Infection of Clinostomum Complanatum in a Black-tailed Gull (Larus Crassirostris) (괭이갈매기의 인두흡충 감염증)

  • Clinostomum (C.) complanatum is one of the trematode helminthes in birds and causes incidental infection in human. A black-tailed gull (Larus crassirostris) was referred to the Jeju wildlife rescue center. Because of the death of bird after two weeks treatment, necropsy was performed to the bird. Grossly, many living worms adhered to the oral mucosa including pharynx and esophagus were observed. The worms removed were identified as C. complanatum after morphological observation. The linguiform adult worms were $5.19{\pm}0.48mm$ long and $1.80{\pm}0.18mm$ wide. Histopathologically, severe necrosis and surrounding inflammation were observed at the infection sites of adult worm in pharynx and esophagus. This is the first report for C. complanatum infection in wild black-tailed gull in Korea.

Pharmacokinetic Interaction between Ticlopidine and Nimodipine in Rats (티크로피딘과 니모디핀과의 약동학적 상호작용)

  • The purpose of this study was to investigate the effect of ticlopidine on the pharmacokinetics of nimodipine in rats. Pharmacokinetic parameters of nimodipine were determined in rats after oral administration of nimodipine (16 mg/kg) with or without ticlopidine (3 or 10 mg/kg). Ticlopidine inhibited cytochrome P450 (CYP)3A4 activity. Ticlopidine significantly (p<0.05, 10 mg/kg) increased the area under the plasma concentration-time curve (AUC) of nimodipine and ticlopidine significantly (p<0.05, 10 mg/kg) prolonged the terminal half-life ($t_{1/2}$) of nimodipine. Ticlopidine significantly (p<0.05, 10 mg/kg) decreased the total body clearance ($CL_t$). The absolute bioavailability (AB%) and relative bioavailability (RB%) of nimodipine by presence of ticlopidine were increased by 14% and by 42%, respectively, compared to the control. Based on these results, the increased bioavailability of nimodipine might be due to inhibition of the metabolizing enzyme cytochrome P450 (CYP)3A4 in the liver or intestinal mucosa and/or reducing total body clearance by ticlopidine.

  • The dimensional stability of tissue conditioners characterizes the ability of the materials to yield accurate functional impressions of oral mucosa. This study evaluated the viscoelastic property and the linear dimensional changes with the factor of time and thickness of tissue conditioners ($COE-COMFORT^{TM}$, Visco-gel. $COE-SOFT^{TM}$, Soft-Liner). The thickness of these materials were changed (1.5mm, 3.0mm) and the percentage changes in dimension were measured at 1h, 12h, 24h, 36h, 3day, 7day after specimen preparation. From the results large differences appear between the various tissue conditioners. The results suggest that the period recommended for forming functional impression would be 2-3days after insertion in the mouth. in addition. it is important to select tissue conditioners suitable for functional impression because of the wide range of dimensional stability among the materials.

Pharmacokinetic Interaction between Verapamil and Tamoxifen in Rats (베라파밀과 타목시펜의 약물동태학적 상호작용)

  • The aim of this study is to investigate the effects of verapamil on the pharmacokinetics of tamoxifen following oral administration of tamoxifen with verapamil to rats. Tamoxifen (10 mg/kg) was administered orally in the presence or absence of verapamil (1, 3 or 6 mg/kg). Compared to the control group (given tamoxifen alone), the presence of verapamil significantly (p<0.05 by 1 mg/kg, p<0.01 by 3 and 6 mg/kg) increased the areas under the plasma concentration-time curve (AUC) and the peak concentrations ($C_{max}$) of tamoxifen. Consequently, the relative bioavailability ($RB\%$) of tamoxifen with verapamil was 1.6-2.1 fold higher than that of the control. But the time to reach peak concentration ($T_{max}$) and the terminal half-life ($t_{1/2}$) of tamoxifen were not altered significantly in the presence of verapamil. The increased AUC and $C_{max}$ of tamox­ifen in the presence of verapamil might be associated with the inhibition by verapamil of the P-glycoprotein and the first­pass metabolizing enzyme CYP3A4 in small intestinal mucosa. The drug interaction should be taken into consideration when tamoxifen is used to the patient with verapamil in the clinical setting.

  • Numerous mycoutaneous island flaps or free flaps have been used to reconstruct a defect which resulted from the wide resection of tumor mass in head and neck region. Since the curative resection of tumor usually include muscles and bones as well as skin and mucosa, the anatomical and functional restoration of the defect depend on which and what amount of tissues were provided to cover the defect; good aesthetic appearance subsequently follows the result. Furthermore, a simultaneous neck nodes dissection usually results in exposure of major neck vessels., which should be protected with sufficient padding. The ideal method to reconstruct a defect in the head and neck region requires a sufficient coverage by muscle layer with good vascularity, a wide arc of rotation, and minimization of donor site defect. The pectoralis major myocutaneous flap which was first decribed by Ariyan and lateral trapezius myocutaneous flap by Demergasso meet these criteria. We describe the use of these myocutaneous flaps in reconstruction of mandible and oral cavity.

  • The lobular capillary hemangioma (LCH) was previously known to pyogenic granuloma and is benign vascular lesion which grows rapidly on skin and mucosa. It arises from whole body, but oral and nasal cavities are most predilection sites in the head and neck area. The laryngeal LCH looks like a granulomatous lesion of posterior glottis and its common etiology are tracheal intubation and laryngopharyngeal reflux disease etc. The LCH in larynx can cause blood tinged sputum and lump sense. The lesions refractory to medical therapy or causing dyspnea may require surgical excision. A 74-year-old man who presented gradually aggravated dyspnea, lump sensation and hoarseness of one month came to our hospital. The stroboscopic examination revealed large well-margined glottic mass. It was excised with $CO_2$ laser and finally diagnosed as LCH. We present a rare unique case of glottic LCH with a review of literatures.

  • Functionally, the lip serves to prevent food and drink from spilling out of the beginning of the gastrointestinal tract, and it is also used for vocalization. In addition, the lip has cosmetic importance as part of the face involved in making expressions, and in many cultures, it is considered to be sexually appealing. The results of lip reconstruction procedures must therefore be both functionally and cosmetically satisfactory. When the orbicularis oris muscle and oral mucosa are excised, functional reconstruction is prioritized. In contrast, if there are no functional problems, cosmetic reconstruction is the main focus. This case involved the reconstruction of a right upper lip defect caused by a dog bite. When the skin defect was covered with a local flap, the right angulus oris shifted medially, so we incorporated a YV flap at the right angulus oris to modify its position and allow for a cosmetically satisfactory result. We believe that this method can be used not only for cases in which asymmetry of the angulus oris is expected to occur at the time of lip reconstruction, but also for cases in which it has already occurred in the initial operation.

Effect of Paraquat on Enteroendocrine Cells in the Gastric and Small Intestinal Mucosa of Developing Rat : Immunohistochemical Study (흰쥐 위와 소장점막의 장내분비세포 발생에 미치는 Paraquat 영향 : 면역조직화학적 연구)

  • The effect of paraquat(1,1´-dimethyl-4,4'-bipyridinium) on the gastrin-, somatostatin-, glucagon-, PP-, CCK-8- and serotonin-immunoreactive cells in the stomach and small intestine of developing rat was examined by peroxidase-an-tiperoxidase method. Oral administration of this herbicide(9mg/Kg per day in 0.2ml of D.W) on days 7 to 14 of gestation revealed some difference, such as first appearance and distribution pattern of immunoreactive cells between control and paraquat-treated group. These results suggest that indirectly treated fetuses display a general developmental retardation on enteroendocrine cells differentiation as well as on gastrointestinal maturation

Granular Cell Tumor in the Lower Lip : A Case Report (하구순의 과립세포종 증례보고)

  • Purpose : Granular cell tumour(GCT) is a rare benign tumour that can arise in anywhere throughout the body. Histopathological diagnosis of malignancy is difficult. We report a case of a granular cell tumor which developed on lower lip. Methods : A 58-year-old male had a palpable mass and whitish plaque in the oral mucosa for three months. Results : Under local anasthesia, the tumor was removed through wide excision including normal skin margin. Immediate reconstruction using local flap was done. Pathologic observation showed nests of polygonal cells with abundant granular, eosinophilic cytoplasm and round nuclei. Immunohistopathologic staining showed positive reaction on S-100 protein Conclusion : The results support the hypothesis that granular cell tumor is derived from Schwann cells. We report here on a case of granular cell tumor of lower lip that was successfully treated with radical resection.

Comparison among diagnostic tools used for differential diagnosis of blood stasis pattern in Korea, China and Japan (한중일 어혈증 감별진단을 위한 도구의 비교)

  • Objectives The purpose of this study is to compare the representative differential diagnosis methods of blood stasis pattern used in Korea, China and Japan, and then to characterize each diagnostic method. Methods Through the journal databases, we have selected representative tools that were developed for differential diagnosis of blood stasis pattern in Korea, China and Japan. In order to characterize the selected check-lists or questionnaires, we investigated the number of items, contents, score calculation method, internal consistency, and accuracy of each selected tool. Results A total of four diagnostic tools were finally selected; quantitative diagnosis scale of blood stasis syndrome (QDSBSS), diagnostic criteria for blood stasis (DCBS), blood stasis questionnaire (BSQ), and blood stasis syndrome questionnaire (BSSQ). The key points in the differential diagnosis for blood stasis were different for each of the diagnostic tool. The key point was oral mucosa (including tongue) status in the QDSBSS. Meanwhile it was abdominal pain/resistance in the DCBS, and general pain in the BSQ. Accuracy of the QDSBSS, the BSQ and the BSSQ were powerful but all of them was not generalized. Conclusions Therefore, it is desirable to select and apply a plurality of appropriate tools according to the characteristics of the blood stasis patients.
