Investigation of Near Infrared Radiation Based Screening for Video-Fluoroscopy Swallowing Studies (비디오투시연하검사 스크리닝을 위한 근적외선 기술 조사)

  • With the recent advances in radiological science, there was radiographic techniques development and several researches to diagnosing dysphagia. We proposed the new Imaging technology based on Near Infrared radiation (NIR) for video fluoroscopic swallowing study (VFSS). To reduce the risk of the VFSS examination for swallowing rehabilitation, multi-NIR camera system comprised. Based on the multi-NIR camera imaging system, Computational simulation was conducted to identify the potential of the multi-NIR camera imaging system as a clinical tool (screening system). As a result of the simulation applied in this study, the proposed system has a potential to be a clinical solution although there is a few of limitations. we believe that it will be a good tool to support the VFSS as a screening technology in clinical fields.

Antioxidative Activity and Chemical Characteristics from Different Organs of Small Black Soybean (Yak-Kong) Grown in the Area of Jungsun (정선산 쥐눈이콩의 부위별 이화학적 특성 및 항산화 효과)

  • Nutritional composition, contents of phenolic compounds, and antioxidative activities of small black soybean were investigated. Proximate composition of the soybean was crude fat, crude protein, carbohydrate, and crude ash at 15.9, 34.7, 38.5, and 3.7%, respectively. Potassium was the predominant mineral, followed by phosphorus, calcium, and magnesium. Glucose and galactose were the major sugars. Twelve different kinds of fatty acids were identified in whole soybean, dehulled soybean, and soybean hull. The 75% methanol and ethyl acetate extract fractions from the soybean hull contained total phenolic compounds at 4.64 and 13.53 g/100 g, respectively. The methanol extract of soybean hull showed strongest antioxidative activity among samples tested. Ethyl acetate fraction of the soybean hull exhibited DPPH radical scavenging activity at level similar to those of ${\alpha}-tocopherol$ and BHT. Phenolic compounds were the major biological components in the soybean hull. These results suggest that small black soybean can be used as a new material for functional food.

Development of a Payment System for Telemedicine (원격진료 보수지불체계 설정방향에 관한 연구)

  • In Korea, telemedicine is still under the beginning stage, but we expect that the developing 'Information Highway' will make this technology more common place and more easily used in coming soon. Currently, three hospitals are providing telemedicine services with their subsidiary hospitals which are far away from their remote place. However, the fee schedule of telemedicine services are not well-settled down, of course not reimbursed through current health insurance system. This study aims to develop new payment system for medical services provided through telemedicaine system. To design appropriate fee schedule for telemedicine services, we, first, review the current insurance payment system and telemedicine system both in domestic and foreign countries focusing on its payment system. A framework of telemedicine payment system is proposed in following steps based on information we acquired from this stage. Second. We decide the span of cost items which should be covered by telemedicine payment scheme. In hear, we suggest payment method for telemedicine services should be designed as dual structure which are telemedicine fee that should be reimbursed through payment scheme and any costs related to capital investment that should not be covered by payment system. Which is, payment system for telemedicine services should cover only service-related costs and any costs related to capital investment should be generated through third party such as government, health insurance association, etc. Finally, we suggest new fee schedules for telemedicine services. The key issues on developing telemedicine fee schedules are related with the determination of appropriate additional rate($\alpha$). The reasonable additional rate($\alpha$) must determine through careful evaluation of any additional efforts(e. g. : additional work hours which are related to providing telemedicine services). This study shows the process of how to determine appropriate additional rate($\alpha$).

  • Respiratory viruses (RVs) cause infections in hospital environments through direct contact with infected visitors. In infection control, it causes major problems of acquired infections in hospitals by respiratory viruses. The surveillance data derived from clinical laboratories are often used to properly allocate medical resources to hospitals and communities for treatment, consumables, and diagnostic product purchases in the institutions and public health sectors that provide health care. An early diagnosis is essential in infection with respiratory viruses, and methods that can be used in diagnostic methods using respiratory samples include virus culture, molecular diagnosis, and analysis. A microchip provides a new strategy for developing a more diverse and powerful technology called point-of-care testing. The importance of the respiratory system should be applied strictly to the infection control guidelines to ensure the occupational health and safety of health care workers. Evidence of clinical efficacy, including this study, is challenging the long-standing paradigm for infection propagation. Additional assistance will be needed for frequent tests to detect respiratory viruses in inpatients who have begun to show new respiratory symptoms indicating infections requiring efforts to control the infection.

Data anomaly detection for structural health monitoring of bridges using shapelet transform

  • With the wider availability of sensor technology through easily affordable sensor devices, several Structural Health Monitoring (SHM) systems are deployed to monitor vital civil infrastructure. The continuous monitoring provides valuable information about the health of the structure that can help provide a decision support system for retrofits and other structural modifications. However, when the sensors are exposed to harsh environmental conditions, the data measured by the SHM systems tend to be affected by multiple anomalies caused by faulty or broken sensors. Given a deluge of high-dimensional data collected continuously over time, research into using machine learning methods to detect anomalies are a topic of great interest to the SHM community. This paper contributes to this effort by proposing a relatively new time series representation named "Shapelet Transform" in combination with a Random Forest classifier to autonomously identify anomalies in SHM data. The shapelet transform is a unique time series representation based solely on the shape of the time series data. Considering the individual characteristics unique to every anomaly, the application of this transform yields a new shape-based feature representation that can be combined with any standard machine learning algorithm to detect anomalous data with no manual intervention. For the present study, the anomaly detection framework consists of three steps: identifying unique shapes from anomalous data, using these shapes to transform the SHM data into a local-shape space and training machine learning algorithms on this transformed data to identify anomalies. The efficacy of this method is demonstrated by the identification of anomalies in acceleration data from an SHM system installed on a long-span bridge in China. The results show that multiple data anomalies in SHM data can be automatically detected with high accuracy using the proposed method.

  • The Golgi apparatus modifies and transports secretory and membrane proteins. In some instances, the production of secretory and membrane proteins exceeds the capacity of the Golgi apparatus, including vesicle trafficking and the post-translational modification of macromolecules. These proteins are not modified or delivered appropriately due to the insufficiency in the Golgi function. These conditions disturb Golgi homeostasis and induce a cellular condition known as Golgi stress, causing cells to activate the 'Golgi stress response,' which is a homeostatic process to increase the capacity of the Golgi based on cellular requirements. Since the Golgi functions are diverse, several response pathways involving TFE3, HSP47, CREB3, proteoglycan, mucin, MAPK/ETS, and PERK regulate the capacity of each Golgi function separately. Understanding the Golgi stress response is crucial for revealing the mechanisms underlying Golgi dynamics and its effect on human health because many signaling molecules are related to diseases, ranging from viral infections to fatal neurodegenerative diseases. Therefore, it is valuable to summarize and investigate the mechanisms underlying Golgi stress response in disease pathogenesis, as they may contribute to developing novel therapeutic strategies. In this review, we investigate the perturbations and stress signaling of the Golgi, as well as the therapeutic potentials of new strategies for treating Golgi stress-associated diseases.

Evaluation of Rare Earths viewed from the Occupational Health (산업보건 측면에서의 희토류 건강영향 평가)

  • Objectives: This study was conducted in order to improve the current understanding of rare earths(RE) and to provide supporting data for establishing occupational health policies by reviewing the toxicological data and issues caused by the use of RE compounds in various fields. Methods: To evaluate the potential toxicity of RE from the viewpoint of occupational health, we summarized extensive reviews of relevant articles in the toxicology(animals and cells), occupational health and safety, and epidemiologic literature. Results: Although occupational RE exposure occurs extensively from ore mining and refining to end users in various industrial applications, epidemiologic study has not been performed among workers up to now. Bioaccumulation and adverse effects of RE have also been mentioned in ore mining regions and nearby residences, but safety standards for each process are insufficient. Moreover, because new commercial recycling technology will soon be applied to various industries, regulation and policies are needed for preventing abuse of recycling. In the results of animal toxicity for a few REs(mostly cerium, lanthanum, and gadolinium), toxicities of liver, lung, blood, and the nervous system were identified due to oxidative stress, but study of long-term RE exposure is required. Understanding the dual effect for RE and discovery of biomarkers pose a scientific challenge in further mechanism studies. Conclusions: In the future, additional hazard evaluation based on animal experiments is required, alongside continuous research for developing analytical methods and discovering biomarkers. Finally, RE occupational health and safety management needs to be integrated into the sustainable use of these materials.

  • In addition to the rapid development of health information technology services for the development of new medical information, a lot of research is underway. Improve health care services for patients are many ways to help them. However, no information about the security, if only the technology advances in health care systems will create an element of risk and threat. Today's issues and access issues are stable over a public network. Ad hocsensor network using secure, non-integrated health information system's security vulnerabilities does not solve the security vulnerabilities. In the development and utilization of health information systems to be subject to greater restrictions. Different security policies in an environment with a medical information system security policy mechanism that can be resolved if people get here are needed. Context-aware and flexible policy of integration and confidential medical information through the resistance should be guaranteed. Other cross-domain access control policy for telecommunications should be protected. In this paper, that the caller's medical information system, diversification, diversification Security agent in the environment, architecture, design, plan, role-based security system are proposed. The proposed system architecture, design work in the field and in the utilization of one model are expected to be.

Pain-related Prescribing Patterns and Associated Factor in Breast Cancer Patients (유방암 환자의 통증 관련 약물 현황과 통증에 미치는 요인)

  • Background: With an increase in the number of breast cancer survivors, greater importance is attached to health-related quality of life, particularly pain and symptom control. This study aimed to identify the factors that are associated with pain in cancer patients based on the patterns of prescribing opioid, non-opioid, and adjuvant analgesics. Methods: This analysis included new patients who had developed breast cancer between 2003 and 2012. The degree of pain was analyzed based on the socio-demographic (age, income quintile, number of hospitalizations, and duration of disease), indicator (Body Mass Index; BMI, Charlson Comorbidity Index; CCI, Cumulative Analgesic Consumption Score; CACS), operation (mastectomy, lymph node dissection), and therapy (chemotherapy, radiation therapy), as well as complication-related variable (lymphedema). Results: As for the patterns of prescribing analgesics by stages, non-opioid and opioid analgesics constituted 30.7 and 69.3%, respectively. The mean value and variance of CACS were 5.596 and 12.567, respectively. The factors that significantly affected the degree of pain were age (≥50; IRR: 1.848, 95% CI 1.564-2.184, p=0.000), income quintile (IRR: 0.964, 95% CI 0.938-0.991, p=0.008), BMI (≥ 25; IRR: 1.479, 95% CI 1.222-1.795, p=0.000), CCI (≥ 4; IRR: 1.649, 95% CI 1.344-2.036, p=0.000), and lymphedema (yes; IRR: 1.267, 95% CI 1.006-1.610, p=0.047). Conclusions: It is necessary to develop systematic and comprehensive pain control measures to improve the quality of life for breast cancer survivors, especially for those who are 50 years or older, lie in the lower-income quintile, have BMI of ≥25 and CCI score ≥ 4, or have lymphedema.

A Study on the Perception of Health Functional Foods: Focusing on the Age Group of 20s (건강기능식품에 대한 20대 소비자의 인식 유형 연구)

  • This study attempted to examine the subjectivity of consumers of the MZ generation in their 20s on health functional foods, which have recently emerged as hot consumption items. A study was conducted using the Q methodology to examine the characteristics, meanings, and values of the younger generation in the health functional food market represented by red ginseng, vitamins, and probiotics. Using the QUANL analysis program, a total of 37 Q samples and 30 P samples were used in the study, and three types with unique tendencies were found. The first type discovered through this was named "YubiMuhwan(Forewarned, forearmed; 有備無患)", the second type was "Bansinbanui(half confident; 半信半疑)", and the third type was called "Silsagushi(empirical tradition; 實事求是)". Reflecting these attributes, various strategic ideas were presented for the classification of new characteristics and consumers in the health functional food market and promotion of sales.
