Antibiotics for bacterial pneumonia in children (세균성 폐렴과 항생제 선택)

  • Pneumonia remains the leading cause of mortality in children. Diagnosis depends on a combination of factors, including clinical assessment, radiological and laboratory findings. Although Streptococcus pneumoniae remains the most important cause of childhood bacterial pneumonia, the great majority of cases of community-acquired pneumonia (CAP) are of viral etiology. A new, rapid, and inexpensive test that differentiates viral from bacterial pneumonia is needed to decide empiric antibiotic treatment. Antibiotics effective against the expected bacterial pathogens should be instituted where necessary. The role of emerging pathogens and the effect of pneumococcal resistance and heptavalent conjugate pneumococcal vaccines are to be considered in practice. There are reports supporting the valid and highly efficacious use of penicillin as a first-line drug for treating CAP. This review raises the issue of the overuse of unnecessary antibiotics in viral CAPs and the use of second or third-line antibiotics for non-complicated pneumonias in most clinical settings.

  • Milking parlor wastewater contains not only high concentrations of organic compounds, but often animal antibiotics. To discharge the antibiotics to public water area cause problem of antibiotics resistant bacteria. Magnetic separation was applied into improvement of milking parlor wastewater treatment process. A new process, composed of a magnetic activated sludge (MAS) process and a contact oxidation (CO) process, was proposed in this study. This process was evaluated by the simulated milking parlor wastewater (4500 mg/L CODCr and 10 mg/L tetracycline) using a bench scale experimental setup. As a result, the process was able to removed 97% CODCr as well as 94% tetracycline. The MLVSS (mixed liquor volatile suspended solids) concentration of MAS was maintained at 12000 mg/L without excess sludge drawing. This process was considered to be useful as treatment process for milking parlor wastewater in which waste-milk including antibiotics is often discharged.

  • It is certainly a difficult work to find out novel-type "practical" antibiotic from natural sources. In fact, chemically modified antibiotics, such as synthetic penicillins, cephalosporins, derivatives of tetracycline, lincomycin, rifamycin and kanamycin are most widely used in medical practice at present. However, as the possibility of success gained by chemical modification is limited, it would be the buty of our applied microbiologists to offer novel antibiotics to chemist and physician without interruption. In this lecture, some practical problems in new antibiotic research will be discussed.(중략)

  • Three polyene macrolide antibiotics including two new compounds were isolated from the culture mycelia of a Streptomyces species. The structures of these metabollites were determined as elizabethin, a previously reported 28-membered macrolide and two analohs, using combined spectroscopic methods. These compounds exhibited antifungal activity and cytotoxicity against a juman leukemia cell.

  • In order to search for new antibiotics from anaerobic bacteria, a large number of samples from domestic soil were collected and processed by apropriate methods. A potential strain, Streptococcus sp. An-21-1, was found to produce antimicrobial compounds. The Results were as follows; 1. During fermentation, the bacteria grew rapidly up to 20hr, thereafter entered the death phase. The optimal temperature and pH for the bacterial growth were $37^{\circ}C$ and pH 7.0, respectively. 2. Antibiotics were purified from culture broth by solvent extraction, silica gel column chromatography and Sepadex L.H 20 column. 3. Physicochemical properties of Ap-1 and Ap-2 were similar ; Their melting points were between $234-237^{\circ}C$. Color reactions of ninhydrin, 2,7-dichlorofluorescein, 4-dimethylaminobenzaldehyde, Dragendroffs reagent and 20% $H_2SO_4$, were positive. Therefore, we assumed that these antibiotics have amine group, immine group, alkaloid, and lipid components. These were stable to heat. UV spectrophotometry showed two peaks at 210 nm and 260 nm. From above results, we assumed these antibiotics are belong to the peptide antibiotic family.

  • Anaerobic bacteria are suggested to be potential source for new antibiotics. In order to search for antibiotics from domestic origin, we collected 800 soil samples across Korean locations and could isolate as many as 989 anaerobic strains. Among them 10, strains were found to have good producing capacity of antibiotics. An anaerobe was finally selected due to secreting antibiotics having high antimicrobial activity towards multiple resistant microorganism(E coli JM 83) transformed by genetic engineering technique. Its morphological, physiological and biochemical charateristics were investigated, together with antimicrobial spectrum therefrom. On antimicrobial spectrum study, substance secreted from this strain, had no activities to fungus and yeast. The selected strain showed G(+) and coccal shape, on Gram, staining and electron scanning microscopy, respectively. Biochemically this strain utilized glucose, fructose lactose, sucrose, but did not arabinose, cellulose, rhamnose, sorbitol, trehalose, mannitol. Catalase test showed negative property. Optimal growth temperature was $37^{\circ}C$. The results obtained above suggest this strain Streptococcus faecium subspp. and we named it Streptococcus sp. An-21-1.

  • Enoyl-Acyl Carrier Protein Reductase (Fabl) of bacteria is hem as an important target for new antibacterial drugs and plays a determinant role in completing cycles of elongation in type-H fatty acid synthase system. In this study, a fabI gene from Staphylococcus aureus 6538p cloned in pET-l4b vector and FabI protein was over-produced in Escherichaia coli BL2l (DE3). $NH_2$-terminal His-tagged FabI protein was purified by nickel-nitrilotriacetic acid (Ni-NTA) metalaffinity chromatography Purified 6xHis-tagged FabI showed a catalytic activity on tram - 2 - octenoyl - N -acethlcysteamine by utilizing NADPH as a cofactor. For the discovery of new FabI inhibitors from chemical libraries, a target-oriented screening system using a 96-well plate was developed. About 10,000 chemical libraries from Korea Chemical Bank wore tested in this screening system, and 26 chemicals (0.25%) among them showed an inhibitory activity against FabI enzyme. This result showed that a new screening system can be used for the discovery of new FabI inhibitors.

Susceptibility and Resistance of Lactobacilli and Clostridia in the Commercially Available Preparations to Antituberculosis Agents and Antibiotics (시판 유산균및 낙산균제제의 항결핵제및 항생물질에 대한 감수성과 내성)

  • To investigate the susceptibility and resistance in case of simultaneous administrations of Lactobacilli and Clostridia prekparations with antituberculosis agents and antibiotics, five strains of Lactobacilli, i.e., Lactobacillus bulgaricus, L. casei, L. helveticus, L. acidophilus, L. sporogenes, and Clostridium butyricum were isolated from commercially available preparation. Minimal inhibitory concentrations (MIC) of nine antituberculosis agents and 21 antibiotics against these strains were determined. The results shwoed that these strains were very sensitive only to rifampicin among antituberculosis agents, and sensitive to other antibiotics. Since the simultaneous oral administration of rifampicin and other antibkiotics with these sensitive strains is ineffective, development of new resistant mutants is desirable.

  • A Streptomyces sp. isolated from a soil sample collected in Taejeon, Korea has previously been found to produce two new polyene antibiotics. The two new antibiotics were named "16-methyloxazolomycin (antibacterial)" and "tetrin C (antifungal)", and their chemical structures are presented elsewhere [10, 11]. In the current study, chemotaxonomy, numerical taxonomy, and ISP methods were all employed for the taxonomic study. The spore chains were spirales and the spore surface was smooth. The spore mass was a gray series and no melanin pigment was produced. On the basis of the morphological and physiological properties, the microorganism was identified to be Streptomyces erumpens, belonging to the gray series of category IV, as defined by Bergey′s Manual. Tetrin C at the concentration of 20 ${\mu}g$/ml demonstrated a potent in vivo (pot test) preventive effect against rice blast, rice sheath blight, cucumber gray mold, wheat powdery mildew, and barley leaf rust.

Momordica charantia and Allium sativum : Broad Spectrum Antibacterial Activity

  • In the Asian sub-continent Momordica charantia and Allium sativum are extensively used as food and are popular in herbal medicine. The two were screened against 15 pathogens and both exhibited broard spectrum antimicrobial activity. As compared to the standard antibiotics, M. charantia demonstrated broader and higher level of activity against most of the organisms. On the other hand A. sativum showed comparable activity to the standard antibiotics. Both M. Charantia and A. sativum are proposed as non toxic, safe, broad spectrum antibacterial agents.
