단일 치아 결손시 TiUniteTM 표면 처리한 임플란트의 생존율에 대한 후향적 연구 (A Retrospective Study of Survival Rate in single Brnemark TiUniteTM Implant)

  • 구강 내 단일 치아를 상실한 경우 기존에는 고정성 보철물을 이용하여 수복하는 방법이 일반적이었지만, 최근에 와서는 임플란트를 이용하여 수복하는 것이 보편화되고 있다. 본 연구는 단일 치아 결손 시 $TiUnite^{TM}$ 표면 처리한 임플란트를 이용하여 수복한 경우를 후향적으로 조사하여 그 생존율을 분석한 것이다. 2002년 9월부터 2006년 12월까지 삼성서울병원에서 단일 치아 결손 부위에 식립된 총 269개의 $TiUnite^{TM}$ 표면 처리한 임플란트 중 21개는 인접한 임플란트가 있거나, 기록이 누락되거나 관찰 기간이 짧아 연구에서 제외되었다. 248개의 임플란트 중 상악에는 129개(52.0%) 하악에는 119개(48.0%) 식립되었다. 수술 부위에 치조골 재생술을 시행한 경우는 총 100개(40.3%)였으며, 상악동 거상술이 시행된 증례는 총 36개(14.5%)였다. 수술 당일부터 관찰 기간은 평균 $26.0{\pm}11.8$ 개월이었으며, 그 기간 동안 실패한 것으로 간주된 임플란트는 12개로 생존율은 95.2%였다. 그 중 상악에서 실패한 경우가 10개, 하악에서는 2개로 각각의 생존율은 92.2%, 98.3% 이다. 단일 치아 결손 시 $TiUnite^{TM}$ 표면 처리한 임플란트를 이용하여 수복한 경우 단기간 동안 높은 생존율을 보였다.

아편유사제 복용 중인 암성 통증 환자들에서 경구 Oxycodone/Naloxone으로 전환 후 발생한 금단증상 (Opioid Withdrawal Symptoms after Conversion to Oral Oxycodone/Naloxone in Advanced Cancer Patients Receiving Strong Opioids)

  • 목적: 경구 oxycodone/naloxone 복합제는 아편유사제에 의해 유발되는 변비를 완화시키거나 예방하는 목적으로 사용되고 있다. Naloxone에 의해 oxycodone의 진통 효과가 상쇄되거나 금단증상이 나타난다는 보고는 거의 없었으나 저자는 실제 임상에서 몇몇 금단증상 예를 경험하였기에 이 환자들에 대한 조사 연구를 수행하였다. 방법: 2012년 1월 1일부터 2016년 12월 31일까지 경남 지역 암센터에 방문했던 진행성 암환자들로 oxycodone/naloxone extended-release tablets를 투약 받고 마약 금단증상이 나타났던 환자들의 의무기록을 후향적으로 조사하였다. 결과: 연구 기간 중 경구 oxycodone/naloxone을 처방 받은 1,641명의 암 환자 중, 총 10예(0.6%) 에서 마약 금단 증상을 겪었다. 금단증상 관련 통증 강도의 변화는 oxycodone/naloxone 투여 전 NRS 3에서 평균 NRS 6점으로 증가하였다. 금단증상 중 오한이 10예 중 7예에서 나타나 가장 많이 나타난 증상이었으며 그 외에 식은땀, 전신 쇠약감, 근육경련, 복부경련(각 5예), 불안(4예), 열, 어지럼증, 의식혼란, 하품(각 2예)의 순으로 빈번하게 관찰되었다. 결론: Oxycodone/naloxone extended-release 복합제에 의한 마약 금단증상은 흔하지는 않아도 적은 수의 환자에서라도 나타날 수 있다. 향후 이에 대한 다기관, 전향적 연구가 필요하다.

Long-term Stability after Reduction of Mandible Fracture by Keyhole Plate: Evaluation at the Time of Plate Removal

  • Background: Various types of miniplates have been developed and used for the reduction of facial bone fractures. We introduced Yang's Keyhole (YK) plate, and reported on its short-term stability. The purpose of this study was to evaluate the long-term stability of the YK plate, as a follow-up study, by examining the patients who had used the YK plate among the patients with the reduction of mandible fractures and who visited for plate removal. Methods: We reviewed the medical records of 16 patients who underwent mandibular fracture fixation using a YK plate (group I) and 17 patients who underwent mandibular fracture fixation using a conventional plate (group II). Assessment was then made on malunion, occlusal stability, discomfort during the application, and clinical symptoms. Results: From January 2015 to December 2017, a total of 36 patients underwent mandibular fracture surgery using a YK plate. A total of 16 patients received plate removal. Among them, 15 were male and 1 female. The average age was 26 years. The applied surgical sites were the 12 on mandibular angle, 4 on mandibular symphysis, and 2 on subcondyle. The application period of YK plate was an average of 335 days. During the same period, 45 people underwent surgery on the conventional plate. A total of 17 patients received plate removal. Among them, 15 were male and 2 females. The average age was 36 years. The applied surgical sites were the 8 on mandibular angle, 4 on mandibular symphysis, and 2 on subcondyle. The application period of the conventional plate was an average of 349 days. No malocclusion occurred at the time of removal, and occlusion was stable. No patient complained of joint disease or discomfort. Conclusion: The YK plate system, in which the screw was first inserted and the plate was applied, for clinical convenience did not cause any particular problem and no significant difference from the conventional plate.

감각통합치료의 역사적 고찰 (Historical Review of Sensory Integration in Korea)

  • The significant historical events and developments in the area of approach for sensory integration in Korean Society of Sensory Integration Therapists(KSSIT) are reviewed to stimulate future planning and clinical application in professional and health promotion. The paper reviewed the purposes of establishment, the role of members, progress of regular meetings and education for members and seminars in KSSIT. This review is placed within the context of the progressive period of KSSIT history, 1997-2003. Historical data are used as primary sources as records of events in KSSIT's homepage, official transcripts, professional articles, and secondary sources such as news letters for occupational therapists and oral information from first members. This review examined current actions and thinking, and suggests further actions and thinking for progressive professional education programs, research, clinical applications and public relations in sensory integration approach in KSSIT.

Hyperplastic conditions of the mandibular condyles

  • Purpose: To evaluate the clinical and radiographic features of unilateral hyperplastic mandibular condyles for some useful preliminary diagnostic recommendations. Materials and Methods : Clinical records, radiographs and histologic diagnoses of 35 cases with asymmetric mandibular condyles due to apparent unilateral condylar hyperplasia were evaluated retrospectively. Results: Among 35 cases, 28 were true hyperplastic conditions of condyles whereas the remaining 7 were unilateral internal derangement occurring on the short side. 17 of the 28 hyperplastic condyles showed a mass or irregular radiographic shadow with histologic diagnosis including osteochondroma and osteoma. Only 5 of these cases showed facial asymmetry. 2 out of the 17 cases showed hyperplastic round shaped irregular condyles consistent with ankylosis and their histologic diagnoses were osteochondromas. 11 of the 28 cases showed smooth enlargement of condylar head with elongation of the neck causing facial asymmetry, but histologic diagnoses were not available because the surgical operation conserved the condyles. Conclusion: The hyperplastic conditions of the mandibular condyles include not only true hyperplasia, osteochondroma, osteoma, and ankylosis, but also unilateral internal derangement occurring on the short side.

초극소미숙아를 위한 조기 퇴원 프로그램의 운영 성과 (The Outcomes of Early Discharge Program for Extremely Low Birth Weight Infants)

  • Purpose: The purpose of this study was to evaluate the actual outcomes of early discharge program for extremely low birth weight (ELBW) infants. Methods: Medical records of 122 ELBW infants admitted in the neonatal intensive care unit from January 2000 to June 2006 and those of their 112 mothers were analyzed retrospectively. Results: After being applied early discharge program to ELBW infants' mothers, their infants' lengths of stay, gestational age and body weight at discharge, duration of completion of oral feeding, number of emergency room visits after discharge were decreased and number of breast milk feeding was increased. Conclusion: Early discharge program for ELBW infants can be an effective intervention for parents and their ELBW infants contributing to neonatal nursing practices.

소아 진정시 Chloral Hydrate, Hydroxyzine복용 후 Midazolam의 점막하 투여 용량에 따른 진정효과에 관한 연구 (Sedative Effect of Submucosal Midazolam after Oral Chloral Hydrate or Hydroxyzine in Children[Author's Correction])

  • Recently combining regimen of minimal doses of chloral hydrate, hydroxyzine and midazolam is good in sedation of children. Midazoalm, water soluble benzdiazepine, has rapid onset and relatively short duration of action. And midazolam has prospective amnesic effect. With these advantages midazolam is popular in conscious sedation for children. This study was to reveal the dose-dependency of behavior and physiologic effects of submucosal midazolam. Sedation records were surveyed retrospectively, of which the patients admitted from April, 2005 to July, 2007. we assigned three groups according the dose of midazolam, 0.1 mg/kg, 0.2 mg/kg and 0.3 mg/kg, respectively and the behavioral evaluation was analyzed with Houpt scale statistically. Combined submucosal midazolam increased the success rate in sedation and the vital signs were stable during sedation.

  • 한국의 근대화 시기는 1920년대 식민지 시기와 1960년대 경제개발시기 두 시기 나누어진다. 근대화시기에 만들어진 공간들은 대다수 국가주도형으로 생산된 공공의 공간들이다. 이러한 근대공간들은 기능성이 강조되는 공공 생산의 장소이자 근대 국가시스템의 장소정체성을 상징하는 국가권력 공간들 이었다. 사회의 변화에 따라 근대공간들은 이제 더 이상 장소적 기능을 상실한 채 모뉴멘트 건축물로 존재하며 과거의 장소정체성에 새로운 가치를 부여할 개념규정이 필요한 시기이다. 간이역, 발전소, 탄광, 물류창고, 폐 공장 등... 근대공간에 새로운 장소정체성이 확립되는 과정과 장소와 기억의 문제를 영상기록 하여 근대화의 큰 특징인 사고의 변화과정과 공간에 투영된 근대적 개념들, 근대사회의 제도, 체계 수립 과정에서 장소성, 공간 속 사람들의 관계성에 주목하고자 한다. 근대공간과 기억의 관계는 < 서울역 >에서, 근대공간과 근대 기록의 관계는 <온양민속박물관>에서, 근대공간과 장소정체성은 <옥포 조선소>에서 찾아본다. 각 장소의 정체성과 기억의 관계는 구술목소리와 영상기록을 통하여 근대공간(장소성)과 개인구술(기억)의 모순된 힘의 관계를 알아본다.

측두하악장애의 예후에 관한 임상 연구 (A CLINICAL STUDY IN THE PROGNOSIS OF THE TEMPOROMANDIBULAR DISORDER)

  • This study is comprised of data obtained from the files of 346 patients with temporomandibular disorders. All patients were diagnosed, treated and followed in the Department of Dentistry at the Inha university hospital, Incheon, Korea. The patients had treated with medications, physical therapy, occlusal splint and arthrocentesis. The study data were obtained from the medical records and telephone interviews that were conducted by research assistants. The results were as follows 1. The patient's main complaint was pain(77%), and mouth opening limitation was 17%. 2. An analysis of the medical records of the 346 patients disclosed that 82% were improved and 17% had no improvement when they were dismissed. 1% of the patients had become worse during therapy. 3. A success rate of 82% was achieved when medication assisted physical therapy was included. In the current status at the telephone interview, 270 patients(89%) reported that they were doing well with 56% describing themselves as asymtomatic and 32% experiencing only minor residual or recurrent symptoms. 11% regarded themselves as unimproved and worse. 4. In the current status of the unsuccessfully treated patients by medications and physical therapy, 64% of patients were doing well(3% as asymptomatic and 56% as only minor residual or recurrent symptom). But 36% of patients was reported as unimproved and worse. 5. TMJ has a remarkable adaptive potential and TMJ disorder has a natural history of spontaneous fluctuations and favorable prognosis during the subsequent natural course. 6. In the treatment of the temporomandibular disorders, there is a treatment ladder, starting with the simplest and least expensive treatment, that is ascended until resolution of the patient's symptoms occur. These findings suggest that conservative reversible therapies are both sufficient and appropriate for management of temporomandibular disorder in most patients. Major alterations of mandibular position or dentoalveolar relationships do not appear to be necessary for obtaining either short term or long term success and therefore they can be generally regards as inappropriate treatment for this disorder. The fact that physical therapy is non-invasive and does not appear to be fraught with irreversible changes, makes it a very applicable vehicle in the area of clinical TMJ disorder management.

Emergency response team activation in the outpatient clinic of a single dental teaching hospital in Korea: a retrospective study of 10 years' records

  • Background: To prepare for possible emergency situations during dental treatment, it is helpful to know how often and what kinds of emergencies may arise. This study set out to evaluate the incidences, causes, treatments, and outcomes of emergency situations in the outpatient clinic of a dental teaching hospital in Korea. Methods: We retrospectively reviewed the records of patients who had experienced an emergency situation and emergency response team activated in a selected outpatient clinic between November 2004 and November 2013. Specific information about the emergency cases was collected, including the patient characteristics and the frequency, types, treatments, and outcomes of the emergency situations. Results: We identified 35 instances of emergency situations in 2,890,424 patients (incidence = 0.012 per 10,000 outpatients). The number of cases was as follows: 10 (28.6%) in the Department of Periodontics, 10 (28.6%) in the Department of Oral and Maxillofacial Surgery, 6 (17.1%) in the Department of Oral and Maxillofacial Radiology, 4 (11.4%) in the Department of Prosthodontics, 2 (5.7%) in the Department of Conservative Dentistry, 2 (5.7%) in the Department of Pediatric Dentistry, and 1 (2.9%) in the Department of Orthodontics. Three (8.6%) of the emergency situations arose before treatment, 22 (62.9%) during treatment, 7 (20.0%) after treatment, and 2 (5.7%) in a patient's guardian. Conclusions: In accordance with the growing elderly population and more aggressive dental procedures, the number of emergency situations may increase in the future. We recommend that clinicians keep in mind airway management and the active control of emergency situations.
