Factors Associated with Unmet Dental Needs among Single-Person Households in Korea

  • Background: The purpose of this study is to provide the data for discussions related to oral health promotion policies for single-person households by analyzing the status of unmet dental needs and related factors in single-person households in Korea, based on the Anderson model. Methods: The data, obtained from 544 single-person households of those over 20 years old who were targeted for the 6th Korea National Health and Nutrition Examination Survey, were analyzed through a complex sample frequency analysis, complex sample cross analysis (Rao-Scott chi-square test), and complex sample binary logistic regression analysis on a complex sampling design. Results: The most frequently given reason for an unmet dental need among single-person households was economic (52.4%). Factors related to the unmet dental needs of single-person households are smoking, which is a predisposing factor; personal income levels, which are an enabling factor; chewing discomfort; and limited daily activities, which are need factors. Smokers, the high-income group, the chewing-discomfort group, and the limited activity group showed high unmet dental care experience. Smokers had a 2.75 times higher rate of unmet dental care than non-smokers, and the high-income group had a 5.29 times higher rate of unmet dental needs than the median group. The rate of unmet dental needs for the chewing discomfort group was 3.27 times higher than the non-chewing discomfort group, and the limited activity group had a 7.87 times higher rate of unmet dental needs than the non-limited activity group. Conclusion: It is necessary to map out policies designed to help maintain and promote met dental needs considered to be internally heterogeneous to single-person householders, based on the Anderson model.

Analysis of trends in the dental care quality improvement contests (치과 의료의 질 향상(Quality improvement) 경진대회 경향 분석)

  • Based on data from the Korean Academy for Dental Administration, which has been conducting the Dental Quality Improvement (QI) Contest since 2010, we aimed to provide basic data for the development of dental quality improvement indicators by analyzing the trends of dental quality improvement activities. A total of 54 articles in the dental QI contest from 2015 to 2021 were used to search for frequently used words and to classify the quality of dental care. The criteria for the quality dimension of dental care were first classified into structure, process, and outcome, and secondary classification was performed into patient safety, timeliness, patient-centeredness, equity, efficiency, effectiveness, and accessibility. The frequently used key terms were satisfaction (five times), efficiency (four times), system (three times), and implant (three times). The activities for process evaluation were high at 62.26%, structural evaluation activities at 35.85%, and outcome evaluation activities at 1.89%. According to the components of dental care quality improvement, the activity performed under efficiency was the highest (33.96%), followed by patient-centeredness (18.87%), effectiveness (16.98%), patient safety (15.09%), accessibility (5.66%), timeliness (1.89%), and the equity (1.89%). QI activities in dental hospitals were mainly activities on improvement in structure and process, as well as activities on efficiency, patient-centeredness, effectiveness, and patient safety.

  • The purpose of this study was to examine the possibility of the existence of high-risk groups of dental caries and influential factors for the high-risk groups, as there was a possibility that certain people might especially be at risk of suffering from dental caries when decayed, missing, and filled teeth index were analyzed, which were the typical indicators of dental caries. The data of the 4th National Health & Nutrition Survey for 2007, 2008 and 2009 were analyzed. Significant caries index (SiC index) were calculated, which were one of the representative devices to indicate high-risk groups of dental caries, and the SiC index and related factors were analyzed by using $x^2$ (chi-square) test. Besides, logistic regression analysis was utilized to find out influential factors, and a statistical package STATA 11.0 was employed. As a result of analyzing what factors would be linked to high-risk groups of dental caries, it's found that women were more likely to belong to high-risk groups than men (p<0.01). How the related factors affected the high-risk groups was analyzed, and it's found that those who didn't find themselves to be in good oral health were more likely to be classified into a high-risk group, and that those who brushed their teeth three times a more a day on a regular basis were more likely to belong to a high-risk group than the others who didn't (p<0.01). Thus, SiC index were calculated in this study to confirm the existence of dental caries high-risk groups, and what factors impacted on the high-risk groups was ascertained. It's required to implement efficient national policies to step up the promotion of national oral health based on the findings of the study.

  • Angelica gigantis (AG), Rehamaninae Radix(RR), Paenia japonica (PJ), and Polygoni multiflori Radix (PM) have been used as medicinal plants to tonify the blood. General function of the drugs have been known to nourish blood and control the heart and liver meridians. Recently, several studies have proposed mechanisms by which some oriental medicinal herbs work on the immune system. However, it is uncertain whether aqueous-extract of these drugs has immunomodulatory effect yet. In this study, I investigated the immune promotive effects of the water-extracted AG, RR, PJ and PM. The water-extracted AG, RR, PJ and PM inhibited NO production and iNOS expression in LPS stimulated RAW 264.7 macrophage cell line. Among these extracts, AG and PM induced expression of IL-2 and IFN${\gamma}$ in mouse spleen cells. In the flow cytometry analysis, PM-stimulated mouse spleen cells showed an increase in B-cell phenotype (CD45R/B220). The oral administration of Polygoni multiflori water-extracts to mice having S-180 abdominal dropsy cancer prolonged life-span more than control mice. These data suggest that among these extracts, PM has cellular and humoral immune-enhancement effect through IL-2 and IFN${\gamma}$ cytokine production, the regulation of NO production in macrophage cells and the B cell production in spleen cells.

Relationship between Health Risk Behaviors and Toothbrushing among School-Going Adolescents in Poor Urban Areas of Peru (페루 도시 빈민 지역 청소년들의 칫솔질 빈도와 건강위험행동의 관련성)

  • The aim of this study was to explore the association between daily toothbrushing frequency and health risk behaviors of school-going adolescents in poor urban areas of Peru. A cross-sectional survey was conducted among 959 school-going adolescents 11~19 years of age in poor urban areas of Peru in 2014. Health risk behaviors we assessed in the study were smoking; drinking; consuming sugar-sweetened soft drinks (more than one cup); leisure time activities including watching television, playing games, and using the Internet; and never or rarely handwashing with soap. Daily toothbrushing frequency was divided into two groups (once and twice daily). For statistical analyses, the chi-square test and hierarchical logistic regression were used at 5% level of significance. Of the total respondents, 63 (14.3%) were boys and 53 (10.3%) were girls and 116 (12.1%) engaged in toothbrushing (${\leq}1$ times daily). According to the adjusted logistic regression analysis for socio-demographic characteristics, two behaviors (leisure time use activities, including watching television, playing games, and using the Internet (odds ratio [OR], 2.29; 95% confidence interval [CI], 1.20~4.35), and never or rarely hand washing with soap (OR, 4.09; CI, 2.48~6.75) were statistically associated with toothbrushing frequency (${\leq}1$ times daily). We found two health risk behaviors (leisure time activities, including watching television, playing games, and using the Internet, and never or rarely handwashing with soap) associated with toothbrushing frequency among adolescents in the study area. Thus, oral health promotion programs should consider these health risk behaviors associated with toothbrushing frequency.

Low Dose Exposure to Di-2-Ethylhexylphthalate in Juvenile Rats Alters the Expression of Genes Related with Thyroid Hormone Regulation

  • Phthalates widely used in the manufacture of plastics have deeply penetrated into our everyday lives. Recently, a concern over the toxicity of phthalates on thyroid, has been raised but in most of cases, the doses employed were unrealistically high. To investigate the effects of phthalates on thyroid, we investigated the effects of the repeated oral exposure to low to high doses (0.3, 3, 30 and 150 mg/kg) di-2-ethylhexylphthalate (DEHP) from weaning to maturity for 90 days in juvenile rats on the thyroid. The histological examination revealed that DEHP significantly induced hyperplasia in the thyroid from the doses of 30 mg/kg, which was confirmed with Ki67 staining. In line with this finding, increased mRNA expression of thyrotropin releasing hormone (Trh) was observed in the thyroid of female at 0.3 mg/kg and 150 mg/kg as determined by RNAseq analysis. Moreover, significantly increased expression of parathyroid hormone (Pth) in the female at 0.3 mg/kg, and thyroglobulin (Tg) and thyroid hormone responsive (Thrsp) in the male at 0.3 mg/kg were noted in the blood, of which changes were substantially attenuated at 150 m/kg, alluding the meaningful effects of low dose DEHP on the thyroid hormone regulation. Urinary excretion of mono-2-ethylhexyl-phthalate (MEHP), a major metabolite of DEHP was determined to be 4.10 and 12.26 ppb in male, 6.65 and 324 ppb in female at 0.3 and 30 mg/kg DEHP, respectively, which fell within reported human urine levels. Collectively, these results suggest a potential adverse effects of low dose phthalates on the thyroid.

Role, Change, Job Satisfaction and Obstacles in Carrying out the Role of Public Health Nurses in Health Center (보건소 보건간호사의 역할변화, 역할수행의 장애요인과 만족도)

  • Based on the questionnaires sent to 270 nurses of public health centers in kyungnam during the period of March 19 to April 11 in 1992, this study was written for the of finding out the grade of satisfaction, obstacles in carrying out duties concerned with nursing services and the change of nurses role needed according to the change of the local public health administration. The first-ranking tasks carried by nurses of public health center are believed to have been family planning activities before the 1970's, nursing services during the 1970's, mother-child health activities during the 1980's, and nursing services during the period of 1990 to 1992. As far as the priority order of all the family planning activities is concerned, the counseling of the insertion of intrauterine contraceptive device, the use of oral pill or the distribution of condom was placed emphasis on before 1970, and publicity activities of family planning after that time. The first priority order of mother-child health activities has been put on the registration of pregnant women since 1970, with prenatal examination and vaccination ranking next to it. The priority order for activities against tuberculosis was laid on finding out and registration of new T.B. patients every year, with patients' control, and medication or injection ranking next to it. As for the priority order of nursing services, traveling medical examination and treatment ranked the first-stressed activity before 1970, with medication and injection ranking next to it. The first priority order management activity of communicable diseases was put on vaccination before 1970, with medication and injection. ranking next. And consultation and education ranked second to it during 1990 to 1992. As for the health services of the aged, traveling examination and treatment ranked the order, with the assistance of medical examination ranking next to it. As far as troubles and obstacles shown in case of family planning, the rate of residents' lack understanding was 28.8%, that of lacking budget 13.6%, and the imperfection of public health administration system 11.9%. In the case of tuberculosis control, residents' lacking understanding was 32.5%, the deficiency of public health administration system 18.2%, over-duty(shortage of hands) 15.6%, and the insufficiency skill and know-how 13.0%. In the case of nursing services, the deficiency of public health administration system was 18.2%, each over-duty(the shortage of hands) and the shortage of facilities and equipment 15.6% respective, and residents' lacking understanding 13.0%. The rate of dissatisfaction with the chance or possibility of promotion for his or her career or capability was shown to be 49.2%, and 65.9% of the health nurses expressed their complaints of the deficiency of the chance of the promotion to a professional or expert. when the public health nurses were asked in the questionably whether they were satisfied or not with current state of equipment and facilities needed for public health service, 49.6% of them answered in the negative. The grade of the satisfaction with the current individual position was shown to be low as much as the status of his or her position was now. 37.6% of those asked in the research answered to have the readiness to switch jobs for the reasons of dissatisfaction and so on with lacking promotion chance as well as bad working condition. Significant correlation between the grade of job satisfaction and the current status of the po as found to be in this research, which showed that the lower the status of position was, the lower the grade of job satisfaction was. But little correlation between the grade of job satisfaction and his or her schooling and career was found. In order to carry out primary health care successfully, it can be said that more education and publicity activities to make public health nurses and residents see it in a new light are requested. In addition to it, it is suggested that the improvement of promotion system for public health nurses and the enlargement of job province should also be taken in consideration of the high dissatisfaction with and complaints of the chance of promotion and the system of position. In order words, it is important that considerations for system improvement enough to make nursing services pleasant and satisfactory should be taken into.

  • FDI World Dental Federation is organization representing more than one million dentists worldwide with its more than 100 years of long history. Now FDI headquarter is located in Geneve, Switzerland and serves for developing health policy and continuing education programs, speaking as a unified voice for dentistry in international advocacy and supporting member association in oral health promotion activities worldwide. Korean Dental Association has been a member of FDI since 1959 and participated actively in the FDI's worldwide activities. Past Dr. Heung-Ryul Yoon was a president of FDI from 2003 for 2 years and we also have an experience in 1997 of hosting FDI Annual World Dental Congress successively in Seoul. The image of a country reflects the country's culture as well as affects to its economical and social development and it decides the national brand toward the world. Currently, Korean people realized that and put their efforts for promoting diverse aspects of Korea to the world such as multimedia culture called Han-Ryu, electric industry, automotive industry, sports represented by soccer. We have advanced technique and skill in various fields and now it's time to show ourself better to the world. It's same to the dentistry. It's true that Korean dentistry is top-class in the world and the dental industry is prosperous and also has best technique in the world. I think and strongly hope that the 2013 FDI Seoul Annual World Dental Congress would be best opportunity for Korean dentistry and dental industry to promote ourselves and make a big step to the world. Not only for the dentistry, it will also be very good chance to all the Korean people to improve Korean national brand. I believe that we can accomplish if we, all dental societies, stand together and join our effort to use this good chance the best. I know we can make it.

The comparison on periodontal attitude and oral health promotion behavior by dental hygiene process applies (치위생과정 수행 전과 후의 치주상태 및 구강건강증진행위 비교)

  • Objectives : This study has made a comparison of the difference between the periodontal parameter before and after applying dental hygiene process on patients requiring supportive periodontal therapy at a private dental clinic. Methods : The entire process was conducted on 74 patients. As a result of analyzing the difference in periodontal parameters such as PPD>4mm, BOP, and O'Leary Index of 29 patients that has completed 1 circle of dental hygiene process. Results : All periodontal parameters in all subject patients had been significantly reduced(p<0.05). Furthermore, as a result of comparing the periodontal conditions of the smoking group and non-smoking group, complier and non-complier, all parameters were reduced with a significant difference in the non-smoking group and the complier, but the smoking group and the non-complier did not show difference in all parameters. As a result of making an assessment of before and after dental hygiene process according to PRA classifications, high risk group has been generally changed to moderate or low risk group. Conclusions : Accordingly, the supportive periodontal therapy applying dental hygiene process has been shown to be effective. The dental hygiene process in periodontal patients who require continuous management is anticipated to be a very efficient process.

Indochinamon ou (Crustacea: Potamidae) as a New Second Intermediate Host for Paragonimus harinasutai in Luang Prabang Province, Lao PDR

  • Paragonimus harinasutai metacercariae were found in a species of freshwater crab, Indochinamon ou, collected in a small stream of Namback District, Luang Prabang Province, Lao PDR. Adult flukes were recovered after experimental infection of the metacercariae to dogs. Metacercariae were round or slightly elliptical, $0.666{\times}0.626\;mm$ in average size, and had a thin cyst wall of about $20{\mu}m$ in thickness, a black excretory bladder, convoluted ceca, and some pinkish materials in the body. Adults were somewhat elongated, $95.2{\times}36.5\;mm$ in average size, covered with single-tipped tegumental spines, had a smaller oral sucker than the ventral sucker, a moderately branched ovary, and 5-6 lobulated testes. Eggs were ovoid and bilaterally symmetrical in shape, $79{\times}45{\mu}m$ in average size, and had a uniformly thickened shell. By the present study, it has been confirmed that I. ou is a new second intermediate host for P. harinasutai.
