항암화학요법을 받는 환자를 위한 웹기반 동영상 감염예방 자가관리 교육프로그램 효과: 예비연구 (Effect of Web-Based Video Self-Management Education Program for Infection Prevention in Patients with Cancer Receiving Chemotherapy: A Preliminary Study)

  • Purpose: The aim of this study was to examine the preliminary effects of applying a developed web-based video education program in patients with cancer receiving chemotherapy. Methods: A one-group pre-post test design was used. The web-based video education program to prevent infection consisted of 5 subjects. The program was confirmed to be valid by an expert group of doctors and nurses. Convenience sampling of 23 subjects was performed to evaluate the effectiveness of the web-based video education program. Data were analyzed based on nonparametric statistics using SPSS 25.0 software. Results: Per the results of the study, knowledge and self-management behavior of infection prevention significantly improved after application of the program; however, there was no significant difference in self-efficacy. Conclusion: It was confirmed that the web-based video education program is effective to improve the knowledge and self-management behavior of infection prevention. However, as a preliminary study, this study did not have external validity. Therefore, it is necessary to verify the effectiveness of the program through randomized controlled trials and confirm the true infection prevention effect that was not considered in this research design.

유방암환자의 피로와 심리사회적 적응에 대한 희망의 매개 (mediation) 작용 (Mediation Effect of Hope between Fatigue and Psychosoical Adjustment in Women with Breast Cancer)

  • The purpose of the present study is to identify the mediation effect of hope between fatigue and psychosocial adjustment in women with breast cancer. The framework for this study was guided by concepts and propositions derived from the theoretical and empirical literature on fatigue, hope and adjustment. The design of this study is a descriptive correlation study using a cross-sectional design. One hundred and twenty two outpatients with early breast cancer, receiving post-surgical radiation therapy or chemotherapy, were selected from three major medical centers in Seoul, Korea. A packet including PABCF (Psychosoical Adjustment to Breast Cancer Factor), revised RPFS (Revised Piper Fatigue Scale), HHI (Herth Hope Index), and self-addressed return envelope was given to the participants at seven to eight weeks post surgery. The questionnaires were to be completed at home and returned to the researcher by mail. The obtained data were analyzed using three regression equations guided by Baron and Kenny (1986); first, hope was regressed on fatigue; second, psychosocial adjustment was regressed on fatigue; and third, psychosocial adjustment was regressed on fatigue and hope, simultaneously. In the first equation, fatigue explained 4% of the variance in hope. In the second equation, fatigue explained 47% of the variance in psychosocial adjustment. In the last equation, hope and fatigue significantly explained the variance in psychosocial adjustment. Therefore, all conditions for the test of mediation effect of hope were satisfied. For the test of the mediation effect, the beta coefficients of fatigue on psychosocial adjustment on the second and third regression equations were compared. The beta coefficients were decreased from .69 (p < .001) on the second regression equation to .63 (p < .001) on the third regression equation. Thus, the hypothesis of this study was supported. As a result of this study, the negative Influence of fatigue on psychosocial adjustment is dampened through the mediator effect of hope in women with breast cancer. Therefore, when planning care for the adverse effect of fatigue on psychosocial adjustment, oncology nurses should consider hope as a mediator between fatigue and psychosocial adjustment to breast cancer.

다직종 암환자 교육·상담에 대한 설문 개발과 환자 만족도 조사 (The Development of Multidisciplinary Cancer Patient Education·Counseling Questionnaire and Satisfaction Survey)

  • Background: Presently, a multidisciplinary team of doctors, pharmacists, nurses, and dietitians provides patient education to impart information on chemotherapy. However, studies on multidisciplinary education satisfaction are inadequate. In this study, we aimed to contribute to the improvement of quality of multidisciplinary education counseling for patients with cancer by developing a satisfaction questionnaire and analyzing the satisfaction survey. Methods: A questionnaire was developed by an expert group, and the responses were recorded using the 5-point Likert scale. After conducting a pre-test, factor analysis was performed to evaluate validity. The reliability of the questionnaire was measured by Cronbach's alpha coefficient. A satisfaction survey was conducted by self-administration method. Results: Based on the results of factor analysis, factors can be divided into two parts: "overall education" and "each team member's education" (total 14 questions). The construct validity and reliability of the questionnaire are sufficiently high. Fifty-one patients took the survey between January 2, 2018 and January 20, 2018. Twenty-six (51%) patients responded that they were "very satisfied" and 22 (43.1%) patients responded that they were "satisfied". Conclusion: By developing a questionnaire on multidisciplinary education counseling for patients with cancer, it is possible to perform evaluation and research of cancer patient education. This study will contribute to the management and improvement of quality of multidisciplinary education.

암 환자를 돌보는 간호사의 부담감, 직무 만족도 및 삶의 질 (Burden, Job Satisfaction and Quality of Life of Nurses Caring for Cancer Patients)

  • 목적: 본 연구는 암 환자를 돌보는 간호사의 부담감, 직무 만족도 및 삶의 질을 파악하고 제 변수들 간의 관계를 알아보기 위한 서술적 조사 연구이다. 방법: 연구 대상은 경기도 및 서울에 있는 500병상 이상의 4개 종합병원 종양 병동의 간호사 237명이었다. 자료 수집 기간은 2005년 2월 2일부터 3월 3일까지였다. 연구도구로 부담감은 이관희(1985), 김연희(2001)와 조계화(2002), 직무 만족도는 이상금(19%), 삶의 질은 노유자(1988)가 개발한 측정도구를 수정, 보완하여 사용하였다. 자료 분석은 SAS 통계 프로그램을 이용하여 t-test, ANOVA, Duncan test, Pearson correlation coefficient, Stepwise multiple regression을 적용하였다. 결과: 1. 부담감의 평균은 5점 만점에 2.78점, 직무 만족도는 2.93점, 삶의 질은 3.22점이었다. 2. 부담감이 가장 높았던 문항은 '환자의 고통을 덜어주기 위해 노력해도 한계를 느낀다'였다. 부담감은 35세 미만 군에서, 임상 경력과 암 환자간호 경력이 각각 $3{\sim}4$년 군에서 유의하게 높았다. 3. 직무 만족도가 가장 낮았던 문항은 직장에서의 승진 가능성 정도였다. 직무 만족도는 배우자가 있는 군, 직위는 수간호사와 책임간호사 군에서 유의하게 높았다. 4. 삶의 질이 가장 낮았던 문항은 '신체적 피로를 느낀다'였다. 삶의 질은 35세 이상 군, 배우자가 있는 군, 수입이 200만원 이상군에서 유의하게 높았다. 5. 부담감과 직무 만족도, 삶의 질은 역상관 관계가 있었다. 6. 삶의 질에 영향을 미치는 주요 요인은 부담감이었다. 결론: 암 환자를 돌보는 간호사의 부담감과 직무 만족도가 높을수록 삶의 질은 낮아지므로 삶의 질을 높일 수 있는 행정적, 제도적 지원과 부담감 감소를 위한 현실적인 대안을 마련하는 것이 필요하다.0.04{\pm}0.20$ P=0.1519), 우울 정도, 불안 정도, 스트레스 정도는 자가치료 순응도가 좋을수록 감소하는 경향을 보였으나, 통계적으로 유의하지는 않았다. 아로마 자가 치료군에서 수축기 혈압은 약간 증가하였으나 통계학적 의의는 없었다($4.53{\pm}14.43mmHg\;vs\;0.0{\pm}7.22mmHg$, P=0.152). 자가 치료군에서 환자들은 두통(20%), 감각이상(6.7%), 오심(6.7%) 등의 부작용을 호소했으나, 이들 모두 일시적인 것으로 자가치료를 중단할 정도는 아니었다. 결론: 유방암 환자들에게 아로마 손 마사지 교육을 시킨 후 자기 스스로 2주간 시행한 결과, 통증강도, 우울 및 불안 정도가 의미 있게 감소하였다.임의 유무와 이유에 대해서는, '망설이지 않았다'(34명)가 '망설였다'(6명)보다 높게 나타났다. 사별가족모임에 참석한 후의 소감, 개선사항, 아쉬운 점에 대한 질문에는, 대부분의 응답자들이 '의미 있는 시간이었다', '사별가족에 대한 배려와 관심에 대해 감사한다.'라는 긍정적인 응답이 있었고, '고인에 대한 회상을 할 수 있는 자리여서 좋았다.' '사별가족모임이 일년에 한 번이 아니라 계속적으로 있었으면 한다.', '한편은 슬프고 한편은 기쁘다.' 등의 의견이 있었다. 사별 후 느끼는 가장 큰 어려움에 대해 참석가족은, '우울감'(10명), '외로움'(7명)의 빈도가 높았고, '그리움'(1명), '경제적/역할상 어려움'(4명), 무응답(6명)으로, '우울감'이 가장 큰 어려움으로 나타났다. 반면에, 비참석 가족에서는, 가장 큰 어려움은 '경제적/역할상 어려움'(6명), '외로움'(5명), '우울감'(3명), '후회감'(1명), '고인에 대한 원망감'(1명), '특별히 어려움이 없다'(1명)라고 답하였다. 결론: 호스피스 서비스를 경험한 사별가족들은 고인이 품위 있는 임종을 맞이하였다고 생각하는 견해가 높았고,

암환자의 삶의 질 도구개발 (Development of Quality of Life Measurement for Cancer Patients)

  • This study was undertaken to develop an instrument to be used for measuring the concept of quality of life of Korean patients with cancer multidimensionary and correctly. It can contribute in holistic nursing care for Korean cancer patients and also provide and validate basic data to help oncology nurses measure the outcome of nursing intervention correctly. To develop this instrument, the researchers first estabilished a conceptual framework based on the results of qualitative data analysis and indepth interview method Development of the scale was conducted using a method in which 31 items were assessed by subjects' self report using linear analogue scales. The subjects were 79 D.M. patients, 103 patients with acute illness, and 91 cancer patients residing in Busan, Korea. Data were collected during the period from July, 24 to August 14, 2000. This instrument consisted of 31 items with a self report scale. This instrument covered 4 dimensions of cancer patients : 1) physical wellbeing 2) psychological wellbeing 3) social wellbeing and 4)spiritual wellbeing. Each item had a possible score of 10. The reliability of the scale was tested with Cronbach's alpha. Validity was evaluated by examining the relationships of this scale, Youn's Quality of Life Questionnare scores and the Simple Quality of Life scale. Two separate runs of multiple regression were used to predict scores on the Simple Quality of Life measurement. Further validation was obtained by examining the correlation between the instrument subscores and Youn's Quality of Life measurement subscore for convergence of this scale. Examination of the discriminant. power of the instrument was done using ANOVA test. The results are summarized as follows: 1. The reliability of the instrument for the quality of life was 0.8321(Cronbach's alpha.), physical wellbeing dimension 0.6343, psychological wellbeing dimension 0.6501, spiritual wellbeing dimension 0.5883. 2. This instrument had a high correlation with Youn's Quality of Life measurement(r= 0.636) in cancer patients, whereas it had a low correlation with Simple Quality of Life measurement(r=0.455) in cancer patients. In the D.M. patients, the instrument correlated with both the Youn's Quality of Life measurement and Simple Quality of life measurement(r=0.313, r= 0.407) and in the acute stage patients, the instrument had no correlation. 3. Multiple regression of individual items on the Simple Quality of Life scores accounted for 56.8% of the variance in the Simple Quality of Life measurement, whereas, Youn's Quality of Life measurement scores accounts for 31.7%. 4. The correlations collected from the three group had the same patterns of variations but especially the instrument developed in this study had higher disciminant power than that of Youn's Quality of Life Measurement.
