BIOMECHANICS OF ABUTMENTS SUPPORTING REMOVABLE PARTIAL DENTURES UNDER UNILATERAL LOADING

  • Statement of problem. In distal extension removable partial denture, the preservation of health of abutment teeth is very important, but abutment teeth are subjected to unfavorable stress under unilateral loading specially. Purpose. The purpose of this study was to investigate the biomechanical effects of mandibular removable partial dentures with various prosthetic designs under unilateral loading, using strain gauge analysis. Material and methods. Artificial teeth of both canines were anchored bilaterally in a mandibular edentulous model made of resin. Bilateral distal extension removable partial dentures with splinted and unsplinted abutments were fabricated. Group 1: Clasp-retained mandibular removable partial denture with unsplinted abutments Group 2: Clasp-retained mandibular removable partial denture with splinted abutments by 6-unit bridge. Group 3: Bar-retained mandibular removable partial denture Strain gauges were bonded on the labial plate of the mandibular resin model, approximately 2 mm dose to the abutments. Two unilateral vertical experimental loadings (30N and 100N) were applied subsequently via miniature load cell that were placed at mandibular left first molar region. Strain measurements were performed and simultaneously monitored from a computer connected to data acquisition system. For within-group evaluations, t-test was used to compare the strain values and for between-group comparisons, a one-way analysis of variance (ANOVA) was used and Tukey test was used as post hoc comparisons. Results. The strain values of group 1 and 2 were tensile under loadings. In contrast, strain values of group 3 were compressive in nature. Strain values increased as the applied load in increased from 30N to 100N (p<.05) except for right side in group 1. Under 30N loading, in left side, group 1 showed higher strain values than groups 2 and 3 in absolute quantity (p<.05). And group 2 showed higher strain values than group 1 (p<.05). In right side, group 1 and 2 showed higher strain values than group 3 in absolute quantity (p<.05). Under 100N loading in left side, group 1 showed higher strain values than groups 2 and 3 in absolute quantity (p<.05). And group 2 showed higher strain values than group 1 (p<.05). In right side, group 1 and 2 showed higher strain values than group 3 in absolute quantity (p<.05). Under 30N loading, group 2 and 3 showed higher strain values in right side than in left side. Under 100N loading, right side strain values were higher than left side ones for all groups. Conclusion. Splinting of two isolated abutments by bridge reduced the peri-abutment strain in comparison with unsplinted abutments under unilateral loading. Bar-retained removable partial denture showed the lowest strain of three groups, and compressive nature.

PARENTAL ATTACHMENT STYLES AND PSYCHIATRIC MANIFESTATIONS IN THEIR $4^{th}\;TO\;6^{th}$ GRADE ELEMENTARY SCHOOL CHILDREN (부모의 애착 유형과 초등학교 고학년 아동의 정신병리와의 상관 관계)

  • Objectives : Few studies have examined the relation between the attachment styles of the primary caregivers and the behavioral problems of their children. This study was performed to identify the impact of the insecure parental attachment patterns on the development of their higher grades elementary school children's psychiatric manifestations and disorders. Methods : 504 higher elementary pupils and their primary caretakers were included in our study. Relationship Questionnaire, Kovacs Children's Depression Inventory, Spielberger's State-Trait Anxiety Inventory for children, and Korean Child Behavior Checklist were applied to them. Results : The grade-school children of the parents who belonged to preoccupied attachment type revealed higher Withdrawn, Anxious/depressed, Social Problems, Attention Problems, Delinquent Problems, Aggressive Problems, Internalizing Problems, Externalizing Problems, Total Behavioral Problems scores than ones of securely attached parents (p<0.01). Internalizing Problems score of preoccupied parents' children was highest, and one of avoidant and secure typed parents' kids followed in order (p=0.004) . More youngsters revealed Total Behavior Problem score high enough to clinically meaningful level in insecure parental attachment style than in secure one (p=0.038). Higher CDI score in insecure parental group was reported than in secure one (p=0.040). Conclusion : Parental insecure attachment can be associated with the development of the behavioral problems and psychiatric illness of their children.

Prognostic and Predictive Value of Hematologic Parameters in Patients with Metastatic Renal Cell Carcinoma: Second Line Sunitinib Treatment Following IFN-alpha

  • Background: Long-term survival is a problem with locally advanced and metastatic renal cell carcinomas. Sunitinib malate is an oral multitargeted tyrosine kinase inhibitor, but data on sunitinib use as a second line treatment in metastatic renal cell carcinoma (mRCC) are limited. Prognostic and predictive value of peripheral blood markers has been shown for many cancers. Materials and Methods: Efficacy and safety profiles of sunitinib after interferon alpha (IFN-${\alpha}$) were evaluated based on retrospective data for 23 patients with mRCC. Hematological parameters (neutrophils, lymphocytes, platelets, mean platelet volume, neutrophil/lymphocyte ratio, platelet/lymphocyte ratio) were recorded at the time of metastasis. It was evaluated whether hematological parameters were prognostic and predictive factors. Results: Median progression-free survival (PFS) time was 16.5 months (95%CI: 0-34.5). Median overall survival (OS) time was 25.7 months (95%CI: 10.8-40.0). Most common side effects were neutropenia (52.2%), stomatitis (26.1%) and hand-food syndrome (26.1%). PFS was found 3.13 vs 17.1 months in patients with neutrophil / lymphocyte ratio (NLR)>3 vs $NLR{\leq}3$ (p:0.012). Median OS was 6.96 vs 27.1 months in patients with NLR>3 vs $NLR{\leq}3$ (p:0.001).While 75% of patients who responded to sunitinib had $NLR{\leq}3$, in 72% of patients with no response to sunitinib NLR>3 was detected (p:0.036). The association between the Memorial Sloan-Kettering Cancer Center (MSKCC) criteria and NLR was statistically significant (p:0.022). Conclusions: Data on second line sunitinib treatment following cytokine in mRCC are limited. In our study, we observed second line sunitinib treatment following IFN-${\alpha}$ to be effective and tolerable. NLRappeared to have prognostic and predictive value.

Hardness and Rebound Properties of Sprayed Green Soil Produced with Functional Additives for the Application to Steep Slopes (기능성 첨가재를 적용한 급경사면용 녹생토의 경도 및 리바운드 특성)

  • In this study, the improved performance of sprayed green soil was evaluated by incorporating functional additives. The optimal mixing ratio of the thickener and super-absorbent polymer, as an additive for moisture supply to the growth of plants within the range of mixing ratios that gives sufficient strength of green soil, was 5% and 1%, respectively. Using Portland cement as a main binder, the pH of the green soil was 9.1. To solve this alkali problem, the mixing proportion was improved so that the pH of the green soil was approximately 7.2 by mixing more than 10% of the chelate resin. The soil conductivity was measured to be 280 ~ 350mS/m under all the mixing conditions. This satisfied the criterion of less than 1000mS/m on the slope surface. As a result of measuring the soil hardness of the green soil prepared under the optimal mixing conditions of functional additives, it satisfied the criteria of 18 ~ 23mm when sprayed under a 1 bar pressure. The rebound rate was less than 15% when spraying green soil on a 75 % slope, and the hardness of the sprayed green soil was more than 18 mm.

EFFECT OF CAVITY DISINFECTANT ON THE BOND STRENGTH AND MICROLEAKAGE OF DENTIN BONDING AGENTS (와동 세척제가 상아질 결합제의 결합에 미치는 영향)

  • Incomplete removal of bacteria contaminated dentin or enamel associated with caries is a potential problem in restorative dentistry Secondary or residual caries, pulpal inflammation and hypersensitivity may result from bacteria left after the initial preparation, especially if an adequate seal against microleakage is not obtained. A possible solution to eliminate residual bacteria left in a cavity preparation would be to treat the cavity with cavity disinfectant wash. But a potential problem with using a cavity disinfectant with dentin bonding agents could be their interference with the ability of the resin to bond to the tooth micromechanically. The purpose of this study was to evaluate the effect of 2% chlorhexidine containing cavity disinfectant ($Consepsis^{(R)}$) on shear bond strength and microleakage of dentin bonding agents, $Adper ^{TM}$ $Scotchbond^{TM}$ Multi-Purpose, $Adper^{TM}$ Single Bond and $Adper^{TM}\;Prompt^{TM}\; L-Pop^{TM}$ Sixty and sixty sound human third molar teeth, respectively, were used for shear bond strength and microleakage test. For experimental group, cavity disinfectant was applied before dentin bonding agents, and was not applied for the control group. The result from the this study can be summarized as follows ; 1. Use of 2% chlorhexidine containing cavity disinfectant($Consepsis^{(R)}$) does not significantly affect the shear bond strength of dentin bonding agents. 2. Use of 2% chlorhexidine containing cavity disinfectant($Consepsis^{(R)}$) does not significantly affect the microleakage of dentin bonding agents.

Effect of Major Depressive Disorder and Insomnia on Somatization (주요 우울증과 불면증이 신체화 증상에 미치는 영향)

  • Introduction: The objective of the present study was to investigate the independent effects of major depressive disorder (MDD) and insomnia on somatization, respectively. Methods: A total of 181 participants (73 males and 108 females ; mean age $41.59{\pm}8.92$) without serious medical problem were recruited from a community and a psychiatric clinic in Republic of Korea. Subjects were divided into 4 groups based on the Structured Clinical Interview for DSM-IV axis I disorder (SCID-IV) and sleep questionnaire : 1) normal controls (n=127), 2) primary insomnia (n=11), 3) MDD without insomnia (n=14), and 4) MDD with insomnia (n=29). All participants were requested to complete the somatization subscores of the Symptom Checklist-90-Revised (SCL-90-R). Results: There were significant between-group differences in somatization score (F=25.30, p<0.001). Subjects with both MDD and insomnia showed higher somatization score compared to normal control (p<0.001), subjects with primary insomnia (p=0.01), or MDD subjects without insomnia (p<0.001). Subjects with primary insomnia had higher somatization score than normal controls (p<0.01), while there was no significant difference between MDD subjects without insomnia and normal controls. In multiple regression, presence of insomnia predicted higher somatization score (beta=0.44, p<0.001), while there was only non-significant association between MDD and somatization (beta=0.14, p=0.08). Conclusion: In the current study, insomnia was associated with somatization independently from major depression. Subjects with primary insomnia showed higher somatization. Within MDD patients, presence of insomnia was related to higher somatization. Our finding suggests that insomnia may partly mediate the relationship between depression and somatization.

How the Science Gifted Connect and Integrate Science Concepts in the Process of Problem Finding (과학영재들이 문제발견 과정에서 나타내는 과학개념 연결방식과 융합적 사고의 특징)

  • The study aimed to investigate how the science gifted connect and integrate science concepts in the process of problem finding. Research subject was sampled from 228 applicants for a science gifted education center affiliated with a university in 2015. A creative problem solving test (CPST) in science, which administered as an admission process, was utilized as a reference to sample two groups. Sixty-seven students from top 30% in test scores were selected for the upper group and 64 students from bottom 30% in test scores were selected for the lower group. The CPST, which was developed by researchers, included one item about how to connect two science concepts among eight science concepts, sound, electricity, weight, temperature, respiration, photosynthesis, weather, and earthquake extracted from elementary science curriculum. As results, there were differences in choosing two concepts among four science major areas. The ways of connecting science concepts were characterized by three categories, relation-based, similarity-based, and dissimilarity-based. In addition, relation-based was characterized by attributes, means, influences, predictions, and causes; similarity-based was by attributes, objects, scientific principles, and phenomena, and dissimilarity-based was by parallel, resource, and deletion. There were significant (p<.000) differences in ways of connecting science concepts between the upper and the lower groups. The upper group students preferred connecting science concepts of inter-science subjects while the lower group students preferred connecting science concepts of intra-science subject. The upper group students showed a tendency to connect the science concepts based on similarity. In contrast, the lower group students frequently showed ways of connecting the science concepts based on dissimilarity. In particular, they simply parallelled science concepts.

Effect of Water Extract of Aloe in RANKL-induced Osteoclast Differentiation (파골세포 분화에 미치는 노회(蘆會) 추출물의 효과)

  • Osteoporosis is the leading underlying cause of fractures, particularly in postmenopausal women, due to the loss of estrogen-mediated suppression of bone resorption. More than 50% of adults 50 years of age or older are estimated to have osteoporosis. Osteoclast which is main target for treatment of osteoporosis is originated from hematopoietic cell line. Aloe has been widely used in worldwide country as a coadjuvant medicine. Extracts of the leaves of Aloe have been used in condition to improve dermatologic problem such as seborrheic dermatitis, aphthous stomatitis, xerosis, lichen planus and has been known to exert anti-inflammatory, anti-oxidant and anti-tumor effects. However, despite the popularity of aloe as a plant food supplements, the evaluation of its efficacy as a possible therapeutic option for osteoporosis remains scarce. Thus, we evaluated the effect of Aloe on receptor activator of nuclear factor-${\kappa}B$ ligand (RANKL)-induced osteoclast differentiation. Here we found that Aloe significantly inhibited osteoclast differentiation induced by RANKL. Aloe suppressed the activation of p38 pathway and $NF{\kappa}B$ in bone marrow macrophages (BMMs) treated with RANKL. Also, Aloe significantly inhibited the mRNA expression of c-Fos, tartrate-resistant acid phosphatase (TRAP), osteoclast-associated receptor (OSCAR), nuclear factor of activated T cells (NFAT)c1 and cathepsin K in BMMs treated with RANKL. Particularly, Aloe greatly inhibited the protein expression of c-fos and NFATc1. Taken together, our results suggested that Aloe may be useful tool for treatment of osteoporosis by inhibition of osteoclast differentiation.

  • Osteoporotic fracture became a serious social problem, which related with mortality and morbidity in old age population. Osteoclast which is responsible for bone resorption is originated from hematopoietic cell line and plays a key role osteoporotic bone loss. Cynanchum wilfordii (Asclepiadaceae) roots have been used in Korean folk medicine for the treatment of diabetes mellitus and aging progression. Also, recent studies have shown that the extract and fractions of Cynanchi Wilfordii Radix have various pharmacological actions including scavenging free radicals, enhancing immunity, reducing high serum cholesterol, and anti-tumor activity. However, the effect of extract of Cynanchi Wilfordii Radix in osteoclast differentiation had not been reported. Thus, we evaluated the effect of Cynanchi Wilfordii Radix on receptor activator of nuclear factor-${\kappa}B$ ligand (RANKL)-induced osteoclast differentiation. Through our study, we found that Cynanchi Wilfordii Radix significantly inhibited osteoclast differentiation induced by RANKL. Cynanchi Wilfordii Radix suppressed the activation of p38 pathway and $NF{\kappa}B$ in bone marrow macrophages (BMMs) treated with RANKL. Taken together, our results demonstrated that Cynanchi Wilfordii Radix may be useful treatment option of bone-related disease such as osteoporosis leads to fracture of bone and rheumatoid arthritis.

Exposure Assessment on Sub-Populations of the Local Community following a Hydrofluoric Acid Accident (불산 누출사고에 따른 지역사회 구성원들의 노출평가)

  • Objectives: The purpose of this study was to propose an indirect exposure assessment method using a questionnaire survey at a time when direct exposure assessment would be impossible after a chemical accident. Methods: About two weeks after an accident, a questionnaire survey was performed with 1,264 persons from the local community. Variables related to exposure were extracted from the survey contents, weighted and then graded for comparison with subjective symptoms in order to evaluate the extent of exposure. Survey items suitable for reflecting the previous exposure level during the accident were extracted, weighted and divided into quartile ranges. Subjective symptoms showed an increasing tendency with higher exposure level when compared with final exposure level (p<0.01). Results: For the relationship between the final exposure grade and subjective symptoms, as the exposure grade was increasing the rates complaining of symptom also showed an increasing tendency. However, when adjusted for demographic characteristics, there was a tendency for the eye irritation symptom to appear higher in women, and respiratory organ irritation appeared higher in smokers. Conclusions: When the problem of recall bias is considered, this study may not have completely unraveled exposure and the characteristics of the participants can affect subjective symptoms. Nevertheless, the exposure rating method of using a questionnaire showed a significant relationship with symptom level. It can be deemed that assessment of past exposure may be successfully evaluated by questionnaire in cases such as chemical accidents.
