An Analysis of Clinical Characteristics of Patients with Cough by Food-accumulation and Yin-Deficiency (식적(食積) 및 음허(陰虛)로 인한 해수 환자의 임상적 특성 분석)

  • Cough is a common clinical problem to which various etiologies are attributable. In Korean medicine there are differentiations in etiology such as food-accumulation and Yin-deficiency. This study was aimed to analyze the relations between the time of cough and Korean syndrome differentiations and to compare the symptoms of Korean syndrome differentiations (food-accumulation and Yin-deficiency). Sixty-two cough patients were analyzed and classified into one of two syndrome differentiations by etiology. We compared the time of coughing and symptomatic characteristics of two: such as symptom differences and change of severity after treatment. Patients with food-accumulation were more prevalent than patients with Yin-deficiency. Among symptoms, anorexia, dyspepsia, nausea and nasal discharge were more prevalent in food-accumulation while pruritus of throat was prevalent in Yin-deficiency. Coughing at night was prevalent in Yin-deficiency and coughing at rising hour was prevalent in food-accumulation.

Clinical features of COVID-19 as presented in journal articles : A Korean Medical Approach (COVID-19 임상표현에 대한 한의학적 접근 -국내외 논문을 중심으로-)

  • Objectives : This paper examines major symptoms representation in COVID-19 patients as groundwork for development of an effective clinical data collection format in Korean Medicine. Methods : Major symptoms representation in COVID-19 related papers published worldwide were collected. Corresponding symptoms in Korean Medicine were then examined, followed by discussion of symptomatic features that require further consideration in regards to a more systematic clinical data collection. Results : Of 256 papers, most papers listed fever and cough while symptoms such as difficulty breathing, diarrhea, muscle pain, headache, nausea, fatigue, chest pain, phlegm, nasal discharge were also mostly listed. Clinical representations could be categorized into general symptoms, throat symptoms, chest symptoms, head and facial symptoms, gastrointestinal symptoms, musculo-skeletal and cutaneous symptoms, psychiatric symptoms and sensory problems. Conclusions : Although each clinical representation could be likened to certain clinical representations of Korean Medicine, the variety of symptoms were too limiting and lacking in detail to be applied in the pattern identification[辨證] of Korean Medicine. For effective clinical data collection and analysis in the future, symptom change according to time, comparison between location, climate and ethnicity, existence of interior symptoms when diagnosing exterior symptoms, deficiency-excessiveness of blood patterns, consciousness levels, etc., need to be considered in establishing criteria for symptom evaluation.

Prevalence study of respiratory pathogens in Korean cats using real-time polymerase chain reaction

  • Pathogens such as feline herpesvirus, feline calicivirus, Bordetella bronchiseptica, Chlamydia felis, Mycoplasma felis and Pasteurella multocida usually cause feline upper respiratory tract disease (URTD). Real-time PCR was used to analyze the detection and prevalence of the most common respiratory pathogens in cats with (n=69) and without respiratory signs (n=31). Pathogens were detected in 53 cats, divided into 37 (69.8%) with a single pathogen, 15 (28.3%) with two pathogens, and 1 (1.9%) with three pathogens. M. felis had the highest detection rate in 29 (42.0%) cats, P. multocida was detected in 18 (26.1%), FHV in 10 (14.5%), FCV in 7 (10.1%), B. bronchiseptica in 3 (4.3%), and C. felis in 2 (2.9%). M. felis was the most frequently detected pathogen in cats living outdoors without vaccination. Of the 37 cats infected with single pathogen, nasal discharge was observed in 13 (35.1%), ocular signs in 6 (16.2%), drooling in 5 (13.5%), dyspnea in 3 (8.1%), and asymptomatic in 10 (27.0%). In 51 outdoor and 49 indoor cats, pathogens were detected in 35 (68.6%) and 18 (36.7%) cats, respectively. Of the 29 cats infected with M. felis, 22 (75.9%) showed respiratory signs, and 7 (24.1%) were healthy. In the age of the 53 positive cats, 10 (18.9%) were under the age of 1 year, 26 (49.1%) were aged 1~3 years, and 17 (32.1%) were aged 3 years or older. Although the number of cats in the study was small, the results can provide valuable data on the prevalence of URTD in Korean cats.

Olfactory Dysfunction in COVID-19 from a Korean Medical Perspective (COVID-19 후각 이상에 대한 한의학적 고찰)

  • Objectives : To analyze symptoms of olfactory dysfunction caused by COVID-19 from a Korean Medical Perspective. Methods : Previous studies dealing with olfactory dysfunction accompanying COVID-19 were studied and analyzed for general characterization. Physiology and pathology of olfactory functions within the classical texts of Korean Medicine were collected and analyzed, through which symptoms of olfactory dysfunction in COVID-19 were examined. Results : Olfactory dysfunction manifested in high ratios in the early stages of confirmed COVID-19 cases, at times independent of other nasal symptoms such as blockage or discharge. There was a high chance of loss of taste being accompanied, while mental problems such as a tendency to have difficulty concentrating were present as well. In most cases, recovery took one to two weeks. From a Korean Medical perspective, physiology of olfactory function is closely linked to the Lungs, Ancetral Qi[宗氣], and the Heart, while its dysfunction could be explained by pathological factors such as Wind-Cold, Fire stagnation, Qi deficiency, Wind stroke, etc. Conclusions : In the context of external contraction disease[外感病], olfactory dysfunction could be caused by problems in the Lungs and Stomach that are responsible for breathing, or the Heart which is involved in recognizing and differentiating scent. General characteristics of COVID-19 imply it to be closely related to the Heart. In clinical application, overall symptoms need to be considered in diagnosis and treatment planning, after which further approaches could made to determine the problem to be of the Lung and Stomach, or of the Heart.

Successful Treatment of Feline Nasopharyngeal Lymphoma by Hypofractionated Radiation Therapy After Surgical Debulking in a Cat

  • A 3-year-old spayed female Russian blue cat was presented for dyspnea, nasal discharge, and stertorous breathing. Plain thoracic radiography revealed no specific findings. Computed tomography (CT) was performed to differentiate upper airway tract disorders. It revealed the presence of an iso-attenuating mass measuring 10.0 × 7.9 × 15.6 mm, with mild homogeneous contrast enhancement occupying the rostral nasopharynx. The mass was surgically debulked via a longitudinal incision in the soft palate. Histopathological and immunohistochemistry analysis of the surgically excised mass revealed CD3-/CD79a+ B cell lymphoma with an incomplete margin. The patient underwent hypofractionated radiation therapy, receiving a total of 36 Gray (Gy) in 6 Gy fractions over a six-week period. A follow-up CT examination was performed after 27 months of irradiation and the patient was confirmed to have achieved a complete response. There were no complications related to irradiation. The patient was alive for 40 months without recurrence. This study suggests that hypofractionated radiation therapy combined with surgical debulking could be considered as a treatment option for feline nasopharyngeal lymphoma.

POSTSEDATION EVENTS IN PEDIATRIC PATIENTS SEDATED FOR DENTAL TREATMENT (진정법을 이용한 치과 치료 시 소아 환자의 진정 후 반응에 대한 연구)

  • The purpose of this retrospective study was to examine twenty four hour postsedation events after discharge in children sedated for dental treatment. The sedation sheets of 355 children were selected. Selection criteria excluded no midazolam or nasal route of midazolam, no ASA class I, absent or incomplete questionnaire. Children received orally chloral hydrate and hydroxyzine, nitrous oxide was maintained during the sedation. Midazolam was injected to buccal submucosa with titration. Parents were interviewed via telephone within twenty four hours after sedation. Questionnaire included sleeping response, sleeping time, sleeping behavior, vomiting response at home. Two hundred twenty four (67.8%) children slept and twenty one(6.3%) children had abnormal sleeping behavior. One hundred four(31.0%) children had abnormal behavior after sleep and ten(3.0%) children had vomiting response. In the children took the higher dose of midazolam, there were the great tendency to sleep after discharge (p<0.05), the more negative response to sleeping behavior(p = 0.055), the longer sleeping time(p=0.054). In this study many children slept after sedation and a few children showed negative response. However there were not any considerable emergency events. Further study is required to determine the postsedation safety for the use of submucosal midazolam combination with chloral hydrate and hydroxyzine.

Epidemiological Characterization of Influenza Virus Isolated from Acute Respiratory Illness in Busan, 2004-2005 (최근 2년간 부산지역에서 급성호흡기 환자로부터 분리한 인플루엔자바이러스의 유행 양상)

  • The occurrence of acute respiratory infections caused by the influenza virus are particularly high during the winter season in Busan, Korea. In 2004 and 2005, a study of the rate of occurrences of the influenza virus was conducted. The results reveal that in 2004, of the 1,869 people with an acute respiratory infection that 154 (8.2%) people were infected by the influenza virus. In 2005, of the 1,579 people infected with an acute respiratory infection that 19 people (1.2%) were infected with the influenza virus. The study shows a decrease in the numbers of an influenza virus infection from 2004 to 2005. Data was collected by inspecting throat swabs and nasal discharge from those with an acute respiratory infection. Further inspection of the throat swab and nasal discharge from the infected individuals during 2004 and 2005 study show the occurrence of the different types of influenza virus in the population: 6 cases (3.5%) of influenza type A/H1N1, 129 cases (74.5%) of A/H3N2, and 38 cases (22.0%) of type B. The study conducted in 2004 and 2005 reveal that children between the ages of two and five were more likely to be infected than any other age group. In the study, about 62.2% of the infected individuals were between two and five years old. The detection rates between males and females are similar. However, it is notable that females are slightly more likely to develop an acute respiratory infection caused by the influence virus compared to their male counterparts.

  • The author analyzed 46 cases of allergic rhinitis patients, who were treated in the Kwang-ju Oriental Medical Hospital of Wonkwang University from January 1997 to April 1996. I've examined the 46 cases in the view of age and sex distribution, seasonal distribution, distribution of the job, duration of the disease, past history, family history, distribution of complicated signs, the degree of recovery about the total patient, the relationship of the ages and the remedial value, the relationship of the duration of disease and the remedial value. The following results are obtained. 1. The ratio of under teenagers was $32.61\%$(15 cases), teenagers was $30.43\%$(11 cases), 2th decade was $17.39\%$(8 cases), 3th decade was $17.39\%$(8 cases), 4th decade was $4.35\%$(2 cases), 5th decade was $2.17\%$(1 case), over sixty was $2.17\%$(1 case). 2. The ratio of the male was $58.70\%$(27 cases) and femal was $41.30\%$(19 cases). 3. The ratio of spring was $21.74\%$(10 cases), summer was $8.70\%$(4 cases), fall was $43.48\%$(20 cases), winter was $43.48\%$(20 cases) and unknown was $10.87\%$(5 cases). 4. The ratio of the students was $45.65\%$(21 cases), house-wives was $15.22\%$(7 cases), office men was $10.87\%$(5 cases), farmers was $4.35\%$(2 cases), driver was $2.17\%$(1 case) and unemployed was $21.74\%$(10 cases). 5. The ratio of the under six months was $8.70\%$(4 cases), under 1 year was $17.39\%$(8 cases), under 3 years was $32.60\%$(15 cases), over 3 years was $41.31\%$(19 cases). 6. The ratio of atopic dermatitis was $32.61\%$(15 cases), allergic dermatitis was $21,74\%$(10 cases), the asthma was $17.39\%$(8 cases), digestion disorder was $6.52\%$(3 cases) and etc. 7. The ratio of paternal line was $21.74\%$(10 cases), maternal line was $13.04\%$(6 cases), paternal and maternal lines was $4.35\%$(2 cases), brother or sisters was $13.04\%$(6 cases) and non-significant was $47.83\%$(22 cases). 8. The ratio of sneezing was $100\%$(46 cases) , nasal discharge was $100\%$(46 cases), nasal obstruction was $86.96\%$(40 cases), pruritus was $45.65\%$(21 cases), headache was $13.04\%$(6 cases), asthma was $10.87\%$(5 cases), digestion disorder was $6.52\%$(3 cases), light phovia was $6.52\%$(3 cases), tears was $4.35\%$(2 cases), nosebreeding was $2.17\%$(1 case). 9. The total remedial value of the 46 patients was revealed $52.17\%$. 10. The remedial value of under teenagers, teenagers were higher than the total remedial value($52.17\%$). 11. The remedial value of under 6 months, under 1 year were higher than the total remedial value($52.17\%$). These results demonstrated that in the point of remedial view, the patients who were younger and shorter of the duration of disease, would be well recovered.

Colonization Rate of Methicillin-resistant Staphylococcus aureus in Neonates: A Single Center Experience (단일병원 신생아 환자의 메티실린내성 황색포도알균 보균율)

  • Purpose: The aim of this study is to investigate the colonization rate of Methicillin-resistant Staphylococcus aureus (MRSA) in neonates by different clinical characteristics, to presume the origin of MRSA acquisition, and to identify the risk factors associated with MRSA colonization. Methods: We retrospectively reviewed the medical records of 1,733 neonates admitted to Seoul Eulji hospital Neonatal Intensive Care Unit between January 2008 and December 2011. Nasal, inguinal and rectal swab specimens were obtained upon admission and each week until discharge. We classified the route of MRSA acquisition as; hospital associated (HA-MRSA) and community associated (CA-MRSA) according to the case definition. Results: Among 1,733 neonates, 415 (23.9%) were colonized with MRSA. Gestational age, birth weight, delivery type, maternal antibiotics usage before delivery, birth place and care place before admission were influencing factors in colonization of MRSA. The colonization rate was significantly high in neonates without maternal prophylactic antibiotics use before delivery than in the other group (relative risk 2.77, 95% CI 1.88-4.07; P<0.01), and outborns showed higher MRSA colonization rate compared to inborns (relative risk 2.28, 95% CI 1.17-4.42; P=0.015). Conclusion: We identified the neonatal MRSA colonization rate to be 23.9%. We estimated HA-MRSA colonization rate to be 10% (51/511) and CA-MRSA colonization rate to be 36% (309/858). We ascertained that risk factors in MRSA colonization in neonates were prophylactic use of antibiotics in mothers and the birth place.

Knowledge and Practice of College Entrants Toward Hepatitis B (일부 대학신입생들의 B형 간염에 대한 지식과 실천)

  • In order to designate a present status necessary for establishment of preventive measures and guidelines of health education against hepatitis B in the course of secondary school education, knowledge and practice toward hepatitis B virus infection was surveyed by a questionnare method on total of 4,855 college entrants in the academic year of 1987 and analyzed the data collected using IBM PC(Trigem 88-II) with SAS package program. About two per cent of college entrants had past history of HBV infections not showing any difference between both sexes and geographical regions. About one third(33.7%) of total students had tested hepatitis B surface antigen(HBsAg), only 4% had tested hepatitis B surface antibody(HBsAb) and vaccination rate amounted to 24.6%, one fourth of total subjects. Both serological tests and vaccination were most commonly performed during adolescence, showing higher rates in female students than in male students. The rates also seemed to be higher in those from urban cities than those from rural cities. Students who had acquired correct knowledge that hepatitis B was infected by virus were amounted to 78.5% of college entrants, and remaining 21.5% had misunderstood that rickettsia, bacteria, fungi or parasites were causal agents. Female students were better aware of the causal agents than male students but there was no difference between places of growth. As for mode of transmission of HBV, 51.5% of male students and 47.7% of female students had correct knowledge. A very few student had known that fact that HBV was transmitted by body fluids such as tear(6.9%), nasal discharge(10.1%) and semen or vaginal secretion(19.2%) and majority(75%) of students had misunderstood that hepatitis B virus would be transmitted per os through food ingestion. Approximately one half(48.9%) of college entrants had knew correctly whom to be vaccinated. Approximately one half of the students knew that hepatr;ma(57.8%) and liver cirrhosis(57.4%) might complicate with hepatitis B virus infection, whereas 12.0% of the students responded that bronchitis was one of the complications of hepatitis B infection. In summary of the above results, we highly recommend that health education program for eradication of hepatitis B virus infection should be introduced in curricula of secondary school education in this country.
