조정산 도주 일가의 만주행록에 관한 재고찰 (A Reconsideration on the Records on Doju Cho Jeongsan and His Family in Manchuria, China)

  • In 2007, according to the records, I made an on-the-spot survey of the place where Doju Cho Jeongsan and his family might have lived in exile in Manchuria and released a paper in which I decided that the site could be the Shuidongchun (village) of Luotongshanzhen in Liuhexian, Jilin Province. Thereafter, sponsored by the Doju Cho Jeongsan's Memorial Project from 2008 to 2013, many times, I carried out the field investigations and researches on Liuhexian, including the visits of the institutions which have the historical documents, in order to find the data related to the participation of him and his family in the anti-Japanese movement. I was, hence, able to reconfirm that the village had been the place of their exile, based on my collected data and the oral reports which the local historians and ethnic Korean elders had provided. In this study, using the historical documents and maps and the oral materials, I made an attempt to prove the historical truth thoroughly once again. First, the existing sources of Doju Cho and his family's settling in Manchuria from March 1909 to 1917, were carefully analyzed which were described in The Jin-gyeong. In doing so, the misspelling of the names and the spatio-temporal errors of the people's activities were corrected. Next, I researched on another town, Shuitungou of Liuhexian in Fengtian Province (in West Gando of Manchuria), which it is known that Doju and his family stayed in, and the Laogushan (mountain), which it is believed that Doju cultivated himself in. Finally, through the attempt, I reached the conclusion that Doju and his family had settled at Shuidongchun (once called Shuidonggou or Shuitongchun) of Luotongshanzhen (once called Datonggou) in Liuhexian, Jilin Province. In the Liuhexian-related documents and maps published in the eras of Republican China and Manchuria, the place name called Shuitungou was not found. However, I discovered a map in the era of Republican China on which Shuitongchun was recorded as Shuidonggou. In addition, considering the administration system of Republic China, tun(屯) and gou(溝) could not be used together in the place names. Accordingly, Shuitungou was more likely misspelled as Korean people in those days mispronounced Shuidonggou. Furthermore, people in China has habitually called the Dagushan(大孤山), located in the north of Gushanzizhen of Liuhexian, as the Laogushan(老孤山). This means that the Korean people who lived in the area then perhaps recorded the mountain as the Nogosan(老姑山), the mountain of the old goddess, according to Korean enunciation, because they had the custom of worshipping the mountain goddess. I tried my best to find the historical documents regarding Doju and his family's anti-Japanese activities to prove the location of exile in which they settled in northeastern China (Manchuria). However, I was not able to reach the initial goal completely due to the shortage of objective evidences, only to leave tasks to be solved. I hope that this study can give a little help to researchers who are interested in this matter.

의료 관련 종사자들의 약물음독에 대한 고찰 -2곳의 응급의료센터 경험을 토대로 (Suicidal Poisoning Methods of Physicians and Medical Related Personnels)

  • Purpose: In recent review of physician suicides] the relative risk of physicians is higher than the general population. The majority of physician suicide were by poisoning. The purpose of this study was to analyse the medical personnels with suicide by poisoning compared with the general population. Methods: We reviewed medical records of 15 medical related personnels with suicide by poisoning who visited the emergency medical centers of St. Mary's and Kangnam St. Mary's hospitals from March 1998 to Aug 2004. For the comparison with general population in analysis] the collected data was acquired from medical records of 677 intoxicated patients in St. Mary's Hospital during the same period. Results: Fifteen suicides of physicians (n=7), nurses (n=4), medical students (n=2), pharmacist (n=1) and medical assistant technician (n=1) was evaluated with regard to the method of poisoning. Preferred methods were ingestion of medications orally ($54\%$) and by infusion/injection ($46\%$). The results were compared with the general population group (n=677: oral ingestion of medications $98\%$, inhalation $1.6\%$). Intoxications by infusion/injection predominated more clearly in physicians than in the total collective. Conclusion: Comparing with the general population, the tendency to a method of suicidal poisoning being typical of the profession rises among physicians and related occupations with the degree of specialization, caused by increasing knowledge, easier access to appropriate drugs and methods.

Stage-Wise Presentation of Non-Metastatic Head and Neck Cancer: an Analysis of Patients from the Kumaon Hills of India

  • Background: Head and neck cancer without distant metastases is amenable to various modalities of treatment. However, the stage at presentation is a very important determinant for treatment success. The present study was conducted to determine the stage-wise presentation of non-metastatic head and neck cancer patients from the hilly regions in Kumaon division of Uttarakhand, India. Materials and Methods: The hospital records for non-metastatic head and neck cancer patients from the only functional cancer centre of the region for the period of two-years (January 2012-December 2013) were included. Nasopharyngeal carcinoma was excluded due to its staging system being different. Non-squamous histopathologies were also excluded. Patients hailing from nearby regions of Uttar Pradesh and Nepal were excluded, as were patients from non-hilly regions of Kumaon. Results: Of the 271 patients of head and neck cancer, 27 with distant metastases at diagnosis were excluded from the analysis. Of the 244 eligible patient records, 90.1% (n=222) were male, and 9.9% (n=22) were female. The proportions of patients with carcinoma of the larynx, oropharynx, oral cavity, hypopharynx and maxillary antrum were 31.9% (n=78), 27.9% (n=68), 20.5% (n=50), 12.7% (n=31) and 1.2% (n=3). A further 5.7% (n=14) were diagnosed as having secondary involvement of neck nodes with unknown primaries. The proportion of patients presenting in stages I, II, III, IVA and IVB were 0.8% (n=2), 2.5% (n=6), 9.4% (n=23), 51.6% (n=126) and 35.7% (n=87) respectively. Conclusions: An abysmally low proportion (3.3%) of non-metastatic head and neck cancer patients presented in the early stages (I and II). A vast majority of the patients (88.1%) presented with stages IVA and IVB. Not only does this reflect a poor therapeutic outlook, but also exposes the dire need for programmes focusing on cancer awareness and early detection in the region.

원발성 온난자가면역용혈빈혈의 치료에 대한 후향적 분석 (Retrospective Clinical Analysis of Treatment for Primary Warm Autoimmune Hemolytic Anemia)

  • Background: Primary warm autoimmune hemolytic anemia (AIHA) is a relatively rare hematologic disorder resulting from autoantibody production against red blood cells. There has been very few studies about primary warm AIHA in South Korea because of its low incidence. We retrospectively analyzed the treatment outcome of primary warm AIHA. Method: We reviewed retrospectively the medical records of 9 primary warm AIHA patients from December 2002 to January 2015. We analyzed the causes and clinical characteristics of primary warm AIHA patients. We retrospectively analyzed the clinical data in electronic medical records for 9 Korean patients with AIHA patients who were diagnosed during the period from December 2002 to January 2015 at the Regional University Hospital in Korea. The study protocol was approved by the Institutional Review Board (IRB #2015-08-007, Chosun University Hospital IRB). Results: The mean age was 52 years (range 27~78), the mean hemoglobin level was 5.0 g/dL (range 2.5~6.4 g/dL). All patients received steroids at therapeutic dosages (corticosteroid 1 mg/Kg) as first line treatment. Eight of them showed complete response (5/8, 62.5%) and partial response (3/8, 37.5%), one patient required second-line treatment with rituximab. Two patients who responded first line treatment were relapsed at 86 weeks and 24 weeks after response, respectively. Only one patient of them was retreated with corticosteroid because of anemic symptoms. Conclusion: This study indicates that oral corticosteroid is an effective therapy for primary warm AIHA.

인진청간탕으로 호전된 스테로이드 부작용을 동반한 자가면역성 간염-원발성 담즙성 간경변증 중복증후군 1례 (Effects of Herbal Medicine Injinchunggan-tang on Steroid-induced Symptoms in a Patient with Autoimmune Hepatitis-Primary Biliary Cirrhosis Overlap Syndrome: A Case Report)

  • Objectives: The medical records, including laboratory test results, of a patient with autoimmune hepatitis (AIH)-primary biliary cirrhosis (PBC) overlap syndrome were reviewed to observe changes in steroid-induced adverse reactions before and after herbal medicine treatment.Methods: We investigated the records of a 44-year-old female patient with AIH-PBC overlap syndrome treated at the Kyung Hee University Korean Medicine Hospital. Injinchunggan-tang and its modified decoctions were administered, and any changes inclinical manifestations as well as laboratory test results were monitored.Results and Conclusions: Significant decreases in ALP, GGT, ALT, and AST levels were observed after oral administration of the herbal medicines. Improvements in steroid-induced symptoms (hyperglycemia, fatigue, and moon face) were also observed. Injinchunggan-tang and its modified prescriptions are promising candidates for proper management of AIH-PBC overlap syndrome.

두경부 이차암의 임상적 고찰 (A Clinical Analysis of Second Primary Malignancy in Head and Neck Cancer Patients)

  • Objectives: Minimal improvement in the long-term survival of head and neck cancer(HNC) patients has occurred despite a multitude of advances in the control of loco regional disease and a second primary malignancy(SPM) contribute to the continued poor prognosis for the HNC patients. This study was performed in order to identify the clinical characteristics of SPM in the HNC patients. Materials and Methods: The medical records of 354 patients of head and neck squamous cell carcinoma that were followed up after initial treatment during the period of 1987 through 1994 were reviewed. This study examines the medical records of 354 patients with squamous cell carcinoma of the head and neck, of whom 26 subsequently developed a second neoplasm. Results: The actuarial SPM rate was 7.3%, and median time to presentation for the SPM was 26.8 months. The SPM were more likely to occur in male patients who had oral cavity index tumors. Patient whose index tumor was small at diagnosis had a greater chance of developing a second tumor as did those with no cervical lymph node metastases to the neck. Initial treatment modality was not associated with an increased risk of developing a second tumor. The commonest sites for the SPM were the lung and other head and neck area. The 3-year survival for patients who developed a secondary tumor from the time of its diagnosis was 27.8%. Conclusion: The SPM in the head and neck cancer patients are not uncommon and early detection of the SPM will contribute to increase the long-term survival of HNC patients.

Induced Abortion and Breast Cancer: Results from a Population-Based Case Control Study in China

  • Aim: To determine whether induced abortion (IA) increases breast cancer (BC) risk. Materials and Methods: A population-based case-control study was performed from Dec, 2000 to November, 2004 in Shanghai, China, where IA could be verified through the family planning network and client medical records. Structured questionnaires were completed by 1,517 cases with primary invasive epithelial breast cancer and 1,573 controls frequency-matched to cases for age group. The information was supplemented and verified by the family planning records. Statistical analysis was conducted with SAS 9.0. Results: After adjusting for potential confounders, induced abortions were not found to be associated with breast cancer with OR=0.94 (95%CI= 0.79-1.11). Compared to parous women without induced abortion, parous women with 3 or more times induced abortion (OR=0.66, 95%CI=0.46 to 0.95) and women with 3 or more times induced abortion after the first live birth (OR=0.66, 95%CI =0.45 to 0.97) showed a lower risk of breast cancer, after adjustment for age, level of education, annual income per capita, age at menarche, menopause, parity times, spontaneous abortion, age at first live birth, breast-feeding, oral contraceptives, hormones drug, breast disease, BMI, drinking alcohol, drinking tea, taking vitamin/calcium tablet, physical activity, vocation, history of breast cancer, eating the bean. Conclusions: The results suggest that a history of induced abortions may not increase the risk of breast cancer.

Epidemiological Trends of GI Cancers in Patients Visiting a Tertiary Care Hospital in Chandigarh, North India

  • Background: Cancer has become an epidemic disease. Nearly ten million new cancer cases are diagnosed annually in the world and out of these about half are from the developing world. To appropriately plan for treatment, management and prevention of the disease, it becomes necessary to study the trends about morbidity caused by cancers. Materials and Methods: Data for patients diagnosed with any form of gastrointestinal (GI) cancers was extracted from records maintained in the outpatient department registers of the Oncology Department of Government Medical College and Hospital in Chandigarh from 1999 to 2012. Trends were analysed for different categories of GI cancers for the period of 12 years. Results: In present study GI cancers accounted for 23 % of all registered cases (n-9603) of carcinomas. Males predominated for all GI cancers except in the gall bladder. Gastrointestinal cancers as a proportion of total cancers increased from 21% in 1999 to 25.9% in 2012 with a significant increasing trend in our series (${\chi}^2$ for linear trend=9.36, p<0.003). Cancers of the tonsil, oral cavity and pharynx taken together showed an increasing trend over the years (${\chi}^2$ for trend=55.2, p<0.001) whereas cancers of the lower GI (${\chi}^2=19.6$, p<0.0001) and gall bladder (${\chi}^2=19.5$, p<0.0001) showed a declining trend in our series. Conclusions: GI cancers form a significant proportion of all cancers reporting to our data. In depth studies to ascertain the reasons for the changing trends are required to design intervention programs. Further information is necessary from cancer registries and from the hospital records of oncology departments.

Analysis of Patients with Facial Lacerations Repaired in the Emergency Room of a Provincial Hospital

  • Background Facial laceration is the most common injury encountered in the emergency room in the plastic surgery field, and optimal treatment is important. However, few authors have investigated this injury in all age groups or performed follow-up visit after repair. In the present study, the medical records of patients with lacerations in the facial area and underwent primary repair in an emergency room over a 2-year period were reviewed and analyzed. Methods Medical records of 3,234 patients with lacerations in facial area and underwent primary repair in an emergency room between March 2011 and February 2013 were reviewed and identified. Results All the 3,234 patients were evaluated, whose ratio of men to women was 2.65 to 1. The forehead was the most common region affected and a slip down was the most common mechanism of injury. In terms of monthly distribution, May had the highest percentage. 1,566 patients received follow-up managements, and 58 patients experienced complications. The average days of follow-up were 9.8. Conclusions Proportion of male adolescents was significantly higher than in the other groups. Facial lacerations exhibit a 'T-shaped' facial distribution centered about the forehead. Careful management is necessary if a laceration involves or is located in the oral cavity. We were unable to long term follow-up most patients. Thus, it is necessary to encourage patients and give them proper education for follow-up in enough period.

Surgical correction for Tessier number 7 craniofacial cleft using a medially overcorrected design

  • Background Various surgical techniques have been used to correct Tessier number 7 craniofacial cleft, which involves macrostomia, ear deformity, and hemifacial microsomia. To achieve symmetrical and satisfactory results in patients with macrostomia, the authors performed a 1-mm medial overcorrection on the cleft side and evaluated the results of this procedure. Methods A retrospective medical record review of patients diagnosed with Tessier number 7 craniofacial cleft from March 1999 to February 2017 was performed. Using clinical photographs, outpatient clinic records, and operative records, information was recorded regarding concurrent congenital anomalies, postoperative complications, and follow-up. Using Photoshop CS2, the length of both sides of the lip was compared. The ratio of these lengths was calculated to evaluate lip symmetry. Results Of the patients treated at the Department of Plastic and Reconstructive Surgery at Kyungpook National University Chilgok Hospital, 11 (male-to-female sex ratio, 7:4) were diagnosed with Tessier number 7 craniofacial cleft. Concurrent congenital anomalies included skin tag, hemifacial microsomia, and cleft palate. The mean duration of follow-up was $78.273{\pm}72.219$ months and the mean ratio of the lengths of both sides of the lip was $1.048{\pm}0.071$. Scar widening occurred as a postoperative complication in some patients. No cases of wound infection, bleeding, or wound dehiscence occurred. Conclusions For the successful correction of macrostomia, plastic surgeons should consider both functional and aesthetic problems of the lip. Adequate repair of the orbicularis oris muscle, skin closure with Z-plasty, and medial overcorrection of the neo-oral commissure led to good results in our patients.
