Effect of Informativeness on the State-Anxiety of Hysterectomy Patient (정보제공이 자궁적출술환자의 상태-불안에 미치는 영향)

  • The purpose of study carried out to determine the effect of informativeness influeucing the state-Anxiety of Hysterectomy patient, the relationship between Hysterectomy patient's general characteristics on State-Anxiety. and to investigation the cause of hysterectomy patient's Anxiety, the relationship between the cause of Anxiety and State-Anxiety. This stuad carried out between April 14. to May 24. 1983. This study sample consisted of 104 Hysterectomy patient who admitted to department of obstetrics of Seoul National University Hospital, Korea University Hospital, Han Yang University Hospital and Seung Nam Hospital. The Sample were divided into two groups; fifty two of experimental and fifty two of control group. The method used for the collection of data were set of information prepared by reseacher and state-Anxiety Inventory, The Questionnaire of state-Anxiety Inventory was made up 20 items. The data was analyzed by computer program. The results of the study were as follows. 1. There was shown state Anxiety to be alleviated by experimental group and control group. 2. There was significant relation between experimental group and control group. (p<0.01) 3. There was relation of state-Anxiety by education Level: Experimental group (p<0.05) control group (p<0.05) religion : only control group (p<0.05)economic state : only control group (p<0.05) The people who encouraged hysterectomy: Experimental group (p<0.05) control group (p<0.0001) 4. The cause of preoperative anxiety about hysterectomy were as follows: 1) destruction of body image(=loss of uterus) 2) husband's attitude 3) Expactation that hysterectomy will adversely affect sexual relation 5. Among the cause of Anxiety, R²for state-Anxiety were as follows: 1) economic state 17.3% 2) deatch 21.6% 3) Expactation that hysterectomy will adversely affect sexual relation 24.5%.

An Analysis of the Physician Productivity in General Hospitals (전국 종합병원 의료인력의 생산성분석)

  • The purpose of this paper is to identify factors affecting the optimum mix of required inputs and other relevant factors which account for the variation in physician's productivity in general hospitals, and to find out their implications for the efficient health planning and management. An extended version of Cobb-Douglas production function and cross sectional data of one day patient census from all general hospitals in Korea in 1988 were used in the analysis. Main results of the analysis and their implications could be summarized as follows : (1) The production function for physician's inpatient service shows the evidence of economies of scale, but the production function for physician's outpatient and adjusted-patient service, which combines both out- and in-patient service, shows that of dis-economies of scale. (2) The physician's role for production for all service is smaller than auxiliary personnel's, which imply that more intensive utilization of nurses, nursing aides and other auxiliary personnel is desirable for improving general hospital productivity (3) In case of physician's inpatient and adjusted-patient service, nurses' role is greater than nursing aides'. Therefore, more extensive utilization of nurses is recommended for the efficient operation of general hospitals. (4) The factor of hospital beds plays the leading role among required inputs in the production for physician's in- and adjusted-patient service. (5) The physician's productivity of general hospitals in rural area is lower than that in urban area. And the productivity of teaching hospitals is lower than that of the other hospitals. Further analysis was made in physician production function based upon the size of hospitals, namely those hospitals below 250 beds and those above. Explained variances by the factor of hospital beds was significantly increased in the case of those hospitals above 250. A more detailed and thorough investigation is needed for verifying factors influencing physician's productivity in general hospitals in Korea.

A Survey of the Medical Treatment Environment in Traditional Korean Medicine Clinics (한의원 환경 및 한의사의 진료 현황에 대한 연구)

  • Objective: To check the status of traditional Korean medical doctors' medical services amid the continual increase in the number of traditional Korean medicine clinics. Methods: A survey of traditional Korean medicine clinics based on questionnaire sheets mailed to 4,200 out of 10,895 clinics, of which 465, or 11.0%, responded, in the June 1, 2008 to December 9, 2008 period. Results: 1. 65.6% of the traditional Korean responding clinics are doing business in a rented space; 92.1%of them are one-person institutions; 24.4% of them, i.e., the largest group of those surveyed, operate in a space sized 41 (123 $m^2$)~50 pyeong (150$m^2$). The number of sick beds installed in their facilities comes to 7.9 on average. 2. Concerning support staff, 190 of them (or 40.9%), i.e., the largest group of those surveyed, employ two people in this capacity. They generally comprise assistant nurses (48.7%) and others (47.6%). 3. The size of the space used by the clinics is showing a tendency to increase. The number of sick beds and support staff, including assistant nurses, reached a peak in 2006, and has been on the decrease since then. 4. The average number of on-days comes to six days a week among 92.6% of those surveyed. Their average daily service hours come to 9 hours and 33 minutes (from 9:17 am to 6:50 pm). 5. Per-patient service time: 14 minutes on average; per-patient acupuncture time: 18.8 minutes; per-patient moxa cautery time: 10.1 minutes per-patient; boil-cupping time: 5.7 minutes; per-patient physical treatment: 28 minutes. Conclusion: Periodical studies should be carried out concerning desirable ways of developing traditional Korean medicine clinics with the focus on the facilities, doctors' service hours, and types of service.

  • Each college of dentistry in Korea is examining and reorganizing its own curriculum to prepare students for the practical examination of the National Dental Licensing Examination. Conducting an assessment of students' competency of communication skills with standardized patients is the first attempt for the examination. Yonsei University College of Dentistry carried out the evaluation of communication attitude, communication written exam, and communication skills practice with standardized patients for Third year predoctoral students. The purpose of this study is a) to find out the relationship between the variables measured in communication skills education and b) to make a new basis for the application of communication education and evaluation in dentistry. Based on the results of the study, the following suggestions for the operation of a dental communication curriculum can be drawn: First, the communication classes and the practice with standardized patients should be based on the wide support of the school. In addition, the patient-dentist interaction (PDI) should be practiced in the same way as practically possible by predoctoral students. Moreover, a space is required to allow predoctoral students to practice PDI. Second, the communication score given by the standardized patients was the most influential on the total score of PDI. This indicates that the communication practice with the standardized patients for the practical examination of the National Dental Licensing Examination is a meaningful process in the course of its communication education.

The Changes in Psychopathological Behavior of Schizophrenics in the Ward (정신분열증환자 간호에 대한 임상적 고찰)

  • A study designed to provide effective nursing care for schizophrenic patient was carried out to 22 patients who were admitted to the psychiatric in-patient service, St. Malays Hospital, Catholic Medical Center. The main purpose of the study was to provide effective means of discriminating the effects of nursing care for schizophrenic patients. The experimental group sampled consisted of 10 male and female patients who have been given patient-centered nursing care while the control group consisted of 12 male and female patients who have been given only routine care. The administration of the WBI manual in both groups obtained the changes in the psychopathological behavior of them. The result were found to be as follows. 1. The greater number of the patient in both groups were below 30 years of age (70%). 2. Uptill 15 days after admission there was no difference between the change of the psychopathological behavior the subject group and that of the control. 3. There as a difference between the change of the psychopathological behavior of the subject group and that of the control uptill 30 days after. admission(p<0.08).

A Families's Burden on a Cerebrovascular Accident Patient and the Demand for Nursing Home (뇌혈관질환자 가족의 부담감과 가정간호 요구도)

  • Purpose: This is to find out the correlation between a families burden on a cerebrovascular accident patient and the demand for nursing home. Method: The data was established by using structured questionnaires for 102 nursing families of cerebrovascular accident patient at three university hospitals. Result: 1) The total burden was showed to be at 3.21, which was some what higher than average. Total average of demand for nursing home was at 3.30, which was higher than average. 2) This showed static correlations between the economic, social, physical dependent and psychological burden, and demand for nursing home. The result represents that the higher burden showed a significantly positive correlation to the higher demand for nursing home. Conclusion: If a systemic support of nursing home is prepared the differentiated nursing home matched perfectly on each burden, the burden on a family of the patient can be decreased effectively.

  • Pain disorder is the chronic syndrome that onset and change of pain is associated with mood change. But the patient of pain disorder can't perceive their inner conflict. We experienced a 58 year-old woman who had general pain, and whose condition was improved through oriental medical treatment. In this case, we assessed the pain disorder patient at MMPI. So we learned out the patient's inner conflict and dynamic. The applicability of understanding with MMPI has positive effects on the patient with Panic disorder.

  • Background & Objectives: Colorectal Cancer is 10% of all cancer incidence and the motality from colorectal cancer is nearly 450,000 a year. Since chemopreventive agents from Herbal medicine is hot issue recently, to prove antitumor effecicacy of Rhus vernifciflua STOKES decoction (Chijong-dan), clinical study was carried out. Here we report one patient with colorectal cancer of stage IV. The patient had a colorectal cancer (Stage IV, T4N2M1), lymph node metastases and underwent chemotherapy. Methods: The patient visited Hana oriental medicine Clinic in 2009 and was treated using Chijong-dan for 1 year at Hana Oriental Clinic. Results: Chijongdan showed no side effect during its treatment and tumor size was retarded based on CT scanning. Overall, this case report suggests that Rhus vernifciflua STOKES decoction can be a potent cancer preventive agents for colorectal cancer but it is still required to verify the scientific and clinical evidences for Rhus vernifciflua STOKES decoction (Chijong-dan).

  • The purpose of this study is to find out the design guideline for women's hospital focusing on the patient needs. This study is performed the literature review of the performance evaluation & spatial satisfaction. And then, the spatial characteristics of selected women's hospitals, which were recently built in Korea, were analyzed. This study is suggested in the spatial satisfaction evaluation model in order to the spatial evaluation performance elements of the patient needs. The evaluation model of space efficiency derived from this study is expected to be used for the actual guide to architecture of women's hospitals as substantial materials. Finally, the model is used to for the planning & design for the future women's hospital.

  • 1. Objectives This study evaluates the effect of oriental medical treatment on a chronic sinusitis patient. 2. Methods The patient was teated with acupuncture, nasal cavity irrigation, and Yangdokbackho-tang(陽毒白虎湯). Previous western medical methods were excluded in order to provide therapeutic methods different from pre-existing ones because we needed different, long-term treatment plan from previous treatment. We applied acupuncture 15 times during treatment period and instructed the patient to carry out nasal cavity irrigation at home. 3. Results & Conclusions Combined application of oriental treatment including administration of Yangdokbackho-tang and nasal irrigation showed remarkable clinical effects on symptom, physical examination and radiological finding.
