암생존자의 주관적 구강건강 및 정신건강이 건강관련 삶의 질(EQ-5D)에 미치는 영향: 제8기(2019-2020년) 국민건강영양조사를 바탕으로 (Effects of subjective oral and mental health on health-related quality of life (EQ-5D) in cancer survivors : The 8th Korea National Health and Nutrition Examination Survey (8th KNHANES, 2019-2020))

  • Objectives: This study aimed to prepare basic data to improve the health-related quality of life of cancer survivors by confirming their oral and mental health statuses and identifying factors affecting their health-related quality of life. Methods: Of all participants in the 8th KNHANES (2019-2020), adults aged 19 years or older who responded 'yes' to the diagnosis of cancer and 404 cancer survivors who responded 'none' to the current cancer prevalence item were selected as the final research participants. Multiple regression analysis was conducted to confirm the effect of cancer survivors' oral and mental health on health-related quality of life. Results: Subjective oral health (p<0.01), chewing problems (p<0.05), subjective health (p<0.001), and depression (p<0.01) had an effect on health-related quality of life from multiple regression analyses. Conclusions: Therefore, oral and mental health promotion may improve health-related quality of life. Thus, it is necessary to recognize the importance of oral and mental health and implement preventive education and programs.

근로자의 정신건강관리를 위한 정신건강 관련 삶의 질에 대한 통합적 고찰 (An Integrative Review on Mental Health-related Quality of Life for the Mental Health Management of Workers)

  • Purpose: Despite the importance of mental health-related quality of life among workers, its comprehensive understanding is lacking. This study aimed to identify influencing factors regarding mental health-related quality of life through a comprehensive literature review. Methods: The integrative review method of Whittemore and Knaf was used. Data search was conducted on papers published before September 2020, and RISS, NDSL, and KISS were used for the search. Results: Fourteen studies were selected for analysis. Five factors affecting the mental health-related quality of life of workers were found. Mental and physical health status, job stress, working environment, health behaviors, and other personal and socio-economic characteristics were significant influencing factors. Conclusion: Findings from the review suggest the necessity of developing a customized intervention program to improve the mental health-related quality of life of workers.

심방세동 환자의 건강 관련 삶의 질 영향요인 (Factors Influencing Health-related Quality of Life in Patients with Atrial Fibrillation)

  • Purpose: The purpose of this study was to investigate the factors that influence health-related quality of life in patients with atrial fibrillation. Methods: The subjects were 150 outpatients with atrial fibrillation who visited the cardiology clinic of a university hospital in U city. The instruments used for this study were Mhel Uncertainty in Illness Scale (MUIS), Center for Epidemiologic Studies-Depression Scale (CES-D), State Trait Anxiety Inventory (STAI), and the Short-Form-36 Health Survey (SF-36) Korean version II. The date were analyzed by ANOVA, Pearson-correlation coefficient, and hierachial multiple regression using SPSS/WIN 18.0. Results: The mean score of physical health-related quality of life (PCS) was $38.92{\pm}6.22$ and mental health-related quality of life (MCS) was $41.49{\pm}5.71$. Physical and Mental health-related quality of life had the significant correlations with uncertainty, anxiety and depression. In multiple regression analysis, physical health-related quality of life was significantly influenced by duration of disease, NYHA class, uncertainty. Mental health-related quality of life was significantly influenced by family income, NYHA class, anxiety and depression. Conclusion: These results suggest that these influencing factors should be consider in developing the nursing interventions to improve the healthrelated quality of life in patients with atrial fibrillation.

청년 1인 가구의 건강 관련 삶의 질 영향요인: 회복탄력성의 매개효과를 중심으로 (Factors influencing health-related quality of life for young single-person households: the mediating effect of resilience)

  • Purpose: To identify factors influencing health-related quality of life for young single-person households, this study investigated physical and mental health status, health behavior, depression, resilience, and health-related quality of life. Methods: An online survey was administered to members of young single-person households from March 22 to 30, 2022. The data were analyzed using the chi-square test, independent t-test, one-way analysis of variance, Pearson correlation coefficients, multiple regression, and a simple mediation model applying the PROCESS macro model 4 with 95% bias-corrected bootstrapped confidence intervals. Results: The participants were 229 members of young single-person households. Health-related quality of life showed significant relationships with residence (t = 2.80, p = .006), month (F = 3.70, p = .026), mental health status (F = 20.33, p < . 001), and high-intensity exercise (F = 7.35, p = .001) among general and health-related characteristics. Health-related quality of life had significant correlations with depression (r = -.72, p < .001) and resilience (r = .58, p < .001). Multiple regression analysis showed that depression (β = -.57, p < .001) and resilience (β = .21, p < .001) influenced health-related quality of life. Moreover, resilience had a mediating effect between depression and health-related quality of life (indirect effect = -0.002, 95% bias-corrected bootstrapped confidence interval = -0.003 to -0.001). Conclusion: Members of young single-person households tended to be more vulnerable to emergency situations, such as during the coronavirus disease 2019 pandemic, when lockdowns and quarantines were frequent. To improve health-related quality of life in young single-person households, people with high levels of depression or low levels of resilience need special attention and support to promote mental health.

한국노인의 구강건강행위 및 상태가 정신건강과 삶의 질에 미치는 영향 (Effects of Oral Health Behavior and Status of Elderly People in Korea on Mental Health and Quality of Life)

  • This study conducted a secondary analysis using original data of performed by Korea Institute for Health and Social Affairs to determine factors affecting oral health-related quality of life and mental health in the elderly. The research subjects were 1,217 people who were over 65 years old and completed an oral examination and oral questionnaire survey. The results were as follows. First, mental health problems arise in people with an oral health status designated as 'bad' or when they feel 'very uncomfortable' chewing or speaking. Second, the quality of life decreases as problems with chewing and speaking increase. The results reveal that to reduce problems of mental health(stress, depression, suicide) and to enhance the quality of life, there is a need to develope diverse programs for preventive treatment and oral health education. What is the most important is consistent policy support.

성인의 생애주기별 정신건강 관련 삶의 질 영향요인 (Factors Affecting the Mental Health related Quality of Life in Adults across the Lifespan)

  • Purpose: The purpose of this study was to identify the influencing factors on mental health related quality of life (MHRQoL) in adults across the lifespan. Methods: A total of 688 Korean adults aged 19 years and older were selected. Data were collected by personal interviews or self report using structured questionnaires. For data analysis, t-test, ANOVA, Pearson's correlation coefficients and Stepwise multiple regression were used. Results: Mental health related quality of life (MHRQoL) differed significantly according to life cycle, especially showing a lower score in the elderly than others. In addition, elderly adulthood reported the highest stress and depression. The most significant predictors of MHRQoL in young and middle aged people were stress, depression, and subjective health status. Predictors of MHQoL in the elderly were stress and religion. Conclusion: The results indicate that MHRQoL of adult is associated with stress and depression. When the programs are developed to enhance mental health in elderly adulthood, religion should be considered as well as stress.

Influencing Factors on Health-related Quality of Life among Japanese Middle-aged Marriage-based Immigrant Women in South Korea: A Cross-Sectional Study

  • Purpose: With the first generation of marriage-based immigrant women in East Asia now reaching their middle or old age, the need to focus investigations on their health-related quality of life has arisen. This study aimed to examine the extent to which physical and mental health, and psychosocial variables can predict health-related quality of life among Japanese middle-aged immigrant women. Methods: This study has a descriptive cross-sectional design. A convenience sample of 197 Japanese middle-aged marriage-based immigrant women from two regions of South Korea were recruited between December 2017 and March 2018. Participants completed self-administered questionnaires on health-related quality of life, menopausal symptoms, depression, perceived health status, disease morbidity, social support, and acculturation. The data were analyzed using hierarchical multiple regression. Results: Depression was the strongest predictor of health-related quality of life, followed by perceived health status, social support, and household income. Menopausal symptoms, presence of disease, and acculturation appeared to have no additional impact on participant's health-related quality of life. Conclusion: In times of rapid growth of global migration and the aging of immigrants in new destination countries, nursing interventions and public health policies for aging marriage-based immigrant women should be prioritized to improve their mental health by facilitating social support and disease management. In addition, social and employment policies that can help immigrant women transition to a healthy midlife are needed.

양측 슬관절 전치환술 여성노인의 건강관련 삶의 질 구조모형 (A Structural Equation Model of Health-Related Quality of Life among Older Women Following Bilateral Total Knee Replacement)

  • Purpose: This study aimed to develop and test a structural equation model of health-related quality of life among older women following bilateral total knee replacement based on a literature review and Wilson and Cleary's model of health-related quality of life. Methods: One hundred ninety three women who were diagnosed with osteoarthritis, were older than 65 years, and were between 13 weeks and 12 months of having a bilateral total knee replacement were recruited from an outpatient clinic. Data were collected from July 2017 to April 2018 using a structured questionnaire and medical records. Data were analyzed using SPSS/WIN 22.0, AMOS 22.0, and Smart PLS 3.2.4. Results: The fitness of the hypothetical model was good, with coefficients of determination (R2) ranging between .28 and .75 and predictive relevance (Q2) between .26 and .73. The standardized root mean square residual of the model fit indices for the hypothetical model was .04; which explained 64.2% of physical and 62.5% of mental health-related quality of life. Self-efficacy, symptom status, functional status, and general health perceptions had a significant direct effect on physical health-related quality of life, while social support, symptom status, and general health perceptions had a significant direct effect on participants' mental-health-related quality of life. Conclusion: To improve the physical and mental quality of life of older women who receive bilateral knee replacement, nursing-based intervention strategies that reduce symptoms, improve functional status, and increase health perceptions, self-efficacy, and social support are needed. The most important factor is the symptom status.

일개 지역사회 주민의 삶의 질 영향 요인 (Factors Influencing the Quality of Life of Community Residents)

  • 본 연구는 경기도 소재 일개 시의 지역사회 주민 509명을 대상으로 삶의 질 영향 요인을 파악하기 위한 목적으로 수행하였다. 구조화된 설문지를 통하여 일반적 특성, 우울, 정신건강수준 및 삶의 질 수준에 관한 자료를 수집하였고, 자료분석은 SPSS20.0 통계프로그램을 이용하였다. 연구결과 첫째, 연구대상자의 일반적 특성에 따른 삶의 질 차이는 성별, 연령, 학력, 결혼상태, 월 총수입, 스트레스 상태에서 통계적으로 유의한 차이가 있었다. 둘째, 우울과 정신건강수준의 세부요인인 전반적 정신건강, 사회적 부적응, 불안/우울감 정도에서 유의한 차이가 있었고, 상관관계를 나타냈다. 셋째, 삶의 질에 영향을 미치는 요인으로는 연령, 스트레스 상태 및 정신건강수준의 세부요인인 불안/우울감 정도 이었으며 선행변수들의 설명력은 47.8%이었다. 본 연구결과는 향후 지역사회 주민들의 삶의 질을 향상시키기 위한 중재와 관리프로그램의 적용방안을 모색하는데 있어 바탕이 되는 자료가 될 것으로 사료된다.

Health-related Quality of Life in Elderly Asian American and Non-Hispanic White Cancer Survivors

  • Objectives: The purpose of this study was to assess predictors of health-related quality of life (HRQoL) in elderly Asian American and non-Hispanic White cancer survivors. Methods: We conducted cross-sectional secondary data analyses using the combined datasets from the Surveillance, Epidemiology, and End Results program and the Medicare Health Outcomes Survey. Results: Elderly Asian American cancer survivors reported a lower mental HRQoL but a comparable physical HRQoL relative to elderly non-Hispanic White cancer survivors. Stress factors, such as comorbidities, difficulties with activities of daily living, and a history of depressive symptoms, along with coping resources like self-rated health and the ability to take the survey in English, were significantly associated with mental and physical HRQoL. Among elderly Asian American cancer survivors, a significantly lower mental HRQoL was observed among those taking the survey in the Chinese language. Conclusions: The findings suggest that race exerts a differential impact on HRQoL. Interventions should be designed to address the distinct cultural, linguistic, and systemic needs of elderly Asian American cancer survivors. Such an approach could assist in reducing cancer-related health disparities.
