Tea plant(Camellia sinensis) is one of a very few plants containing caffeine, a kind of narcotics, originates from Yunnan in China and was utilized as a medicine at the beginning and then as a drink as well as a food. The tea was introduced to Japan from China. Japanese people well developed tea manufacturing technology to produce unique teas such as Matcha(抹茶) and Sencha(煎茶), resulting in creating a culture, Tea ceremony [Chado or Sado(茶道)].
Objective : The purpose of this study is to take around the oriental medical treatment about substance related disorder in china. Method : We review the studies which are published by six different journal in China since 1992 to 2002 involved in substance related disorders. Result 1. The kinds of substance which is the subject of each study. It suggest that the narcotics-withdrawal patients in china take kinds of opium many more than phillopon or barbiturate, cocain etc. especially the heroine takes the most portion in the kinds of opium. 2. The type of chinese medicine demonstration which is about the addiction and withdrawal. There are many symptoms in the each period of withdrawal, According to the each period demonstration, the herbal formula must be different. 3. The formula used in treatment and the substance which is used in common Codonopsis radix is widely used, and pinellia ternata, aractylodes japonica, citrus nobilis, vegetable worms, angelica gigas, zizyphus jujuba, panax ginseng, astragalus membranaceus etc are also used in treatment. 4. The methods of acupunture treatment Hapkok(LI-4), Naegwan(PC6), and Sanyinjiao(SP6) are the widely used acu-points. in addition to these acupoints, there are waegwan(TE5) choksamli(ST36) hanggan(LR2)etc. Conclusion : We expect that this review about substance related disorders in TCM help the clinical study of substance related disorders in Korean medicine.
Recently, continuous epidural infusion of narcotics and local anesthetics have been used for postoperative pain relief. This study was designed to compare the analgesic efficacy and side effects of continuous epidural infusion of narcotics and local anesthetics with those of intramuscular administration of meperidine, for postoperative pain relief after cesarean section. Forty patients were divided into 2 groups of 20 patients each ; Continuous epidural group and control (IM meperidine) group. Before each operation, the epidural group had an epidural catheter placed (L1-2) and following each operation, a bolus of 1%~8ml of lidocaine was injected, followed by continuous infusion of morphine 3 mg/day, fentanyl 300g, 2% mepivacaine 20 ml, 0.5% bupivacaine 20 ml and normal saline 40 ml. The control group received meperidine 50mg IM injection as needed. We evaluated analgesic efficacy with VAS (Visual analogue scale) and side effect at 1, 6, 12, 24, 36 and 48 hour intervals after the operation. The results were as follows: 1) Continuous epidural group was superior to the control group with respect to postoperative analgesia. 2) Side effects (pruritus, nausea & vomiting) were more frequent in the epidural group.
This study was designed to investigate the situation of adolescent drug use in Seoul, and to com-pare this with the 1991 survey in order to better understand the present situation. The subject for this study were 1000 students in High School and Middle School. The data were collected during the period from september 1, 1992 to December 30, 1992. The guestionnaire developed by Kim So Ya Ja (1991) to survey adolescent drug use was used. The data was analyzed using descriptive statistics, 1-test and ANOVA with the SPSS program. The results of this study can be summarized as follows 1. Prevalence of Substance use : Antihistamines were used by 0.3% of adolescent, Sedatives 0.6%, Stimulants 8.1%, Hallucinogens 0.9%, Inhalants 3.2%, Narcotics 1.7%, and Analgetics 154.3%. 2. Trend in substance use compared to the 1991 surbey : Alcohol use increased from 52.8% to 63.7% and narcotics from 0.6% to 1.7%, while Smeking, Analgetics, Antihistamines, Sedatives, and Inhalants showed a decrease. 3. Smoking and Alcohol use : Twenty five percent of school adolescent had experienced cigarette smoking and 63.7% of school adolescent had experienced alcohol use. 4. Motives for drug use : The highest was avoidence of sleep at 49.4% and the next highest was adventure seeking at 27.7%. As to feeling after drug use, 34.3% felt apathy, 22.8% had feelings of sleepiness and unconsciousness. 5. Places were dreg were purchased : The most frequent was the drug store (78.3%) and 84.4% of the respondents answered that drug purchase was easy and 86.7% that drug use was mainly at home. 6. Related Variables : There was a statistically significent high score for drug and alcohol use by adolescents whose fathers used drugs. (PC.05) In conclusion, adolescents in Seoul showed in decrease in the tendency to use drugs compared to the 1991 survey, however drugs which are habit-forming and lead to dependency are still being abused. Therefore, counter-plans and preventive stratiges are important.
Ginseng saponin has been shown to inhibit the development of dependence on morphine, cocaine, methamphetamine, but the antinarcotics effects of ginseng on nalbuphine remains still largely unknown. Ginseng administration attenuated the naloxone-induced jumping behavior on nalbuphine dependent mice. The development of morphine dependence was mediated through ${\mu}-opioid$ receptor, however, development of nalbuphine dependence was mediated through ${\kappa}-opioid$ receptor. However, it was found that the efficacy of analgesic antagonism of GTS was mediated through the serotonergic mechanism, not mediated through the opioid receptor. In addition, ginseng administration modulated cellular signal transduction in the brain. The increased NMDA receptor subunit (NR1, pNR1), phosphate extracellular signal regulated protein kinase (pERK), phosphate cAMP response element binding protein (pCREB) expression by nalbuphine was decreased by the administration of ginseng powder in cortex, hippocampus, striatum of rat brain. These results suggest that ginseng could be one of the targets of antinarcotic therapies to reduce the development of tolerance and dependence on nalbuphine as well as morphine.
Purpose : This study examined the prevalence of delirium-related factors in a surgical intensive care unit (SICU). Methods : This retrospective study enrolled 73 patients who were admitted to an SICU from October 1, 2016 to March 20, 2017 and who had been hospitalized for more than 72 hours. Data was collected by reviewing electronic medical records. Results : Delirium occurred in 46 (63.0%) patients. Its related factors were age, education, mechanical ventilator, sleep, narcotics, physical restraint, and central line catheters. Conclusion : The results indicate that sleep and physical restraint are significant factors related to delirium occurrence. The results of this study can help in developing guidelines for the prevention of delirium.
Long QT syndrome is a cardiac repolarization disorder and is associated with an increased risk of torsades de pointes. The acquired form is most often attributable to administration of specific medications and/or electrolyte imbalance. This review provides insights into the risk for QT prolongation associated with drugs frequently used in the treatment of chronic pain. In the field of pain medicine all the major drug classes (i.e. NSAIDs, opioids, anticonvulsive and antidepressant drugs, cannabinoids, muscle relaxants) contain agents that increase the risk of QT prolongation. Other substances, not used in the treatment of pain, such as proton pump inhibitors, antiemetics, and diuretics are also associated with long QT syndrome. When the possible benefits of therapy outweigh the associated risks, slow dose titration and electrocardiography monitoring are recommended.
The use of epidural narcotics to treat cancer pain was first described by Behar et al in 1979. More recently, a variety of implantable INDSs have been described for long-term intraspinal narcotic administration. Especially, among these systems INDS typeIII which is designed by Poletti et al is relatively low cost and less risk of infection, therefore this system has been widely accepted but the clinical experience is insufficient yet. 1, Problems, 1) thorough education of patients and care-givers about this system the method of drug delivery and the situations could be happen in using this system. 2) high cost of continuous drug delivery system 3) legal problems about morphine carry-out in the case of bolus infusion by syringe 1. Complications; 1) by morphine; Significant respiratory depression was not found in all 21cases. other morphine-related complications were occurred occasionally but improved within a few days by appropriate treatment. 2) by system, Blockage or leakage of catheter was occurred in 2cases and wound infection was occurred in 2cases and so reimplantation was done.
Epidural injection of narcotics for postoperative pain relief has been well reported. Caudal nalbuphine was assessed as a postoperative analgesic in a randomized double blind study of 80 patients after perianal surgery. Caudal block was carried out with 1.5% lidocaine 25 ml (Group 1) in 20 patients, and mixed with nalbuphine 3 mg (Group 2) in 20 patients, nalbuphine 5 mg (Group 3) in 20 patients, and nalbuphine 10 mg (Group 4) in 20 patients. Pain relief was evaluated by the subsquent need for systemic analgesics (Pethidine). In group 4, the use of systemic analgesics was significantly reduced for the first 24 hours postoperatively. Urinary retention was not correlated with nalbuphin dose.
