국내 지역사회 거주 만성질환 노인 대상 자기관리 프로그램의 효과 및 측정도구에 대한 고찰 (The Effects of Self-Management Program and Its Measurement for the Elderly with Chronic Illness in the community)

  • 본 연구는 지역사회에 거주하는 만성질환을 가진 노인을 대상으로 하여 자기관리 프로그램의 효과 및 사용하는 측정도구가 무엇인지 확인하는데에 목적을 두고 있다. 문헌검색을 위해 사용된 데이터베이스는 Google Scholar, RISS, Dbpia였으며, PICO 기준틀을 기반으로 연구질문을 선정하였다. 2010년 1월부터 2019년 9월까지 발표된 문헌을 검색하여 포함기준과 배재기준을 적용하여 최종 6편의 문헌을 선정하였다. 연구결과, 선정된 문헌은 Level 1-2의 질적수준을 가지고 있었다. 프로그램의 일반적 특성에서는 간호사가 가장 많이 프로그램을 진행 하였으며, 프로그램은 노인 50명 이하를 대상으로 운영되었다. 또한, 주로 보건소와 노인정, 경로당에서 프로그램이 진행되었다. 연구대상자는 고혈압을 가진 만성질환자를 대상으로한 문헌이 가장 많았으며, 프로그램의 종속변수는 모든 문헌에서 인지적 영역을 다루었고, 그 뒤로 신체적 영역을 종속변수로 측정한 문헌이 많았다. 본 연구결과는 지역사회에 거주하는 만성질환을 가진 노인에게 자기관리에 대한 중재의 효과성을 제공하고, 지역사회에 만성질환을 대상으로 하는 프로그램 개발에 대한 필요성을 강조한다. 또한, 노인의 다양한 인지적, 신체적, 정신적, 사회적, 삶의 질의 영역에 관련된 측정도구를 제시하며 다학제간 연구의 필요성을 제시힌다.

노인요양시설에 있는 노인환자의 구강실태 및 치료수요도 (Oral health status and treatment need of institutionalized elderly patients)

  • 문제제기: 노인요양시설은 치과진료영역 가운데 가장 취약한 상태에 놓여 있고 신체적, 정신적 장애로 인해 주관적 치료요구도를 파악할 수 없는 상태지만 이에 대한 기초적인 실태조사가 전무한 상태이다. 목적: 본 연구는 노인요양시설에서 노인환자들의 치아우식증에 의한 구강실태를 조사하고, 기능치아와 보철물로 이루어진 교합단위를 분석하여 그에 따른 치과치료수요도를 분석하는데 목적이 있으며 향후 적절한 진료방향을 수립하는데 기여하고자 한다. 연구재료 및 방법: 노인요양시설에서 758명의 노인환자와 D치과의원에 내원한 212명의 65세 이상인 노인을 대상으로 2000년 국민구강건강실태조사에서 작성된 치아우식검진지침에 입각하여 구강실태조사를 시행하였다. 결과: 노인요양시설의 노인에게서 고령으로 갈수록 대조군에 비해 악화되고 있는 구강상태를 알 수 있었고 매우 높은 치료수요를 파악할 수 있었다. 고찰: 노인환자의 구강건강관리를 위해서는 무엇보다도 이들을 보살피는 간병인과 간호 인력들이 가지고 있는 구강건강의 중요성에 대한 인식을 바꾸기 위한 노력이 절실히 필요한 상황이다. 이제는 무치악 환자에게 시행되고 있는 일회성 의치사업 일변도의 복지정책방향을 보건복지부와 일선 지방자치단체에서 사후관리를 원칙으로 한 전문가에 의한 무치악 환자의 의치사업으로 강화해야 하며 유치악의 노인환자들에 대한 정기적인 치과검진을 시행하고 이동진료 및 왕진시스템이 보완된 구강건강관리 및 보존, 수복진료 형태로의 전환을 고려할 시점이라 생각된다.

간호 교육에 대한 일 연구 -2년제 초급대학 과정 중심으로- (A Study of The Nursing Education Concerning Two Years Associate Degree Nursing Program)

  • 1. The purpose of this study. The purpose of this study is to plan and investigate short-term nursing education of two-years associate degree program to produce middle-level professional nurses which are needed by-society and nation. Current nursing education in Korea is divided into four years degree program, three years diploma program Even (though) there are differences in the aims of their education, the curriculums are not much different between the education for producing leaders which is its basic purpose and training middle-level professional nurses. Therefore the purpose of associate degree program lies in minimizing the waste of time and finance which are invested for long-term education for middle-level professional nurses. And also this coincide with the policy of national technical training and definite supply of nurse manpower according to health policy for effective role and ability of nurse. 2. The method of study. This is based on the study of literature, research on the actual condition and investigation of opinion- through questionnaire. L) The study of literature: Domestic and foreign literatures for two years associate degree program were studied and investigated. 2) Research on the actual condition : Current three years nursing education program was collected and analysed. 3) Investigation of opinion. The problem of curried nursing education system and the possibility of two years associate degree program were investigated through questionnaire. 3. The result of the study. 1) The trend of recent nursing education. a. The aims of nursing in past chiefly taking care of physical disease of patient has recently changed to nursing of character including physical, mental, socio-economic, educational and psychological condition. b. For the performing systematic and effective nurse's duty according to her role, the-change of educational system which is classified as a range of education the period of education and certificate after graduation has been enforced or fulfilled. c. Nursing education also has a trend to become a collage or two years associate degree program which can get same legal protection as other educational institutions whose basic purpose is education. Attached nursing school to hospital is getting disappeared because of disadvantage of educational system. 2) Problems. Depending upon research on actual condition of current 3 years nursing education program. a. There are too many subjects. b. Contents of education could be doubled because major subjects are subdivided in detail. c. The credits for graduation are too heavy comparing to the period of study or the ability of students. (The necessary credits are 150.8 for three years according to actual investigation 4. There is no certain standard in organizing curriculum therefore there are too much differences between schools. 4. Basic Plan. The plan for two years associate degree program in nursing education depending on demand of professional nursing field of society is based on following items. 1) Training middle-level professional nurse lay emphasis on liberal arts and basic major field. 2) Liberal arts are divided into required and optional subjects and students could take courses by choice. 3) Major subjects are compound together by fields and they become the sciences of nursing Ⅰ,Ⅱ,Ⅲ,Ⅳ and every items has its educational purposes and contents major study includes laboratory practice and clinical experience. 4) The required credits for graduation are to which means 17-18 credits a semester. The above has been planned to solve the problems of current three years nursing education program. In conclusion for the achievement of this system, following items are needed. 1) It is necessary to change educational administration and system such as amendment of educational law or order of educational application of law. 2) Qualified professors should be available to understand and develope the idea or purpose of this educational system. 3) Local medical institutions should be opened widely and educational for clinical training. 4) The job after graduation should he secured positively.

문제음주자 자녀에 관한 국내 간호연구의 통합적 고찰 (An Intergrative Review on Korean Nursing Studies Related to Problematic drinker's children)

  • 본 연구는 국내 간호학 분야에서 보고된 문제음주자 자녀에 관한 연구를 살펴보고 그 의미 있는 결과를 통합함으로써 문제음주자 자녀들의 특성을 고찰하고자 시행되었다. 연구방법은 통합적 문헌고찰이며 연구대상으로는 2000년부터 2014년까지 3개의 검색 데이터베이스에서 '문제음주', '알코올중독', '알코올의존' '자녀', '간호'로 각각 조합하여 검색 후 선정된 11편의 논문이다. 연구결과는 다음과 같다. 문제음주자 자녀에 관한 국내 간호연구는 설문조사 방법을 이용한 서술적 조사연구가 많았는데 문제음주자 자녀들의 특성과 삶을 깊이 알 수 있는 다양한 연구 방법의 도입이 필요하다. 문제음주자 자녀의 주요 특성은 일반가정 자녀들 보다 극복력이 낮고, 정신건강문제가 많으며 대인관계에서도 예민하고 적대감도 많았다. 또한 문제음주자 자녀에게서 문제음주가 더 발생하고 우울수준도 높으며 정서인식 및 표현, 사회적지지, 사회능력 등이 낮게 보고 되었다. 본 연구 결과를 바탕으로, 문제음주자 자녀에 관한 연구가 극히 미비한 것을 고려해 볼 때 앞으로 더 활발한 간호연구가 필요하겠고, 정확한 결과를 도출한 연구를 함으로써 임상에서나 지역사회에서 간호사의 역량을 발휘할 수 있는 지침이 될 것이다.

정신질환자의 성지식, 성태도 및 성행동 (The sexual knowledge, sexual attitude and sexual behavior of psychiatric patients)

  • 본 연구는 정신질환자의 성지식, 성태도 및 성행동 정도와 차이를 파악하고 변수 간 관계를 파악하기 위해 시도되었다. 연구대상자는 J도 K시에 소재한 정신전문병원에 입원한 성인 정신질환자 222명이었다. 자료는 구조화된 설문지를 사용하여 2016년 3월 11일부터 4월 15일까지 수집되었다. 수집된 자료는 SPSS/WIN 20.0 프로그램을 이용하여 independent t-test, One-way ANOVA, $Scheff{\acute{e}}$ test, Pearson's Correlation Coefficients로 분석하였다. 연구 대상자의 성지식 평균점수는 2.60점, 성태도는 2.74점, 성행동은 0.49점이었다. 대상자의 성지식은 부모의 양육태도에 따라 유의한 차이를 보였고(${\rho}$<.01), 성행동은 진단명에 따라 유의한 차이를 나타났다(${\rho}$<.05). 성지식은 성태도(r=.75, ${\rho}$<.001) 및 성행동(r=.15, ${\rho}$<.05)과 유의한 양의 상관관계가 있었다. 성태도는 성행동 하부영역 중 적극적 성행동 간에 유의한 양의 상관관계가 있었다(r=.20, ${\rho}$<.01). 본 연구결과는 정신질환자의 성 관련 문제에 관한 정신간호중재방법의 개발에 기초자료로 활용될 수 있을 것이다.

세브란스 호스피스 추후관리 프로그램의 효과에 관한 연구 (A STUDY OF THE EFFECTIVENESS OF THE BEREAVEMENT PROGRAM OF SEVERANCE HOSPICE)

  • Grief that is not acknowledged and worked through may manifest itself in some emotional, mental or physical problem. In recent years much has been learned about coping with grief which the hospice program can utilize to help family members cope with their grief. This study was carried out to determine the helpfulness of the bereavement care of Severance Hospice and to learm more about the grief response of the bereaved. The tools used to collect data were an assessment form used in the bereavement program and the Grief Experience Inventory developed by Sanders and revised and translated 'by the researcher. Data was obtained from bereaved family members(54 for the final grief assessment and 39 for the grief response assessment) receiving bereavement follow-up, from July 1989 to March 1991. Results of the study were as follows: 1. Final Grief Assessment Regarding the resolution of their grief the majority of the bereaved accepted the reality of the death of their family member, while slightly more than three-quarters were able to express their feelings toward their loss. A large majority had returned to activities of daily living well or fairly well and had reinvested their energy in a person other than the deceased. In addition, the physical condition of the majority was good or fairly good. A majority of the bereaved considered the bereavement care to be helpful and almost three-quarters were not considered to be in need of more follow-up. 2. Grief Response Assessment Age was found to have a modoerately positive correlation to appetite disturbance(r=.41, P<.Ol) and loss of vigor(r=.37, P<.Ol) A moderately positive correlation was found between the number of contacts and sleep disturbance(r=2.38, P<.01) Significant differences were found between men and women in regard to guilt(t=2.38, P<.05), social isolation(t=2.44, P<.05) and depersonalization(t=2.07, P<.05) with men having the more intense grief. Significant differences were found in the grief responses of somatization(F=5.82, P<.001), physical symptoms(F=5.87, P<.OOl), appetite disturbance(F=4.40, P<.Ol), despair(3.79, P<,Ol), anger(Fp2.83, P<.05), social isolation(F=3.61, P<.05), guilt(F=3.62, P<.05) and depersonalization (F = 2.58, P <.05). In the first six of these grief responses mothers scored highest, followed by husbands and then wives, In the grief response of guilt, daughters scored highest and on the grief response of depersonalization sons scored highest. Only one grief response, that of sleep disturbance(t= -2.19, P<.05) was found to be statistically significant, with those family members who died at home having the higher scores. Based on the results of this study several suggestions are presented as follows: 1. Since unresolived grief can have a detrimental effect on the bereaved person's mental and phys. ical health it would be good for the nurse, to include questions related to death of family members and the bereaved person's response to the grief, in her nursing assessment. And in the case of unresolved grief the nurse should encourage the person to talk with a trusted friend or counselor and express their fellings of grief. 2. A study to determine the degree of resolution of the grief of those in the bereavement program could be carried out by use of the Grief Experience Inventory early in their bereavement and again 13 months after the death of their family member. 3. A comparison of the grief response of the bereaved in the bereavement program and bereaved not in the program could be carried out using the Grief Experience Inventory. 4. After bereavement programs have been started in other hospice programs it would be good to carry out a joint study of bereavement outcomes of those in the bereavement programs.

청소년의 소진 경험에 관한 연구 -고3 수험생을 중심으로- (A Study Exhaustion Experiences in Adolescents - Samples of 12th Grade Examinees-)

  • This study identifies a basic framework to analyze specific experiences of examinees so that which nurses are better able to understand reactions of examinees and, further, to provide better nursing treatments to them. This study follows Strauss & Corbin's analysis tool (1990). This basic framework identifies the specific mental reactions in a process away from the "being tightened" state. The main factors explaining this process include the 28 following variables : anxiety, being burdened, being suppressed, lack of agency, being suffocated, term pressure, expectation pressure, grade pressure, firmness, feebleness, helping others, being disturbed, watching others, off ficiousness, staightening on, depending on, getting along with, getting out of, shooting out, giving up, being alone, devoting, bracing up, being industrious, being harassed, being distressed, troubles, and quiting. Twelve variables are factored from the above factors which include pressure, being tighened, terms, expectations, grades, will, friends, family, open-minded confrontation, close-minded confrontation, pursuing, and wandering around. The Examinees' state of being tightened is first developed, watched by others, and finally resolved causing students either to pursue (positive reactions) or to wander around (negative reactions). Based on the theoretical framework, the following sixteen hypotheses are developed : 1. Students are less tightened in the first term. 2. Students are more tightened in the second term. 3. Students with higher grades feel less tightened. 4. Students with lower grades feel more tightened. 5. Students with higher expectations feel more tightened. 6. Students with lower expectations feel less tightened. 7. Students who are less tightened confront open-mindedly. 8. Students who are higher tightened confront close-mindedly. 9. Students with a strong will confront open-mindedly. 10. Students with a weak will confront close-mindedly. 11. Students who have more sincere friends confront open-mindedly. 12. Students who have less sincere frieds confront close-mindedly. 13. Students with family support confront open-mindedly. 14. Students with little family support confront close-mindedly. 15. Students confronting open-mindedly pursue. 16. Students confronting close-mindedly wander. We identify the following four relations from the data analysis : 1. Students with average grades in their first term are more tightened from high expectations and have a tendency to wander if they have less supporting families even if they also have a strong will and sincere friends. 2. Students with low grades in their second term are less tighened because of low expectations and have a tendency to wander if they have a weak will and few sincere friends even if they have strong family support. 3. Students with high grades in their second term are more tightened with high expectations and have a tendency to pursue confrontations open-mindedly if they have supporting families and a strong will even if they have few sincere friends. 4. Students with average grades in their first term are less tightened from fewer expectations and have a tendency to pursue confrontations open-mindedly when they have more supporting families and sincere friends even if they have a weak will. A Student's degree of being tightened are affected by grades, terms, and expectations. Being tightened is resolved positively or negatively based on will, friends, and family. We conclude that will, family support, and sincere friends cause students to confront "being tightened" open-mindedly. We also conclude that practical nursing for students who are tightened have to focus on creating environments in which students are able to confront open-mindedly.
