Purpose: This study examined the characteristic of the Injury Severity Score (ISS) of Korean geriatric patients with a traumatic injury in a nationally representative sample to determine the optimal cutoff of ISS of mortality according to age. Methods: The subjects were 3,018 non-elderly patients and 1,584 elderly patients with an ISS and Korean Triage and Acuity Scale (KTAS) in 2016 from the data of the Health Insurance Review and Assessment Service. The traumatic characteristics of the elderly and non-elderly were compared by stratifying the ISS. Receiver Operating Characteristic (ROC) curve analysis was used to find the optimal cutoff of ISS of mortality according to age. Results: The elderly were more prone to severe trauma than the non-elderly were. The distribution of KTAS grades was lower, even though the severity of ISS was as high as that of the non-elderly. The optimal cutoff score of the ISS for mortality in the ROC curve was lower in elderly over 65 years than in the other age group. Conclusion: The elderly are more prone to severe trauma and death than non-elderly, even though their ISS is low. Therefore, a strategy to prevent elderly from experiencing serious trauma and managing their geriatric trauma actively is needed.
Current low-altitude radar system often fails to detect small unmanned aerial vehicles (UAV) because of their small radar cross section (RCS) compared with larger targets. As a potential alternative, a passive bistatic radar system has been considered. We study an optimal deployment problem for the passive bistatic radar system. We model this problem as a covering problem, and develop an integer programming model. The objective of the model is to maximize coverage of a passive bistatic radar system. Our model takes into account factors specific to a bistatic radar system, including bistatic RCS and transmitter-receiver pair coverage. Considering bistatic RCS instead of constant RCS is important because the slight difference of RCS value for small UAVs could significantly influence the detection probability. The paired radar coverage is defined by using the concept of gradual coverage and cooperative coverage to represent a realistic environment.

Objectives: We examined the agreement between the Autism Diagnostic Observation Schedule (ADOS) and the Childhood Autism Rating Scale (CARS) in the diagnosis of autism spectrum disorder. Methods: The ADOS and CARS scores of 78 children were retrospectively collected from a chart review. A correlation analysis was performed to examine the concurrent validity between the two measures. Using the receiver operating characteristic (ROC) curve, we determined the optimal cut-off score of the CARS for identifying autism spectrum disorder. Results: The CARS score was significantly correlated with the ADOS score (r=0.808, p<0.001). Taking ADOS as the ideal standard, the optimal cut-off scores of CARS for identifying autism and autism spectrum were 30 and 24.5, respectively. Conclusion: We determined the optimal cut-off scores of CARS for screening and diagnosing autism spectrum disorder.
Purpose: This study aimed to assign weights for subscales and items of the Post-Intensive Care Syndrome questionnaire and suggest optimal cut-off values for screening unplanned hospital readmissions of critical care survivors. Methods: Seventeen experts participated in an analytic hierarchy process for weight assignment. Participants for cut-off analysis were 240 survivors who had been admitted to intensive care units for more than 48 hours in three cities in Korea. We assessed participants using the 18-item Post-Intensive Care Syndrome questionnaire, generated receiver operating characteristic curves, and analysed cut-off values for unplanned readmission based on sensitivity, specificity, and positive likelihood ratios. Results: Cognitive, physical, and mental subscale weights were 1.13, 0.95, and 0.92, respectively. Incidence of unplanned readmission was 25.4%. Optimal cut-off values were 23.00 for raw scores and 23.73 for weighted scores (total score 54.00), with an area of under the curve (AUC) of .933 and .929, respectively. There was no significant difference in accuracy for original and weighted scores. Conclusion: The optimal cut-off value accuracy is excellent for screening of unplanned readmissions. We recommend that nurses use the Post-Intensive Care Syndrome Questionnaire to screen for readmission risk or evaluating relevant interventions for critical care survivors.
There are few studies reporting optimal waist circumference that can be utilized to prevent the incidence of cardiovascular disease (CVD). We evaluated the association of waist circumference and waist and hip circumference ratio (WHR) with incident cases of CVD developed over 6 years in a population-based prospective study including Korean adults. Analyses for receiver-operating characteristic (ROC) curve were performed with data for 1,733 men and 1,579 women who were aged 40 to 69 years and were free of a physician-diagnosis of CVD at baseline. Information on the diagnosis of CVD was periodically reported using interviewer-administered questionnaires and anthropometric measures were obtained by biennial health examinations. We newly identified 77 cases of CVD during a follow-up period between 2003 and 2008. On the basis of measures of diagnostic accuracy including minimum distance to ROC curve and Youden index, waist circumference of 85 cm for men, in particular for male nonsmokers, and of 80 cm for women and WHR of 0.88 to 0.90 for men and of 0.83 for women were found to be optimal cutoff points to identify individuals at CVD risks. The study also found that the use of the suggested optimal values for waist circumference show higher sensitivity and lower specificity compared with 90 cm for men and 85 cm for women, which are waist cutoff points given by the Korean Society for the Study of Obesity to define abdominal obesity for Korean adults. Although lower cutoff points of waist circumference (83 cm) and WHR (0.87) were observed to be optimal for male smokers compared with male nonsmokers, whether suggesting waist cutoff points specific to smokers is needed warrants further studies. After taking into account other cardiovascular risk factors including smoking, men with waist circumference of 85 cm or greater and women with 80 cm or greater were at an increased risk of CVD. Thus, these cutoff points of waist circumference may be able to capture more individuals at CVD risks contributing to the prevention of future development of CVD.
Objectives: The purpose of this study was to evaluate the optimal distance and angle of observers by modulation transfer functions(MTFs) and receiver operating characteristics(ROCs), Material and Methods: Digital periapical radiograms were taken from 43 patients who have dental diseases(19 patients : dental caries. 12 patients : periapical lesions, 12 patients : periodontal diseases). Segmental images(4×4cm) were evaluated by 4 MTFs and ROC analysis. Results: The optimal distance(magnification) using MTF by Mannos & Sakrison was 12.97. and those by Nill. Ngan and Rao were 8.39, 4.78. 5.84 respectively. The optimal distance obtained from 4 radiologists by ROC analysis was 32cm(Az value: 0.89). and it was 40cm(Az value: 0.78) from 4 non-radiologists. There were significant differences of Az values between 4 radiologists and 4 non-radiologists at 24. 32 and 40cm (P<0.05). No significant differences of optimal distances were observed using 4 MTFs among +20, +10, 0, -10, -20 degrees(P>0.05). The optimal angle obtained from 4 radiologists by ROC analysis was +20 degree(Az value: 0.91). and no significant differences of Az values were observed among +20, +10, 0, -10 and -20 degrees(P>0.05). The optimal angle obtained from 4 non-radiologists by ROC analysis was 0 degree(Az value : 0.81). and no significant differences of Az values were observed among +20, +10, 0, -10 and -20 degrees(P>0.05). And there was significant difference of Az value between 4 radiologists and 4 non-radiologists at +20 degree(P<0.05). but no significant differences of Az values were observed among +10, 0, -10 and -20 degrees(P>0.05).
