유아성 흑백 신경외배엽성 종양 (Melanotic neuroectodermal tumor of infancy)

  • The melanotic neuroectodermal tumor of infancy is a rare neoplasm arising in the first year of life. A 33-day-old female infant had an enlarged alveolar ridge on the right anterior maxilla. Intraoral examination revealed a nonulcerative swelling at the site. An intraoral radiograph showed an ill-defined radiolucency on the right anterior maxilla and displacement of primary incisors from their alveolar sockets. CT scans revealed an expansion of the surrounding bone and partial destruction of the anterior wall of the premaxilla. Histopathologic examination showed the cytoplasm of neuroblastic cells and eosinophilic, epithelioid cells frequently contained a dark brown granular pigment that stained positively to vimentin and HMB45, focally positive to NSE and cytokeratin. Four weeks after the operation, CT scans showed a rapidly growing soft tissue mass occupying right maxillary sinus encroaching to the orbit and nasal cavity. The final diagnosis was made as a malignant melanotic neuroectodermal tumor of infancy.

혀에서 발생한 일차성 결핵 (Primary Tuberculosis of the Oral Tongue: A Case Report and Literature Review)

  • Tuberculosis is primarily a pulmonary disease and extrapulmonary involvement of the oral cavity is an infrequent occurrence accounting for 0.2 - 1.5% of all extrapulmonary sites. The authors report a case of primary tuberculosis of the oral tongue in a 60-years-old male patient who visited the clinic for ongoing tongue pain that he experienced for several months. The lesion was initially indistinguishable from the malignancy, but differential diagnosis was performed on the biopsy with Ziehl-Neelseen stain. After treatment with anti-tuberculosis, the oral lesion healed completely without surgery.

  • Metastases to the jawbones are found predominantly in the mandible and are rare in relation to the overall spectrum of oral malignancy. Analysis of the literature shows that the most frequent primary sites are the breast, lung, kidney, thyroid, and prostate. Adenocarcinoma of the mandible, whether primary or metastatic, are usually difficult to diagnose clinically. We report a case illustrating the clinical, radiographic, and histologic findings of a metastatic lung adenocarcinoma of the anterior mandible in a 58-year-old male.

Importance of immediate surgical intervention and antifungal treatment for rhinocerebral mucormycosis: a case report

  • Rhinocerebral mucormycosis (RCM) is an opportunistic, potentially life-threatening fungal disease. This infective disease invades not only the facial sinuses, but also the maxilla, zygoma, and rhino-cerebral structures with a massive destruction of the facial skeletons and soft tissue. This disease progresses within various underlying diseases, such as diabetes mellitus, hematologic malignancy, renal failure, and systemic immunodepression. The relationship between mucormycosis and these underlying conditions have been discussed extensively in the literature. The authors studied 6 cases of RCM diagnosed by a tissue biopsy and treated at the department of oral and maxillofacial surgery, from 1997 to 2012. Patients were treated with several kinds of surgical interventions and antifungal agents, and their clinical & radiological signs, underlying conditions, surgical methods, and outcomes were analyzed.

  • Leukemia is the most common neoplastic disease of the white blood cells which is important as a pediatric malignancy. Oral manifestations occur frequently in leukemic patients and may present as initial evidence of the disease or its relapse. The symptoms include gingival enlargement and bleeding, oral ulceration, petechia, mucosal pallor, noma, trismus and oral infections. Oral lesions arise in both acute and chronic forms of all types of leukemia. These oral manifestations either may be the result of direct infiltration of leukemic cells (primary) or secondary to underlying thrombocytopenia, neutropenia, or impaired granulocyte function. Despite the fact that leukemia has long been known to be associated with oral lesions, the available literature on this topic consists mostly of case reports, without data summarizing the main oral changes for each type of leukemia. Therefore, the present review aimed at describing oral manifestations of all leukemia types and their dental management. This might be useful in early diagnosis, improving patient outcomes.

Head and Neck Cancer: Global Burden and Regional Trends in India

  • The actual burden of head and neck cancer in India is much greater than reflected through the existing literature and hence can be regarded as a 'tip of iceberg' situation. This has further been evident by the recent reports of 'Net-based Atlas of Cancer in India'. South-east Asia is likely to face sharp increases of over 75% in the number of cancer deaths in 2020 as compared to 2000. Since the percentage increase of Indian population has been nearly twice that of the world in last 15 years there is a likelihood of increase in cancer burden with the same proportion. The distribution of population based cancer registries is grossly uneven with certain important parts of the country being not represented at all and hence the current cancer burden is not reflected by registry data. However, the pathetic situation of health care system in major parts of the country as also emphasized by the World Bank, is not suitable to provide anywhere near accurate data on cancer burden. Head and neck cancer (including thyroid lesions) is third most common malignancy seen in both the sexes across the globe but is the commonest malignancy encountered in Indian males. Also oral cavity cancer is the most prevalent type amongst the males and one of the highest across the globe. This article reviews the latest global and national situation with an especial emphasis on head and neck cancer. Furthermore this review focuses on burden in different sub sites at national and global levels.

  • Carcinoma ex mixed tumor is a mixed tumor in which a second neoplasm develops from the epithelial component that fulfills the criteria for malignancy. This tumor occurs frequently in the parotid glands. Individuals in whom carcinoma ex mixed tumor have a past history of benign mixed tumor. These lesions contain both a benign mixed tumor as well as a malignant neoplasm. We report a case of carcinoma ex mixed tumor which occurred in the parotid gland. A 67-year-old woman presented with movable right mass. she complained pain and facial paralysis. The clinical presentation, several diagnostic images, and histopathologic findings are presented.

Necrotizing Sialometaplasia: 두 증례보고 (Necrotizing sialometaplasia: Report of 2 cases)

  • Necrotizing sialometaplasia (NS) was defined by Abrams et al. in 1973 as a reactive necrotizing inflammatory process involving minor salivary glands of the hard palate. Before that recognition, many patients with this condition had been improperly treated because of its clinical and histologic resemblance to malignancy such as mucoepidermoid carcinoma and squamous cell carcinoma. We report two cases of necrotizing sialometaplasia. One case involved a 58-year-old male who had an ulcerative palatal lesion exposing underlying bone which has the typical features of the above mentioned condition. Another case involved a 59-year-old male who developed a necrotizing sialometaplasia in association with a dome-shaped palatal swelling which was proved as an adenoid cystic carcinoma after operation biopsy.

High grade carcinoma ex pleomorphic adenoma of parotid gland: a case report

  • Pleomorphic adenoma is the most prevalent benign tumor of the parotid gland, and shows potential malignancy. Carcinoma ex pleomorphic adenoma (CXPA) can occur in 3%-15% of pleomorphic adenoma cases. Owing to its clinical similarity to benign tumors, critical information related to CXPA can be easily overlooked, leading to frequent misdiagnosis of the condition. In this article, we report a rare case of CXPA found in the 55-year-old male patient with characteristic clinical, radiographic, and histological features, and subsequent treatment.

  • Fibrous histiocytomas are commonly occurred in the skin of the extremeties and rare in the head and neck region. Fibrous histiocytomas in general are considered benign tumors. But deep fibrous histiocytomas have more poor prognosis than cutaneous counterparts and tendency of local invasion and recurrence. Wide surgical excision is the treatment of choice due to high recurrence rate and potential malignancy. We presented a rare case of benign fibrous histiocytoma occurred in the superficial lobe of the right parotid gland, showing palpable mass with pain.
