서울시 거주 성인의 라이프스타일에 따른 건강신념 수준과 건강행동 실천 비교 (Comparison of Health Belief Levels and Health Behavior Practices according to Lifestyle among Adults Residing in Seoul)

  • This study compared levels of health beliefs and health behavior practices according to lifestyle pattern among adults in Seoul. A self-administered survey questionnaire was collected from a total of 1,004 Seoul residents aged 30-59 years. The levels of perceived benefit, perceived barrier, and self-efficacy from health belief model and health behavior practices were measured across multiple health behavior areas including dietary behavior, drinking, smoking, exercise, functional food consumption, and weight control behavior. Factor analysis and subsequent cluster analysis based on 28 lifestyle questions divided the subjects into four lifestyles of society-, economy-, trend-, and health-oriented lifestyle. Some general characteristics were significantly different by lifestyles. The society-oriented lifestyle was significantly higher in proportions of men and overweight. The trend-oriented lifestyle was significantly younger and spent more monthly allowance. Health-oriented lifestyle was older. The levels of health belief variables and health behavior practices significantly differed by lifestyles. Overall the health-oriented lifestyle showed more desirable levels of health belief variables and health behavior practice in various health behavior areas compared to the other lifestyles, whereas the society-oriented lifestyle was found the other way. Health belief model variables including perceived benefit, perceived barrier, and self-efficacy were generally significant in predicting the levels of various health behavior practice, with somewhat differences by lifestyle pattern and health behavior type. The study findings suggest it may be useful to segment target subjects according to lifestyle pattern in planning and administering health education programs.

  • Objectives: To understand relations general characteristics, lifestyle habits including smoking, alcohol habit, exercise, eating habit and menopausal symptoms measurement indicators during treatment for hot flush in menopausal women. Methods: The participants were 159 women (45~60 yr) who were not currently on hormone therapy, and had reached hot flash scores of 10 or higher. The evaluating indexes of this trial are hot flush score, hot flush visual analogue scale(VAS), Hot flush consistence time, sweating visual analogue scale(VAS), Menopause Rating Scale(MRS), Menopause-specific quality of life questionnaire(MENQOL), Kupperman's Index. Results: 1. When the participants divided according to body mass index(BMI), overweight group showed a high score in all indicators. Therefore, obesity can aggravate the symptoms of menopause. 2. When the participants divided according to drinking habits, Hot flush consistence time of non-alcohol group showed a statistically significant difference. However, the sample size is uneven. Additional studies will be needed. 3. When the participants divided according to eating habits, Hot flush consistence time of regular group showed a statistically significant difference. However, the sample size uneven. Additional studies will be needed. 4. When the participants was divided depending on whether exercise, non-exercise group showed a high score in all indicators. Thus, exercise will be able to improve menopausal symptoms. Conclusions: Improvement of obesity and exercise will be able to improve menopausal symptoms.

20대 여성들의 변비가 허리통증에 미치는 영향 (Influence of Constipation in Women in Their Twenties on Low Back Pain)

  • Background: In this study, the importance of constipation and back pain was assessed by regression analysis of the effects of stress, dietary habits, and water intake on constipation in women in their twenties and the influence of constipation and body mass index (BMI) To provide basic data. Methods: This study selected 109 having constipation of 120 students attending G University in Gwangju and eating habits, water intake level, stress and BMI. Trigger point at the tip of erector spina was palated with tenderness set in order to examine whether muscle tenderness and actual low back muscle tenderness level were same and left and right parts were measured three times and average of Max values was used. We examined the effect of constipation on low back pain and examined constipation and BMI to determine whether they affected low back pain. Results: There was no significant difference in eating habit although there was a significant difference in the effect of stress and water intake on constipation. Constipation had significant difference in back pain. However, there was no significant difference in BMI, normal weight, overweight, and obesity except for low body weight. Conclusions: This study found that constipation was associated with stress levels, water intake, and back pain. In conclusion, this study suggests basic data to prevent and treat constipation-related back pain, and recommends plenty of water intake, proper exercise and stress management to prevent constipation.

The Combined Effect of Subjective Body Image and Body Mass Index (Distorted Body Weight Perception) on Suicidal Ideation

  • Objectives: Mental health disorders and suicide are an important and growing public health concern in Korea. Evidence has shown that both globally and in Korea, obesity is associated with an increased risk of developing some psychiatric disorders. Therefore, we examined the association between distorted body weight perception (BWP) and suicidal ideation. Methods: Data were obtained from the 2007-2012 Korea National Health and Nutritional Evaluation Survey (KNHANES), an annual cross-sectional nationwide survey that included 14 276 men and 19 428 women. Multiple logistic regression analyses were conducted to investigate the associations between nine BWP categories, which combined body image (BI) and body mass index (BMI) categories, and suicidal ideation. Moreover, the fitness of our models was verified using the Akaike information criterion. Results: Consistent with previous studies, suicidal ideation was associated with marital status, household income, education level, and perceived health status in both genders. Only women were significantly more likely to have distorted BWP; there was no relationship among men. In category B1 (low BMI and normal BI), women (odds ratio [OR], 2.25; 95% confidence interval [CI], 1.48 to 3.42) were more likely to express suicidal ideation than women in category B2 (normal BMI and normal BI) were. Women in overweight BWP category C2 (normal BMI and fat BI) also had an increased OR for suicidal ideation (OR, 2.25; 95% CI, 1.48 to 3.42). Those in normal BWP categories were not likely to have suicidal ideation. Among women in the underweight BWP categories, only the OR for those in category A2 (normal BMI and thin BI) was significant (OR, 1.34; 95% CI, 1.13 to 1.59). Conclusions: Distorted BWP should be considered an important factor in the prevention of suicide and for the improvement of mental health among Korean adults, especially Korean women with distorted BWPs.

Relationship Among Body Mass Index, Nutrient Intake and Antioxidant Enzyme Activity of Postmenopausal Women

  • To elucidate the relationship among body mass index, nutrient intake and blood antioxidant capacity in the postmenopausal period, 60 women residing in Iksan area were recruited. Body mass index (BMI) was calculated base on height and weight, and food and nutrient intakes were estimated by 24-hour recalls of 3 non-consecutive days. Parameters of antioxidant capacity including the activities of superoxide dismutase (SOD), glutathione peroxidase (GPx), catalase (CAT) and total antioxidant capacity (TA) were measured in fasting blood samples from the subjects. The average age, height, weight and BMI of the subjects were 65 years, 151.1cm, 59.5 kg and 26.0 m/kg$^2$, respectively. The macronutrient intake rate of carbohydrate : protein : fat were 65:17.5:17.5; the mean intakes of energy and protein were 1532.7 kcal (86.3% of RDA) and 67.1 g (122.0% of RDA) respectively. The mean intakes of phosphorus, vitamin A, niacin and vitamin C were higher than Recommended Daily Allowance (RDA) for Koreans. On the other hand, calcium and riboflavin intakes were only 84.6% and 70.4% of RDA. Among the parameters of antioxidant capacity, SOD activity was significantly lower in lean subjects (BMI<20) than in the normal or overweight subjects (BMI${\geq}$20) (p<0.05). TAs of the subjects with the highest intakes of vegetables and fruits were significantly higher than those of subjects with lower intakes (p<0.05). Antioxidant capacity was compared among subjects according to 3 different nutrient intake levels according percentage of RDA for Koreans for selected nutrients with the following results: The high protein and niacin groups exhibited significantly lower TA status than those of the other intake groups (p<0.05). In conclusion, the low BMI was associated with lower SOD activity in postmenopausal women. Higher consumption of fruits and vegetables was associated with higher TA. When protein and niacin intakes were excessive, SOD activity and TA tended to be low. SOD and TA, among antioxidant indexes, seemed to be mostly influenced by other factors. Therefore, more studies on the effects of nutritional intake and the activity of antioxidant enzyme should be conducted.

Effects of caloric restriction with varying energy density and aerobic exercise on weight change and satiety in young female adults

  • This study examines the combined effects of caloric restriction on body composition, blood lipid, and satiety in slightly overweight women by varying food density and aerobic exercise. Twenty-three women were randomly assigned to one of two groups for a four-week weight management program: the high-energy density diet plus exercise (HDE: n = 12, $22{\pm}2$ yrs, $65{\pm}7$ kg, $164{\pm}5$ cm, $35{\pm}4%$ fat) and low-energy density diet plus exercise (LDE: n = 11, $22{\pm}1$ yrs, $67{\pm}7$ kg, $161{\pm}2$ cm, $35{\pm}4%$ fat) groups. Subjects maintained a low-calorie diet (1,500 kcal/day) during the program. Isocaloric ($483{\pm}26$ for HDE, $487{\pm}27$ kcal for LDE) but different weight ($365{\pm}68$ for HDE, $814{\pm}202$ g for LDE) of lunch was provided. After lunch, they biked at 60% of maximum capacity for 40 minutes, five times per week. The hunger level was scaled (1: extremely hungry; 9: extremely full) at 17:30 each day. Before and after the program, the subjects' physical characteristics were measured, and fasting blood samples were drawn. The daily energy intake was $1,551{\pm}259$ for HDE and $1,404{\pm}150$ kcal for LDE (P > 0.05). After four weeks, the subjects' weights and % fat decreased for both LDE (-1.9 kg and -1.5%, P < 0.05) and HDE (-1.6 kg and -1.4%, respectively, P < 0.05). The hunger level was significantly higher for HDE ($2.46{\pm}0.28$) than for LDE ($3.10{\pm}0.26$) (P < 0.05). The results suggest that a low-energy density diet is more likely to be tolerated than a high-energy density diet for a weight management program combining a low-calorie diet and exercise, mainly because of a reduced hunger sensation.

  • The objective of this study was to analyze body measurements, BMI and body type according to age in the male and female groups based on 3D scan body measurement data of adult men and women in their 20's~30's and to provide basic information usable in the clothing and fashion industry. For this purpose, we analyzed 3D scan body measurement data of 848 adults measured in the 6th Size Korea project and the results were as follows. First, there were differences in 3D scan body measurements according to age in the male and female groups, and in general the measurements of the height items were larger in younger adults and the measurements of the circumference, breadth and depth items were larger in older ones. Second, there were differences in 3D scan body measurements according to BMI in the male and female groups, and in general body measurements were largest in the obesity group and lowest in the underweight group. Third, BMI was different according to gender and in general BMI was higher in men and obesity and overweight were more frequent in men than in women. Moreover, BMI was significantly different according to age and was higher in the 30's than in the 20's. Fourth, the mean difference in the drop and lower drop values according to age in the male and female groups was significant, and the drop and lower drop values were lower in the 30's than in the 20's. These results are considered meaningful as data for the development of clothing size systems, patterns and grading in the areas of clothing and fashion targeting consumers in their 20's~30's.

Blood Triglycerides Levels and Dietary Carbohydrate Indices in Healthy Koreans

  • Objectives: Previous studies have obtained conflicting findings regarding possible associations between indices measuring carbohydrate intake and dyslipidemia, which is an established risk factor of coronary heart disease. In the present study, we examined crosssectional associations between carbohydrate indices, including the dietary glycemic index (GI), glycemic load (GL), total amount of carbohydrates, and the percentage of energy from carbohydrates, and a range of blood lipid parameters. Methods: This study included 1530 participants (554 men and 976 women) from 246 families within the Healthy Twin Study. We analyzed the associations using a generalized linear mixed model to control for familial relationships. Results: Levels of the Apo B were inversely associated with dietary GI, GL, and the amount of carbohydrate intake for men, but these relationships were not significant when fat-adjusted values of the carbohydrate indices were used. Triglyceride levels were positively associated with dietary GI and GL in women, and this pattern was more notable in overweight participants (body mass index [BMI] ${\geq}25kg/m^2$). However, total, low-density lipoprotein and high-density lipoprotein cholesterol levels were not significantly related with carbohydrate intake overall. Conclusions: Of the blood lipid parameters we investigated, only triglyceride levels were positively related with dietary carbohydrate indices among women participants in the Healthy Twin Study, with an interactive role observed for BMI. However, these associations were not observed in men, suggesting that the association between blood lipid levels and carbohydrate intake depends on the type of lipid, specific carbohydrate indices, gender, and BMI.

비만 지표 (Body Mass Index)가 만성 무배란 여성의 혈중 기저 호르몬치와 포도당 대사에 미치는 영향 (The Effects of Body Mass Index on Baseline Hormonal Status and Glucose Metabolism in Women with Chronic Anovulation)

  • Objective: To assess the difference of baseline hormonal status and pathophysio logy, and confirm the risk factors for long term complication according to Body Mass Index in women with polycystic ovary syndrome. Materials and Methods: Serum level of LH, FSH, Estradiol, Prolactin, Testosterone, DHEA-S, fasting insulin were measured and 100 gm oral glucose tolerance test and endometrial biopsy were performed in total 75 chronic anovulation patients and 20 normal cycling infertility patients. 95 evaluated patients were divided into 3 groups including patients with chronic anovulation having BMI below 25, BMI beyond 25.1, normal cycling infertility patients, Group 1 (n=39), Group 2 (n=36), Group 3 (n=20), respectively. Statistical analysis was performed respect to relationship between BMI and measured hormone level, sum of glucose level during 100 gm OGTT, insulin resistance using t-test, ANOVA test, Post Hoc test, Mann-Whitney test. p<0.05 was considered as statistically significant. Results: Serum LH level and LH/FSH ratio was significantly higher in Group 1, compared than Group 2 or 3 (p<0.05), BMI and LH, LH/FSH ratio was negatively correlated (r=-0.351, r=-0.318). There was no significant difference according to BMI in FSH, testosterone, estradiol, prolactin, DHEA-S level. Fasting insulin and sum of glucose level during 100 gm OGTT were significantly higher in Group 2 compared than Group 1 or Group 3 (p<0.05), there was no significant difference between Group 1 and Group 3. Insulin resistance was more frequently identified in Group 2 compared than Group 1 (p=0.001). Conclusions: BMI and LH, LH/FSH ratio were negatively correlated, so clinical significance of LH, LH/FSH ratio in diagnosis of PCOS may be attenuated by increasing body weight. Overweight patients with chronic anovulation may be the risk group for developing insulin resistance, hyperinsulinemia, glucose intolerance, later type 2 DM. Hyperinsulinemia may operate mainly in overweight chronic anovulation patients in development of hyperandrogenism.

Interactions Between Genetic Risk Score and Healthy Plant Diet Index on Cardiometabolic Risk Factors Among Obese and Overweight Women

  • People with higher genetic predisposition to obesity are more susceptible to cardiovascular diseases (CVDs) and healthy plant-based foods may be associated with reduced risks of obesity and other metabolic markers. We investigated whether healthy plant-foods-rich dietary patterns might have inverse associations with cardiometabolic risk factors in participants at genetically elevated risk of obesity. For this cross-sectional study, 377 obese and overweight women were chosen from health centers in Tehran, Iran. We calculated a healthy plant-based diet index (h-PDI) in which healthy plant foods received positive scores, and unhealthy plant and animal foods received reversed scores. A genetic risk score (GRS) was developed based on 3 polymorphisms. The interaction between GRS and h-PDI on cardiometabolic traits was analyzed using a generalized linear model (GLM). We found significant interactions between GRS and h-PDI on body mass index (BMI) (p = 0.02), body fat mass (p = 0.04), and waist circumference (p = 0.056). There were significant gene-diet interactions for healthful plant-derived diets and BMI-GRS on high-sensitivity C-reactive protein (p = 0.03), aspartate aminotransferase (p = 0.04), alanine transaminase (p = 0.05), insulin (p = 0.04), and plasminogen activator inhibitor 1 (p = 0.002). Adherence to h-PDI was more strongly related to decreased levels of the aforementioned markers among participants in the second or top tertile of GRS than those with low GRS. These results highlight that following a plant-based dietary pattern considering genetics appears to be a protective factor against the risks of cardiometabolic abnormalities.
