Reliability of the Korean Version of the Trunk Impairment Scale in Patients With Stroke

  • The purpose of this study was to establish the reliability of the Trunk Impairment Scale (TIS) translated into Korea in patients with stroke. It also aims to score the quality of trunk movement and to be a tool for the treatment. The TIS consists of three subscales that static sitting balance, dynamic sitting balance and co-ordination. The TIS score ranges from a minimum of 0 to a maximum of 23. Twenty-five stroke patients (13 males, 12 females) were examined by two physiotherapists. Interrater and test-retest reliability were assessed. Kappa and weighted kappa values for the items of the trunk assessment of the TIS ranged from .67 to 1.00. Intraclass correlation coefficients for interrater and test-retest agreement were .95 and .97. Cronbach alpha coefficients for internal consistency range from .87 to .97. The TIS provide reliable assessments for the trunk and are valid scales for measuring trunk performance in patients with stroke. TIS can be used as a guideline for treatment and the assessment of quality of trunk activity.

Development of an Optimal Convolutional Neural Network Backbone Model for Personalized Rice Consumption Monitoring in Institutional Food Service using Feature Extraction

  • This study aims to develop a deep learning model to monitor rice serving amounts in institutional foodservice, enhancing personalized nutrition management. The goal is to identify the best convolutional neural network (CNN) for detecting rice quantities on serving trays, addressing balanced dietary intake challenges. Both a vanilla CNN and 12 pre-trained CNNs were tested, using features extracted from images of varying rice quantities on white trays. Configurations included optimizers, image generation, dropout, feature extraction, and fine-tuning, with top-1 validation accuracy as the evaluation metric. The vanilla CNN achieved 60% top-1 validation accuracy, while pre-trained CNNs significantly improved performance, reaching up to 90% accuracy. MobileNetV2, suitable for mobile devices, achieved a minimum 76% accuracy. These results suggest the model can effectively monitor rice servings, with potential for improvement through ongoing data collection and training. This development represents a significant advancement in personalized nutrition management, with high validation accuracy indicating its potential utility in dietary management. Continuous improvement based on expanding datasets promises enhanced precision and reliability, contributing to better health outcomes.

간호사 배치기준에 대한 정책적 함의 (Policy Implications of Nurse Staffing Legislation)

  • 급성기 병원의 간호사 배치수준은 환자안전 및 성과에 영향을 미치며, 간호사 배치기준의 법제화는 환자안전에 필요한 최소 인원을 보장하는 중요한 수단이 될 수 있다. 우리나라는 의료법에 간호사 정원기준을 제시하고 있으나, 환자안전 보장 및 간호의 질 향상을 위해서 다음의 몇 가지 사항에 대한 개선이 필요하다. 첫째 1962년 제정된 현행 의료법의 간호사 정원기준은 보건의료환경 변화에 따라 적정성에 대한 재검토가 필요하다. 둘째 의료법의 간호사 정원은 의료기관이 준수하여야할 최소 인원으로 모든 의료기관이 준수하도록 관리되어야 한다. 셋째, 간호사 배치기준은 환자의 이해를 돕고, 관리가 용이하도록 근무조당 입원환자수 대 간호사 수를 기준으로 하도록 한다. 넷째, 병원 간호단위별 근무조별 간호인력 배치정보를 공개하여야 한다.

부정확한 융복합 온라인 태스크들의 스케쥴가능성을 향상시키기 위한 지연 전략 (A deferring strategy to improve schedulability for the imprecise convergence on-line tasks)

  • 부정확한 실시간 스케쥴링은 실시간 시스템에 일시적인 과부하가 발생할 때, 보다 덜 중요한 태스크들을 희생시킴으로서 시간적 오류들의 결과로 발생될 수 있는 나쁜 효과들을 최소화시키기 위하여 사용될 수 있다. 부정확한 실시간 스케쥴링에 있어서, 모든 경성 실시간 태스크는 논리적으로 필수적 태스크와 선택적 태스크로 나누어 질 수 있다. 최근들어서, 선택적 태스크들을 지연시킴으로서 총오류를 최소화시키면서도 필수적 태스크들의 스케쥴가능성을 향상시키는 연구들이 진행되어 왔다. 그러나, 이러한 연구들에서의 스케쥴가능성은 각 각의 선택적 태스크의 실행요구시간이 대응하는 필수적 태스크의 실행요구시간보다 작거나 같을 때 에만 적용된다는 제약이 있었다. 그래서, 본 논문에서는 이전의 연구들의 필수적 및 선택적 태스크들에 대한 실행요구시간 제약조건과 정 반대되는 제약조건 하에서도 적용될 수 있는 새로운 지연 전략을 제시하였다. 그럼에도 불구하고, 본 논문에서 제시한 지연 전략은 총오류를 최소화시키면서도 이전 연구들에 비하여 유사하거나 더 우수한 스케쥴가능성 성능을 보여 주었다.

의학교육에서 컴퓨터바탕검사와 문항은행 데이터베이스 구축 (Computer-Based Testing and Construction of an Item Bank Database for Medical Education in Korea)

  • A number of medical schools in Korea have been using computer-based testing (CBT) for evaluating their students' scientific and/or clinical performance since the early 1990s. Introducing CBT to medical education would have several advantages: first, presenting figures and audio-video files of clinical content is simple with CBT, making it possible to evaluate medical students' competency with navigating more realistic clinical situations at minimum cost; second, CBT enables automatic item analysis and score reporting. To establish CBT, constructing an item bank with item parameters such as difficulty or discriminating parameters will be needed. To select more psychometrically sound items, analysis of the items according to item response theory is necessary. CBT has already been introduced in high stakes tests like the United States Medical Licensing Examination and the Medical Council of Canada Qualifying Examination. The National Health Personnel Examination Board in Korea is also planning to introduce a CBT-based version of the National Medical Examination soon. Thus all medical schools in Korea will need to introduce CBT and construct item banks to prepare their students for their licensing examinations and to measure the students' competency more accurately.

의과대학 교원 현황과 업적평가제도 특징 분석 (Current Status and Performance Evaluation Systems of Faculty in Korean Medical Schools)

  • The aim of this study is to analyze the current status and performance evaluation systems of faculty in Korean medical colleges and professional graduate medical schools (called medical schools). We developed a research tool based on previous studies and distributed it to 40 medical schools from July to October 2017. The response rate was 100%. We calculated the number of faculty members and analyzed the faculty evaluation systems and awareness according to national and private medical schools. As of 2017, the number of medical faculty in Korea was 11,111 (4,973 faculty were employed by their alma mater, which is 44.76% of the total), with non-medical doctor faculty accounting for 754 of the total. The medical schools reflect research achievements as most important for re-appointment and screening to promote faculty, and the area of education is secondary excepting clinical faculty of private medical schools. However, important issues in the faculty evaluation deal with the relevance of research achievement and the need for qualitative assessment. Some medical schools revised or have been revising the faculty evaluation system in areas such as minimum standards of education for promotion and separation of promotion and tenure review. Opening non-tenure track lines for faculty show positive effects such as increasing the number of positions for hire and easing the financial burdens of medical schools. Downfalls include inconsistencies between the responsibilities and actual practices of tenure not being available and the instability of faculty's status. In conclusion, medical schools need to prepare a faculty evaluation system that fits the position of faculty members and attempt to establish a reasonable compensation system.

Overall damage identification of flag-shaped hysteresis systems under seismic excitation

  • This research investigates the structural health monitoring of nonlinear structures after a major seismic event. It considers the identification of flag-shaped or pinched hysteresis behavior in response to structures as a more general case of a normal hysteresis curve without pinching. The method is based on the overall least squares methods and the log likelihood ratio test. In particular, the structural response is divided into different loading and unloading sub-half cycles. The overall least squares analysis is first implemented to obtain the minimum residual mean square estimates of structural parameters for each sub-half cycle with the number of segments assumed. The log likelihood ratio test is used to assess the likelihood of these nonlinear segments being true representations in the presence of noise and model error. The resulting regression coefficients for identified segmented regression models are finally used to obtain stiffness, yielding deformation and energy dissipation parameters. The performance of the method is illustrated using a single degree of freedom system and a suite of 20 earthquake records. RMS noise of 5%, 10%, 15% and 20% is added to the response data to assess the robustness of the identification routine. The proposed method is computationally efficient and accurate in identifying the damage parameters within 10% average of the known values even with 20% added noise. The method requires no user input and could thus be automated and performed in real-time for each sub-half cycle, with results available effectively immediately after an event as well as during an event, if required.

요소-멜라민수지로 접착된 파티클보드의 열압동안 포름알데히드 배출량 측정 (Measurement of Formaldehyde Emissions during Hot-Pressing of Particleboard Bonded with Melamine-Urea-Formaldihyde Resin)

  • 요소-멜라민수지 접착제를 실험실에서 수지 접착제 고형분량에 대해 5% 멜라민을 첨가하여 파티클보드 제조용으로 합성하였다. 합성한 요소-멜라민 수지를 이용하여 전건 목재파티클의 무게에 대해 접착제 첨가량 6, 8%와 열압시간 3, 4, 5분에서 실험실 파티클보드를 접착하는 열압시간동안 배출되는 가스를 증류수에 용해·포집하여 얻어진 용액으로부터 포름알데히드 배출량을 미국 National Institute of Occupational Safety and Health (NIOSH) 3500의 방법에 의해 분석·비교하였다. 포름알데히드 배출량을 측정하여 분석한 결과는 파티클보드를 열압하는 시간동안 배출되는 가스 중에서 포름알데히드량은 요소-멜라민수지 접착제의 첨가량과 열압시간에 대해 통계적으로 유의하게 차이가 있다는 것을 보여줬다. 파티클보드의 성능평가 결과는 3분의 열압시간에서 접착된 파티클보드의 박리강도는 KS F 3104 파티클보드 8.0형의 최소요구치를 만족시키는 것을 보여줬다.

Antimicrobial and cytotoxic properties of calcium-enriched mixture cement, Iranian propolis, and propolis with herbal extracts in primary dental pulp stem cells

  • Objectives: In this study, natural substances were introduced as primary dental pulp caps for use in pulp therapy, and the antimicrobial and cytotoxic properties of these substances were investigated. Materials and Methods: In this in vitro study, the antimicrobial properties of calcium-enriched mixture (CEM) cement, propolis, and propolis individually combined with the extracts of several medicinal plants were investigated against Enterococcus faecalis, Escherichia coli, Pseudomonas aeruginosa, and Staphylococcus aureus. Then, the cytotoxicity of each substance or mixture against pulp stem cells extracted from 30 primary healthy teeth was evaluated at 4 concentrations. Data were gathered via observation, and optical density values were obtained using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-2H-tetrazolium bromide (MTT) test and recorded. SPSS software version 23 was used to analyze the data. Data were evaluated using 2-way analysis of variance and the Tukey test. Results: Regarding antimicrobial properties, thyme alone and thyme + propolis had the lowest minimum inhibitory concentrations (MICs) against the growth of S. aureus, E. coli, and P. aeruginosa bacteria. For E. faecalis, thyme + propolis had the lowest MIC, followed by thyme alone. At 24 and 72 hours, thyme + propolis, CEM cement, and propolis had the greatest bioviability in the primary dental pulp stem cells, and lavender + propolis had the lowest bioviability. Conclusions: Of the studied materials, thyme + propolis showed the best results in the measures of practical performance as a dental pulp cap.

Constructing a digital twin for estimating the response and load of a piping system subjected to seismic and arbitrary loads

  • In recent years, technological developments have rapidly increased the number of complex structures and equipment in the industrial. Accordingly, the prognostics and health monitoring (PHM) technology has become significant. The safety assessment of industrial sites requires data obtained by installing a number of sensors in the structure. Therefore, digital twin technology, which forms the core of the Fourth Industrial Revolution, is attracting attention in the safety field. The research on digital twin technology of structures subjected to seismic loads has been conducted recently. Hence, this study proposes a digital twin system that estimates the responses and arbitrary load in real time by utilizing the minimum sensor to a pipe that receives a seismic and arbitrary load. To construct the digital twin system, a finite-element model was created considering the dynamic characteristics of the pipe system, and then updating the finite-element model. In addition, the calculation speed was improved using a finite-element model that applied the reduced-order modeling (ROM) technology to achieve real-time performance. The constructed digital twin system successfully and rapidly estimated the load and the point where the sensor was not attached. The accuracy of the constructed digital twin system was verified by comparing the response of the digital twin model with that derived by using the load estimated from the digital twin model as input in the finite-element model.
