A Study on Actual Conditions of Leisure Time and Service Satisfaction of Community-dwelling Elderly -Focused on Buyeo (지역사회거주 노인의 여가시간활용과 바우처사업 서비스 만족도에 관한 연구-부여군을 중심으로)

  • Objective : This study conducted in rural areas, elderly services voucher business and leisure satisfaction survey based on data from the use of community-dwelling older people considering service needs was to find ways to take advantage of leisure time. Methods : Buyeo county residents in this study 71 patients were elderly, the characteristics of elderly gender, the presence of spouse, health status, education level, monthly living expenses, number of children, occupation, and the variables were included. Subjects' self-esteem, life satisfaction, quality of life, program satisfaction questionnaire for the survey were used, t-test and correlation analysis using the correlation between each factor were analyzed. Results : In a survey of participants in leisure activities, TV watching, meeting friends, such as phone calls to static and time-consuming activities were surveyed about satisfaction of the voucher business, the elderly oral health care services, health screening services, sports massage such as health-related services is relatively high degree of satisfaction. Conclusion : The results of this study of elderly people in rural communities through meaningful leisure time were utilized to determine what is lacking, a strong desire for health-related services that could be confirmed. Therefore, further experience and knowledge about activities by providing opportunities for meaningful leisure time needs to improve, and it based on a variety of programs to meet the needs of the elderly should be provided.

Primary implant stability in a bone model simulating clinical situations for the posterior maxilla: an in vitro study

  • Purpose: The aim of this study was to determine the influence of anatomical conditions on primary stability in the models simulating posterior maxilla. Methods: Polyurethane blocks were designed to simulate monocortical (M) and bicortical (B) conditions. Each condition had four subgroups measuring 3 mm (M3, B3), 5 mm (M5, B5), 8 mm (M8, B8), and 12 mm (M12, B12) in residual bone height (RBH). After implant placement, the implant stability quotient (ISQ), Periotest value (PTV), insertion torque (IT), and reverse torque (RT) were measured. Two-factor ANOVA (two cortical conditions${\times}$four RBHs) and additional analyses for simple main effects were performed. Results: A significant interaction between cortical condition and RBH was demonstrated for all methods measuring stability with two-factor ANOVA. In the analyses for simple main effects, ISQ and PTV were statistically higher in the bicortical groups than the corresponding monocortical groups, respectively. In the monocortical group, ISQ and PTV showed a statistically significant rise with increasing RBH. Measurements of IT and RT showed a similar tendency, measuring highest in the M3 group, followed by the M8, the M5, and the M12 groups. In the bicortical group, all variables showed a similar tendency, with different degrees of rise and decline. The B8 group showed the highest values, followed by the B12, the B5, and the B3 groups. The highest coefficient was demonstrated between ISQ and PTV. Conclusions: Primary stability was enhanced by the presence of bicortex and increased RBH, which may be better demonstrated by ISQ and PTV than by IT and RT.

  • The diagnosis of iron deficiency rests upon the correct evaluation of body iron stores. Morphological interpretation of blood film and the red cell indices are not reliable and often absent in mild iron deficiency. Serum iron levels and iron-binding capacity are more sensitive indices of iron deficiency, but they are often normal in iron depletion and mild iron deficiency anemia. They are also subject to many variables which may introduce substantial errors and influenced by many pathologic and physiologic states. Examination of the bone marrow aspirate for stainable iron has been regarded as one of the most sensitive and reliable diagnostic method for detecting iron deficiency, but this also has limitations. Thus, there is still need for a more practical, but sensitive and reliable substitute as a screening test of iron deficiency. Pollack et al. (1965) observed that the intestinal absorption of cobalt was raised in iron-deficient rats and Valberg et al. (1969) found that cobalt absorption was elevated in patients with iron deficiency. A direct correlation was demonstrated between the amounts of radioiron and radiocobalt absorbed. Unlike iron, excess cobalt was excreted by the kidney, the percentage of radioactivity in the urine being directly related to the percentage absorbed from the gastrointestinal tract. Recently a test based on the urinary excretion of an oral dose of $^{57}Co$ has been proposed as a method for detecting iron deficiency. To assess the diagnostic value of urinary cobalt excretion test cobaltous chloride labelled with $1{\mu}Ci\;of\;^{58}Co$ was given by mouth and the percentage of the test dose excreted in the urine was measured by a gamma counter. The mean 24 hour urinary cobalt excretion in control subjects with normal iron stores was 6.1% ($1.9{\sim}15.2%$). Cobalt excretion was markedly increased in patients with iron deficiency and excreted more than 29% of the dose. In contrast, patients with anemia due to causes other than iron deficiency excreted less than 27%. Hence, 24 hour urinary cobalt excretion of 27% or less in a patient with anemia suggets that the primary cause of the anemia is not iron deficiency. A value greater than 27% in an anemic subject suggests that the anemia is caused by iron deficiency. The cobalt excretion test is a simple, sensitive and accurate method for the assessment of body iron stores. It may be particularly valuable in the epidemiological studies of iron deficiency and repeated evaluations of the body iron stores.

A study on dental hygiene department students' attitude toward infection in a dental hygiene workshop (치위생 실습실에서 치위생과 학생의 감염에 관한 태도 연구)

  • Objectives : This study was to study dental hygiene department students' management of infection and their attitude toward infection. Methods : This study was conducted from August 24, 2009 to September 20, 2009. 269 sophomore and junior college students enrolled in the department of dental hygiene from schools located in Gyeonggi-do and Gangwon-do participated in the study. A self-administered questionnaire survey was conducted. Results : 1. As for dental hygiene department students' recognition of the causes of infectious diseases, 80.7% of the respondents said hepatitis B was the infectious disease highly likely to occur in laboratories. 35.3% was correct about the antecedent variables of infection in the workshop, 24.9% about the oral symptoms of HIV and 18.6% about the diseases induced by HBV. The recognition rates were generally low. 2. About whether the respondents ask patients questions about infectious diseases, 80.7% of them answered Yes, but only 56.2% of them said they do so every time, and 17.8% of them said they never do so. 3. As far as washing hands to prevent infectious diseases is concerned, 97.4% of the respondents said hand washing helps prevent infection. 72.5% of them said they wash their hands every time before they practice on a patient, while 84.0% of them said they washed their hands after the lab practice. 90.7% said they use liquid soap containing anti-microbial agents, and 81.8% of them said they use paper towels. 4. With regard to protective gear for prevention of infectious diseases, 98.9% of the respondents said it is desirable to use disposable protective gear for each patient. When it comes to what they actually used as protective gear, 91.1% said aprons, 89.2% gloves, and 87.7% masks. However, a low percentage of the respondents actually use goggles and replace masks when they got damp, 11.2% and 24.2% respectively. Conclusions : As for treating the surface of equipments to prevent infectious diseases, most of the respondents exhibited a high recognition rate. Relative fewer respondents actually treat the surface of equipments than those respondents who are aware of the need to do so. A high percentage of the respondents also said they use alcohol sponge to treat the surface of each equipment in order to prevent infectious diseases.

  • Purpose: This study was to analyze the trends and content of all the research (171 research papers) published in the Journal of Fundamentals of Nursing from 1994, when it began to 2000 using an objective tool developed by the researches, and to identify the direction for the future research in Fundamentals of Nursing. Method: Research published in the Journal of Fundamentals of Nursing from 1994 were reviewed. Result: Most of research (94.2%) was non-degree based and funded research was very limited. The average number of researchers per research study was 2.2 and collaborative research has been gradually increasing. The total number of key words was 331 and the average was 2.6 per research. The major key words were not different from other nursing departments and included home care nursing (10 times), health promotion (8 times), self-care (7 times). self-efficacy (7 times), and homo-dialysis (6 times). Eighty seven percent of the research was quantitative research. and there were very few qualitative studies. Considering theory level, it was found that 40% were factor related research For study design, non-experimental studies were most frequent (66%), and the rate, especially of surveys, has remarkably decreased. Selection of subjects by convenience sampling, was most frequent and there were very few studies that provided the rationale for the calculation of sample size. The major subjects of study were patients (44.8%) with various diagnoses. The subjects usually gave oral consent to take part in that study. Giving information (46.9%) and exercise (26.5%) were common nursing interventions, and physiological indices (16.5%). vital signs (10.3%), physical functioning (8.2%), level of knowledge/skill (7.2%) and level of activity (6.2%) were frequently measured as outcome. Variables questionnaires were the major approach used to collect data, and 57.8% of the research provided the Cronbach alpha to guarantee internal consistency of study instruments. Data were analyzed with computerized statistical packages using, ANOVA (42.0%), T test(39.5%), and chi-square test. For the last seven years, nursing research in Fundamentals of Nursing has gradually improved in both quantify and quality. Conclusion: It was difficult to find any uniqueness or difference compared to other departments of nursing. In fact, because the history of the Journal of Fundamentals of Nursing is rather short, we can expect that there will be further improvement in qualify and content in the future.

Accuracy of linear measurement using cone-beam computed tomography at different reconstruction angles

  • Purpose: This study was performed to evaluate the effect of changing the orientation of a reconstructed image on the accuracy of linear measurements using cone-beam computed tomography (CBCT). Materials and Methods: Forty-two titanium pins were inserted in seven dry sheep mandibles. The length of these pins was measured using a digital caliper with readability of 0.01 mm. Mandibles were radiographed using a CBCT device. When the CBCT images were reconstructed, the orientation of slices was adjusted to parallel (i.e., $0^{\circ}$), $+10^{\circ}$, $+12^{\circ}$, $-12^{\circ}$, and $-10^{\circ}$ with respect to the occlusal plane. The length of the pins was measured by three radiologists, and the accuracy of these measurements was reported using descriptive statistics and one-way analysis of variance (ANOVA); p<0.05 was considered statistically significant. Results: The differences in radiographic measurements ranged from -0.64 to +0.06 at the orientation of $-12^{\circ}$, -0.66 to -0.11 at $-10^{\circ}$, -0.51 to +0.19 at $0^{\circ}$, -0.64 to +0.08 at $+10^{\circ}$, and -0.64 to +0.1 at $+12^{\circ}$. The mean absolute values of the errors were greater at negative orientations than at the parallel position or at positive orientations. The observers underestimated most of the variables by 0.5-0.1 mm (83.6%). In the second set of observations, the reproducibility at all orientations was greater than 0.9. Conclusion: Changing the slice orientation in the range of $-12^{\circ}$ to $+12^{\circ}$ reduced the accuracy of linear measurements obtained using CBCT. However, the error value was smaller than 0.5 mm and was, therefore, clinically acceptable.

Importance of Job Tasks, Job Training Requirement and Work Satisfaction felt by Dental Hygienists (치과위생사의 수행업무의 중요성·업무에 대한 교육훈련의 필요도와 만족도 조사연구)

  • The study aimed to estimate the importance of job tasks, job training requirement and work satisfaction felt by dental hygienists to help dental hygienists work efficiently and effectively. A total of 142 dental hygienists working in dental hospitals located in J participated in the study. A SPSS 10.10 for Windows was used for statistical analysis. The statistical significance was defined as ${\alpha}$=.05. The results of the study are summarized as follows: 1. The mean score for the importance of job tasks was 4.09. The respondents considered dental clinic management and assistance to dentists the most important among their job descriptions showing the mean score of 4.44. The need for job training was 4.15 in average. The respondents wanted to have training on how to make a dental health insurance claim to receive payment other than any training with the mean score of 4.42. The work satisfaction of the respondents was 3.65 in average. The respondents were most satisfied with their work in dental assistance. 2. By the category of job tasks, the levels of importance of job tasks, job training requirement and work satisfaction were higher in plague removal with an ultrasonic scaler related to clinical oral prophylaxis, pit and fissure sealants related to preventive dental treatment and instruction in the correct usage of toothbrush related to public dental health education 3. In dealing with dental radiation machines, the radiation protection was considered the most important. While the training for periapical radiography was most needed, the respondents were satisfied with working in the same area. The levels of job importance, training requirement and work satisfaction were high in preventive dental care in the community performed in relation to the public dental health care. 4. In the dental health insurance claim category, the levels of importance of job task, training requirement and work satisfaction were higher in insurance claiming. The levels of job importance and training requirement were high in dental clinic management and assistance to dentists performed for preventive dental treatment. The work satisfaction was higher in maintaining medical chart records. The levels of job importance, training requirement and work satisfaction were higher in dental assistance in relation to dental protection. 5. There was significant relationship between the level of importance and work experience. Those with a longer period of work experience had higher levels of job importance in dental health insurance claim, dental clinic management and assistance to dentists and the overall task(pE0.05). 6. A significant relationship was observed between the need for job training in dental cleaning and preventive dental treatment and work experience. Those with more than 8 years of work experience demonstrated that job training is most needed(pE0.05). 7. The work satisfaction level was higher in the group with more than 8 years of work experience, compared with those in other work experience periods(pE0.05). 8. A positive correlation was seen between the level of importance and the need for job training among three variables.

Relationships between subjective teeth awareness and satisfaction on self-esteem (주관적인 치아에 대한 인식 및 만족도와 자아존중감과의 관련성)

  • The purpose of the study are expected to provide some basic information on counseling, diagnosis and treatment planning for patients related to teeth care. A self-reported questionnaire was filled out by 320 subjects in Seoul and Gyeonggi from May to August, 2015 after permission from Institutional Review Board (IRB). Except 18 copies, 302 data were analyzed using SPSS WIN 19.0 program. The questionnaire consisted of four questions of the general characteristics of the subjects, ten questions of the awareness toward of subjective teeth, five questions of the influence of teeth on appearance, and ten questions of self-esteem. The dependent variables made a 23.4% prediction of self-esteem. If they have higher education(p<0.001), influence of teeth on appearance(p<0.001) and the awareness toward of subjective teeth(p<0.001) had significant influence on self-esteem. The findings suggest that heightened subjective awareness toward teeth seems to be of use for the improvement of self-esteem. Therefore it's required to make an effort to take good care of teeth in terms of color or arrangement as well as oral health.

Vitamin D Level in Blood of Menopausal Women over 50 and the Relation with the Proportion Requiring Dental Scaling (50세 이상 폐경여성의 혈중 비타민 D 농도와 치면세마필요자율의 관련성)

  • The purpose of this study was to examine the relationship between vitamin D and periodontal diseases based on the raw data of the 5th National Health & Nutrition Examination Survey of 2010. The subjects in this study were 1,327 people, and those whose data on major variables were missing and who suffered from diabetes and/or osteoporosis were excluded. As for data analysis, R2.15.1 program and PASW Statistics 18.0 were utilized. The findings of the study were as follows: 1. As for all the respondents aged 50 and up, there was no significant relationship between vitamin D and periodontal diseases. 2. As for the post-menopausal women including the women who underwent bilateral ovariectomy, the vitamin D-deficit group 1 (<10) were 6.66-fold more likely to suffer from periodontal diseases than the vitamin D-sufficient group (${\geq}30$) (OR, 6.66; 95% CI, 1.004~44.19). The above-mentioned findings ascertained that vitamin D had a significant negative correlation to periodontal diseases among the post-menopausal women including the women who underwent bilateral ovariectomy. This finding should be taken into account in terms of the prevention and management of periodontal diseases.

The Household Characteristics and Underage Children's Dental Clinic Use - Focusing on the Mother's Characteristics - (가구특성과 미성년 자녀의 치과이용 - 모의 특성을 중심으로 -)

  • The purpose of this study was to use the Korea Health Panel to determine how the mother's characteristic and household characteristic variables were associated with dental clinic use of children in childhood and adolescence. The logistic regression analysis found that mother's education level was statistically significantly positively correlated with underage child's dental clinic use and children in single-parent family were less likely to use a dental clinic than those in two-parent family. As for dental clinic use by diseases, the higher level of mother's education, the more likely to use a dental clinic due to dental caries and children in single-parent family were less likely to use a dental clinic due to dental caries than those in two-parent family. No difference in dental clinic use due to tooth extraction was found by any of mother's characteristics or household characteristics, probably because age has an absolute impact during the period between milk teeth and permanent teeth. Lastly, mother's education level and household income were significantly positively correlated with dental clinic use due to orthodontics. On the basis of these results, it is necessary to develop a customized strategy reflecting children's growth stages and household characteristics in making a plan for promoting oral health of children and adolescents.
