A Study of the Recent Diseases in Korean Pediatrics and Adolescent Patients Treated with Oriental Medicine (최근 국내 한방 진료를 받은 소아·청소년 환자의 질환 진단명 분석)

  • Objectives The purpose of this study is to investigate recent trend of diseases in Korean pediatrics and adolescent patients treated with oriental medicine. Methods Using data from the Korean statistical information service and healthcare bigdata hub, top 500 diseases pediatrics and adolescents that were treated with oriental medicine from 2012 to 2016 in admission and outpatient department was collected. Results From the inpatient study, majority of the subjects were between 15 to 19 years old (62.74%), followed by 10 to 14 years old, 5 to 9 years old and under 5 years old. In the outpatient department study, majority was 15 to 19 years old (36.51%), followed by 10 to 14 years old, under 5 years old, 5 to 9 years old. In systemic division of admission part, the most common disease was musculoskeletal related which was 72.32%, followed by brain, nerve, respiratory, dermatology and digestive related diseases. In systemic division of outpatient department, respiratory disease was the most common (41.81%), followed by digestive, dermatology, brain and nerve diseases. For under 5 years old patient group, respiratory disease was the most common, 29.86%, followed by dermatology, musculoskeletal, digestive related diseases. For 5 to 19 years old group of patients, musculoskeletal disease was most common. For the 5 to 14 years old patient group, respiratory related disease was the most common followed by dermatology and digestive diseases. For 15 to 19 years old patient group, digestive disease was the most common followed by respiratory and dermatology related diseases. For under 5 to 9 years old outpatient group, respiratory disease was the most common, and for under 5-year-old group, digestive, growth development, and dermatology disease were common. For the 5 to 9 years old group of patients, musculoskeletal related disease was the most common followed by digestive and dermatology related diseases. For the 10 to 19 years old patient group, musculoskeletal was the most common. For the 10 to 14 years old patient group, respiratory related disease was the most common followed by digestive, dermatology disease. For the 15 to 19 years old patient group, digestive related disease was the most common followed by respiratory, dermatology diseases. Musculoskeletal disease increased every year, in both inpatient and outpatient. Respiratory, brain, nerve, digestive related diseases were generally decreased. In outpatient, respiratory diseases were increased every year but brain, nerve, digestive related diseases were generally decreased. Conclusions More studies about the oriental medicine in chronic disease, such as allergy, metabolic syndrome, in Korean pediatrics and adolescents are needed.

  • Objectives The purpose of this study was to analyze the current status of utilization of Korean medical institutions. Methods This study analyzed the Health Insurance Review and Assessment Service data (2016-2020). We included patients with one of the five most frequent diseases in Korea and examined the variation in healthcare utilization. Results This study analyzed the Health Insurance Review and Assessment Service data (2016-2020). We included patients with one of the five most frequent diseases in Korea and examined the variation in healthcare utilization. Conclusions As a result of this study, It was possible to understand the impact of Chuna treatment and COVID-19 on the use of Korean medical institutions. Further studies are needed to establish a definite conclusion regarding the relevance of COVID-19.

Change in Healthcare Utilization by Disease Severity after Case Management for Medicaid (의료급여 사례관리 후 질병 중증도에 따른 의료이용 변화)

  • Purpose: This study examined change in healthcare utilization by disease severity after case management (CM) for Medicaid. Methods: Data were extracted from survey data on "Healthcare utilization and health status of Medicaid beneficiaries" conducted in 2007 and 2008 by the Ministry for Health, Welfare and Family Affairs. This study was designed to compare change in healthcare utilization between the CM group and the non-CM group. The subjects were 528 Type I Medicaid beneficiaries who utilized healthcare more than 365 days during 2006. Results: In beneficiaries having fewer than 3 among the 11 notified diseases, the CM group showed a significantly larger decrease in outpatient day, outpatient expense, medication day, and medication expense than the non-CM group. In beneficiaries having 3 or more among the 11 notified diseases, however, there was no significant difference in healthcare utilization between the CM group and the non-CM group. Conclusion: CM worked effectively on Medicaid beneficiaries outpatient healthcare utilization for mild diseases. However, its effects on hospitalization, which is a major cause increasing the total expense, were not observed. Therefore, a future study is needed to develope strategies to reduce hospitalization and care for Medicaid beneficiaries with severe diseases.

  • Objective : To use and its affecting factors of patients' outpatient treatment that have single and multiple chronic illnesses Method : We used the 2011 study by the Ministry of Health and Welfare, "Report on Usage and Consumption of Korean Medicine." This report was conducted on outpatients and inpatients that visited Korean traditional medicine, from August 25th, 2011, to September 30th, 2011. Our research was based on 1729 patients with chronic diseases aged 30 and over who received outpatient treatment during the last three months. Results : There were 1365 patients with non-complex chronic diseases, while there were 364 patients with complex chronis diseases. Patients had 1 - 8 chronic diseases, and the average number (standard deviation) was 1.26 (0.59). There were statistically significant differences by sex(P<0.0001), age(p=0.0045), marriage (p=0.0060), education level(p<0.0001), income level(P=0.0063), and types of health insurance(p=0.0023). The diseases most common among patients with non-complex chronic diseases were: low back pain, arthritis, gastrointestinal disorder, frozen shoulder, side effects from motor accidents, high blood pressure, fracture, stroke, diabetes, cancer, atopic dermatitis, and asthma. The diseases most common among patients with non-complex chronic diseases were: arthritis+low back pain, low back pain+gastrointestinal disorder, low back pain+side effects from motor accidents, low back pain+frozen shoulder, arthritis+gastrointestinal disease, gastrointestinal disease+frozen shoulder, arthritis+low back pain+gastrointestinal disease, high blood pressure+arthritis, arthritis+low back pain+frozen shoulder, arthritis+fracture, and arthritis+side effects from motor accidents. There were statistically significant differences among the usage of medical clinics by: frequently used clinic (p<0.0001), number of treatment (p<0.0001), the cost of outpatient treatment (p=0.0073), the satisfaction rate (p=0.0171), whether the clinic is the preferred clinic (p=0.0040). In model 1, men than women, people who had local benefits instead of type 1 medical aid, and patients with complex chronic diseases were more likely to use Korean medical clinics. In model 2, men than women, people who had local benefits than people with types 1 and 2 medical aids, people who went to pharmacies and Korean medicine pharmacies than people who went to hospitals, people who went to get treatment 1-10 times than people who visited 11-20 times and more than 41 times, and people who spent less than ten thousand Korean won than people who spent 1 to 2 million Korean won, and people without complex chronic diseases were more likely to use Korean medical treatment. Conclusion : There were differences in sociodemographic characteristics and the usage of medical clinics between patients with non-complex chronic diseases and patients with complex chronic diseases. Among patients with complex chronic diseases, women, patients with types 1 and 2 medical aid, patients who used Korean medical clinics, patients who were treated 11-20 times and more than 41 times, and patients who spent 1 million to 2 million Korean won on outpatient treatment used less treatment than patients with non-complex chronic diseases. However, patients with complex chronic diseases used pharmacies and Korean medicine pharmacies more.

A Study on the Establishment of Facility Guidelines for Infectious Diseases Hospitals - Focusing on Operational Methods and Architectural Planning (감염병전문병원의 시설 가이드라인에 관한 연구 - 운영방식과 건축계획을 중심으로)

  • Purpose: In terms of efficiency and safety, this study attempted to organize data on the operation methods and architectural planning of infectious diseases hospitals. Methods: The results obtained through on-site and interview surveys with hospital officials and medical staffs at four infectious diseases hospitals under construction were summarized based on those original business plans and facility guidelines. Results: First, the operational methods to secure safety and operational efficiency were summarized for each department which are major hospital functions of infectious disease hospitals. Second, as the architectural planning, the characteristics of space and circulation of each department are summarized. For safety of medical staff, negative pressure and non-negative pressure zone have to completely separated. In addition medical staff wears PPE and enters the negative pressure zone and returns in the order of admiral, shower, and gowning in the PPE undressing room after patient treatment. In case of operational efficiency, flexible operation is required in normal and crisis situations. For example, it is important for The Ward to gradually switch to negative pressure beds in times of crisis from normal situation and the outpatient department considers the composition of negative pressure and non-negative pressure outpatient spaces that can operate in parallel even in crisis situations. Implications: Infectious disease hospitals require flexible operation and appropriate facilities for normal and crisis situations.

  • Various changes in medical environments including growing elderly population, chronic diseases, deepening competition among hospitals since opening to foreign medical service, economic strategy for improvement of profit system have caused hospitals to be specialized. In this backgrounds, the purpose of this study is to receive basic data for architectural planning on the spatial organization of outpatient department in general hospital. The results of this study were as follows; First, major changes of outpatient department in general hospital are composed of 'co-work in medical examination and treatment', 'decentralization of diagnosis and treatment(D/T)' and 'patient-oriented medical service'. Changes by co-work system include appearance of medical offices for co-work, activation of specialized clinics, grouping of E/T section for outpatient and various types of specialized centers. Second, the grouping of E/T sections means the modification of E/T system and organization in general hospitals, and a new spatial organization will be needed. Third, the types of specialized centers are getting varied. they are classified into several types including disease-resource, social stratum-resource, human organ-resource, health-resource, rehabilitation-resource, alternative medical center and so on.

Regional Difference in Outpatient Service Utilization for Chronic Diseases among the Elderly (고령층 만성질환 외래이용의 지역 간 변이)

  • Background: With ageing and growing importance of disease management system, it is necessary to investigate the extent of regional difference in service utilization for chronic diseases among the elderly and to reflect it in designing the system. Methods: A multiple regression analysis and descriptive statistics analyses were employed using patient survey, which covers nationwide health facilities and their users. Results: While the differences in the rate of service utilization/utilization outside living area between urban and rural areas or between income levels are not large, considerable variations are observed within urban or rural areas and within income groups. Conclusion: This results suggest that it is important to subsidize economically disadvantaged segments of the population and residents of less-favored areas to be better-equipped for chronic disease management in order to prevent the development of severe ailments and the need for treatment at higher-level medical institutions. Improvements to the service infrastructure in vulnerable regions are essential.

Analysis on the Multiple Frequency Disease Trend of Yeongbuk in Gangwon-do (강원도 영북권역 다빈도 질환 추세분석)

  • The purpose of this study was to analyze the frequency of use of hospitalization/outpatient care of patients with addresses in Yeongbuk-gu, Gangwon-do to identify the medical demand and status of medical use in Yeongbuk-do. The National Health Insurance Corporation (www.data.go.kr) conducted an analysis of health insurance and medical benefits (inpatient/outpatient) claims data from 2003 to 2017. The order of the highest frequency of chronic disease was ranked from 1st to 60th according to 'Healing Case and %'. In addition, the use of patients at Sokcho Medical Center, the only hospital in the Yeongbuk region, was analyzed at the same time. According to the analysis, the use of outpatient treatment and In-patients in the Yeongbuk area of Gangwon-do is increasing due to chronic non-infectious diseases from the past acute diseases. In particular, it is necessary to expand the functions of schizophrenia and obstetrics and gynecology and to carry out specific health projects necessary for the health care of the local community. Through this, we should improve the quality of life in Yeongbuk, Gangwon Province.

The relationship of sperm DNA integrity with serum vitamin levels (folate and cobalamin) and food consumption in infertile men

  • Objective: The aim of this study was to investigate the relationships of serum folate (vitamin B9), cobalamin (vitamin B12) levels and diet with semen parameters (semen standard parameters [SSP] and DNA fragmentation index [DFI]) in infertile men. Methods: Sperm samples were assessed for SSP and DFI (using the sperm chromatin dispersion test). Serum vitamin concentrations were measured with an immuno-electrochemiluminescence assay, and men completed a semi-quantitative food frequency questionnaire (FFQ). Results: Serum folate levels were positively correlated with sperm progressive motility and DFI. A comparison of SSP between two groups of patients according to serum folate concentration (B9 <4.840 ng/mL and B9 ≥4.840 ng/mL) showed significantly higher sperm concentration and sperm progressive motility in the latter group. However, there was no difference between these groups regarding DFI. Interestingly, serum folate levels were significantly higher in patients with a high DFI (using the cut-offs of 30% or 18%). FFQ data showed that the consumption of fruits and egg yolk correlated positively with sperm concentration and sperm motility, respectively. Conclusion: Serum folate levels showed significant associations with sperm concentration and sperm progressive motility. However, the positive association of serum folate with DFI raises the need for careful prescription of folate supplements.

Determination of the type-2 diabetes risk status of the individuals who applied to the internal diseases outpatient clinic

  • The purpose of this study is to determine the type-2 diabetes risk status of the individuals applying to the internal diseases outpatient clinic and the affecting factors. This descriptive study was conducted on 500 individuals who applied to the internal diseases department of a hospital in Ordu Turkey, between May and June 2010 without diabetes diagnosis. The questionnaire form and Type 2 Diabetes Risk Assessment Form are used to collect the data. For the evaluation of data; independent samples t-test and One-Way Anova are used to evaluate the data. It is determined that the type-2 diabetes risk an score of the individuals participating in the study is $10.4{\pm}5.5$ and 32.4% of them have low type-2 diabetes risk score, 26.6% have slightly elevated score, 18% have moderate score, 16.6% have high and 6.4% have very high type-2 diabetes risk score. It is found that the type-2 diabetes risk mean score of the women participating in the study who are adults, are married, have low education level, are unemployed, are non-smoker, do not drink alcohol and do not have any information about type-2 diabetes is statistically significantly high (p < 0.05). It is determined that 16.6% of the individuals participating in the study have high type-2 diabetes risk and 6.4% have very high type-2 diabetes risk.
