Cardiac function associated with home ventilator care in Duchenne muscular dystrophy

  • Purpose: Cardiomyopathy is becoming the leading cause of death in patients with Duchenne muscular dystrophy because mechanically assisted lung ventilation and assisted coughing have helped resolve respiratory complications. To clarify cardiopulmonary function, we compared cardiac function between the home ventilator-assisted and non-ventilator-assisted groups. Methods: We retrospectively reviewed patients with Duchenne muscular dystrophy from January 2010 to March 2016 at Gangnam Severance Hospital. Demographic characteristics, pulmonary function, and echocardiography data were investigated. Results: Fifty-four patients with Duchenne muscular dystrophy were divided into 2 groups: home ventilator-assisted and non-ventilator-assisted. The patients in the home ventilator group were older ($16.25{\pm}1.85years$) than those in the nonventilator group ($14.73{\pm}1.36years$) (P=0.001). Height, weight, and body surface area did not differ significantly between groups. The home ventilator group had a lower seated functional vital capacity ($1,038{\pm}620.41mL$) than the nonventilator group ($1,455{\pm}603.2mL$). Mean left ventricular ejection fraction and fractional shortening were greater in the home ventilator group, but the data did not show any statistical difference. The early ventricular filling velocity/late ventricular filling velocity ratio ($1.7{\pm}0.44$) was lower in the home ventilator group than in the nonventilator group ($2.02{\pm}0.62$. The mitral valve annular systolic velocity was higher in the home ventilator group (estimated ${\beta}$, 1.06; standard error, 0.48). Patients with Duchenne muscular dystrophy on a ventilator may have better systolic and diastolic cardiac functions. Conclusion: Noninvasive ventilator assistance can help preserve cardiac function. Therefore, early utilization of noninvasive ventilation or oxygen may positively influence cardiac function in patients with Duchenne muscular dystrophy.

Quantitative Relationship between Body Composition and Muscular Strength Measured with the Biodex System 3 (Biodex System3을 사용한 근력측정과 체조성의 양적조건과의 관련성)

  • This study measured body composition and muscular strength, to examine their relationship, using the Bioelectrical Impedance (IB) method and biodex system3, respectively, in 44 healthy male(20) and female(24) university students. 1. Muscular strength of the upper extremities correlated significantly with fat-free mass(FFM) with $r=.604{\sim}.630$ and intracellular fluid(ICF) with $r=.672{\sim}.668$ in males and FFM with $r=.416{\sim}.552$, ICF with $r=.432{\sim}.564$ and extracellular fluid(ECF) with $r=.429{\sim}.463$ in females. 2. Muscular strength of the lower extremities correlated significantly with FFM with $r=.522{\sim}.785$, ICF with $r=.501{\sim}.739$ and ECF with $r=.498{\sim}.796$ in males and FFM with $r=.642{\sim}.660$, ICF with $r=.627{\sim}.671$ and ECF with $r=.572{\sim}.623$ in females. 3. Muscular strength of the trunk correlated with FFM with $r=.595$, ICF with $r=.627$, ECF with $r=.448$ in males, but did not correlate significantly with body composition in females. These results suggest that total body water(TBW) and ICF may be factors directly associated with muscular strength as well as physical fitness.

Relationship between Exercising Activity, Muscle Mass, Muscular Strength and Cognitive Function in the Elderly (노인의 운동참여, 근육량, 근력 및 인지기능과의 관련성)

  • The purpose of this study was to investigate the relationship among Exercising Activity, muscle mass, Muscular strength and cognitive function of 274 elderly people aged 65 years and older living in Seoul and Gyeonggi-do As for the data collection methods, were measured the Exercising Activity, muscle mass and muscular strength and the Korean version of Mini Mental State Examination(MMSE-K) and digit span test were measured. The results are as follows First, there was a statistically significant correlation between Exercising activity, muscle mass, muscular strength, and cognitive function of the elderly. Second, Exercising activity of the elderly showed a positive (+) influence on muscle mass, muscular strength, and cognitive function. Third, the muscle mass of the elderly showed a statistically significant (+) influence on muscular strength and cognitive function. Fourth, the strength of the elderly showed a positive (+) influence on cognitive function.

Evaluation of Plantarflexion Torque of the Ankle-Foot Orthosis Using the Artificial Pneumatic Muscle (인공공압근육 엑츄에이터를 이용한 족관절 보조기의 족저굴곡 토크 평가)

  • Ankle-foot orthosis with an artificial pneumatic muscle which is intended for the assistance of plantarfelxion torque was developed. In this study, power pattern of the device in the various pneumatics and the effectiveness of the system were investigated. The pneumatic power was provided by ankle-foot orthosis controlled by user‘s physiological signal, that is, muscular stiffness in soleus muscle. This pneumatic power can assist plantarflexion torque of ankle joint. The subjects performed maximal voluntary isokinetic plantarflexion motion on a biodexdynamometer in different pneumatics, and they completed three conditions: 1) without wearing the orthosis, 2) wearing the orthosis with artificial muscles turned off, 3) wearing the orthosis activated under muscular stiffness control. Through these experiments, we confirmed the effectiveness of the orthosis and muscular stiffness control using the analyzing isokinetic plantarflexion torque. The experimental results showed that isokinetic torques of plantarflexion motion of the ankle joints gradually increased in incremental pneumatic. The effectiveness of the orthosis was -7.26% and the effectiveness of the muscular stiffness control was 17.83% in normalized isokinetic plantarflexion torque. Subjects generated the less isokinetic torques of the ankle joints in wearing the orthosis with artificial muscles turned off, but isokinetic torques were appropriately reinforced in condition of wearing the orthosis activated under muscular stiffness control(17.83%) compared to wearing the orthosis(-7.26%). Therefore, we respect that developed powered orthosis is applied in the elderly that has weak muscular power as the rehabilitation equipment.

The Effect of Muscular strength, Sensation, and Spasticity of the Hip Joint on Balance in Chronic Stroke Patients (엉덩관절의 근력, 감각과 경직이 만성 뇌졸중 환자의 균형에 미치는 영향)

  • The purpose of this study was to identify the effect of muscular strength, sensation, and spasticity of the hip joint on balance using clinical tools in chronic stroke patients. Thirty-two subjects participated. It were measured muscular strength of the hip flexor, extensor, abductor, and adductor using manual muscle testing, light touch sensibility, and spasticity of the quadriceps using the modified Ashworth scale in the paretic lower extremity. Also, static and dynamic balance were measured by the functional balance scale. Collected data was analyzed by stepwise multiple regression. Muscular strength of the hip abductor, light touch sensibility, and spasticity were associated with static balance(p<0.05) and explanatory power was 80.5%. Muscular strength of the hip abductor and light touch sensibility were associated with dynamic balance(p<0.05) and explanatory power was 77.9%. Thus, it is suggested that muscular strength of the hip abductor and light touch sensibility is considered when providing evaluation and intervention programs for the future in the balance.

Study on the Activity Patterns of Masticatory Muscles according to the Levels of Occlusal Force (교합력 수준에 따른 저작근 근활성도에 관한 연구)

  • In order to evaluate muscular activity patterns of masticatory muscles and asymmetry patterns of muscular activity according to the levels of occlusal force, twenty-one individuals of age ranged from 23 to 27 years were examined. They were selected according to the following criteria : 1) no symptoms of temporomandibular disorder, 2) complete dentition except third molars, 3) normal or Angle's class I molar relationship and 4) no experience of dental treatment. The electromyographic amplitudes was measured for evaluation of muscular activity and asymmetric patterns of masseter and anterior temporal muscle during unilateral clenching at the levels of 10%, 20%, 40% and 50% of the maximum occlusal force by use of electromyogram and bite force meter. The obtained results were as follows : 1. The muscle activity indices of masticatory muscles of clenching side at the clenching levels of 10%, 20% and 30% of the maximum occlusal force were -19.12, -9.87, -0.49%, so that activity of anterior temporal muscle was dominant than that of masseter muscle. At the levels of 40%, 50% of the maximum occlusal force, muscle activity indices were 4.68%, 6.70%, so that activity of masseter was dominant at all level and as the levels of occlusal force was increased, muscular activity index was tend to decrease. 2. In masseter, asymmetry indices of muscular activity at the levels of 10%, 20% of maximum occlusal force were -10.34 and -1.24%, so that muscular activity of non-clenching side were dominant and at the levels of 30%, 40% and 50% each of maximum occlusal force, muscular activity was dominant on clenching side as 4.68, 7.18 and 10.9%. In anterior temporal muscle, asymmetry indices were 33.38%, 25.46, 2095, 10.23 and 15.45% at the levels of 10%, 20%, 30%, 40% and 50% each of maximum occlusal force, so that activity of clenching 15.45% at the levels of 10%, 20%, 30%, 40% and 50% each of maximum occlusal force, so that activity of clenching side was dominant than that of non-clenching side at all levels, but as the levels of occlusal force was increased, asymmetry indices of muscular activity was tend to decrease. 3. Between both sides, average electromyographic amplitudes of masseter and anterior temporal muscle were correlated, so that as the levels of occlusal force was increased, average electromyographic amplitudes of both side in same muscle were increased proportionally. But asymmetry indices between muscular activities of masseter and anterior temporal muscle were not correlated.

Development of Jacket Pattern for Muscular Men (근육형 남성용 재킷 패턴설계)

  • Recently, young men have become more muscular as they become more interested in physical figure. However, most of these muscular men have fit problems regarding ready-made clothes. In view of this, this study aimed to develop a prototype jacket pattern for muscular men. For this study, five muscular men were selected to put on existing jacket pattern for wearing tests. The regression formula, in which muscular men body measures were adopted, was applied to unsuitable parts, especially the areas determined not to be appropriate in the evaluation of existing jacket pattern wearing tests. After the first and the second jacket pattern wearing tests, the final jacket pattern suitable for muscular men was developed. The results of the study were as follows: In order to make up for the problem of the loosening of the lapel area, due to the development of the chest muscle, the chest circumference line on the chest area of the pattern was cut to be 1.0cm wide; thus, the front length was modified with an increase. The wearing tests found that a wearers felt discomfort from the tight armhole area, so the armhole depth was set to be a little lower than that of ready-made clothes. A muscular men needs much more extra quantity in this area because the upper part of the back side is projected due to the greater development of the trapezius muscle and the deltoid than in average men. Hence, concerning the standard line for the length of the back interscye, ease of 1.0cm was added to the regression equation formula {(0.371${\times}$chest circumference+3.145)/2} in order to resolve the discomfort with the back area. Also, for the biacromion length, the upper arm protruded more than the shoulder point of the jacket because of the development of the deltoid and the upper arm muscle, and it was set to be wider than the actual shoulder. In order to solve the problem of discomfort from the narrow neck area during the wearing of a jacket owing to the development of the trapezius muscle, extra ease of 0.5cm was added to chest circumference/12-0.5cm in the existing jacket prototype to the width of back of the neck, and it was corrected to be chest circumference/12.

Effects of Different Cool-down Exercise Methods on Muscle Strength and Endurance of the Lower Extremities

  • The purpose of this study was to investigate the effects of various cool-down exercises on muscular strength and endurance. After receiving a treadmill training for main exercise, the subjects conducted isotonic and isometric cool-down exercises four times for three weeks. Isotonic exercise with leg press of 10kg was repeated by 20 times and isometric exercise was conducted at flexion of hip joint and knee joint with leg press of 10kg by maintaining it for 6 sec and resting for 2 sec by 20 times. Muscular strength after exercise was measured with 1 RM by times and muscular endurance with maximum repetition frequency using time to keep for loading the weight of 10 RM and 65% of maximum muscular strength. As a result of comparing and analyzing measured values, exercise recovery shape of isotonic and isometric cool-down group were more effective than rest recovery shape of the control group. The isometric cool-down group was more effective than isotonic cool-down group. In conclusion, isometric exercise was more effective than isotonic exercise or simple rest on muscular strength and endurance.

Studied on Garment Restraint(III) - Relation between Clothing Pressure and Muscular Activity of Foundation - (의복의 구속성에 관한 연구(III) - 화운데이션의 의복압과 근활동과의 관계를 중심으로 -)

  • We studied relation between the clothing pressure applied by foundations(waist nipper, girdle, body suit) on the waist of bodies and muscular activity, feeling of tightness. The main results were summerized as follows ; 1. Clothing pressure applied by foundations was high in order of girdle>waist nipper>body suit, also clothing pressure was higher back than front and side, sitting on the chair than standing posture, ventral flection than repose. Individual differences, even if size of body was equal, were shown in clothing pressure applied by foundations with subcutaneous fat's amount in measuring region. 2. The muscular activity of rectus abdominis than obliquus externus abdominis was more affected by foundations in all kinds of postures and motions. Amplitude of electromyogram was high sitting on the chair than standing posture, but there was little difference with motion variation. The wearing girdle strongly affected on the muscular activities of rectus abdominis and obliquus externus abdominis as compared with waist nipper and body suit. 3. The value for feeling of tightness by wearing waist nipper was higher than girdle and body suit. Also the case when sitting on the chair and ventral flection, the value for feeling of tightness was high. When the foundations were on the body, most tightened on the region of the body was anterior abdominal region.

The effects of kinesio taping of thigh muscle on isokinetic muscular function during closed kinetic chain exercise of lower extremity (하지의 닫힌 사슬 운동시 대퇴부 테이핑 적용이 등속성 근기능에 미치는 영향)

  • Purpose : The purpose of this study was to investigate effects of kinesio taping of thigh muscle on isokinetic muscular function during closed kinetic chain exercise of lower extremity. Methods : Twenty healthy males were participated in this study. The isokinetic muscular function of closed kinetic chain were measured for peak torque, peak torque % body weight, average power, max rep tot work before and after application of kinesio taping on thigh muscles. Results : At $60^{\circ}/s$ peak force and peak force % body weight were significant increased in both extension(p<0.01) and flexion(p<0.001) after applying kinesio taping. At $180^{\circ}/s$ average power was significant increased in both extension and flexion after applying taping(p<0.001). At $240^{\circ}/s$ max rep tot work was significant increased in both extension(p<0.001) and flexion(p<0.05). Conclusion : This study suggests that application of kinesio taping on thigh muscle was more effective to improve isokinetic muscular function in closed kinetic chain exercise of lower extremity.
