Injury of the Medial Rectus Muscle by Using a Microdebrider During Endoscopic Sinus Surgery: A Case Report

  • A microdebrider is increasingly used in endoscopic sinus surgery. Although it has many advantages over conventional instruments, it has been associated with severe complications. We treated a case of rupture of the left medial rectus muscle after use of a microdebrider during endoscopic sinus surgery in a 50 year-old female patient who complained of binocular diplopia and exotropia. The patient showed marked limitation on adduction and about 40 prism diopters of left exodeviation. The orbital computed tomography showed a bony defect at the left medial orbital wall, and injury of the medial rectus muscle. The exodeviation was corrected after ophthalmologic surgery. We report a case of the rupture of the medial rectus muscle after use of a microdebrider during endoscopic sinus surgery and review the medical literature.

Dendropanax morbifera Extract Inhibits Intimal Hyperplasia in Balloon-Injured Rat Carotid Arteries by Modulating Phenotypic Changes in Vascular Smooth Muscle Cells

  • The plant Dendropanax morbifera Léveille is effective folk medicines for the treatment of several conditions, such as infectious diseases, skin diseases, and other illnesses. Although the inhibitory effects of D. morbifera on the proliferation and migration of vascular smooth muscle cells (VSMCs) have been shown in our previous study, its effects in vivo remain to be elucidated. In this study, we aimed to investigate the protective effects of the extracts from D. morbifera (EDM) on neointimal hyperplasia of rat carotid artery and explore the underlying mechanisms. We observed that the ratio of intima to media thickness (I/M) was significantly decreased in the EDM-treated groups by ~80% compared to that of the control. The expression of Ki-67 and proliferating cell nuclear antigen was decreased by ~70% in the EDM-treated groups compared to that of the control. In addition, matrix metalloproteinase (MMP)2 and MMP9 significantly reduced in the neointimal layer of the EDM-treated groups. Moreover, the decreased levels of contractile phenotypic markers of VSMCs, such as α-smooth muscle actin, myocardin, and smooth muscle-myosin heavy chain, were successfully restored by EDM treatment. Furthermore, the levels of synthetic phenotypic markers, cellular retinal binding protein 1 and connexin 43 were also restored to normal levels. These results suggest that EDM inhibits vascular neointimal hyperplasia induced by balloon injury in rats via phenotypic modulation of VSMCs. Therefore, EDM may be a potential drug candidate for the prevention of restenosis.

  • The purpose of this study is to provide basic data for injury prevention and performance improvement by developing a flywheel training program for handball players. The results of analyzing the effects of the 12-week complex flywheel training program on body composition, physical fitness factors, anaerobic power, and isokinetic muscle function are as follows. There was a significant difference between groups in push-ups. Body fat percentage and anaerobic power showed significant differences in the interaction by period. Long jump, slow jump, grip strength, back muscle strength, anaerobic average power, and isokinetic muscle function tests showed significant differences by period in the left and right flexors. Therefore, it is considered that 12-week flywheel training results can be used as the basic data for injury prevention and improvement of performance in handball players.

  • Objectives The purpose of this study is to know the effect of acupuncture on brachial plexus palsy due to birth injury. Methods We decided to treat one week of Vojta therapy and occupational therapy, but other were combined, and then add three weeks of acupuncture treatment as well. Results Right clavicle fracture at birth brachial plexus injury due to decreased locomotion of the right upper extremity, muscle weakness in children aged 7 months to Vojta therapy, occupational therapy and acupuncture, when performed in conjunction mobility increase of the shoulder joint, elbow support improvement, recovery of motor function of the hand grip, etc. to obtain a significant motor function recovery improvement. Conclusion Acupuncture was a good treatment of choice for the better shoulder, joint and hands movements.

  • Objectives : This study is to investigate the effects of oriental medical treatment of traumatic brain injury. Methods : We treated the patient with Herb medicine, acupuncture, moxa, fumigation, rubdown and negative and occupational therapy. And we evaluated the case with Modified Bathel Index, Activity Index, Jebsen Test. Results : There is improvement in his symptom (manual muscle power, finger apraxia, memory loss, dysarthria, urinary frequency), after oriental medical treatment. Conclusions : We report the good result of oriental medical treatment on this case. The more clinical study of oriental medical treatment for traumatic brain injury is needed.

Acute Cervical Spinal Subdural Hematoma Not Related to Head Injury

  • We report an extremely rare case of traumatic cervical spinal subdural hematoma not related to intracranial injury. There has been no report on traumatic cervical spinal subdrual hematoma not related to intracranial injury. A 27-year-old female patient was admitted to our emergency room due to severe neck pain and right arm motor weakness after car collision. On admission, she presented with complete monoplegia and hypoesthesia of right arm. Magnetic resonance imaging (MRI) revealed subdural hematoma compressing spinal cord. Lumbar cerebrospinal fluid (CSF) analysis revealed 210,000 red blood cells/$mm^3$. She was managed conservatively by administrations of steroid pulse therapy and CSF drainage. Her muscle power of right arm improved to a Grade III 16 days after admission. Follow-up MRI taken 16th days after admission revealed almost complete resolution of the hematoma. Here, the authors report a traumatic cervical spinal SDH not associated with intracranial injury.

A Clinical Case Study on Postural Brachial Plexus Injury with Whole Body Articulation-Mechanics Technique (추나요법을 적용한 체위성 상완신경총 손상 치험 1례)

  • This study was performed to report the effect of oriental medical treatment and general biomechanic manipulation on a patient with upper limb palsy caused by postural brachial plexus injury. The patient was treated with acupuncture, herb medicine and general biomechanic manipulation. The effectiveness of treatment was evaluated with range of motion, visual analogue scale and manual muscle test. After 11 times treatment, motion and muscular force were progressed, pain was decreased. This result suggests that oriental medical treatment and general coordinative manipulation are effective to care the postural brachial plexus injury.

Effects of Red Ginseng Intake on Muscle Injury Due to Eccentric Exercise

  • This study examined the effects of Korean red ginseng (RG) on muscle injury induced by eccentric exercise. Eighteen college male students were randomly assigned to either the RG group or the placebo group (P). The RG group ingested RG extract at 20 g/day (mixed with 200 mL of water) for 7 days prior to performing a downhill treadmill exercise and for 3 days after performing the downhill treadmill exercise, while the P group ingested 200 mL of water containing Agastachis Herba for 7 days prior to and 3 days following a downhill treadmill exercise. All subjects performed 2 bouts of a downhill treadmill exercise (6 km, -14 degree slope, 12 km/hr speed) with a 5 minute resting interval between bouts. Blood samples were drawn immediately before and after exercise, and at 1 hr, 24 hr, 48 hr, and 72 hr. Two-way repeated ANOVA documented that creatine kinase (CK) levels in the RG group were significantly reduced at 48 hr after exercise, as compared with P group, and remained constant until 72 hr after exercise. Lactate levels in the RG group were also significantly lower at 24 hr and 72 hr after exercise as compared to the P group. We conclude that supplementation of RGreduces CK levels and may prevent muscle injury induced by eccentric exercise.

  • This study has purpose to perform leaving the hospital nursing plan during hospital treatment of patient with the knee ligament Injury on the basis of the concerns between the time of leaving the hospital after 2 weeks in hospital and the concerns through practical life after leaving the hospital systematically. This study is also tried to make exercise plan to prevent atrophy of Quardriceps muscle of the thigh caused by the limitation of activities. The subjects of study are 12 people who were in the hospital in affiliation to a university in Seoul, were operated on the knee ligament, and were discharged from the hospital within 2 weeks. Data collection time was for about 4 months from December, 94 to March, 95. Research method was performed 2 times by unorganized open questions. The collection of first data is through direct interview about the concerns before leaving the hospital, and secondary data is through direct interview about the concerns by practical life and experiences after leaving the hospital. The data was categorized by classifying several common elements among similar contents according to data by the situational analysis of Giorgi. The result of this study is as follows : 1) The concerns before leaving the hospital are like that the concerns of recovery possibility, the limitation of activities, the grim realities of life, the lack of confidence about self-nursing, the eyes of the surroundings, the burden to the others, and so on. 2) The concerns after leaving the hospital are like that the demands of the inconvenience by the limitation of self-nursing, the support about self-nursing by the difficulties of performing treatment order, the support of physical activities limitation by physical tiredness and the limitation of social activities, the support of information by the necessities of education in using crutches before leaving the hospital and continuing informational needs, psychological and mental support of normal physical recovery by the worries of physical recovery and physical change, and social support by thankfulness for his family and relatives. I suggest to make more systematic leaving the hospital nursing plan and exercise education plan for nursing plan after leaving the hospital of the patients with the knee ligament injury on the basis of this content.

  • The aim of this study was to investigate the effect of combine exercise on the fall injury related index and myokine in elderly women. Sarcopenia leads to a loss of strength, alter on to a decreased functional status, impaired mobility, a higher risk of falls, and eventually an increased risk of mortality. Aerobic exercise characterized by rhythmic and repetitive movements of large muscles, for sustained periods that depends primarily on the use of oxygen to meet energy demands through aerobic metabolism, and that is structured and intended to generate improvements in cardiopulmonary fitness, body composition, and cardiorespiratory health. Resistance training has performance in the elderly. As combined exercise therapy can be used to enhance muscle functions and cardiopulmonary functions, it is being highlighted as an effective health management methods for the aged. The myokine has been regarded an important factor of exercise how muscle communicate adipose tissue, bone and muscle to exert beneficial effects at the whole body level.
