Discomfort on Wearing Thigh-Length Compression Stockings in Orthopedic Surgical Patients (정형외과 수술환자의 대퇴길이 압박스타킹 착용 불편감)

  • Purpose: This study aimed to investigate the degree of discomfort and to identify factors related with discomfort in patients wearing thigh-length compression stockings after orthopedic surgery. Methods: The participants were 105 patients wearing thigh-length compression stockings after orthopedic surgery at two hospitals in B city. Data were collected by the scale of discomfort in wearing compression stockings from July 5 to October 20, 2011. Results: Mean score of discomfort on wearing compression stockings was significantly higher at post-op. day 3 than day 1 (t=-2.30, p=.004). Discomfort scores for 'itching skin' and 'tightened leg' items were higher than others. There was significant difference in discomfort at post-op. day 1 according to surgical regions. The total score of discomfort at post-op. day 3 was positively related with the body mass index (r=.20, p=.041). Conclusion: Wearing thigh-length compression stockings to prevent deep vein thrombosis may cause physical and psychological discomfort for orthopedic surgical patients. To reduce discomfort in wearing compression stockings, nurses need to intervene discomfort and problems related to the stockings post-operatively.

Treatment of Fifth Metatarsal Base Fracture Using Tension Band Wiring (인장대 강선 고정술을 이용한 제5 중족골 기저부 골절의 수술적 치료)

  • Purpose: To evaluate the clinical and radiological results of internal fixation with tension band wiring for the fracture at the base of fifth metatarsal bone. Materials and Methods: From January 2008 to December 2009, 15 cases with displaced fracture at the base of fifth metatarsal were analyzed and average follow up period was 13.8 months. Lawrence classification was used to classify fracture type. We evaluated clinical results by American Orthopedic Foot Ankle Society (AOFAS) midfoot score and radiological results by union time. Complications was also checked. Results: According to classification, zone I fracture were 11 cases and zone II fracture were 4 cases. Bony union was achieved in all cases after 7 weeks. In the final follow-up, average AOFAS score was 94. There were no complications except hardware irritation. Conclusion: Satisfactory results were obtained after tension band wiring for the fifth Metatarsal base fracture in zone I fracture or comminuted zone II fracture for which it is not easy to be fixed with screw.

Treatment of Forearm Deformity caused by Hereditary Osteochondromatosis using Free Vascularized Fibular Epiphyseal Transplantation (생비골 성장판 이식술을 이용한 선천성 다발성 골연골증에서 전완부 변형의 치료)

  • It is difficult to manage the growing deformity of forearm bone caused by hereditary osteochondromatosis in children, because deformity and discrepancy of limb length is progressive. The are many treatment methods of these problems including excisio of osteochondroma, lengthening of ulna, shortening of radius, corrective osteotomy with or without lengthening apparatus. Among many treatment methods, we tried free vascularized epiphyseal transplantation with the proximal fibular epiphysis in 3 patients of hereditary osteochondromatosis for inducement of continuous bone growth and deformity correction. The average duration of follow up was 7 years and 1 month, the shortest duration being to 4 years and 5 months and the longest 10 years and 8 months. Serial radiologic and clinical evaluation were carried out during follow up and there were satisfactory length gain, deformity correction and improvement of adjacent joint motion in 2 cases. According to our follow up evaluation, free vascularized epiphyseal transplantation is valuable procedure in forearm deformity of hereditary osteochondromatosis although it needs skillful and experienced operative technique.

Comparison of Survival in Stage IIB Osteosarcoma According to the Chemotherapeutic Protocols (Stage IIB 골육종환자에서 항암 화학요법의 종류에 따른 생존율 비교)

  • From Sept. 1986 to Dec. 1992, seventy three cases of Enneking's stage IIB osteosarcoma of extremities, which were proved histologically, took neoadjuvant chemotherapy and completed our protocol. Their average age was 16.7 years(7 to 57). For neoadjuvant chemotherapy, 37 cases took high dose methotrexate(HDMTX)-adriamycin(ADR)-cisplatin(CDDP) regimen(HDMTX group) and 36 cases took ADR-CDDP(ADR-CDDP group). The average follow up was 17 months(2-63). According to Kaplan-Meier's plot, 5-year continuously disease free survival for whole 73 cases of neoadjuvant group was 45.2%, for HDMTX group 68.4%, for ADR-CDDP group 26.6%. There was significant stastical difference between these two groups(p<0.001), with log-rank test. There can be a different survival according to the chemotherapeutic protocols. Better results can be achieved through refined protocol and effective chemotherapeutic agents.

  • The osteopoikilosis is a rare disorder characterized by multiple radiodensities in the metaphysis or in the epiphysis of long tubular bones. The etiology and pathogenesis remain obscure, generally transmitted as an autosomal dominant fashion. The osteopoikilosis is asympotomatic and it is usually found radiologically as an incidental finding. Our case shows a typical clinical feature of the osteopoikilosis, and the biopsy was done to differentiate other disease from the osteopoikilosis.

  • An infection after total knee arthroplasty has many complications such as severe bone defect, skin and soft tissue problems, devastated general condition, so arthrodesis is preferred as treatment option. However, poor bony contact due to severe bone defect and inadequate conditions of the soft tissue often cause nonunion or severe limb shortening after arthrodesis. More over these conditions, it is not easy to choose appropriate fixative devices. In these situations, the arthrodesis using vascularized fibular graft can be the solution. Vascularized fibular graft (VFG) can playa role as a suitable material for the treatment of bone defects. And VFG can overcome poor blood circulation caused by scar tissues, and can be relatively more durable and adequate length. In the long term, VFG can be hypertrophied by weight bearing, and will give mechanical stablility. The purpose of the paper is to report the successful results of arthrodesis using VFG in a patient who got extensive bone defect after failed revision total knee arthroplasty with infection.

  • Purpose: To compare clinical characteristics of ganglions in hand & Foot. Materials and Methods: Seventeen cases of ganglions located in foot and fifty-five cases in hand. Excised from Mar.1988 to Apr.2003, were included in the study. The clinical characteristics and recurrence ratio were evaluated Results: The mean size of 2.2 cm in hand and 2.5 cm in foot. The most common area of ganglions are dorsum of foot and wrist. The cosmetic problem of palpable mass is the primary chief complaint of ganglions on hand and the pain is that of foot. The recurrence was found in 5 cases in hand and 4 cases in foot. The recurrence was related to incomplete excision of ganglion in foot and the large size of ganglion and incomplete excision of ganglion in hand. Conclusion: recurrence ratio in the cases of foot is higher than that of hand. The ganglions in foot and hand need to treated by meticulous surgical excision to prevent the recurrence.

  • The first metatarsophalangeal joint injury is common in professional soldiers and athletes. But this was rarely reported. A professional soldier has varus instability in the first metatarsophalangeal joint due to hyperextension. In the MR Imaging, weavy appearance in lateral collateral ligament and high signal change in plantar plate was shown. So he has surgical treatment using reconstructive procedure. At first, $4^{th}$ extensor digitorum longus tendon was splitted longitudinally and harvested, second triangular shape reconstruction on lateral joint line was done using harvested tendon. One year later, fifteen degrees was limited compared with intact side. Reconstruction using $4^{th}$ extensor digitorum longus tendon in traumatic dynamic hallux varus was good method.

  • Although calcific tendinitis around hip joint is a rare condition, patients with symptomatic calcific tendinitis serve very severe pain and are disturbed their daily activities. We report four calcific tendinitis patients who had acute pain around the hip with limitation of motion of the joint and were treated with ultrasonography-guided multiple needling with dramatic pain relief.

  • The anterior interosseous syndrome is an entrapment neuropathy which can be caused by several different etiologies. It is also clinically known that it weakens flexor pollicis longus, flexor digitorum profundus to the index and long fingers without loss of finger sensory. We have experienced anterior interosseous syndrome which found to be rare to ordinary conservative treatment, i.e. application of Kenny-Howard brace, given to a patient with acromioclavicular separation type III. We also review related articles.
