Measurement of Nursing Service Quality using SERVQUAL Model (SERVQUAL 모델을 이용한 간호 서비스 질 측정)

  • This study is a descriptive analytic research measuring nursing service quality, using SERVQUAL model, to make fundamental data and strategies for nursing service improvement. Data were collected by self-reported questionnaire from 202 patients and 142 nurses, from June 7 to 14, 1999. The reliability of instrument were adequate(Cronbach ${\alpha}=.94$). SAS program was utilized for statistical analysis of collected data. The results were as follows; 1. There was a gab between patient's expectation and perception on nursing service(Gap B). Gap D was indicated an affecting factor to decide nursing service quality. Gap C was indicated an indirect affecting factor of nursing service quality. Because it was not statistically significant in total item analysis, but in individual item analysis, 7 items were appeared statistically significant. Gap A was not a gap occurrence factor of nursing service quality. 2. Focuses of nursing service quality improvement strategies were; (1) to direct qualitative improvement of nursing service in order to correspondence patient's nursing service expectation. (2) to make nurse's service activity modified because nurse's practice were not reached patient's expectation level. (3) to need internal, external factor analysis affecting nurse's service activity. 3. Nursing service quality was decided by rather environmental inappropriateness provided nursing service than itself. Therefore, to make nursing service quality improvement, it is required to improve nursing service environment. For this, followings are required; (1) to strengthen nurse's education on lower part of nursing service satisfaction and QI activities. (2) to balance demand and supply of nursing personnel. 3) to fix computerized system for reducing other duties weight except nursing care through analysis of nursing activity. (4) to construct rational cooperating system among related departments. 4. The important parts for nursing service quality improvement were indicated as follows: (1) Gap B: 'prompt reaction', 'examination symptom before patient's complaint', 'hearted nursing service reducing patient's dissatisfaction', 'explanation goals of nursing activities', 'having special Knowledge enough', 'maintenance position comfortably', 'management of patient's physical hygiene'. (2) Gap C: 'maintenance physical safety', 'explanation about hospital rules and facilities'. (3) Gap D: 'tender, safe injection and wound care'. Because above items are mostly improved through nurse's attitude change and quality improvement, it is required to establish nursing standardization and to strengthen nurse's clinical education. As the based on above results, followings are suggested; 1. SERVQUAL model is very useful to make strategies for nursing service quality improvement because it indicates multiple factors affecting hap occurrence. 2. At individual items analysis of Gap C, statistically significant 7 items appeared higher nurse's perception level than patient's perception level on nursing service were trouble perception level on nursing service quality improvement. So. it need further research to analysis about these difference occurring factors. 3. At analysis of Gap D, it is indicated that in nursing service performance process, multiple factors lowing nursing service quality were intruded. So it needs further research to analysis what these factors are and how each factors affect on nursing performance process. 4. nursing service quality measurement is changeable according to sample select time or sampled subject's characteristics. So to develope strategy for nursing service quality improvement is based on the results of periodical analysis.

On Determinants of Physician Utilization -A causal analysis- (의료이용(醫療利用)에 영향(影響)을 미치는 요인(要因)에 관(關)한 분석(分析))

  • This study seeks to provide a framework for understanding differential access to medical care. The framework is provided by Anderson Model, a model of health services utilization which suggests a sequence of predisposing, enabling, illness-morbidity characteristics that determine the number of times people will visit a physician. The framework in this study is composed of two models, one is for Adults and the other is for Non-Adults. Models are operationalized using stepwise multiple regression analysis and path analysis. The data come from a national health survey conducted in 1983. The findings of the analysis can be summarized as follows : First, the causal models used in this study are able to explain only a small amount of the variance in medical care utilization(Adjusted $R^2$ is .144 in the Model for Adults and .243 in that for Non-Adults). This finding suggests that we reconsider the utility of such existing model using the predisposing, enabling, and illness-morbidity characteristics in light of their poor correspondence with these data. Second, while small amount of the valiance in medical care utilization is explained, most of the explained variance is due to the illness-morbidity characteristics. The path coefficients of study variables except illness-morbidity variables show these characteristics to be substantially unrelated to medical care utilization, and the indirect effects of the predisposing and edabling characteristics on medical care utilization are also negligible. This casts doubt on the importance of the predisposing and enabling characteristics in explaining medical care utilization. Third, among the predisposing and enabling characteristics, Medical Security variable is the only one having significant direct effect on medical care utilization in both models for Adults and for Non-Adults. Fourth, the amount of the variance explained in the Model for Non-Adults is more than in the Model for Adults. This suggests that medical care utilization of adults is more influenced by behavioral factors than that of children.

A Study on the Traffic Accident Offenders' PTSD Occurrence and Analysis of the Changing Mechanism of Driving Attitudes (교통사고 야기자의 외상 후 스트레스 장애 발생과 운전태도 변화 메커니즘 분석)

  • This study has been conducted to provide the basic materials about the occurrence structures, causes and choice of treatments of the traffic accident offenders' PTSD(Posttraumatic Stress Disorder). The results of this study can be summarized as follows. First, accident offenders', not victims', PTSD conditions, which has been only estimated until now, are measured by nationwide questioning survey. Second, accident offenders' changes of driving attitude after accidents could be typed by factors analysis in the 20 driving situations closely connected to the occurrence of traffic accidents. Third, by the t-test, the difference of driving attitudes in primary driving situations according to existence of PTSD is distinguished. Six driving situations, which are found significant by t-test, are applied to structural equation. So the variables related to PTSD and the models which analyze the attitude changes of driving are constructed. This study provides the basic materials to help the reduction and prevention of the occurrence of PTSD.

  • This research analyzed. knowledge structure and its effect factor by analysis of co-author and keyword network in Korea's health policy and administration sector. The data was extracted from 339 articles listed in the Korean Journal of Health Policy and Administration, and was transformed into a co-author and keyword matrix. In this matrix the existence of a link was defined by impact factors which were calculated by the weight value of what the role was and the rate of how many authors contributed. We demonstrated that the research achievement was dependent on the author's status and network index. Analysis methods were neighborhood degree, correspondence analysis, multiple regression and the difference of weight distribution by research fields. Co-author networks were developed as closeness centrality as well as degree centrality by a few high productivity researchers. In particular, power law distribution was discovered in impact factor and research productivity. The effect of the author's role was significant in both the impact factor calculated by the participatory rate and the number of listed articles. Especially, this journal shared its major researchers who had a licensed physician with the Journal of Preventive Medicine and Public Health. Therefore, social scientists were likely to be small co-author network differently from natural scientists. It was so called 'two cultures' phenomenon. This study showed how can we verified academic research structure existed in the unit of journal like as citation networks. The co-author networks in the field of health policy and administration had more differentiated and clustered than preventive medicine and epidemiology fields.

A Study on Visitor Characteristics for Trail Program Development (숲길프로그램 개발을 위한 이용객 특성 분석에 관한 연구)

  • Trail is a main infrastructure for various forest recreational activities. In this study, the recreational supply (trail) and demand (trail program according to visitor characteristics, preferred forest activities) were studied. Through the application of a multiple correspondence analysis and cross tabulation analysis, 3 trails (Bukhansan trail, Uljin trail, Jirisan trail) were identified. On this study, visitor demands for trail program participation, preferred forest recreational activities, program contents and program operations. Visitors expressed positive attitude toward trail program participating and showed characteristics with the differences. Young visitors preferred active experience program and middle aged visitors more liked health program such as forest therapy. On site program and self guided interpretation were preferred for program operation. In Bukhansan trail, continuous health program (1~2 hours) would be proper and it needs to adapt various nature activities in the Uljin trail. For the Jirisan trail, self guided program would be developed in many ways.

A Development of Summer Seasonal Rainfall and Extreme Rainfall Outlook Using Bayesian Beta Model and Climate Information (기상인자 및 Bayesian Beta 모형을 이용한 여름철 계절강수량 및 지속시간별 극치 강수량 전망 기법 개발)

  • In this study, we developed a hybrid forecasting model based on a four-parameter distribution which allows a simultaneous season-ahead forecasting for both seasonal rainfall and sub-daily rainfall in Han-River and Geum-River basins. The proposed model is mainly utilized a set of time-varying predictors and the associated model parameters were estimated within a Bayesian nonstationary rainfall frequency framework. The hybrid forecasting model was validated through an cross-validatory experiment using the recent rainfall events during 2014~2017 in both basins. The seasonal precipitation results showed a good agreement with the observations, which is about 86.3% and 98.9% in Han-River basin and Geum-River basin, respectively. Similarly, for the extreme rainfalls at sub-daily scale, the results showed a good correspondence between the observed and simulated rainfalls with a range of 65.9~99.7%. Therefore, it can be concluded that the proposed model could be used to better consider climate variability at multiple time scales.

  • The coffee shop market in Korea has been rapidly increased in quantitative terms over 10 years since the time when Starbucks opened their first shop in 1999. In recent years, the mature market characteristics that the serious competition of the coffee shop brands make the difference in taste or service reduce has been shown in that market. It's important to build the differentiated brand image for the differentiation of products and services and obtaining a price premium in serious competitive mature market. This study presented practical implications based on the results of empirical analysis of current positioning using 'Store Personality Scale.

Analysis Study on the use of Frequency and the Cooking Method of Leaf and Stem Vegetables in High School Foodservice (고등학교 급식식단의 엽경채류 식재료 사용 빈도 및 조리방법 분석 연구)

  • This study was conducted to extract the factors affecting the microbial safety of leaf and stem vegetables in the high school foodservice and to provide information for supplying the safe foodservice menu. The lunch and dinner menu (1,945 data) of the total 6 high schools at the Central and the South Region in March, June, September, and December were collected. The frequency analysis and the multiple correspondence analysis (MCA) based on the 3 factors (potentially hazardous food (PHF), leafy and stem vegetables in the menu, the cooking methods) were conducted. The most frequent PHF was the menu of blanched vegetables, salads, seaweeds and fried chicken. The most frequent consumed leaf and stem vegetables were spinach, chive, lettuce, Western cabbage, perilla leaf, iceberg lettuce, chicory, leek and broccoli. MCA based on the leaf and stem vegetables, the region, and the cooking method (cooked/non-cooked) showed that garlic stem and spinach were more used in the Central Region, while water drop-wort were more used in the South Region. Iceberg lettuce, Bok choy and leek were included frequently in the PHF menu. Plant products frequently used in PHF menu requires the food safety system such as Good Agricultural Practice (GAP) to reduce the microbial risk. The menu database according to raw materials based on cooking methods (heating or mixing) as well as the development and verification of menu based on the microbial safety will be contributed to provide the safer foodservice menu.

Automatic Generation of GCP Chips from High Resolution Images using SUSAN Algorithms

  • Automatic image registration is an essential element of remote sensing because remote sensing system generates enormous amount of data, which are multiple observations of the same features at different times and by different sensor. The general process of automatic image registration includes three steps: 1) The extraction of features to be used in the matching process, 2) the feature matching strategy and accurate matching process, 3) the resampling of the data based on the correspondence computed from matched feature. For step 2) and 3), we have developed an algorithms for automated registration of satellite images with RANSAC(Random Sample Consensus) in success. However, for step 1), There still remains human operation to generate GCP Chips, which is time consuming, laborious and expensive process. The main idea of this research is that we are able to automatically generate GCP chips with comer detection algorithms without GPS survey and human interventions if we have systematic corrected satellite image within adaptable positional accuracy. In this research, we use SUSAN(Smallest Univalue Segment Assimilating Nucleus) algorithm in order to detect the comer. SUSAN algorithm is known as the best robust algorithms for comer detection in the field of compute vision. However, there are so many comers in high-resolution images so that we need to reduce the comer points from SUSAN algorithms to overcome redundancy. In experiment, we automatically generate GCP chips from IKONOS images with geo level using SUSAN algorithms. Then we extract reference coordinate from IKONOS images and DEM data and filter the comer points using texture analysis. At last, we apply automatically collected GCP chips by proposed method and the GCP by operator to in-house automatic precision correction algorithms. The compared result will be presented to show the GCP quality.

Successful seroconversion against diphtheria and tetanus induced through maternal vaccination in a region of Colombia

  • Purpose: This study aims to compare protection against diphtheria and tetanus conferred on the mother and the neonate before and after maternal vaccination against tetanus, diphtheria, and acellular pertussis (Tdap), transfer of antibodies, and the variables that could impact on the protection. Materials and Methods: The study followed a cohort of 200 pregnant women from a region in Colombia, contacted during prenatal control before vaccination and upon delivery. The work determined immunoglobulin G antibodies against diphtheria and tetanus of pregnant women and umbilical cord. The proportion of protection, the geometric mean of the concentration, and the transfer of maternal antibodies were calculated. The protection profile of the pregnant women was explored by using multiple correspondence analysis. Results: The concentration of antibodies against diphtheria was significant before and after vaccination of the pregnant women (p=0.000) with proportions of 85.0% and 97.5%, respectively, and of 98.6% in the umbilical cord, with significant antibody correlation (Spearman's coefficient=0.668, p=0.01). Sero-protection against tetanus before vaccination was at 71.0%, after at 92.6%, and in the umbilical cord at 95.9%, with significant antibody concentration before and after vaccination (p=0.000) and antibody correlation (Spearman's coefficient=0.936, p=0.01). Sero-protection was higher when the pregnant women were vaccine 8 to 11 weeks before delivery. Unprotected pregnant women were those not vaccinated during pregnancy. Conclusion: The high proportion of protection against diphtheria and tetanus and the placental transfer support the need to promote maternal immunization with Tdap.
