Energy Consumptions and Daylight Illumination levels of a Multi-beded Patient Room according to the Window Shapes and Shading (창의 형태 및 차양 계획에 따른 다인 병실의 에너지소비량과 주광조도의 평가 및 분석)

  • Window and shading designs have a great influence on energy consumption and daylighting in buildings. As far as energy is concerned, small window area is advantageous. But it is not good to the patient healing in hospital. So it is important to find out the optimum window shape which is favorable for both energy consumption and patient healing. In this study, annual energy consumption and daylight illumination levels were analyzed according to the window shapes and shading devices for a multi-beded patient room in hospitals. The simulations were conducted for 19 different cases by COMFEN 4.0 computer simulation program. The results of this paper are as follows. First, window to wall area ratio and shading devices have great influences on annual energy consumption. But it is a problem in that they decrease significantly daylight level in bed room. Second, considering the same energy consumption, reducing the width of window rather than the hight of window is desirable for the secure of daylight level. Third, increase of the number of horizontal shade is not desirable in south face of the building for the energy consumption and daylight level. Fourth, sun shade is not necessary in north face of the building for the energy consumption and daylight level.

A Idiopathic Oculomotor Nerve Palsy Patient Treated by Acupuncture Complex Therapy (특발성 동안신경 마비환자 치료 1례에 대한 증례 보고)

  • Objectives : This study is designed to find out the improvement of eyelid ptosis and eyeball movement trouble caused by idiopathic oculomotor nerve palsy after diagnosing the patient to Ghan-shil (肝實), Bee, Shin-hur(脾腎虛) and treating the patient with acupuncture and herb medicine. Methods & Results : The changes in clinical symptoms of eyelid ptosis and eyeball movement trouble were described as the patient was treated with acupuncture therapy and herb medicine, Shae-ghan-san gagam(洗肝散加減) and Yukmi-jihwang-tang gagam(六味地黃湯加減). Conclusions : The patient was treated by acupuncture, herb medicine on idiopathic oculomotor nerve palsy for 6 weeks and showed good effect.

Depression of Stroke Patient자s Family Caregivers and the Relating Factors (뇌졸중 환자 가족 간호자의 우울 및 영향 요인에 관한 연구)

  • In this paper, we examined the depression of stroke patient's caregivers and analyzed influencing factors of the depression. The subjects were 215 caregivers who have takencare of stroke survivors in their home. The conceptual model of this study consisted of the caregiver's depression, perceived burden, illness intrusiveness, and patient's ADL. Modified Korean CES-D, modified subjective and objective Burden Scale, Illness intrusiveness(II), and Instrumental Activity of Daily Living(IADL) were used to measure concepts. Path analysis was used to test the model of this study. The results were as follows: 1. The mean depression score was 11.6 which was below the cut-off score of the CES-D. This score indicates that the subjects were higher than normal adults' mean score but not depressive. Eighty-six out of 215 caregivers(40%) were above the cut-off score. This finding was different from previous research results, and the reason might be the patients' capability of ADL. In a group of low capability patient's activities of daily living, caregiver's depression score was 15.5. 2. Caregiver's depression was positively related to caregiver's burden and illness intrusiveness, but negatively related to patient's activities of daily living. 3. The caregiver's perceived burden and illness intrusiveness directly influenced on their depression. Furthermore, the and caregiver's illness intrusiveness led to depression indirectly through their burden. A patient's activities of daily living didn't influence directly on depression but indirectly through caregiver's illness intrusiveness and burden.

A case of Patient with epigastralgia which appeared after Stroke Treated with Baekho-tang (뇌경색 이후 발생한 위완통의 백호탕 치험 1례 증례보고)

  • Objectives: The purpose of this study is to show a case of patient with epigastralgia which appeared after Stroke improved by Baekho-tang. Methods: We treated the patient with Herbal med, Acupuncture, Moxa. Results: Every Herbal med was no effect on Epigastralgia of patient. But The epigastralgia of patient was disappeared after using Baekho-tang. Conclusion: We can find out the effect of the Baekho-tang for epigastralgia which appeared after Stroke in this study.

  • Objective : This study was to investigate diagnosis and treatment based on an overall analysis of signs and symptoms on a hyperthyroid patient. Methods : The observation of the clinical progress was carried out by conducting diagnosis and treatment based on an overall analysis of signs and symptoms with the patient diagnosed hyperthyroidism. Results : Treatments such as the invigoration of qi (益氣) nourishing m (養陰) was given because the patient showed the qi deficiency (氣虛) and m deficiency (陰虛). Conclusion : Diagnosis and treatment based on an overall analysis of signs and symptoms of a hyperthyroid patient had moderate effects on the improvement of the patients condition.

A Study on Dietary Therapy for Patient with Peptic Ulcer (식이요법에 의한 위궤양의 치료 연구)

  • A serious case of the stomach ulcer was cured completely through a dietery treatment. The patient was listed as one of worse cases by the doctors. The patient refused to accept doctors' advises to have al surgical operation and sticked to his idea of dietetics. The doctors openly expressed their scepticism to a successful out come of a dietery treatment. However, it has been a success. A special ulcer diet menu prepared for the patient was based on those principles as follow: 1. Ulcer diet contained high protein (70g/day) and high energy (2,000cal/day). 2. To maintain a minimum degree of pressure on digestive process of the stomach, several small meals more than three times a day were served. Fat was avoided except that contained in milk and small amount of butter on toast. 3. To help to neutralize acid of the stomach, ample amount of milk was served. 4. Irritating food and beverage were prohibited. It was concluded that milk was one of the best diet for a peptic ulcer patient, for it could neutralize acid of the stomach as well as could provide ample amount of high protein which expedite healing craters formed by the stomach or duodenal ulcer. It was reported that most of unsucessful peptic ulcer cases of the local patients could be attributed to their failure in observance of an ulcer diet during their dietery treatment period, particularly their unfriendly attitude toward a milk diet.

Effects of Somatic Mutations Are Associated with SNP in the Progression of Individual Acute Myeloid Leukemia Patient: The Two-Hit Theory Explains Inherited Predisposition to Pathogenesis

  • This study evaluated the effects of somatic mutations and single nucleotide polymorphisms (SNPs) on disease progression and tried to verify the two-hit theory in cancer pathogenesis. To address this issue, SNP analysis was performed using the UCSC hg19 program in 10 acute myeloid leukemia patients (samples, G1 to G10), and somatic mutations were identified in the same tumor sample using SomaticSniper and VarScan2. SNPs in KRAS were detected in 4 out of 10 different individuals, and those of DNMT3A were detected in 5 of the same patient cohort. In 2 patients, both KRAS and DNMT3A were detected simultaneously. A somatic mutation in IDH2 was detected in these 2 patients. One of the patients had an additional mutation in FLT3, while the other patient had an NPM1 mutation. The patient with an FLT3 mutation relapsed shortly after attaining remission, while the other patient with the NPM1 mutation did not suffer a relapse. Our results indicate that SNPs with additional somatic mutations affect the prognosis of AML.

  • The u-health service is using portable device such as smart device and it consists of small computing device. The u-health service carry out same performance with desktop computer. We designed message structure based on Bluetooth HDP. This message structure is used to transmit patient's biometric data on the smart device of medical team, patient and family over the mobile network environment. ISO/IEEE 11073 PHD standard was defined based on the method of communication between the agent and the manager. And We are confirmed the reliable transmission of biometric data at the smart device by implementing the android OS based patient information monitoring application to check the status of patient for medical team, patient and family.

Clinical study on one patient with oculomotor nerve palsy accompanied by Diabetes mellitus complication history (당뇨 합병증의 과거력이 있는 동안 신경마비 환자1례에 대한 임상적 고찰)

  • Objective : To carry out the oriental medicine treatment on a patient with oculomotor nerve palsy accompanied by Diabetes mellitus complication history Methods : We conducted one patient with oculomotor nerve palsy at Won-Kwang University Kwang-Ju Oriental Medical Hospital. We treated patient by herbal medicine, acupuncture, etc. Results : We had an effect that one patient's symptom had improved. Conclusion : We suggest that the oriental medical treatment and management have an effect with oculomotor nerve palsy accompanied by DM complication history.

Comparison of Anesthesiologist Controlled Sedation and Patient Controlled Sedation during Neurolytic Pain Block and Regional Anesthesia (통증치료를 위한 신경차단과 부위 마취시 Anesthesiologist Controlled Sedation과 Patient Controlled Sedation의 비교)

  • The purpose of this study is to evaluate the feasibility, advantages/disadvantages of patient-controlled sedation (PCS) compared to anesthesiologist-controlled sedation (ACS) during neurolytic pain block and regional anesthesia. Forty patients were divided randomly into two groups of 20 patients each. Group 1(ACS) received 0.01 $mg{\cdot}kg^{-1}$ intravenous midazolam and 0.5 ${\mu}g{\cdot}kg^{-1}$ fentanyl intravenously by anesthesiologist just before, 30, and 60 minutes after the procedure to acheive sedation; Group 2 (PCS) patients self-administered a mixture of midazolam (0.4 mg) and fentanyl ($20{\mu}g$) using a syringe type infusion pump (Terumo, Japan) to acheive sedation. Considering the dermographics of patients, the types and durations of procedure performed, the level of average sedation the comfort level were similar in both groups. But the doses of midazolam and fentanyl administerd in group 2 were smaller than those in group 1 (p<0.01). Patients in PCS group showed their level of sedation more proper than did those in ACS group. However, patients in ACS group rated their level of comfort higher than did those in PCS group. The findings of this study indicate that PCS using a combination of midazolam and fentanyl is a fafe and effective technique. More studies are, however, needed to determinc the best choice of drug(s), doses, lock-out intervals, and possible use of continuous infusion with patient-controlled sedation.
