Enteral nutrition for optimal growth in preterm infants

  • Early, aggressive nutrition is an important contributing factor of long-term neurodevelopmental outcomes. To ensure optimal growth in premature infants, adequate protein intake and optimal protein/energy ratio should be emphasized rather than the overall energy intake. Minimal enteral nutrition should be initiated as soon as possible in the first days of life, and feeding advancement should be individualized according to the clinical course of the infant. During hospitalization, enteral nutrition with preterm formula and fortified human milk represent the best feeding practices for facilitating growth. After discharge, the enteral nutrition strategy should be individualized according to the infant's weight at discharge. Infants with suboptimal weight for their postconceptional age at discharge should receive supplementation with human milk fortifiers or nutrient-enriched feeding, and the enteral nutrition strategy should be reviewed and modified continuously to achieve the target growth parameters.

  • 목적: 미숙아에게 필요한 영양성분을 알기 위해서는 그들의 모유 성분을 분석하는 것이 도움이 된다. 저자들은 미숙아 모유의 장기적인 철분함량의 변화를 관찰하고 만삭아 모유와 비교하였으며, 이를 토대로 모유수유만 하는 미숙아에서 철분 보충의 필요성과 지침에 대해 알아보고자 하였다. 방법: 재태연령 34주 미만이거나 출생체중 1,800 g 미만의 미숙아를 출산한 산모 103명으로부터 생후 1, 2, 4, 6, 8, 12주의 모유를 수집하였고, 만삭아의 모유는 24명의 산모로부터 생후 1주째에 수집하여 철분함량을 분석하였다. 결과: 생후 2주에서 8주 사이의 미숙아 모유내의 철분함량은 유의한 차이가 없었으며(36.3${\pm}$23.1~45.8${\pm}$26.0 $\mu$g/dL), 이는 생후 1주째의 미숙아모유 (23.1${\pm}$14.6 $\mu$g/dL)나 만삭아 모유(25.2${\pm}$7.55 $\mu$g/dL)의 철분함량보다는 유의하게 높았다. 교정나이로 만삭이 된 미숙아의 모유(42.2${\pm}$19.4 $\mu$g/dL)는 만삭아의 모유보다 유의하게 높은 철분함량을 나타내었다. 결론: 미숙아 모유의 철분함량은 생후 3개월까지 큰 차이가 없었으며, 이는 필요량보다 적어 모유수유만 할 경우 AAPCON(2004)에서 권유하는 대로 2 mg/kg/day의 철분을 보충하는 것이 좋겠다.

Effect of Vitamin E Supplementation on Hematological and Plasma Biochemical Parameters during Long Term Exposure of Arsenic in Goats

  • The present investigation was designed to determine whether supplementation of different level of vitamin E for 12 months to arsenic exposed goats (50 ppm as sodium arsenite) affords protection against the blood hemato-biochemical parameters caused by the metalloid. A total of 24 crossbred (Alpine${\times}$Beetal) lactating goats were assigned randomly into 4 equal groups (control, $T_1$, $T_2$ and $T_3$) of 6 in each, on the basis of average body weight ($36.10{\pm}0.11$ kg) and milk yield ($1.61{\pm}0.04$ kg/d). The animals in $T_1$, $T_2$ and $T_3$ were given 50 ppm arsenic, while in $T_2$ and $T_3$, additionally; vitamin E at the rate of 100 IU and 150 IU/kg dry matter (DM) respectively was additionally supplemented for the period of 12 months. Hemoglobin (Hb), total leukocyte (TLC) and blood lymphocyte % were decreased (p<0.05) in arsenic fed groups and vitamin E supplementation in the experimental group showed a protective potential. Significant increases (p<0.05) in aspertate transaminase (AST) and alanine transaminase (ALT) activities among arsenic supplemented groups were recorded, however vitamin E supplementation at higher doses showed a protective effect (p<0.05) against AST but in the case of ALT no ameliorating effect was found in either of the doses. Plasma total protein was decreased (p>0.05) but creatinine level was periodically increased in all As supplemented groups and vitamin E supplementation did not produce any protective effect. It can be concluded that arsenic exposure resulted in varying degree of changes in hemato-biochemical parameters and activities of antioxidant enzymes in goats but concomitant treatment with Vitamin E is partially helpful in reducing the burden of arsenic induced effect.

Effect of Plants Containing Secondary Compounds with Palm Oil on Feed Intake, Digestibility, Microbial Protein Synthesis and Microbial Population in Dairy Cows

  • The objective of this study was to determine the effect of rain tree pod meal with palm oil supplementation on feed intake, digestibility, microbial protein synthesis and microbial populations in dairy cows. Four, multiparous early-lactation Holstein-Friesian crossbred (75%) lactating dairy cows with an initial body weight (BW) of $405{\pm}40$ kg and $36{\pm}8$ DIM were randomly assigned to receive dietary treatments according to a $4{\times}4$ Latin square design. The four dietary treatments were un-supplementation (control), supplementation with rain tree pod meal (RPM) at 60 g/kg, supplementation with palm oil (PO) at 20 g/kg, and supplementation with RPM at 60 g/kg and PO at 20 g/kg (RPO), of total dry matter intake. The cows were offered concentrates, at a ratio of concentrate to milk production of 1:2, and chopped 30 g/kg of urea treated rice straw was fed ad libitum. The RPM contained condensed tannins and crude saponins at 88 and 141 g/kg of DM, respectively. It was found that supplementation with RPM and/or PO to dairy cows diets did not show negative effects on feed intake and ruminal pH and BUN at any times of sampling (p>0.05). However, RPM supplementation resulted in lower crude protein digestibility, $NH_3$-N concentration and number of proteolytic bacteria. It resulted in greater allantoin absorption and microbial crude protein (p<0.05). In addition, dairy cows showed a higher efficiency of microbial N supply (EMNS) in both RPM and RPO treatments. Moreover, NDF digestibility and cellulolytic bacteria numbers were highest in RPO supplementation (p<0.05) while, supplementation with RPM and/or PO decreased the protozoa population in dairy cows. Based on this study, supplementation with RPM and/or PO in diets could improve fiber digestibility, microbial protein synthesis in terms of quantity and efficiency and microbial populations in dairy cows.

Effects of dietary supplementation of glucose oxidase, catalase, or both on reproductive performance, oxidative stress, fecal microflora and apoptosis in multiparous sows

  • Objective: The objective of this experiment was to investigate the effect of dietary glucose oxidase (GOD), catalase (CAT), or both supplementation on reproductive performance, oxidative stress, and apoptosis in sows. Methods: A total of 104 multiparous sows were randomly assigned to four groups (n = 26) with each group given a basal diet, basal diet plus GOD at 60 U/kg, basal diet plus CAT at 75 U/kg, and basal diet plus GOD at 60 U/kg and CAT at 75 U/kg. Sows were fed the experimental diets throughout gestation and lactation. Results: Dietary GOD supplementation increased average daily feed intake of sows and litter weight at weaning (p<0.05). Dietary CAT supplementation reduced the duration of parturition, stillbirth, and piglet mortality and increased growth performance of weaned piglets (p<0.05). Dietary GOD and CAT supplementation enhanced antioxidant enzyme activities and lessened oxidative stress product levels in plasma of sows and elevated antioxidant capacity of 14-day milk and plasma in weaned piglets (p<0.05). Dietary GOD supplementation increased fecal Lactobacillus counts and reduced Escherichia coli counts of sows (p<0.05). Compared with the basal diet, the GOD diet reduced fecal Escherichia coli counts of sows, but the addition of CAT did not reduce Escherichia coli counts in the GOD diet. Dietary GOD and CAT supplementation reduced the apoptosis rate of the liver, endometrium, and ovarian granulosa cells in sows (p<0.05). In the liver, uterus, and ovary of sows, the mRNA expression of caspase-3 and caspase-9 was downregulated by dietary GOD and CAT supplementation (p<0.05). Conclusion: Dietary GOD and CAT supplementation could improve the antioxidant capacity of sows and weaned piglets, and alleviate hepatic, ovarian and uterine apoptosis by weakening apoptosis-related gene expression. Glucose oxidase regulated fecal microflora of sows, but supplementation of CAT to GOD could weaken the inhibitory effect of GOD on fecal Escherichia coli.

식품섭취에 따른 유방암의 발생 위험 : 환자- 대조군 연구 (Food Intake and Breast Cncer Risk: A Case-Control Study)

  • We performed case-control study to evaluate relationship between food intake and risk of breast cancer. We interviewed breast cancer cases(n=108) who were newly histologically identified and selected from Hanyang and Soonchunhyang University Hospital in Seoul. We used hospital-based control subjects(n=121), who were selected from the patients in the department of plastic surgery, general surgery and opthalmology of the same hospital by frequency matching. Matching variables were age($\pm$4 age) and menopausal status. We collected information on general characteristics of subjects, history of disease, family history of breast cancer, vitamin supplementation, alcohol intake, and food intake through individual interview. Dietary information was ascertained via a food frequency questionnaire method(total item=98). All analyses was conducted according to menopausal status. Especially, pepper and grape intake in premenopausal women, showed protective effect in breast cancer. Otherwise consumption of meat, pork, and fish was not associated with breast cancer risk in this study. At milk and dairy products, milk intake was associated with lower risk of breast cancer in postmenopausal women and overall intake of soy products was associated with breast cancer risk, but not significant. These findings suggested that consumption of some fruit and vegetable intake was a protective factor on breast cancer and further study with more number of subjects should be need to evaluate the breast cancer risk. Rheological, Physicochemical, Microbiological, and Aroma Characteristics of Sour Creams Supplemented with Milk Protein Concentrate

  • Milk protein concentrate (MPC) is widely used to enhance the stability and texture of fermented dairy products. However, most research has focused on yogurt products, and the effects of MPC on sour cream characteristics remain unknown. Therefore, we investigated the effects of different MPC levels (0%, 1%, 2%, and 3% w/w) on the rheological, physicochemical, microbiological, and aroma characteristics of sour creams in this study. We found that MPC supplementation stimulated the growth of lactic acid bacteria (LAB) in sour creams, resulting in higher acidity than that in the control sample due to the lactic acid produced by LAB. Three aroma compounds, acetaldehyde, diacetyl, and acetoin, were detected in all sour cream samples. All sour creams showed shear-thinning behavior (n=0.41-0.50), and the addition of MPC led to an increase in the rheological parameters (ηa,50, K, G', and G"). In particular, sour cream with 3% MPC showed the best elastic property owing to the interaction between denatured whey protein and caseins. In addition, these protein interactions resulted in the formation of a gel network, which enhanced the water-holding capacity and improved the whey separation. These findings revealed that MPC can be used as a supplementary protein to improve the rheological and physicochemical characteristics of sour cream.

Effects of Monensin on Metabolism and Production in Dairy Saanen Goats in Periparturient Period

  • This trial evaluated the effects of dietary supplementation with monensin sodium on dry matter intake, metabolic parameters and milk yield and milk composition of dairy Saanen goats in the periparturient period. Twelve Saanen pregnant dairy goats were assigned to a treatment and a control group. Saanen goats were fed monensin as its 10% sodium salt in the amount of 33 mg/kg of total dry matter intake during 30 d before parturition till d 42 in milk. Blood samples were collected from each goat at d 30, 15 and 7 before expected kidding time and also in d 1, 7, 13, 19, 21, 28, 35 and 42 postpartum. The serum concentrations of ${\beta}$-Hydroxybutyrate (BHBA), non-esterifed fatty acid (NEFA), calcium, magnesium, inorganic phosphorus, cholesterol, triglyceride, urea, total protein, albumin and glucose and aspartate aminotransferase (AST) activity were determined. The BHBA concentration significantly decreased in goats, which received monensin in comparison to controls in the postpartum period (p = 0.049). The concentration of sodium (Na) was significantly influenced by monensin treatment in the postpartum period (p = 0.048). In addition, the difference in dry matter intake was highly significant between the two groups during the pre-partum period. Controls had more dry matter intake (DMI) than treatment goats (p = 0.0001). Mean 3.5% fat corrected milk production was not influenced by monensin treatment. However, milk fat percentage was significantly decreased in monensin treated goats (p = 0.0017).

두유(豆乳)에 첨가된 Methionine이 유산균의 생육과 산생성에 미치는 영향 (Effects of Methionine Supplemented to Soy Milk on Growth and Acid Production by Lactic Acid Bacteria)

  • 본 연구는 농축대두단백으로 만든 두유에 L-methionine을 여러가지 농도로 첨가하여 6종의 유산균 (Lactobacillus 4종, Leuconostoc 1종, Streptococcus 1종)의 생육과 산생성에 미치는 영향을 조사하고 아울러 DL-methionine, D-methionine 또는 L-cysteine이 L. acidophilus에 미치는 영향을 조사한 것으로서 그 결과를 요약하면 다음과 같다. 두유에 첨가된 L-methionine은 6종의 유산균 가운데 4종 (L. bulgaricus, L. casei, Leuconostoc mesenteroides, S. lactis)의 생육과 산생성에 거의 영향을 미치지 않았으나 L. acidophilus 2종의 산생성을 억제시켰으며, 같은 L. acidophilus 중에서도 KFCC 12731은 AKU 1122보다 더 큰 영향을 받는 것으로 나타났다. 두유에 첨가된 DL-methionine은 L-methionine과 동일하게 L. acidophilus(KFCC 12731)의 산생성을 현저하게 억제시켰으나, D-methionine은 높은 농도에서도 생육과 산생성에 전혀 영향을 미치지 않았다. 한편 L-cysteine에 의하여 L. acidophilus (KFCC 12731)의 산생성이 다소 억제되었으나 그 정도는 L-methionine이 비하여 매우 완만한 것이었다.

Components of human breast milk: from macronutrient to microbiome and microRNA

  • Human breast milk (HBM) is essential for the infant's growth and development right after birth and is an irreplaceable source of nutrition for early human survival. Various infant formulas have many similarities to HBM in many components, but there is no perfect substitute for HBM. Recently, various breast milk components and their roles have been studied according to the development of various analysis techniques. As is already well known, HBM contains about 87%-88% water, and 124-g/L solid components as macronutrients, including about 7% (60-70 g/L) carbohydrates, 1% (8-10 g/L) protein, and 3.8% (35-40 g/L) fat. The composition may vary depending on the environmental factors, including maternal diet. Colostrum is low in fat but high in protein and relatively rich in immuneprotective components. Although HBM contains enough vitamins to ensure normal growth of the infant, vitamins D and K may be insufficient, and the infant may require their supplementation. Growth factors in HBM also serve as various bioactive proteins and peptides on the intestinal tract, vasculature, nervous system, and endocrine system. In the past, HBM of a healthy mother was thought to be sterile. However, several subsequent studies have confirmed the presence of rich and diverse microbial communities in HBM. Some studies suggested that the genera Staphylococcus and Streptococcus may be universally predominant in HBM, but the origin of microbiota still remains controversial. Lastly, milk is the one of most abundant body fluid of microRNAs, which are known to play a role in various functions, such as immunoprotection and developmental programming, through delivering from HBM and absorption by intestinal epithelial cells. In conclusion, HBM is the most important source of nutrition for infants and includes microbiomes and miRNAs for growth, development, and immunity.
