편평발에서 Low-dye 테이핑이 한발 서기 동안 근활성도 변화에 미치는 영향 (The Effect of Low-dye Taping on Muscle Activity during Single-leg Standing in People with Flatfoot)

  • PURPOSE: This study was performed to investigate the effect of Low-dye Taping on muscle activity during single-leg standing in subjects with flexible flatfoot. METHODS: Thirteen able-body volunteers who had flexible flatfoot were recruited for this study. Subjects were measured navicular drop test to evaluate pronation of foot and muscle activity during single-leg standing before and after taping. The muscle activity was recorded using surface EMG from the tibialis anterior and the peroneus longus during single-leg standing on stable and unstable surface. RESULTS: The results show that the navicular drop height and the tibialis anterior muscle activity were significantly decreased after Low-dye taping. CONCLUSION: The results suggest that Low-dye taping could be useful in managing overuse of the tibialis anterior by reducing their level of activation during single-leg standing.

라이프케어 증진을 위한 발내재근 훈련과 발배뼈 가동술이 유연성 편평발에 미치는 효과 (Impact of Intrinsinc Foot Muscle Training and Navicular Mobilization on Flexible Flatfeet to Improve Life-care)

  • 본연구의 목적은 발내재근 훈련과 새로운 접근 방법인 발배뼈 가동술 중 유연성 편평발에 효과적인 중재방법을 알아보기 위해 연구를 진행하였다. 32명의 대상자를 발내재근 훈련과 발배뼈 가동술군 두 그룹으로 설정 연구를 진행하였다. 연구결과 발배뼈 높이 검사에서 두군 모두 유의한 샹항을 보였고(p<.01), 발내재근 훈련군이 발배뼈가동술군보다 유의한 효과를 보였다(p<.01, 95% CI: .768-4.607). 족저압 압력분포의 변화에서도 두군 모두 유의한 감소를 보였으며(p<.01), 발내재근 훈련군이 발배뼈 가동술보다 유의한 감소를 보였다(p<.01, 95% CI: 3.404-14.90 8). 본 연구결과 유연성 편평발에 발배뼈 가동술보다 발내재근 훈련이 더욱 효과적이었다. 편평발을 경험하고 있는 환자들에게 더욱더 효과적인 중재방법을 제공할 수 있을 것이며, 나아가 편평발로 인한 2차적인 근골격계 질환 또한 예방할 수 있을 것이다.

과도하게 엎침된 발을 가진 뇌졸중 환자에게 적용된 수정 발바닥활 지지 테이핑의 효과 (Effects of Modified Low-Dye Taping on Stroke Patients with an Excessive Pronated Foot)

  • PURPOSE: Stroke patients may develop an over-pronated foot, resulting in a lower medial longitudinal arch. This can lead to a structural change of the foot due to deformation of the musculoskeletal system. The purpose of this study was to examine the effects of modified low-dye taping on stroke patients with an excessively pronated foot. The effect of the taping on the foot after light daily activity was also examined. METHODS: The subjects consisted of 21 stroke patients with an excessively pronated foot, as measured by the navicular drop test. First, their navicular heights were measured at a relaxed standing position (measure 1) and while standing in the subtalar neutral position (measure 2). Modified low-dye taping was applied to each subject's affected foot and the navicular height was then measured for the standing posture (measure 3). Finally, each subject walked around for 10 minutes and the navicular height was measured again (measure 4). RESULTS: Statistical analyses showed that the navicular height value at the relaxed standing position (measure 1) was significantly lower than for the other 3 measurements. That is, the modified low-dye taping was effective in maintaining a subtalar neutral position, even after a 10-minute walk, for stroke patients with an excessively pronated foot. CONCLUSION: The results suggest that modified low-dye taping applied to stroke patients with an excessively pronated foot could be an effective way to place the subtalar joint in a neutral position, and that its effect can be sustained for light daily activities.

만성 발목 불안정성(CAI) 환자와 건강 대조군 간 종합적 발목관절복합체의 생체역학적 특성 (Biomechanical Characteristics of Comprehensive Ankle Joint Complex between Chronic Ankle Instability (CAI) and Healthy Control)

  • Objective: To investigate the static and dynamic analysis of ankle joint complex between subjects with chronic ankle instability (CAI) and healthy controls. Method: A total of 38 subjects and CAI group (N=19) and healthy control (N=19) participated in this first study. Variables that were measured in this study were as follows: 1) Subtalar joint axis inclination and deviation 2) Rearfoot angle 3) Navicular drop test 4) Heel alignment view in alignment analysis. Intra Correlation Coefficient (ICC) is used for reliability. A secondary 17 subjects are recruited including 9 of CAI and healthy for gait analysis between group. Lower extremity sagittal, frontal, and transverse kinematics were measured. All data were analyzed to ensemble curve analysis. Results: 1) There were statistically significant differences in standing rearfoot, navicular drop, heel alignment view, subtalar joint (STJ) inclination and deviation. 2) Only in sagittal, meaningful difference is showed during walking in gait analysis. Conclusion: Morphological problem can affect ankle sprain in aspect of structure with no relation to compensation of neuromuscular.

Preliminary Study on the Comparison of Calcaneus Taping and Arch Taping Methods for Flexible Flatfoot Subjects

  • Background: The flexible flatfoot is characterized by a flattening of the foot arch due to excessive bodyweight. The use of shoe insoles or taping methods has been identified as effective in realigning the navicular or calcaneus bones and addressing supination in pronated feet. Objects: This study aimed to analyze the difference between the arch taping attachment method, introduced in a previous study, and a novel taping method designed to provide support to the inner aspect of the heel bone in cases of flexible flatfoot. Methods: A navicular drop test was performed to discriminate flexible flatfoot. To analyze the differences in pressure distribution during walking for each taping method, the subjects underwent testing in the barefoot state with no attachments. The procedure included a sequence of arch taping and heel taping. Subsequent analysis of pressure distribution during walking utilized the GaitRite® system (GAITRite Gold, CIR Systems Inc.). Results: Arch taping and calcaneus taping significantly reduced the integrated pressure over time and peak pressure on the medial side of the midfoot for both feet compared to the barefoot state. Conclusion: The findings of this study suggest that supporting the inside of the heel through calcaneus taping, without direct stimulation to the longitudinal arch and navicular bone, is an effective intervention for flexible flatfoot.

시각적 피드백을 병행한 복합운동프로그램이 편평발 환자의 발배뼈 높이, 족저압 및 다리 정렬에 미치는 영향 (The Effects of Complex Exercise Program with Visual Feedback on Navicular Bone Height, Plantar Pressure and Low Extremity Alignment in Flat-Footed Patients)

  • Purpose : The most effective intervention for flat foot is strengthening exercises for the intrinsic and extrinsic of the foot. Additionally, visual feedback is necessary for movement accuracy. However, the effectiveness of the intervention when combined with visual feedback was not revealed. To confirm this, the research was to investigate the effect of visual feedback and a complex exercise program on navicular bone height, plantar pressure, and lower extremity alignment. Methods : The twenty eight adult men and women with flat foot were randomly assigned to group 1 (n=14) and group2 (n=14), group1 performed complex exercises with visual feedback, and group 2 performed only complex exercises. Both groups performed a 40 minute compound exercise program three times a week. Navicular drop test, plantar pressure test, and lower extremity alignment test were performed equally in both group. Results : As a result of comparing the change in navicular height within the group according to the intervention, both groups showed a significant difference before and after the exercise (p>.05). There was not significant difference comparing the difference between the groups in the navicular height (p>.05). Comparing the change in plantar pressure within groups, there was not significant difference in the change in plantar pressure in both groups (p>.05). Coparing the difference before and after exercise between groups, there was not significant plantar pressure (p>.05). Comparing the change in leg alignment within the group, there was a significant difference in the change in ankle before and after exercise in group 1 (p<.05), but there was not significant difference in group 2. There was not significant difference in pelvic tilt and knee tilt before and after exercise in both groups (p>.05). Comparing the before and after exercise difference between groups, there were not significant in all variables of leg alignment (p>.05). Conclusion : The results of this study showed that complex exercise applied to patients with flat foot were effective in increasing the height of the navicular bone and ankle angle, but there was no effect due to visual feedback.

Comparison of Flatfoot on the Activities of Medial and Lateral Plantar Flexor Muscle During Heel Rising

  • Background: Flat feet can be identified by assessing the collapse of the medial longitudinal arch (MLA) and these conditions can trigger epidemiological changes in the feet. Many of previous studies compared the muscle activity of lower body in terms of intervention and dynamics to treat the structural defect of flat feet. However, few studies have investigated or analyzed the muscle activity of gastrocnemius muscle in the subjects with flat feet. Objects: This study investigated the differences in changes of medial and lateral plantar flexors in subjects with flat feet during bipedal heel-rise (BHR) task and analyzed the differences in muscle activity between two groups by measuring the electromyography (EMG) of abductor hallucis (AH), tibialis anterior (TA), medial gastrocnemius (MG), lateral gastrocnemius (LG). Methods: A total of Twenty one adult females participated in this experiment. Subjects were assigned to groups according to the navicular drop test. The task was applied to the leg, where the heel lifting action prevailed. The muscle activity of the medial and lateral feet plantar flexors was evaluated, and the % maximum voluntary isometric contractions (%MVIC) of these were compared. Results: For the difference between groups the muscle activity (%MVIC) of LG muscle was statistically significantly low in flat feet group compared to healthy feet group (flat feet: 64.57, healthy feet: 90.17; p < 0.05). Conclusion: The results of this study will contribute to identifying the muscle activities of medial and lateral feet plantar flexors among subjects with flat feet, which can cause abnormal epidemiological changes in the feet.

유연성 평발에 적용한 로우다이 테이핑에 의한 균형 수행력의 변화 (The Changes of Balance Performance by Low-dye Taping Application on Flexible Flatfoot)

  • PURPOSE: The purpose of this study was to determine the immediate effect of low-dye taping on balance performance in subjects with flexible flatfoot. METHODS: Fifteen asymptomatic volunteers who had flexible flatfoot (age, $21.7{\pm}1.81$years; height, $164.80{\pm}7.57cm$; weight, $56.47{\pm}10.48kg$) were participated for this study. Navicular drop test was used to evaluate pronation of foot in three different conditions; non-weight bearing position without low-dye taping, weight bearing position without low-dye taping, weight bearing position with low-dye taping. And balance performance (anterio/ posterior, medial/lateral, and overall) was evaluated using the Biodex Balance System in three different conditions; one-leg standing without low-dye taping, one-leg standing with low-dye taping, and one-leg standing with low-dye taping in one week later. Repeated-measures analysis of variance (ANOVA) was used to assess navicular height and balance performance across the three testing conditions. RESULTS: Significant and clinically meaningful improvement in navicular height was found after application of the low-dye taping. However, there was no statistically significant change in balance performance. CONCLUSION: The results of this study provide evidence to suggest that low-dye taping does not affect balance performance.

The Effect of Hallux Valgus Correction Taping on Foot Arch and Static Balance of Young Adult with Hallux Valgus

  • Purpose: The purpose of this study was to identify the effect of hallux valgus (HV) correction taping on the foot arch and balance of young adults with HV of the big toe. Methods: Forty-eight adults volunteered to participate in this study; of them, the 37 feet (11 men, 26 women) with ${\geq}15^{\circ}$ lateral bending of the metatarsophalangeal joint of the big toe, on a goniometer, were selected as the target foot. Non-elastic correction tape was applied to the foot with HV, while the target foot was used to evaluate the changes, before versus after taping, in the lateral bending angle of the big toe, navicular drop level, Clarke angle, and static balance using a Gaitview system. Each measurement was performed three times, and the average of each set was used in the analysis. The patients'general characteristics were compared by using an independent t-test, and the measurement values were analyzed by using a paired t-test. Results: After taping, the lateral bending angle of the toe significantly decreased (p<.05), difference in navicular drop level significantly decreased (p<.05), the Clarke angle significantly increased (p<.05), and static balance significantly improved (p<.05). Conclusion: We showed that HV taping resulted in an immediately improved HV angle, arch, and single-leg standing balance on the foot with HV. However, further studies should investigate the correlation between HV and foot arch.

  • PURPOSE: This study evaluated an intrinsic foot strengthening exercise method for flat feet by comparing the effects of intrinsic exercises on a stable surface and intrinsic exercises on an unstable surface. METHODS: Twenty-four people with flat feet were divided into two groups. The control group performed short foot exercises and toe towel curl exercises on the stable support surface. The experimental group performed short foot exercises and toe towel curl exercises on an unstable support surface using Aerostep. The navicular drop test, and the static and dynamic balance were measured before and after the intervention. RESULTS: In the experimental and control groups, there was a significant difference in the navicular drop test and static balance before and after the intervention (p < .05). On the other hand, there was no significant difference between the two groups (p > .05). In the dynamic balance, the experimental group showed significant differences in all directions after the intervention (p < .05). The control group showed significant differences in the postero-lateral direction and posteromedial direction (p < .05). In particular, in the forward direction, the experimental group had a more significant change than the control group (p < .05). CONCLUSION: Intrinsic foot muscle strengthening exercises performed on unstable surfaces can be an effective clinical exercise method to improve the dynamic balance ability of people with flat feet.
