A Study on Acute Oral Toxicity of Pyungwi-san and Fermented Pyungwi-san in ICR Mice (ICR 마우스를 이용한 평위산과 발효평위산 급성독성 연구)

  • Objectives: This study was conducted to investigate the acute toxicity of Pyungwi-san(Pingwei-san in Chinese) in ICR mice, according to KFDA and OECD guideline. Methods: In the present study, 15 male and female ICR mice administrated singly by gavage at dose levels of 0 and 2000 mg/kg of Pyungwi-san. During the experimental period, no treatment-related death was observed. There were no adverse effects on clinical signs, body weight, and gross findings at all treatment groups. Results: These results showed that the single oral adminstration of Pyungwi-san (Pingwei-san) did not cause any toxic effect at the dose levels of 2000 mg/kg in rats. Conclusions: Taken together, the median lethal dose($LD_{50}$) of Pyungwi-san (Pingwei-san) was considered to be over 2000 mg/kg body for both sexes.

A Study on Factors Influencing Medical Treatment Decision-Making for Overactive Bladders in Female Patients over 40 - Data from Clinical Trial Participants - (40세 이상 과민성방광 여성 환자의 의료이용 결정요인에 관한 연구 - 임상시험 참여자를 중심으로 -)

  • Purpose: The aim of this study is to determine associated factors affecting the utilization of Medical Care for an overactive bladder. Methods: Total 114 women with overactive bladder participated in this study. Patients were categorized in two groups, the patients with treatment experiences for an overactive bladder(n=40) and without treatment experiences for an overactive bladder(n=74). Patients completed Overactive Bladder Severity Score, Beck Depression Inventory, King's Health Questionnaire, and questionnaires on treatment experience. The results were analysed using SPSS for Windows 12.0k. All statistical tests were two-sided with a p-value < 0.05 being significant. Results: Patients with treatment experience had a greater score on KHQ, OABSS, BDI and had longer symptom duration period than those without treatment experience. And only Impact of Life(P=0.004), Role Limitation(P=0.004), Social Limitation(P=0.007), Emotion(P=0.035), Sleep/Energy(P=0.001), Incontinence Severity Measures(P=0.012) among KHQ domains were statistically significant difference between two groups. Conclusions: The most influencing factor associated with utilization of Medical Service in overactive bladder is the impact of the patient's quality of life.

Analysis of Blood Pressure Pulse in Female High School Students with Menstrual Disorders (월경장애를 가진 여고생의 맥진분석)

  • Objectives: The purpose of this study is to find out the patterns of teenage girls who is easy to suffer from Menstrual Disorders. Methods: The data from the 1681 participants were collected using a structured menstrual history questionnaire. Based on the survey responses, we had 97 adolescents with menstrual disorder as the test group and 97 adolescents without menstrual disorder as the control group. The clinical trials subjects were asked to respond to another questionnaire for identifying Blood Pressure Pulse Analyzer tests. We identified significant variables for dysmenorrhea from comparison between the test and control group; a binary response variable from the test group's responses for ever experiencing dysmenorrhea and explanatory variables from Blood Pressure Pulse Analyzer. Results: The reselts of Blood Pressure Pulse Analyzer test showed that the score of Energ_R, EIX_R were lower in dysmenorrhea group than the control group. The reselts of Blood Pressure Pulse Analyzer test showed that the score of Energ_R, EIX_R were lower, and T4_R were higher in hypomenorrhea group than the control group. The reselts of Blood Pressure Pulse Analyzer test showed that the score of Energ_R, EIX_R were lower in Irregular Menstruation group than the control group. Conclusions: Blood Pressure Pulse Analyzer test showed that the score of Energ_R, EIX_R were lower in the test group than the control group.

Review on RCT Research Papers Related Nausea and Vomiting in Pregnancy - Focusing on Acupoint Treatment - (임신오조(姙娠惡阻) 관련 무작위대조군연구(RCT)에 관한 고찰 - 경혈을 이용한 치료를 중심으로 -)

  • Objectives: To observe randomized controlled trials(RCTs) related to investigating the effects of acupoint treatments on nausea and vomiting in pregnancy to establish the basis of korean medical treatments, and suggest the problems of future clinical research. Methods: After searching for national and international papers published after 2000 and restricting the parameters to 'clinical trial', 'controlled clinical trial', 'randomized controlled trial', then comparing the subjects, methods, research tools, and results to understand the effects and re-comparing the conflicting results to assume the cause. Results: Of the 9 research papers that applied acupressure, four of them used control and experimental groups, and three(75%) research papers indicated significant reduction in vomiting and nausea, and of the seven research papers that utilized the experimental and placebo groups, five(71.4%) reported significant reducing effects. Of the four research papers the used acupuncture treatments, four research papers using control and placebo groups, two showed improvements in symptoms but did not have significant results compared to sham acupuncture groups, and two showed significant reduction of symptoms compared to the control groups. The one research paper that used electrical stimulation showed significant results in the reduction of vomiting and nausea compared to the control group. Conclusions: Acupressure and electrical stimulation showed efficacy reducing the symptoms. Acupuncture treatments did not indicate efficacy some experiments. Therefore it is thought that future acupuncture treatments should use methods that resemble actual clinical medicine to establish efficacy to produce significant results.

A Survey Study on Use Condition of Korean Medical Institution and Demand of Korean Medical Treatment Project of Infertility in Infertile Couple (난임부부의 한방의료기관 이용 실태 및 한방난임치료 지원사업 수요에 관한 설문조사연구)

  • Objectives: The purpose of this study is to investigate use condition of Korean medical institution and demand of Korean Medical Treatment Project of Infertility in infertile couple. Methods: Questionnaires were distributed to all members of the incorporated association AGAYA(http://www.agaya.org/) via e-mail and to infertile women who visited to Korean medicine OB & GY outpatient clinic of university Korean medicine hospitals of the country and an infertility specialized Korean medicine hospital. One hundred twenty four of completed questionnaires were analyzed. Results: 1. Korean medical institution utilization rate of infertile couple was 89.6% and was similar with western medical institution utilization rate of 87.1%. 2. 96.8% of infertile couple answered that Korean Medical Treatment Project of Infertility is necessary. And 90.3% of infertile couple answered that herbal medicines should be supported first of all. 3. 90.3% of infertile couple answered that they will participate if Korean Medical Treatment Project of Infertility go ahead. Conclusions: It was confirmed that most of the infertile couple have been received Korean medical treatment for infertility, and think that Korean Medical Treatment Project of Infertility is necessary.

A Study on Factors Influencing Pregnancy in the Pilot Project for Korean Medical Treatment for Subfertility (한방 난임 치료 시범사업에 참가한 여성의 임신 성공에 영향을 미치는 요인에 관한 연구)

  • Objectives: This study aims to identify associated factors in female infertility treatment using herbal medicine, acupuncture and moxibustion. Methods: Acupuncture and moxibustion treatments were performed with intake of herbal medicines (Jokyungjongoktanggagam-bang, Sutaehwangagam-bang) for six menstrual cycles in 32 subfertile women. The variables selected for initial analysis were age, duration and type of infertility, frequency of previous In Vitro Fertilization (IVF) and live births, weekly frequency of sexual intercourse, number of treatment cycles, endometrial thickness on MCD15 and hormonal parameter (anti-mullerian hormone, follicle-stimulating hormone, luteinizing hormone, estrone). Results: Six patients became pregnant (18.75%) and seven patients withdrew. Factors influenced pregnancy were age (${\leq}35$ years), a shorter duration of infertility (<6 years), frequency of IVF (${\leq}3$), anti-mullerian hormone level (>1 ng/ml), and number of treatment cycles(${\leq}3$ cycles). Conclusions: Korean medicine is a useful and optimized treatment option for women with younger age, fewer previous IVF cycles, shorter infertile duration and normal anti-mullerian hormone level at first 3 cycles of treatment.

Acute Oral Toxicity of Root of Polygala teunifolia Willd. Extract (원지(Root of Polygala teunifolia Willd.) 추출물의 급성 경구투여 독성 연구)

  • Objectives: In this study, it was carried out to evaluate the acute oral toxicity of Root of Polygala teunifolia Willd. in Sprague-Dawley (SD) rats. Methods: Male and female rats were administered orally with Root of Polygala teunifolia Willd. water extract of 1,000 mg/kg (low dosage group), 2,000 mg/kg (middle dosage group) and 4,000 mg/kg (high dosage group). We daily observed number of deaths, clinical signs and gross findings for 7 days. After 7 days, we measured body and organs weight. Also we analyzed hematological changes. Results: No dead SD rats and no clinical signs were found during the experiment period. Also other specific changes were not found between control and treated groups in hematology and serum biochemistry. But we found out histopathological changes in liver fat tissues of female. In addition, there were no significant changes of gross body and individual organs weight. Conclusions: These results suggest that water soluble extract of Root of Polygala teunifolia Willd. has not acute oral toxicity and oral $LD_{50}$ value was over 4,000 mg/kg in SD rats.

Relationships of High School Girls Menstrual Disorders with the Inbody Test Results by Sasang Constitutions (사상체질(四象體質)에 따른 여고생의 월경장애와 Inbody 검사결과와의 관계)

  • Purpose: The purpose of this study is to find out the patterns of teenage girls who is easy to suffer from dysmenorrhea with the Inbody test results by Sasang constitutions. Methods: The data from the 1681 participants were collected using a structured menstrual history questionnaire. Based on the survey responses, we had 97 adolescents with menstrual disorder as the test group and 97 adolescents without menstrual disorder as the control group. The clinical trials subjects were asked to respond to another questionnaire for identifying their constitutional types and undergo Inbody test. Results: The result of a comparison of the test and control groups showed that there' no relevance to the body fat mass and body fat percentage with menstrual irregularities. The lesser yang person with menstrual irregularities was no relevance to the body fat percentage. The greater yin person with menstrual irregularities was especially lacking in body fat mass and body fat percentage. The lesser yin person with menstrual irregularities was poor in body fat mass. Conclusions: As for study, female high school students with menstrual disorders have nothing to do with muscle mass. Body fats shortage could pose problems. According to the study, Taeumin female high school students usually needed to higher body fat than a general standard. It seems to be needed more body fat and weight than modern standards in period of poor sexuality for having a normal menorrhea especially Taeumin. It will take some continuing study that BMI standards should be changed or not on the Sasang constitutions.

A Study for GnRH Antagonist (Cetrotide) Short Protocol in Controlled Ovarian Hyperstimulation (GnRH Antagonist (Cetrotide) Short Protocol의 임상적 유용성에 관한 연구)

  • Objective : The aim of this study was to evaluate the outcome the GnRH antagonist (Cetrotide) short protocol in controlled ovarian hyperstimulation comparing with GnRH agonist long protocol. Materials and Method: From July 2000 to November 2001, 26 patients, 28 cycles were performed in controlled ovarian hyperstimulation by GnRH antagonist and GnRH agonist. GnRH antagonist (Cetrotide) was administered in 12 patients (14 cycles, Group 1) and GnRH agonist (Lucrin, Sub Q, Group 2) in 14 patients (14 cycles). Ovulation induction was performed by hMG (Pergonal) in group 1, and by Combo (Metrodine HP + Pergonal) in group 2. We compared the fertilization rate, good quality embryo, and clinical pregnancy rate between the two groups. Student-t test and Chi-square were used to determine statistical significance. Statistical significance was defined as p<0.05. Results: Ovarian hyperstimulation syndrome did not occurred in which estradiol (E2) level was $3874{\pm}809\;pg/ml$ and the number of retrieved oocytes was $18.4{\pm}2.4$. The number of used gonadotropin ampules was significantly decreased in Group 1 (26.0 vs. 33.1, p<0.04). There were no significant difference in the number of preovulatory oocyte ($10.6{\pm}6.9$ vs. $10.0{\pm}6.1$), fertilization rate ($74.8{\pm}23.4$ vs. $72.2{\pm}21.8$), good quality embryo ($58.7{\pm}23.6$ vs. $38.7{\pm}36.6$), and embryo transfer ($4.3{\pm}1.6$ vs. $4.4{\pm}1.6$). Although the age of the group 1 was older than the group 2 (34.4 vs. 30.8), there was no significant difference in clinical pregnancy rate (50.0% vs. 57.1%). Conclusions: We suggest that GnRH antagonist was a safe, effective, and alternative method in the controlled ovarian hyperstimulation, especially in PCOD patients who will be develop the ovarian hyperstimulation syndrome.

The Dimension of Trichomonas vaginalis as Measured by Scanning Electron Microscopy

  • It is known that physicochemical conditions (e.g., pH, temperature, and ionic strength) affect the size of trichomonads. In this study, the sizes of 4 isolates of Trichomonas vaginalis cultured for more than a year (called "old T") and 3 isolates freshly isolated from vaginitis cases (called "fresh T") were compared by scanning electron microscopy. Although the fresh T had shorter body length, body width, and flagellar length than old T, total length (about $26{\mu}m$), including body length, flagella length, and axostyle length was almost the same in the 2 groups. A striking difference was observed between the axostyles of the 2 groups; the axostyle length of the fresh T ($8.2{\mu}m$) was more than twice as long as that of the old T ($4.0{\mu}m$). However, in several parasitology textbooks, the length of T. vaginalis is said to vary widely from 7 to $32{\mu}m$, and its undulating membrane is said to extend about half way (53.5%) to the posterior end of the body. On the other hand, in our study, the undulating membrane was observed to extend more than 3/4 of the body length (72.1%) in old T, whereas in fresh T it could not be measured. Taken together, we suggest that T. vaginalis averages 26 (21-32) ${\mu}m$ in total length, with 9.5 (7.4-11.4) ${\mu}m$ of body length and 6.8 (5.3-7.7) ${\mu}m$ of width, and its undulating membrane extending 3/4 of its body length. Therefore, these findings may provide useful information for morphological characteristics of T. vaginalis.
