한국형 DASH 식이 교육과 칼슘/비타민 D 보충 영양중재 프로그램이 노인여성의 영양소 섭취량 및 식품 섭취 빈도, 골표지자, 골밀도에 미치는 효과 (The Effects of Korean DASH Diet Education with Calcium/Vitamin D Supplements on Nutrient Intakes, Food Consumption, Bone Turnover Markers and Bone Mineral Density among Korean Elderly Women)

  • Purpose: This study was aimed to investigate the effects of the nutritional intervention program including DASH diet education and calcium/vitamin D supplements in Korean elderly women. Methods: This was a quasi-experimental study employing non-equivalent control group pretest-posttest design. The experimental group (n=26) was given DASH diet education and supplements (calcium 1200mg and vitamin 800 IU) while the control group (n=16) was given only general health consults. After the interventions, differences were analyzed in nutrient intake, bone turnover markers and bone mineral density between the two groups. Results: After one year, bone mineral density was found reduced in both groups, but showed higher levels (p=.003) in the experimental group than the control group. After research, nutrient intakes of participants improved generally, yet there was no significant difference between the two groups. The experimental group was divided into subgroups after interventions according to the level of calcium intake, and bone density and bone markers were compared between the subgroups. In a subgroup whose calcium intake was in the normal range, bone mineral density was significantly high (p=.002) while CTx and osteocalcin were significantly low (p=.003, p=.006, respectively). Conclusion: This study is significant in that it provided a nutritional intervention program for one year to elderly women who are susceptible to osteoporosis and severely low in dietary calcium intake and it proved to be effective.

채식과 비채식인의 모발 내 무기질 함량과 영양상태의 관련성 (Association between Hair Mineral Content and Nutritional Status in Vegetarians and Non-Vegetarians)

  • Previous studies have shown that plant-based vegetarian diets, which typically contain a variety of antioxidants and dietary fiber, help reduce the risk of heart disease, diabetes, obesity, and cancer. However, some studies have reported that vegetarian diets can lead to deficiencies in protein and trace minerals compared to non-vegetarian diets. This study was conducted to compare anthropometric measurements, blood parameters, dietary intake, and hair mineral status in long-term vegetarians (MV; moderate vegan, LV; lacto-ovo vegetarian) and non-vegetarians (NV). Thirty MV (12 males, 18 females; mean age, $50.58{\pm}5.05$ years), 15 LV (11 males, four females; mean age, $49.45{\pm}4.97$ years), and 30 NV (15 males, 15 females; mean age, $48.90{\pm}3.62$ years) participated. No significant differences were observed for age, height, weight, or body mass index, but body fat was significant lower in MV and LV males than that in NV males. White blood cell counts of MV, LV, and NV male subjects were significantly different. Dietary intake data showed that plant protein and plant iron intake were significantly higher in MV and LV than those in NV. Animal protein, animal fat, and animal iron intake were significantly higher in NV than those in MV and LV. A hair mineral analysis showed that calcium and iron were higher in the hair of MV and LV than those in NV. Zinc concentration in hair was not significantly different among the groups. The results suggest that vegetarian diets are adequate to sustain mineral status to at least the same degree as that of non-vegetarian diets.

대두 이소플라본 보충 섭취가 폐경 후 여성의 골대사 지표와 소변 무기질 배설량에 미치는 영향 (Effect of Soy Isoflavone Supplementation on Bone Metabolism Marker and Urinary Mineral Excretion in Postmenopausal Women)

  • We studied the effects of soy isoflavone supplements on bone metabolism marker (serum osteocalcin, urinary deoxypyridinoline) and urinary mineral excretion (urinary Ca, Mg, Zn) in 47 postmenopausal women. There were 24 participants in the treatment group and 23 in the control group. The treatment group consumed isoflavone extract capsules daily (which contained 90 mg of soy isoflavones) for 12 weeks. The study compared before and after isoflavone intake in the following areas: Physical examination, diet survey, bone metabolism marker and urinary mineral excretion. The average age of the treatment group was 64.6 years and that of the control group was 66.5 years. There were no significant differences between the two groups in terms of height, weight and body mass index. Both groups maintained a regular diet pattern in terms of their average daily nutrient intake. There were no significant differences between the treatment group (23.9 mg) and the control group (25.4 mg) in terms of daily isoflavone intake based on diet. The analysis of bone metabolism marker changes in the treatment group after 12 weeks of taking the isoflavone supplements demonstrated significant differences in the following: Serum osteocalcin (13.7 ng/mL in befor versus 6.8 ng/mL in after) and urinary deoxypyridinoline (5.9 nmol/mmol Cr in befor versus 4.5 nmol/mmol Cr in after). The subjects in the treatment group showed no significant difference in urinary Ca excretion. But the subjects showed a significant difference in urinary Mg (131.9 mg/day in befor versus 115.6 mg/day in after) and Zn (400.5 $\mu\textrm{g}$/day in befor versus 310.2 $\mu\textrm{g}$/day in after) excretion in the isoflavone treatment group at the levels of p<0.001, p<0.01, respectively. No changes were made in the intake of minerals. The composition of serum osteocalcin and urinary deoxypyridinoline, and indicators of bone metabolism, including the excretion Mg and Zn, significantly decreased. Nutrient Intakes and Hair Mineral Contents of Young Children

  • Purpose: This study was performed to evaluate the dietary nutrient intake status and hair mineral content of Korean young children. Methods: Fifty-five children who visited Seoul National University Bundang Hospital were divided into three groups by age: infants, toddlers, and preschoolers. The 24-hour recall method was used to collect the food intake data of the subjects. Hair mineral analysis was conducted using a Mass Spectrometer. Serum iron, ferritin, and calcium were also measured. Results: The mean energy intakes of the subjects were 730.3 kcal, 994.3 kcal, and 1,482.9 kcal for each age group. The mean percentage of energy intake compared to recommendation was 101.4% and was not different by age group. Toddlers of 37.8% and preschoolers of 54.5% consumed less than the Estimated Average Requirement (EAR) of calcium. Infants of 28.6%, toddlers of 10.8% and preschoolers of 9.1% consumed less than the EAR of iron. In the case of zinc, copper, and selenium, only 0% to 5% of toddlers and none of the preschoolers consumed less than the EAR of those minerals. The hair calcium, iron and copper concentrations were lower in toddlers and preschoolers than those in infants. Serum calcium levels of preschoolers were significantly lower than those of infants, whereas serum iron and ferritin levels were not. Conclusion: Hair calcium, iron, and copper concentrations were significantly lower in toddlers and preschoolers than in infants. Insufficient dietary intake of calcium and iron seems to be related with decreased hair mineral contents in young children.

이소플라본 보충이 폐경 후 여성에서 골밀도와 성호르몬에 미치는 영향 (Effects of Isoflavones Supplementation on Bone Mineral Density and Sex Hormones in Postmenopausal Women)

  • Soy isoflavones have been hypothesized to exert hormonal effects and to enhance bone mineral density in postmenopausal women. To test this hypothesis, we studied the effects of soy isoflavones supplements on bone mineral density and sex hormones (serum estradiol, sex hormone-binding globulin and testosterone) in 47 postmenopausal women. There were 24 participants in the treatment group and 23 in the control group. The treatment group consumed the isoftavones extract capsule daily (which contained 90 mg of soy isoflavones) for 12 weeks. The study compared pre- and post-isoflavones intake in the following areas: physical examination, diet survey, bone mineral density and serum sex hormone levels. The average age of the treatment group was 64.63 years and that of the control group was 66.48 years. There were no significant differences between the two groups in terms of height, weight, and body mass index. Both groups maintained regular diet patterns in terms of their average daily nutrient intake. There was no significant difference between the treatment group (18.49 mg) and the control group (21.27 mg) in terms of daily isoflavones intake based on diet. The 12-week analysis of bone mineral density change after taking isoflavones supplements demonstrated no significant differences in the following : lumbar spine BMD (0.82 g/$\textrm{cm}^2$ in pre versus 0.81 g/$\textrm{cm}^2$ in post), femoral neck BMD (0.58 g/$\textrm{cm}^2$ in pre versus 0.57 g/$\textrm{cm}^2$ in post) in the treatment group. There was no significant difference in serum estradiol in the isoflavones treatment group. The subjects indicated no significant difference in serum testosterone in the isoflavones treatment group. But the subjects indicated a significant difference in sex hormone-binding globulin (60.04 nmol/L in pre versus 52.39 nmol/L in post) in the isoflavones treatment group at the levels of p < 0.05. The significant decrease in sex hormone-binding globulin did indicate the need for long-term study on isoflavones supplementation as well as its positive effect on bone mineral density.

  • To investigate the effects of dietary protein and calcium levels on calcium and bone metabolism Sprague-Dawley male growing rats weighting approximately 91.4g were divided into four groups and fed one of the following four experimental diets-15% protein 0.2% calcium ; 15% protein 0.5% calcium ; 30% protein 0.2% calcium ; 30% protein 0.5% calcium-for five weeks. Calcium intake and excretion, apparent calcium absorption were measured and bone densities and mineral contents of femur and scapula were analyzed. Calcium excretion through feces and urine was significantly greater in animals receiving diets of higher calcium. Fecal calcium but not urinary calcium excretion was greater when the protein level was increased from 15% to 30%. Apparent calcium absorption rate was significantly higher with lower calcium intakes. Serum alkaline phosphatase activity was significantly higher in 0.2% calcium group than in 0.5% calcium group, while urinary hydroxyproline excretion was essentially same among all experimental groups. Weights and mineral contents or protein. Bone weights were greater, but calcium and ash contents of femur and scapula were lower in animals on the diet containing low calcium and high protein, which suggests that bone metabolism may be affected by the interaction between calcium and protein intake. These results indicate that during growth high protein intake might be beneficial to bone health if the diet is sufficient in calcium, however, if the diet fails to provide an optimum amount of calcium, such practice might be detrimental.

성인여성의 식습관과 영양섭취상태와 골밀도 및 골무기질함량과의 관계 (The Relationship between Food Habit, Nutrient Intakes and Bone Mineral Density and Bone Mineral Content in Adult Women)

  • Bones are important parts in sustaining the shape of the body, but they are also metabolic organs which undergo bone remodeling by constant bone resorption and formation. Osteoporosis, the typical metabolic bone disease, is characterized by a reduction in bone mineral density (BMD). Women more than men are at risk fir osteoporosis-related fractures, especially in the lumbar spine, wrist, and hip region. Risk of fracture depends on one's BMD, which open determined by the peak bone mass value achieved at skeletal maturity and followed by subsequent age-and menopause-related bone loss. Genetic and environmental factors are known to play a key role in bone metabolism and diet is considered as one of the important environmental factors. The purpose of the present study was to assess the status of BMD and bone mineral content(BMC) to clarify the relationships between dietary intakes and the risk of osteoporosis in adult women in Taegu. Subjects were 130 healthy females in between 20 and 69 years of age. BMD and BMC of the lumbar spine(venebrae L2-4) of the subjects were measured by dual energy X-ray absorptiometry. The average age of the subjects was 47.4${\pm}$11.7 years old, the average weight was 57.2${\pm}$8.4kg, the average age of menarche was 16.6${\pm}$1.9 years old and the average age of menopause was 48.4${\pm}$5.3 years old. The nutrient intakes of the subjects measured by the convenient method were generally lower than the level of RDA. The result of nutrient intake assessed has shown that the average energy intake was 1701${\pm}$316kca1 which is 85.1% of the RDA and the average calcium intake was 485.4${\pm}$172.3mg which is 69.3% of the RDA. The intakes of protein, vitamin A, vitamin B$_1$, niacin were greater than the RDA, whereas the remaining nutrient intakes were lower than the RDA. The average BMD of the subjects was shown to be 1.06${\pm}$1.09g/$\textrm{cm}^2$. The highest BMD of 1.24${\pm}$0.14g/$\textrm{cm}^2$ was noticed in the subjects of 30s compared to 20s, 40s, 50s, 60s. The BMD values were compared by the relative body weight(R3W) of the menopause subjects, and it was found that the underweight group had significant lower BMB while the rest of the groups did not have any differences in BMD. The most strongly correlated nutrient with BMD among the menopause subjects appeared to be calcium. The women whose Ca intakes were higher than 500mg showed the significantly higher BMD than those with Ca intakes lower than 500mg. This study suggests that the most effective way to prevent osteoporosis and to reduce the incidences of fractures seems to be minimizing bone loss through the adequate intake of calcium as well as avoiding underweight, especially in menopausal women. 무기질 출납에 있어서의 섬유질 및 Vitamin 급원 식품의 영향 (Effects of Fiber and Vitamin Food Sources on Mineral Balance)

  • 종류가 다른 채소 3종을 임의로 선정하여 마우스의 Na, K, Ca, Fe, Cu, Zn, Mg 밸런스에 어떠한 영향을 미치며, 또한 채소의 종류나 양, 사육기간이 영향을 미칠 수 있는 요인인지 조사하였다. 실험동물은 8주된 SPF계 마우스 130마리를 사용했으며 평균 체중은 $29{\sim}38g$이었다. 마우스 10마리를 대조군으로, 나머지 120마리를 3군으로 나누어 이를 다시 두군으로 나누어 한마리씩 분리 사육시켰다. 대조군은 물(2차 증류수)과 식이를 제한없이 주고 다른군은 물 대신 임의로 선정한 채소 3종(당근, 껍질콩, 호박)으로 대체하고 각군 20마리 중 10마리는 1주(단기간) 10마리는 2주(장기간) 사육시켜 실험 완료일 까지의 실험식이 섭취량이 대조군과 유의적인 차가 없는 마우스만을 실험대상으로 하였다. 또한 이들 채소의 양에 따라서 당근대량군(12마리), 당근소량군(10마리), 껍질콩대량군(11마리), 껍질콩소량군(9마리), 호박대량군(10마리), 호박소량군(14마리)으로 분류하였다. 체중증가는 대조군에 비하여 껍질콩대량군이 높게 나타났으며, 식이 효율은 대조군에 대한 차이가 없었다. 분변과 요량은 채소 급여에 의하여 증가하였다. 체중증가와 식이효율, 분변과 요량에는 채소의 종류나 양, 사육기간등이 작용인자로는 확인되지 않았다. 무기질 섭취량은 대조군에 비하여 Na, Ca, Fe, Cu가 호박대량군에서 가장 낮게 나타났고, K는 당근대량군이 가장 높게 나타났으며 Zn, Mg는 껍질콩대량군에서 가장 높게 나타났다. Na, K, Ca, Fe, Cu, Zn, Mg의 흡수율과 밸런스는 대조군이 플러스의 균형을 나타내는데 반하여 채소 급여군은 모두 마이너스의 균형을 나타내었다. 채소의 종류, 양, 사육기간이 Na, K, Ca, Cu, Zn의 섭취량에, Cu, Zn의 흡수율에, K, Cu, Zn 평형에 각각 영향을 미치는 요인인 것으로 확인되었다. K, Zn, Mg는 대조군에 비하여 실험군의 섭취량이 증대 했음에도 불구하고 흡수율은 마이너스이였다.

우리나라 폐경전 여성에서 칼슘, 단백질, 인의 섭취상태가 골밀도에 미치는 영향 (Effects of Dietary Calcium, Protein, and Phosphorus Intakes on Bone Mineral Density in Korean Premenopausal Women)

  • Effects of dietary calcium(Ca), protein, and phosphorus(P) intake on bone mineral density (BMD) were investigated in 129 Korean premenopausal women(age 31-54 years) without diagnosed disease. BMD was measured at the spine(vertebrae L2-4) and femur(neck, Ward's triangle and trochanter). By stepwise multiple regression analysis it was shown that protein, Ca, and P intakes affected most significantly on BMD at the vertebrae L2-4, protein and P intakes affected most significantly on BMD at the femoral neck and Ward's triangle, and body mass index(BMI) affected most significantly on BMD at the trochanteric region. When ate-matched BMD % at the vertebrae L2-4 and all femoral sites was grouped by three levels(<90%, 90-99%, >=100%), only at the vertebrae L2-4>=100% and 90-99% groups had higher Ca intakes than <90% groups. When Ca, protein and P intakes of the recommended level for Korean(RDA) were grouped by three levels (Ca or P ; <=650mg/d, 650-750mg/d, >=750mg/d, Protein ; <=55g/d, 55-60g/d, >=65g/d), only at the vertebrae L2-4>55g/d of protein intake had higher age-matched BMD % than <=55g/d intake, >=750mg/d of Ca and P intakes, age-matched BMD % than <=650mg/d. In RDA range of Ca, protein, and P intakes, age-matched BMD % of the vertebrae L2-4 and all femoral sites was greater than 90%. Correlation between Ca intake and vertebral BMD was examined closer. There was more significant linear correlation between vertebral BMD and Ca intake below 800mg/d(r=0.346, p<0.0001)than above(r=0.376, p<0.019), implying a threshold effect and vertebral BMD was better expressed as a function of the logarithm of calcium intake(r=0.3881, p<0.0001). These results suggest that Ca, protein, and P intakes greater than RDA help to maintain proper BMD in middle-aged prementopausal women. Especially dietary Ca have important role in increasing the vertebral BMD and 800mg/d of Ca intake is optimum amount.

노년기 농촌여성의 비타민 D 수용체 유전자형에 따른 칼슘 섭취량과 골밀도와의 관계 (Association of Calcium Intake and Bone Mineral Density by Vitamin D Receptor Genotype among Elderly Women Living in Rural a Area)

  • Recent studies have shown that environmental, biochemical, nutritional or genetic factors affect bone mineral density (BMD). The purpose of the present study was to investigate the effect of vitamin D receptor (VDR) gentotype and nutritional status on BMD of elderly women living in a rural area. Three hundred thirty five elderly women over 65 years in Asan participated the study. Data for demographic and nutrient intakes were obtained by survey with a two day 24 hr recall method. BMD was measured by broadband ultrasound attenuation (BUA) using quantitative ultrasound (QUS). VDR genotypes of the subjects analyzed with Bsm I restriction enzyme were bb (92%), Bb (7%) and BB (1%). No differences were found between genotypes bb and Bb/BB in age, menopausal age, body mass index and body fat. BUA of bb genotype was higher (62.5 $\pm$ 15.6 dB/MHz) than Bb/BB genotype (56.1 $\pm$ 17.6 dB/MHz) by Student's t-tests. Correlation analyses showed strong negative correlation of BMD and age, but positive correlation with BMI, energy and calcium intake. When subgroup analyses were conducted after stratification by the median calcium intake level (412.9 mg/d), the above median calcium intake group showed significant difference in BUA by VDR genotype while the lower median calcium intake group did not show significant difference. The current study confirmed interaction of calcium intake and VDR genotype in association with BMD. Further nutritional intervention will be needed to improve calcium status of the elderly women living in rural areas.
