How will surface treatments affect the translucency of porcelain laminate veneers?

  • PURPOSE. The purpose of this study was to evaluate whether surface treatments affect the translucency of laminate veneers with different shades and thicknesses. MATERIALS AND METHODS. A total of 224 disc-shaped ceramic veneers were prepared from A1, A3, HT (High Translucent) and HO (High Opaque) shades of IPS e.max Press (Ivoclar Vivadent) with 0.5 mm and 1.0 mm thicknesses. The ceramics were divided into four groups for surface treatments. Group C: no surface treatments; Group HF: etched with hydrofluoric acid; Group SB: sandblasted with 50-${\mu}m$ $Al_2O_3$; and Group L; irradiated with an Er;YAG laser. A translucent shade of resin cement (Rely X Veneer, 3M ESPE) was chosen for cementation. The color values of the veneers were measured with a colorimeter and translucency parameter (TP) values were calculated. A three-way ANOVA with interactions for TP values was performed and Bonferroni tests were used when appropriate (${\alpha}=0.05$). RESULTS. There were significant interactions between the surface treatments, ceramic shades and thicknesses (P=.001). For the 0.5-mm-thick specimens there were significant differences after the SB and L treatments. There was no significant difference between the HF and C treatments for any shades or thicknesses (P>.05). For the 1-mm-thick ceramics, there was only a significant difference between the L and C treatments for the HT shade ceramics (P=.01). There were also significant differences between the SB and C treatments except not for the HO shades (P=.768). CONCLUSION. The SB and L treatments caused laminate veneers to become more opaque; however, HF treatment did not affect the TP values. When the laminate veneers were thinner, both the shade of the ceramic and the SB and laser treatments had a greater effect on the TP values.

도재용착용 Ni-Cr 합금과 Co-Cr 합금의 열처리에 따른 전단결합강도 비교 (Comparison of Shear Bond Strength of Ceramic Fused to Ni-Cr and Co-Cr Alloy by Heat Treatment)

  • Purpose: This study was to evaluate the shear bond strength of the ceramic fused to Ni-Cr alloy(Bellabond plus) and Co-Cr alloy(Wirobond C) by heat treatment. Methods: Metal specimens were divided into 5 groups for each alloy according to heat treatment conditions prior to porcelain application. Fifteen specimens from each group were subjected to a shear load a universal testing machine using a 0.1mm/min cross-head speed and one specimen from each group was observed with EDX line profile. Results: The diffusion of metal oxide observed far in the specimen heat treated than no heat treated in the opaque layer. The shear bond strength measured highest to BP3(50.50MPa), WC2(50.49MPa) groups and measured lowest from BP1(35.1MPaa), WC1(39.66MPa) groups which were not treated with heat, and there was a significant difference (p<0.05). Conclusion: The shear bond strength of Ni-Cr alloy(Bellabond plus) and Co-Cr alloy(Wirobond C) measured similar 5 groups all.

금속코핑 설계에 따른 Collarless Metal Ceramic Crown의 파절강도에 관한 연구 (A STUDY ON FRACTURE STRENGTH OF COLLARLESS METAL CERAMIC CROWN WITH DIFFERENT METAL COPING DESIGN)

  • The metal ceramic crown is currently the most popular complete veneer restoration in dentistry, but in many cases, the metal cervical collar at the facial margin is unesthetic and unacceptable. Facial porcelain margin has been used in place of it. But this dose not solve the problems, such as dark gingival discoloration and cervical opaque reflection of porcelain veneer. Recently, metal copings which were designed to terminate its labio-cervical end on the axial walls coronal to the shoulder have been clinically used to solve the esthetic problem of metal ceramic crown. But in this design, porcelain veneer of labio-cervical area which is not supported by metal may not be able to resist the stress during cementation and mastication. The purpose of this study was to evaluate fracture strength and fractured appearance of crowns according to different coping designs. A resin maxillary left central incisor analogue was prepared for a metal ceramic crown, and metal dies were made with duplication mold. Metal copings were made and assigned to one of four groups based on facial framework designs: group 1, coping with 0.5mm metal collar; group 2, metal extended to the shoulder; group 3, metal extended to 1mm coronal tn the shoulder: group 4, metal extended to 2mm coronal to the shoulder. Copings and crowns were adjusted to be same size and thickness, and cemented to metal dies with zinc phosphate cement by finger pressure. Fracture strength was measured with Instron Universal Testing Machine. Metal dies were anchored in Three-way-vice at 3mm below finish line and at $130^{\circ}$ inclined to the long axis of the crown. Load was directed lingually at 2mm below midincisal edge. Load value at initial crack and at catastrophic fracture was recorded. The results obtained were as follows : 1. Fracture strength values at initial crack were higher in groups 1, 2 than in groups 3, 4 but this difference was not statistically significant(P<0.05). 2. Conventional metal collared crown had greater catastrophic fracture strength than any other collarless crowns. 3. The greater the labial metal coping reduction, the lower the catastrophic fracture strength of crowns but when more than 1mm of labial metal reduction was done, the difference in strengths was not statistically significant(p<0.05). 4. The strongest collarless coping design was group 2.

A STUDY ON THE MARGINAL FIT OF COLLARLESS METAL CERAMIC FIXED PARTIAL DENTURES

  • Statement of problem. Collarless metal ceramic fixed partial dentures(FPDs) had an esthetic problem such as opaque reflection in cervical region. To overcome this, modified coping which removed its facial cervical metal could be used. The marginal quality could be worsen according to the amount of its facial metal reduction. Purpose. The purpose of this study was to evaluate marginal fits of collarless metal ceramic FPDs with retainers of modified copings. Material and method. Dentoform maxillary left central incisor and right lateral incisor were prepared for 3-unit collarless metal ceramic FPD and fixed in yellow stone. This model was duplicated to PBT resin dies via CAD/CAM and injection molding. Four different facial margin design groups were investigated. Group A was a coping with a thin facial metal collar, group B was a collarless coping with its facial metal to the shoulder, group C was a collarless coping with its facial metal 1 mm short of the shoulder, and group D was a collarless coping with its facial metal 2 mm short of the shoulder. Seven collarless metal ceramic FPDs per group were fabricated. They were cemented to PBT resin dies with resin cement. After removal of pontics, each retainers were separated and observed under Accura 2000 optical microscope. Then, retainers were embeded in orthodontic resin and cross sectioned faciopalatally. Internal marginal fits of midfacial porcelain margins were observed under FE-SEM. Result and conclusion. Within the limitations of this in vitro study. The following conclusions were drawn. 1. Mean marginal gaps of collarless FPDs were in the $50-60{\mu}m$ range. 2. In midfacial margin, marginal discrepancies were greater in group A than in the experimental groups(p<0.05). 3. In midpalatal margin, marginal gaps were greater in group C and D than in group A and B(p<0.05). 4. Marginal fits of porcelain margins were better than those of metal margins in collarless metal ceramic FPDs. 5. In both teeth, internal marginal gaps of group C and D were greater than those of group A and B(p<0.05).

치과용 지르코니아의 발전 (Development of dental zirconia)

  • 1990년대 말부터 치과보철 수복에 응용된 초창기 지르코니아는 코핑 형태로 사용되었다. 이렇게 만들어진 보철 수복물은 기존의 다른 심미 금속 수복물의 구조물이나 프레임 역할을 하는 금속이 없어서 자연스러움이 필요한 전치부에 성공적으로 사용되었으나 구치부에서는 기능중에 상부 비니어링 포세린의 파절로 그 사용에 한계가 있었다. 그후 여기에 대한 해결 방안으로 강한 코핑 재료를 전체 보철물의 형태로 제작하는 단일구조 지르코니아 수복물이 제안되었다. 이것은 강한 힘에 대한 저항은 뛰어나지만 매우 희고 불투명한 지르코니아의 특성상 조금은 비심미적인 결과를 나타내었다. 그러나, 현재는 지르코니아의 투명도를 높이는 기술적인 진보로 구치부뿐만 아니라 전치부에서 강하면서도 심미적인 지르코니아 수복물 제작이 가능하게 되었다. 이 논문에서는 지금까지 개발된 여러 종류의 지르코니아 재료의 구조적 특징을 알고 실제로 사용하고 있는 지르코니아 제품이 어디에 속하는지 설명한다.

착색지르코니아 코어와 전장 도재 사이의 전단결합강도에 관한 연구 (A study on the shear bond strengths of veneering ceramics to the colored zirconia core)

  • 연구목적: 지르코니아-도재 수복물에 있어 상부도재와 코어 사이의 결합 실패가 종종 보고되어 왔으며 특히 착색지르코니아 코어는 기존의 백색 지르코니아보다 상부 도재와의 결합력이 약하다고 보고된 바 있다. 이 연구의 목적은 착색 지르코니아 코어 위의 상부도재를 적층식과 열가압식으로 제작하여 그 전단결합강도를 알아보고, 이를 전통적인 금속-도재간 결합강도와 비교하여 그 임상적 안정성을 평가하는 것이다. 연구 재료 및 방법: 금속도재군 (MC)을 대조군으로 하였다. 전통적인 금속도재군 (MC)과 지르코니아 코어를 사용한 두가지 군 (ZB, ZP)에 대하여 각 시스템별로 10개씩, 총 30개의 시편을 제작했다. CAD/CAM을 이용해 직경 12 mm, 높이 2.8 mm의 원판형 지르코니아 코어 (Katana zirconia)를 제작하고, 그 상부에 직경 2.8 mm, 높이 3 mm의 도재를 축성했다. ZB군은 CZR을 이용하여 적층법으로 상부도재를 제작했으며 ZP군은 NobelRondo Press ingot를 열가압하여 제작했다. Shear bond test machine (R&B Inc. Daejeon, Korea)을 이용하여 분당 0.50 mm의 속도로 파절이 일어날 때까지 전단력을 가하여 최대적용력 (N)을 측정하여 전단결합강도를 계산하고, 일원배치 분산분석을 사용하여 유의수준 5%에서 검정하였다. 파절양상을 알아보기 위하여 전자주사현미경을 통해 파절단면을 관찰했다. 결과: 평균 전단강도 (SD)는 MC 대조군 29.14 (2.26); ZB 29.48 (2.30); ZP 29.51 (2.32) 이었다. 실험군과 대조군 사이에 유의한 차이는 없었다. 모든 실험군에서 접착성 실패와 응집성 실패가 혼재된 양상을 보였으며, 응집성 실패가 우세했다. 결론: 1. 착색지르코니아 코어와 상부도재들 간의 전단결합강도는 금속 도재간 전단결합강도와 유의한 차이가 없었다. 2. 착색지르코니아 코어의 상부도재를 제작하는 방식에 있어 적층법과 열가압법 간의 전단결합강도에 유의한 차이는 없었다 (P > .05). 3. 파절양상은 응집성 파절이 우세한 가운데 접착성 파절과 응집성 파절이 혼재되어 나타났다.
