the Cox maze procedure concomitant with another cardiac procedure. They were followed-up with serial electrocardiographic and echocardiographic studies. We used 24-hour Holter monitoring tests to evaluate postoperatively symptomatic patients. Results: At the mean follow-up time of 36.4 months, 81 patients (94.2%) had sinus rhythm and two were on anti-arrhythmic medication (one on a beta-blocker and the other on amiodarone). Five patients (5.8%) with postoperative recurrent and persistent atrial fibrillation never experienced sinus rhythm conversion; however, they did not require any medication for rate control. On postoperative echocardiography, the left atrial A waves were more frequently observed after concomitant mitral valve repair than after concomitant mitral valve replacement (82.4% vs. 40.4%, respectively; p<0.001). Conclusion: For the Cox maze procedure, narrow mazes and atrial volume reduction resulted in excellent sinus rhythm conversion without the preventive use of anti-arrhythmic drugs, and they did not affect the presence of the left atrial A waves on echocardiography.

  • In order to understand the effect of duck hepatitis B virus (DHBV) on the economic performance of ducks, Three groups (DHBV congenitally infected, experimentally infected and DHBV negative) Brown Tsaiya ducks (Anas platyrhyncha) were used for experimental animals. Artificial insemination and pedigree hatching were applied in the propagation of ducklings, and the efficiency of vertical transmission and experimental infection was analyzed through the detection of DHBV DNA in the sera of 8-week-old offspring. The observation of the records of the first year indicated that the persistent infection had no significant effects on the performance of ducks, except the egg number of survival ducks up to 40 week of age. Thus DHBV infection did not appear to give ill effects to the economic performance of ducks in first laying year. A higher infection rate (85.3%) was obtained in congenital transmission than that (75.5%) of experimental infection. Both modes of infection did not reach 100% infectious rate, although some ducks developed transient viraemia in a tracing of DHBV DNA for 24 weeks to 11 challenged ducklings.

  • Atmospheric concentrations of persistent organic pollutants (POPs) were measured at Gosan, Jeju in November 2001 and spring 2002, each time for two weeks. Primary target pollutants were organochlorine pesticides, coplanar polychlorinated biphenyls (co- PCBs), and dioxin/furans listed in the Stockholm Convention adopted in May 2001. Polycyclic aromatic hydrocarbons (PAHs) were also measured in order to understand the overall characteristics of the POPs distribution as well as PM$_{2.5}$, a potent carrier of POPs. In the latter part of the measurement period of November 2001, almost every pollutant of combustion origin including dioxin/furans went high probably due to influence of emissions in the nearby area. The characteristics of atmospheric environment at Gosan in this period were rather close to urban areas far from those of a background area. A severe dust storm swept for three days at the end of the measurement period of spring 2002. However, changes in pollutant concentrations were relatively small except PM$_{10}$. Nevertheless, increases in particulate PAHs and OCDD (octachlorinated dibenzo-p-dioxins), mostly present in fine particles, were observed. Trends in organochlorine pesticide variations were mixed although possible volatilization of DDT residues from soil was inferred from the measurements of spring 2002.2.2.

Microdecompression for Extraforaminal L5-S1 Disc Herniation; The Significance of Concomitant Foraminal Disc Herniation for Postoperative Leg Pain

  • Objective : To analyze the relationship of concomitant foraminallumbar disc herniation (FLDH) with postoperative leg pain after microdecompression for extraforaminallumbar disc herniation (EFLDH) at the L5-S1 level. Methods : Sixty-five patients who underwent microdecompression for symptomatic EFLDH at the L5-S1 level were enrolled, According to the severity of accompanying FLDH, EFLDH was classified into four categories (Class I : no FLDH; Class II : mild to moderate FLDH confined within a lateral foraminal zone; Class III : severe FLDH extending to a medial foraminal zone; Class IV : Class III with intracanalicular disc herniation). The incidence of postoperative leg pain, dysesthesia, analgesic medication, epidural block, and requirement for revision surgery due to leg pain were evaluated and compared at three months after initial surgery. Results : The incidences of postoperative leg pain and dysesthesia were 36.9% and 26.1%, respectively. Pain medication and epidural block was performed on 40% and 41.5%, respectively, Revision surgery was recommended in six patients (9.2%) due to persistent leg pain, The incidences of leg pain, dysesthesia, and requirement for epidural block were higher in Class III/IV, compared with Class I/II. The incidence of requirement for analgesic medication was significantly higher in Class III/IV, compared with Class I/II (p=0,02, odds ratio=9,82). All patients who required revision surgery due to persistent leg pain were included in Class III/IV. Conclusion : Concomitant FLDH seems related to postoperative residual leg pain after microdecompression for EFLDH at the L5-S1 level.

  • Follicular cystic follicles (FCFs) show delayed regression with persistent follicle growth. However, the mechanism by which follicles are persistently grown remains unclear. Glutathione S-transferases (GSTs) are drug-metabolizing and detoxification enzymes that are involved in the intracellular transport and metabolism of steroid hormones. In this study, a proteomic analysis was performed to identify whether GST expression is changed in bovine FCFs and to predict the interactions between GST and other proteins. Normal follicles and FCFs were classified based on their sizes (5 to 10 mm and 25 mm). In bovine follicles, GST mu 1 (GSTM1) was detected as a differentially expressed protein (DEP) and significantly up-regulated in FCFs compared to normal follicles (p<0.05). Consistent with the proteomic results, semi-quantitative PCR data and western blot analysis revealed an up-regulation of GSTM1 in FCFs. Expression levels of aromatase and dehydrogenase proteins were changed in FCFs. These results show that the up-regulation of GSTM1 that is observed in bovine FCFs is likely to be responsible for the persistent follicle growth in FCFs as the activity of aromatase and the dehydrogenases.

  • The importance of XML data and their database supports in a mobile terminal system is on the increase. It leads to active research for embedded XML database systems. In order to handle in documents, DOM API should be supported. The existing DOM API is not suitable for the embedded system with limited resource because the DOM tree requires a large space on main memory. Considering poor resource of embedded systems, we present persistent DOM which is implemented on Berkeley DB and DOMdbm which manages it.

  • In 60 dairy cows with inactive ovaries, follicular cyst, luteal cyst, persistent corpus luteum and silent heat as diagnosed by rectal palpation, and those that had not resumed ovarian cycles until 60 days postpartum, progesterone concentrations for differential diagnosis of reproductive disorders were measured and were compared in matched plasma, skim milk and milk fat samples at 10 days interval. The incidence rate of reproductive disorders were as follows; inactive ovaries 20(33.3%), silent heat 11(18.3%), follicular cyst 7(11.7%), luteal cyst 7(11.7%), persistent corpus luteum 7(11.7%), pyometra 4(6.7%), vaginitis 2(3.3%), cystic corpus luteum 1(1.7%), and endometritis 1(1.7%), respectively. Cows having a progesterone concentration in plasma and skim milk < 1.0 ng/ml, and in milk fat < 80.0 ng/ml were considered to have inactive ovaries or follicular cyst. Those with concentrations in plasma and skim milk ${\geq}1.0ng/ml$, and in milk fat ${\geq}80.0ng/ml$ were regarded as the cases of luteal cyst or persistent corpus luteum. Progesterone concentrations in above cows did not differ significantly between the time of initial determination and the 10 days after initial determination. But progesterone concentrations in cows with silent heat did differ significantly between the time of initial determination and the 10 days after initial determination(P<0.05). The accuracy of rectal palpation for making a differential diagnosis of ovarian dysfunction, as defined on basis of progesterone concentrations, were as follows; follicular cyst 55.6%, luteal cyst 50.0%, inactive ovaries 90.5% and persistent corpus luteum 60.0%, respectively. It may be concluded that progesterone determinations at 10 days interval is practical as an aid to diagnosing ovarian dysfunction, particularly follicular cyst and luteal cyst.

Mitogen-activated Protein Kinases in the Development of Normal and Diseased Kidneys

  • Mitogen-activated protein kinases (MAPKs) play important roles in various cellular functions including proliferation, differentiation, and apoptosis. We showed that MAPKs are developmentally regulated in the rat kidney. p38 MAPK (p38) and extracellular signal-regulated kinase (ERK) were strongly expressed in the fetal kidney, whereas c-Jun N-terminal kinase (JNK) was detected predominantly in the adult kidney. The inhibition of p38 or ERK in organ culture resulted in reduced nephron formation with or without reduced kidney size. On the other hand, persistent fetal expression pattern of MAPKs, i.e., upregulation of p38 and ERK and downregulation of JNK, was observed in the cyst epithelium of human renal dysplasia, ovine fetal obstructive uropathy, and pcy mice, a model of polycystic kidney disease. Furthermore, activated p38 and ERK induced by cyclic stretch mediated proliferation and $TGF-{\beta}1$ expression in ureteric bud cells, probably leading to cyst formation and dysplastic changes. Inhibition of ERK slowed the disease progression in pcy mice. Finally, ERK and p38 were inactivated in the early embryonic kidney subjected to maternal nutrient restriction, characterized by reduced ureteric branching and nephron number. Thus, MAPKs mediate the development of normal and diseased kidney. Their modulation may result in novel therapeutic strategies against developmental abnormalities of the kidney.

Impact of COVID-19 Infection on Work Functioning in Japanese Workers: A Prospective Cohort Study

  • Background: The impact of COVID-19 infection on workers' work function persists even after the acute phase of the infection. We studied this phenomenon in Japanese workers. Methods: We conducted a one-year prospective cohort study online, starting with a baseline survey in December 2020. We tracked workers without baseline work functioning impairment and incorporated data from 14,421 eligible individuals into the analysis. We estimated the incidence rate ratio for new onset of work functioning impairment due to COVID-19 infection during follow-up, using mixed-effects Poisson regression analysis with robust variance. Results: Participants reporting infection between January and December 2021 showed a significantly higher incidence of new work functioning impairment (adjusted incidence rate ratio: 2.18, 95% confidence interval: 1.75-2.71, p < 0.001). The formality of the recuperation environment correlated with a higher risk of work functioning deterioration in infected individuals (p for trend <0.001). Conclusion: COVID-19-infected workers may continue to experience work difficulties due to persistent, post-acute infection symptoms. Companies and society must urgently provide rehabilitation and social support for people with persistent symptoms, recognizing that COVID-19 is not just a transient acute infection.
