Geographic Analysis of Neurosurgery Workforce in Korea

  • Objective : In respect of the health and safety of the public, universal access to health care is an issue of the greatest importance. The geographic distribution of doctors is one of the important factors contributing to access to health care. The aim of this study is to assess the imbalances in the geographic distribution of neurosurgeons across Korea. Methods : Population data was obtained from the National Statistical Office. We classified geographic groups into 7 metropolitan cities, 78 non-metropolitan cities, and 77 rural areas. The number of doctors and neurosurgeons per 100000 populations in each county unit was calculated using the total number of doctors and neurosurgeons at the country level from 2009 to 2015. The density levels of neurosurgeon and doctor were calculated and depicted in maps. Results : Between 2009 and 2015, the number of neurosurgeons increased from 2002 to 2557, and the ratio of neurosurgeons per 100000 populations increased from 4.02 to 4.96. The number of neurosurgeons per 100000 populations was highest in metropolitan cities and lowest in rural areas from 2009 to 2015. A comparison of the geographic distribution of neurosurgeons in 2009 and 2015 showed an increase in the regional gap. The neurosurgeon density was affected by country unit characteristics (p=0.000). Conclusion : Distribution of neurosurgeons throughout Korea is uneven. Neurosurgeons are being increasingly concentrated in a limited number of metropolitan cities. This phenomenon will need to be accounted when planning for a supply of neurosurgeons, allocation of resources and manpower, and the provision of regional neurosurgical services.

  • Background: Despite increasing the number of newly licensed nurses across Korea, shortages caused by geographical imbalances remains a significant concern. Therefore, understanding nursing students' attitudes to working and living, factors influencing where they first choose to work after graduation is useful in formulating appropriate interventions to retain nurses in regional areas. Methods: A total of 329 senior nursing students from areas outside Metropolitan Seoul completed self-report questionnaires. Data were analyzed using t-test, chi-square test and multiple logistic regression analysis. Results: Of the respondents, 57.8% reported that they planned to work in the region in which their school was located. The three factors ranked as having the greatest influence on their decision to work in non-metropolitan regions were: the cost of living, housing costs, and the proximity to family. Enjoyable aspects of rural life contributed positively to students' intentions to work in non-metropolitan regions, whereas isolation and socialization problems negatively affected their intentions to work in such areas. Conclusions: Greater consideration should be given to improving working conditions and housing environments in non-metropolitan regions.

A Survey on Prevalence of S.T.D for Prostitutes in Seoul District - About Young Dung Po and Sung Buk Areas- ('윤락여성의 성병감염에 관한 조사' -영등포 및 성북지역을 중심으로-)

  • The incidence of Sexually Transmitted Disease has risen despite the progress in its diagnosis and treatment methods. For the effective control programs of S.T.D it should be required medical and social approaches. This survey was conducted in Young Dung Po and Sung Buk areas from June, 1986 to September, 1986. The 1185 prostitutes were studies in order to determine the prevalences of sexually transmitted disease. The results were as follows: 1. The average age of prostitutes was $24.0\pm3.9$ years. The age group of 21-25 years old was the highest level of about $70.3\%$. 2. The positive rate on non Gonococcal urethites was $9.1\%$ at Sung Buk area and $15.3\%$ at Young Dung Po area. 3. The positive rate of Neisseria Gonorrhoeae on culture of TM media was $6.4\%$ or total level and incidence ratio of Neisseria Gonorrhoeae: Non Gonococcal urethritis was 1: 1.7. 4. The positive rate of PPNG was occupied about $26.3\%$ among the total NG positive case. 5. The positive rate of VDRL Slide Test was $2.9\%$, and antibody Titer level of 1:2 was $38.8\%$. it was the highest of all Titer distribution.

The Impact of Regional Characteristics on Martial Status and Childbirth (지역 특성이 결혼 및 자녀 출산에 미치는 영향)

  • The purpose of this study is to identify the difference of regional characteristics affecting 'marriage stage' and 'childbirth stage' and to compare how these factors differ between metropolitan and non-metropolitan areas using a heckman two-stage selection model. The main findings are as follows. First, regional characteristics affecting marriage stage and childbirth stage are different. Second, in the metropolitan area, regional characteristics such as population density, the proportion of female employee, and housing prices have a relatively greater impact on 'marriage', and in the non-metropolitan area, regional characteristics such as population density and unemployment rate have more impact on 'childbirth'. The empirical analysis suggests that marriage and childbirth stage need to be distinguished when establishing low fertility policies, and these approaches should be differentiated at regional level.

  • This article measures equal weighted voting to evaluate the fairness of a redistricting system operated in the $4^{th}$ to the $6^{th}$ metropolitan council elections using a new index. The cosine square index using in the article would be useful on what we see the ratio of the equality of population among metropolitan regions and the fairness of the whole electoral system, along with its simple calculation. The results of the fairness of the $4^{th}$ to the $6^{th}$ metropolitan council elections calculated by the cosine square index are as follow: Because the $4^{th}$ metropolitan council election uniformly elects two members for each electoral district regardless of the size of the population, it has a low equality of population between districts. But as a result of the decision of the Constitutional Court in 2007, standard of population variation in electoral district has been strengthened to 4 : 1 from the $5^{th}$ metropolitan council election. It increases significantly equality of population between districts from the $5^{th}$ metropolitan council election. But in the elections from the $4^{th}$ to the $6^{th}$ metropolitan council elections, Rural electoral districts continuously show the lowest equality of population between districts. It also shows the growing disparity between urban and rural areas as well as between capital and non-capital. This paper emphasizes that electoral apportionment in local council elections should reflect regional diversity and community identity.

Trends of Alcohol Attributable Mortality in Korea: 1995-2000 (한국의 음주기인 사망수준의 변화 : 1995-2000)

  • Although alcohol misuse contributes substantially to mortality from diseases, injuries and adverse effects, a few attempts have been made to figure out size of adverse consequences attributable to alcohol in Korea. This study was conducted to describe trends of estimated deaths attributable to alcohol in Korea. Estimations were made by employing Korean alcohol aetiological fraction(AEF) into deaths from alcohol-related diseases, injuries, and adverse effects from year of 1995 through 2000. Korean AEF was derived from previous studies on AEF applied to USA and Canada (Schultz et al.,1991; English et al., 1995) with reflecting peculiar drinking patterns in Korea. An average number of deaths attributable to alcohol was 21,123, accounting for 8.76% of all deaths reported to National Statistical Office during the period. Death rates attributable to alcohol tended to decrease from year of 1995 to 1997 and then increased with peak at year of 1999. Sex-age standardized alcohol attributable death rates varied among areas, with those of metropolitan areas being lower than those of non metropolitan areas. Years of potential life lost (YPLL) were estimated to reflect qualitative aspect of deaths attributable to alcohol. Similar change patterns during the year were observed between number of deaths and YPLL. Average YPLL of men was longer than that of women by about 4 years. Some implications for future study have been discussed.

Spatial-Temporal Pattern Analysis of Unplanned Factory Locations in the Seoul Metropolitan Area Using FEMIS Data (수도권 개별입지 공장의 시공간적 입지특성 분석)

  • The purpose of this study is to analyze the spatial-temporal proliferation of individual factories in the Seoul Metropolitan Area and to provide policy implications. The location of the factory is divided into individual locations and planned locations. In the case of individual locations, it takes a long time to establish factories. Also, due to high cost and regulation, the establishment of individual factories is not easy. However, since the establishment process is simple and the price of land is low, the establishment of individual factories has led to an increase in the number of individual factories. The problem of the undeveloped factories is that the lack of infrastructure such as the road and environmental pollution treatment facilities around the factory deteriorates the pleasant environment and the cityscape and deteriorates the health of the residents in the surrounding area. In this study, we analyzed the location of individual factories established in the Seoul metropolitan area from 2001 to 2016 by using ArcGIS Pro. The results of the analysis are as follows. First, individual location factories are formed around existing industrial complexes and industrial sites. The reason for this is considered to be the external effect that can be obtained from the surrounding area. Secondly, since the Seoul city's individual location factories are established in many residential areas, it shows the conflicting result of the mixture of residence and factory. Third, the Gyeonggi province's individual location factories have a high proportion of non-urban areas. This is because the Gyeonggi province's management area occupies a larger proportion than other areas in Gyeonggi province. This study analyzed the spatial - temporal spreading process of individual factories and the unfolding of individual factories in the metropolitan area, and it can provide policy implications to control the over development of individual factories in the future.

The Impact of Moving into an Industrial Park on a Company's Management and Innovation Performance : Comparing Capital Region to Non-Capital Region (산업단지 입주여부가 기업의 경영·혁신 성과에 미치는 영향 분석 : 수도권과 비수도권 간 비교를 중심으로)

  • This study analyzed the effect of moving into an industrial park on the performance of a company using data from individual companies. In addition, regional variables were additionally set, and industrial complexes were divided into metropolitan and non-metropolitan areas for comparison. Data were collected through KIS-2022 (manufacturing) to verify the hypothesis. In the case of the analysis method, multiple regression analysis and Propensity Score Matching(PSM) were first used. The analysis results are as follows. First, it was found that companies that moved into industrial park had a positive effect on innovation performance than companies that did not move in. Second, in terms of financial performance, there was no statistically significant difference between companies that moved into the industrial park and those that did not. Third, there was a significant difference between companies that moved into industrial park and those that did not, depending on the metropolitan and non-metropolitan areas. Based on these analysis results, policy and academic implications could be presented.

A Study on the Technology Transfer Efficiency for Public Institutes Using DEA Model (DEA 모형을 이용한 공공연구기관의 기술이전 효율성 분석에 관한 연구)

  • This study measured technology transfer efficiency for public institutes. The study made use of DEA being one of the non-parametric linear programming to evaluate technology transfer efficiency for public institutes and to measure technology efficiency, pure technical efficiency and scale efficiency. The measurement of the technology transfer efficiency for public institutes was as follows: The cause of the technology transfer inefficiency was affected by pure technical inefficiency more than by scale inefficiency. Public institutes' RTS(Return To Scale) value varied depending upon the features of the organizations than the features of the regions. Public research institutes' RTS value is more effective than universities' RTS value. We compared the RTS group with the RTS of Projected DMU groups. The RTS group had constant returns to scale effect while the RTS of the Projected DMU had increasing returns to scale effect. The technology transfer efficiency of public institutes varied depending upon the features of the organizations and regions : The technology transfer efficiency of public institutes were as follows : public research institutes at the metropolitan area, public research institutes at the local areas, universities at the metropolitan area and universities at the local areas. In other words, the technology transfer efficiency was affected by organizational characteristics more than by regional characteristics at the place where public institutes were located.

Simulating the Impacts of the Greenbelt Policy Reform on Sustainable Urban Growth: The Case of Busan Metropolitan Area

  • The greenbelt of South Korea has been under the process of adjustment and removal since its first designated year. This research is aimed at predicting the effect that the removal of the greenbelt has on urban growth. The SLEUTH model was executed via three calibration phases using historical data between 1990 and 2010. The urban growth of Busan Metropolitan City was predicted under its historical trend, as well as two different scenarios including development and compact development up to the year 2030. The accuracy of model, as verified by ROC, was 85.7%. The historical trend scenario showed the smallest increase, with the urban area expanding from 175.96 km2 to 214.68 km2 in 2030. Scenario 2, the development scenario, showed the most increase, with a 39.9% growth rate from 2010 to 2030. However, according to scenario 3, the compact development scenario, the urban area decreased in comparison to scenario 2. Accordingly, it is necessary to have effective urban growth management to provoke eco-friendly development on the removed areas, and to strengthen the non-removed areas for sustainable development. The results obtained in this study showed that the SLEUTH model can be useful for predicting urban growth, and that it can help policy makers establish proper urban planning as a decision-support tool for sustainable development.
