A case of primary central nervous system lymphoma diagnosed with cerebrospinal fluid analysis: replacement brain biopsy with cerebrospinal fluid immunohistochemistry and immunoglobulin gene rearrangement

  • Primary central nervous system lymphoma (PCNSL) is a type of non-Hodgkin lymphoma confined to the central nervous system. Its diagnosis requires a stereotactic biopsy, which is an invasive procedure. Cerebrospinal fluid (CSF) analysis is less invasive and easier to perform than a stereotactic biopsy. We hereby report a PCNSL case diagnosed using CSF analysis and treated with systemic chemotherapy.

  • The Cyr61/CTGF/NOV (CCN) family is dynamically expressed in various tissues, including the nervous system, from the prenatal period to adulthood. However, major studies have been conducted only in limited fields, such as the cardiovascular and muscular systems, skeletal development, and cancer. In addition, although the CCN family is a secretory protein, very few studies have described its mechanism of secretion. Recently, it has been suggested that overexpression of CCN3 or intracellular accumulation due to problems in the secretory pathway can inhibit neuronal axonal growth. In this review, we have briefly summarized the structure and characteristics of the CCN family and its related diseases, with particular emphasis on the secretory mechanism and modifiers of the CCN family, newly identified in the nervous system.

한국산(韓國産) 하고초류(夏枯草類)의 약물학적(藥物學的) 연구(硏究)(II) -중추신경(中樞神經) 및 이뇨작용(利尿作用)에 대하여- (Pharmacological Studies on Prunellae Herba and Thesii Herba (II) -On Central Nervous and Diuretic Actions-)

  • This study is an attempt to investigate the effects of the water extracts of the whole plants of Prunella vulgaris (Labiatae) and Thesii chinense (Santalaceae) on the acute toxicity, the activities on central nervous system and the diuretic action. The acute toxicities shown by $LD_{50}$ were estimated to be more than 3000 mg/kg p.o. and 1,000 mg/kg s.c. in the extracts of Prunellae Herba and Thesii Herba, respectively, in mice. The extracts at a dose of 2,000 mg/kg p.o. did not show any activities on central nervous system, i.e, sedative, analgesic, hypothermic and anticonvulsant actions. The urination in rats was increased by 45.5% and 57.6% when 100mg/kg of each of the extracts were given orally. The results obtained revealed that the water extracts possessed weak diuretic actions without any of central nervous system activities. Furthermore, it is considered that the potassium in the extract may play a role in the diuretic action.

  • The development of the central nervous system is a continuous process during the embryonic and fetal periods. For a better understanding of congenital anomalies of central nervous system, three major events of normal development, i.e., neurulation (3 to 4 weeks), brain vesicle formation (4 to 7 weeks) and mantle formation (over 8 weeks) should be kept in mind. The first category of anomalies is neural tube defect. Neural tube defects encompass all the anomalies arise in completion of neurulation. The second category of central nervous system anomalies is disorders of brain vesicle formation. This is anomaly that applies for "the face predicts the brain". Holoprosencephaly covers a spectrum of anomalies of intracranial and midfacial development which result from incomplete development and septation of midline structures within the forebrain or prosencephalon. The last category of central nervous system malformation is disorders involving the process of mantle formation. In the human, neurons are generated in two bursts, the first from 8 to 10 weeks and next from 12 to 14 weeks. By 16 weeks, most of the neurons have been generated and have started their migration into the cortex. Mechanism of migration disorders are multifactorial. Abnormal migration into the cortex, abnormal neurons, faulty neural growth within the cortex, unstable pial-glial border, degeneration of neurons, neural death by exogenous factors are some of the proposed mechanism. Agyria-pachygyria are characterized by a four-layerd cortex. Polymicrogyria is gyri that are too numerous and too small, and is morphologically heterogeneous. Cortical dysplasia is characterized by the presence Q[ abnormal neurons and glia arranged abnormally in focal areas of the cerebral cortex. Neuroglial malformative lesions associated with medically intractable epilepsy are hamartia or hamartoma, focal cortical dysplasia and microdysgenesis.ysgenesis.

젊은 비만 여성의 심박변이도 특성에 대한 연구 (A Study on Characters of Heart Rate Variability in Young Overweight and Obese Woman)

  • Objectives Obesity is the major risk factor of the heart disease and the metabolic disease. The autonomic nervous system is a key contributor in the regulation of energy balance, so the blunted activity may contribute to the maintenance of the obese state. So we evaluated the function of the autonomic nervous system in young overweight and obese women with heart rate variability. Methods The subjects were 26 overweight and obese young women(BMI〉$23kg/m^2$) and 25 lean women who visited Kangnam Kyunghee hospital for obesity management from March 2006 to April 2008. Bioimpedance Analysis (BIA), short-term spectral analysis of heart rate variability (HRV) were performed. Results HRV of obese young group was lower than the lean young group, but there is no statistical significance. BMI had significantly a negative correlation with Low Frequency(LF) of HRV. LF power is mediated by sympathetic nervous system activity. These results indicate a decrease of sympathetic modulation in overweight and obese young women. Conclusions Overweight and obese young women have decreased sympathetic nervous system activity. In clinical practice, an assessement of HRV would be noninvasive and sensitive methods for sympathetic nervous system of young overweight and obese women.

물리치료사에 의해서 실시되는 도수수동 운동이 자율신경계에 미치는 영향 (The Effects of Manual Passive Exercise Performed by Physical Therapists on Autonomic Nervous System)

  • PURPOSE: The purpose of this study was to investigate the effects of ten minutes of manual passive exercise performed by physical therapists on autonomic nervous system. METHODS: Thirty-five healthy adults, both male and female, were chosen as subjects for this study. The autonomic nervous system was measured by heart rate variability (HRV),before and after passive exercise using the following measurements: mean heart rate (mean HRT), low frequency (LF) and high frequency (HF) components, the LF/HF ratio, root mean square of the successive differences (RMSSD), and the HRV index. The exercise was performed on the subjects by a physical therapist with seven years of experience specializing in the nervous system. The exercise was conducted at the mid-range of motion on the upper and lower limbs of the subjects for two minutes and thirty seconds and for a total of ten minutes. RESULTS: There was a significant increase in the LF component. The mean HRT and the LF/HF ratio both increased, but these increases were not significant. The HF component, RMSSD, and HRV index all decreased, but these decreases were not significant. CONCLUSION: In conclusion, mid-range manual passive exercise does not induce stress on the autonomic nervous system. It can safely be performed by a physical therapist.

  • Adaptive lengthening or shortening of the nervous system is essential in order for a part of the body to move without restriction or resistance. Up until now, most treatment concepts have focused on normalizing muscle tone and preventing contractures in muscles and joint, but, as Butler and Gifford have brought to the attention of therapists, when a part of the body moves, nerves are required to move as well, so that the integration of nervous system mobilization in treatment is equally important. As the main function of the nervous system is impulse conduction, it is obvious therefore that adaptive lengthening, both centrally and peripherally, is essential in order to accommodate the enormous variety and extent of body movements and postures used in everyday life impeding news conduction. In present day manual therapy, most physical therapist mobilize the nervous system inadvertently. Probably what may make it more difficult is that, for most physical therapist it is a relatively new tissue to contemplate, at least in biomechanical terms, and basic knowledge of structure is generally less than if joints and muscles. The purpose of this study was to introduce the nervous system mobilization to a physical therapist who is primary a clinician and who has concerned in the areas of biomechanics and pathology looking far answers to the clinical Problems.

대사증후군 환자에게 적용한 12주간의 타이치 운동이 자율신경계 활성도와 대사증후군 위험요인에 미치는 효과에 대한 예비 실험연구 (A Pilot Study Examining the Effects of 12-week Tai chi Exercise on the Activity of Autonomic Nervous System and Risk Factors of Metabolic Syndrome in Patients with Metabolic Syndrome)

  • Purpose: This study was to examine the effects of 12-week Tai Chi exercise on the autonomic nervous system activity and risk factors of metabolic syndrome in patients with metabolic syndrome. Methods: A single group pre-posttest was used. Subjects were 16 participants aged 51~71 years with metabolic syndrome. All of the subjects were met the criteria of the National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III). The Tai Chi exercise consisted of 19 movements from Yang and Sun styles provided twice a week for 3 months. Metabolic syndrome risk factors including waist circumference, blood pressure, glucose, triglyceride, high density lipoprotein cholesterol (HDL-cholesterol) were measured. A heart rate variability device was used to measure the activity of autonomic nervous system. Results: After 12-week Tai Chi exercise, systolic pressure, waist circumference, and HDL-cholesterol improved significantly. Tai Chi exercise had no effect on the autonomic nervous system activity. Conclusion: The 12-week Tai Chi exercise was partially effective in managing risk factors of metabolic syndrome. A larger study with longer period to improve the autonomic nervous system activity in patients with metabolic syndrome is needed.

비강의 삼차신경 화학수용 (Nasal Trigeminal Chemoreception)

  • The sensory function of the trigeminal nerve is to provide tactile, proprioceptive, and nociceptive afference by chemical stimulation. Various physical responses of the trigeminal nerve to stimuli help to defend against harmful substances. Recently, many studies have been conducted on solitary chemoreceptor cells innervated by trigeminal nerve. Most volatile organic compounds stimulate both the olfactory and trigeminal nervous systems. In general, the trigeminal nervous system is less sensitive than the olfactory nervous system. Studies have shown that sensation of the trigeminal nerve by chemical stimulation results in inhibition of olfaction. This indicates that the olfactory and trigeminal nerves interact with each other in the central nervous system. It is important to study various noxious stimuli acting on the trigeminal nerve in modern society where environmental concerns are intensifying.

죽염의 일반약리작용 (General Pharmacology of Bamboo Salt)

  • This study has been carried out to investigate general pharmacological action of bamboo salt (jukyom) in terms of effects on central nervous system and cardiovascular system in experimental animals. After bamboo salt, crude salt or reagent-grade NaCl were orally administered into male ICR mice with dose of 2.0 g/kg, general behavioural syndromes such as body weight and locomotor activity, spontaneous motor activity, pento-barbital-induced sleeping time, muscle incoordination, electroshock-induced convulsion, body temperature and writhing response caused by 0.6% acetic acid solution were observed. Bamboo salt had no influences in these indices for examinition of effect on central nervous system. Additionally, conscious male Sprague Dawley rats fastened overnight won ere treated with bamboo salt, crude salt or reagent-grade NaCl (2.0 g/kg, p.o.) to examine the effect of these salts cardiovascular system. Systolic, median and diastolic food pressure and heart rate were dertemined using tail cuff indirect method. Treatment with Hydralazine (50 mg/kg, p.o) as a positive control produced the decreases in systolic, median and diastolic blood treasure and an increase in heart rate. whereas no changes were observed in bamboo salt, crude salt and reagent-grade NaCl treated groups. These results strongly suggest that bamboo salt may have no effects on general pharmacology of central nervous systems and cardio-vascular systems.
