발보조기가 자세동요에 미치는 효과 (Effects of Foot orthotic on Postrual sway)

  • The purpose of this study was to investigate the effect of foot orthotic on postural sway. Subjects were 15 patients(7 men and 8 women) who prescribed foot orthotic at B hospital and L rehabilitation medicine clinic in Seoul. Postural sway during a single limb stance was measured using the CMS 10 Measuring System when subjects positioned on the balance trainer under two treatment conditions(orthotic and nonorthotic). RCSP(resting calcaneal stance position) while subjects were standing on the glass plate was measured using the angle finder after subjects were positioned in prone to divide equally lower leg and calcaneus using the goniometer. The result was as follows. There were significant difference between postural sway of orthotic and nonorthotic conditions(t=4.888, 4.589, p<0.001), and the leg of the small RCSP within a subject has the small postural sway index($x^2=26.000$, p<0.001). In conclusion, foot orthotics provide structural support for detecting and controlling postural sway in patients prescribed foot orthotics.

4절 연쇄 보조기무릎관절의 개발 (Development of 4-Bar Linkage Orthotic Knee Joint)

  • 본 연구의 목적은 소아마비, 하지 근력약화 등의 장애인이 사용하는 잠금형 보조기무릎관절을 대체하기 위하여, 4절 연쇄 방식의 입각기조절형 보조기무릎관절을 개발하는 것이다. 본 연구에서 개발한 4절 연쇄 보조기무릎관절은 기존의 입각기조절형 보조기무릎관절과 달리, 기하학적으로 잠금상태가 제어되어 전기제어 부품 등이 필요하지 않으며 부피를 최소화하였다. 평가를 위하여, 소아마비로 진단받고 잠금형 보조기무릎관절을 사용해온 소아마비자가 대상자로 참여하였다. 대상자의 걸음걸이를 3차원으로 분석한 결과, 4절 연쇄 보조기무릎관절을 착용했을 때 입각기 동안 무릎관절의 신전이 유지되고 유각기 동안 무릎관절이 굴곡되며 자연스러운 걸음걸이를 보여주었다.

자세성 사두증의 진단과 치료 (Diagnosis and Treatment of Deformational Plagiocephaly)

  • It is important to distinguish deformational plagiocephaly from craniosynostosis, the two conditions are different with respect to clinical progression and treatment options. Deformational plagiocephaly is diagnosed based on the patient's medical history and physical examination. Until recently, there has been no standardized method of evaluation. Visual assessment, anthropometric assessment, digital scanning, and radiologic evaluation are mostly commonly used modalities for diagnosis and assessment. Treatment of deformational plagiocephaly requires an understanding of natural progression of the disease. Deformational plagiocephaly should be classified according to its severity before the proper method and time for treatment are determined. Treatment includes repositioning, physiotherapy and remodeling with the use of orthotic devices. In general, repositioning is preferred for patients younger than six months old while treatment with the use of orthotic devices such as helmet is preferred for patients over six months old. Moreover, treatment with the use of orthotic devices is also favored for severe plagiocephaly. There is continuing research on the relation between deformational plagiocephaly and developmental delay.

50~60대 평발 여성의 컴프레션 팬츠 개발을 위한 착용자 필요 조사 (User Needs of Women with Pes Planus in Their 50s and 60s for Compression Pants Development)

  • This study examined user needs for compression pant development for women with pes planus in their 50s and 60s. A total of 355 women aged 50 to 69 participated in the survey and interview. Questions were asked if they had pes planus, the using condition of foot orthotic, inconveniences during gait, and wearing condition of compression pants. The results showed that 42 (11.8%) women had pes planus. Orthotic insole and arch support were used most frequently. The most uncomfortable aspect of foot orthotic (n=146) was that it was difficult to use unless they were going outside. Participants with pes planus responded that they felt discomfort on the inner area of propodium, metatarsus, ankle, and knee during gait. The purchase and wearing rate of compression pants were not high; however, compression pants were purchased with specific needs and purposes. Respondents mainly wore the compression pants for sports activities. M size was the most frequently worn size. They preferred high waist type leggings and there was a need to increase the compression strength of the waist, thigh, knee and ankle. Additionally, the ease of donning and doffing were discussed.

골반 경사가 동반된 만성 통증 환자에게 족부 보조기를 적용한 증례 4례 (The Application of Foot Orthotic to Chronic Pain Patient with Pelvic Obliquity : 4 Cases Report)

  • Objectives : To evaluate the effect of foot orthotic in patient with chronic pain and pelvic obliquity in standing. Methods : Four cases of functional spinal scoliosis, were investigated for the changes in the calcaneal stance position angle, pelvic height, pelvic angle, Cobb's angle and walking pattern. Standing full spine X-ray for measuring the pelvic height, pelvic angle, Cobb's angle were checked before and after application of foot orthosis. The foot orthosis was composed of polyprophylen and chamude cover. Results : 1. There was no change in resting calcaneal stance position. 2. Difference of pelvic height and pelvic angle was reduced after application of a foot orthosis. 3. Cobb's angle in 2 cases was reduced after application of a foot orthosis. 4. Walking balance was improved. 5. Visual analogue scale was decreased. Conclusions : The study showed that foot orthosis seemed to be effective for chronic pain, spinal curve, pelvic obliquity and walking balance.

  • 본 연구에서는 3D 스캐닝에서 획득한 이미지 정보를 바탕으로 3차원 그래픽 제품설계와 3D 프린팅 기술을 적용한 보조기 제작방법을 제시하였다. 뇌성마비 환자를 대상으로 4가지 종류의 보조기를 기존의 수작업이 아닌 전산화된 작업 기술을 적용하여 제작해봄으로써 방법과 절차에 대한 기준을 마련하고, 이 기술이 임상적으로 사용 가능한지 여부에 대해 확인했다. 보조기 사용 시 환자만족도와 보조기 제작시간, 인장강도 등의 비교를 통하여 정량적, 정성적으로 제작방법의 적용 가능성을 검증한 결과 기존의 열가소성 수지가 표출했던 문제를 보완하고 대체가능한 방법이 될 가능성이 있음을 확인하였다.

Gait Angle Prediction for Lower Limb Orthotics and Prostheses Using an EMG Signal and Neural Networks

  • Commercial lower limb prostheses or orthotics help patients achieve a normal life. However, patients who use such aids need prolonged training to achieve a normal gait, and their fatigability increases. To improve patient comfort, this study proposed a method of predicting gait angle using neural networks and EMG signals. Experimental results using our method show that the absolute average error of the estimated gait angles is $0.25^{\circ}$. This performance data used reference input from a controller for the lower limb orthotic or prosthesis controllers while the patients were walking.

  • This paper deals with gait assisting rehabilitation robot which helps SCI patient walk again. We propose new concept of orthotic for robot considering motions of Hip and Knee Joints, and how to fit the robot to a user in terms of weight balance and comfortable standing. Then we describe our first engineering sample being designed based on the passive orthotic and show how to make the robot work for SCI patient in basic operation.

밀착형 외측 쐐기 스트랩 깔창의 높이에 따라 대퇴경골각에 미치는 영향 (The Effects of Femorotibial Angle of Contact laterally Wedged Insoles With Strapping of varying elevations)

  • The purpose of this study is to assess the radiographic effects of normal person with contact laterally wedged insoles with strapping of varying elevations. Eight person who were randomized into group according to their birth dates and wedge elevation, participants wore contact laterally wedged insoles with strapping with elevation of 9, 15, 21mm. Standing radiographs were used to analyze the femorotibial angle for each subject, The result of repeated measures ANOVA's reveled that laterally wedged insoles with strapping of varying elevations produced significantly the femorotibial angle. The degree of change in femorotibial angle with the insole with strapping was effected by the tilt of the lateral wedge(P<0.05). We suggest that these results may be beneficial for manufacturing foot orthotic devices, such as wedged insoles, to control medial and later compartment forces in the knee varus-valgus deformity.

Equivalent reinforcement isotropic model for fracture investigation of orthotropic materials

  • In this research, an efficient mixed mode I/II fracture criterion is developed for fracture investigation of orthotropic materials wherein crack is placed along the fibers. This criterion is developed based on extension of well-known Maximum Tensile Stress (MTS) criterion in conjunction with a novel material model titled as Equivalent Reinforced Isotropic Model (ERIM). In this model, orthotropic material is replaced with an isotropic matrix reinforced with fibers. A comparison between available experimental observations and theoretical estimation implies on capability of developed criterion for predicting both crack propagation direction and fracture instance, wherein the achieved fracture limit curves are also compatible with fracture mechanism of orthotic materials. It is also shown that unlike isotropic materials, fracture toughness of orthotic materials in mode $I(K)_{IC}{\mid})$ cannot be introduced as the maximum load bearing capacity and thus new fracture mechanics property, named here as maximum orthotropic fracture toughness in mode $I(K_{IC}{\mid}^{ortho}_{max})$ is defined. Optimum angle between crack and fiber direction for maximum load bearing in orthotropic materials is also defined.
