자녀의 초경 경험을 통한 어머니의 초경 경험 재구성 : Parse의 연구방법 적용 (Understanding the reconstruction experiences of mothers through their child's menarcheal experiences by using Parse's human becoming theory)

  • Purpose: The purpose of this study was to understand the reconstruction experience of mothers who had recently experienced a late-school age daughter's menarche by utilizing a hermeneutic phenomenological research methodology based on Parse's human becoming theory. Data were collected using purposive and snowball sampling methods to recruit the mothers of elementary fifth- and sixth-grade girls recently experiencing menarche. The data were analyzed using Parse's research methodology. As a result, the mothers' experiences appeared as "the process of reconstructing the menarche-related values created by themselves by facing the child's menarche experiences and related responses along with the change of era." The key concepts of there construction experiences of the mothers were "experiencing an ambivalent feeling of pity and pride," "celebrating her instead of expressing concerns," "giving a traditional lesson of protecting oneself to a new generation," and "feeling empathy as a woman and also alienation due to the generational gap." These findings reveal the important roles and competencies of community and school nurses in terms of achieving a healthy and positive reconstruction experience for mothers and their daughters. Also, it is necessary to develop educational programs for mothers who are about to encounter their daughters' menarche.

  • Studies on menstruation have focused only on menstruation itself and menstrual disorders. The menstruating girls or women have been neglected. So, the purpose of this study was to understand menstrual experience of adolescent girls in their perspective and build a theory on it, The specific purpose of this study were to find initial reaction of the girls, their strategies to adapt to menstruation. consequences of their efforts, influencing factor, and patterns of experience. The subjects of this study were eleven adolescent girls who experienced menarche three months to twenty-six months before the interview time. They were selected purposively. Their ages were in range of twelve and sixteen. One of them was a elementary school girl, three high school girls, and seven middle school girls. Two girls were handicapped because of cerebral palsy. All of them had some knowledge about menstrual physiology and hygiene during menstruation. Data were collected from September, 1994 to July, 1995. Data collection & analysis were done according to the grounded theory methodology by Strauss & Corbin(1990). Data collecting method was the long interviews and observation. Each interview took from 1 hour to 2 hours. Interview were tape-recorded and transcribed later by author. Data were analyzed immediately after interviews. Based on the results of previous interview, next interview were planned until gathered data reached the saturation point. Results were as follows. One hundred and six concepts were found. Those concepts were grouped into twenty eight categories and then fourteen higher categories. Twenty eight categories were as follows. “want to hide”, “bewildered”, “sense of burden”, “sense of heterogeneity”. “gladness”. “sense of superiority”, “negative empathy”, “positive empathy”, “limited hygenic control”, “sense of timing”, “lack of knowledge”, “lack of support”, “advance knowledge”, “informational support”, “emotional support”, “endurance”, “prayer”, “disclosing”, “avoidance”, “diversion”, “sense of powerlessness”, “discovery of sex identity”, “sense of maturation”, “sense of stability”, “acceptance of menstruation ”. fourteen higher categories were as follows. “negative feeling”, “posive feeling”, “exchange of feeling”, “limited hygenic control”, “sense of timing”, “accumulated experience”, “dysmenorrhea”, “level of knowledge”, “need for support”, “perceived support”, “sharing of feeling”, “self-control”, “passive acceptance”, “active acceptance”. The core category was “emotional shaking”, which consisted of “positive feeling” and “negative feeling”. “Emotional shaking”comes up to every adolescent girls experiencing menarche, independently of any contextual conditions, and its dimension has two directions : positive one and negative one. Its influencing factors were time of menarche, advance knowledge, support from the significant persons, expression and self-regulation. Even if they showed different process of adaptation to menstruation, general process of adaptation were as follows : 1. stage of emotional shaking 2. stage of acceptance 3. stage of internalization of the menstrual experience. Seven patterns existed on the process of adaptation to menstruation after menarche. Those are as follows. 1. If girls thought their menarche came too early and they had not much knowledge on menstruation, they had a kind of negative feeling. If they did not get enough support and dysmenorrhea superimposed, they came to accept menstruation passively. 2. If girls had menarche too early. they had negative feeling, even though they had enough advance knowledge. But support helped them accept menstruation easily. 3. If girls had menarche too early, they had negative feeling, even though they had enough advance knowledge on menstruation. But by experiencing subsequent menstruations and disclosing feeling, they began to accept menstruation. 4. If girls had menarche too lately and they had enough advance knowledge on menstruation. they had positive feeling. If dysmenorrhea superimposed later, their feeling turned in to negative one. But they came to accept menstruation positively by disclosing feeling and getting support. 5. If girls had menarche too early, they had negative feeling, even though they had enough advance knowledge on menstruation. In addition to this. if dysmenorrhes superimposed while they did not get enough support, they felt powerless and came to accept menstruation passively. 6. If girls had menarche too early and did not get enough advance knowledge, they had negative feeling. But disclosing feeling and support made them get sense of homogeneity and began to accept menstruation. 7. If girls had handicap, they had negative feeling, even though they had enough advance knowledge and menarche was late. But Menarche made them get feel sexual identity. Their limited hygenic control and negative empathy from their mothers made them accept menstruation passively. To let adolescent girls take their menstrual experience as a part of their lives forming a positive sense of feminine identity, it needs qualified teaching and, support and deep concern of the significant others. Nurses including school nurses should try to develop an educational program, which include menstrual physiology. hygiene during menstrual period, meaning of menstruation and impact of menstruation on the development of female sexual identity.

초등학교 여학생의 초경시기와 관련된 결정요인 분석 (Determinants of age at menarche in Korean elementary school girls)

  • 본 연구는 초경시기의 결정요인을 규명하기 위하여 대구 3개 지역 초등학교 5, 6학년 여학생 190명을 대상으로 개별 짝짓기 환자-대조군 연구를 실시하였고, 본 연구 결과를 요약하면 다음과 같다. 첫째, 비초경군에 비하여 초경군의 평균 수면시간이 더 적었다. 둘째, 어머니의 초경 나이가 빠를 수록 같은 또래에 비해 더 이른 초경 경험을 할 확률이 높았다. 셋째, 초경시기와 관련된 요인 중 비만이 가장 강한 연관성을 보였다. 전 세계적으로 초경 나이가 낮아지는 경향을 보이고 있으며, 우리나라 여성의 초경 나이도 현저하게 낮아지는 상황에서 본 연구의 결과는 중요한 기초자료로 활용될 것이다.

Risser 증후와 역연령, 골연령, 초경 시기 및 성인 예측신장 (AHP-TW3)과의 관계 (The Study on Correlations of Risser Sign with the Chronological Age, Bone Age, Menarche, and Adult Height Prediction according to TW3 Method)

  • Objectives The purpose of this study was to find out the clinically reliable relationships between the Risser sign and chronological age, bone age, menarche, and adult height prediction (AHP) and to evidence the reliability of the Risser sign. Methods This study had been carried out with 50 children who had their growth checked in an oriental medical hospital from January 2015 to February 2017. We investigated Risser sign in AP X-rays with iliac crest, bone age, AHP for all 50 children and the timing of menarche from the 22 girls in the study subjects. We also investigated a correlation between the Risser stage and the other indicators to analyze statistical data. Results The mean chronological ages of Risser 1, 2, 3 and 4 were 11.2, 12.6, 14.4, and 15.5 years respectively for the boys and 10.8, 12.2, 13.8 and 14.8 years respectively for the girls. The mean bone ages of Risser 1, 2, 3 and 4 were 12.3, 13.6, 15.7 and 16.5 years respectively for the boys and 11.7, 13.8, 14.3 and 14.9 years respectively for the girls. We analyzed 22 girls' Risser stages in accordance with the duration from menarche. The result showed that in the first six months after menarche, all girls were in Risser 1 and 2; in the next six months, the girls were in Risser 2 on average; in the next 12 months, all girls were in Risser 3 and 4; after more than two years from menarche, all girls were in Risser 4. The mean remaining growth height of Risser 1, 2, 3 and 4 were 27.8, 17.3, 4.4 and 1.0 cm respectively for the boys and 14.5, 5.1, 3.1 and 1.1 cm respectively for the girls. The Risser stage was correlated strongly with chronological age (Spearman's rho=0.707 (boy), 0.841 (girl)), bone age (Spearman's rho=0.869 (boy), 0.875 (girl)), duration from menarche (Spearman's rho=0.909) and remaining growth height (Spearman's rho=-0.784 (boy), -0.878 (girl)). Conclusions This study showed that the Risser sign can be useful in assessing skeletal maturity and predicting remaining growth height based on the Risser stage and the other growth indicators.

도시에 거주하는 중년 여성들의 골밀도와 이에 영향을 미치는 인자들에 관한 연구 2. 골밀도에 영향을 미치는 요인에 관한 연구 (Bone Densities of the Middle Aged Women Residing in the City and Related Factors)

  • This study was conducted to investigate the effect of physiological and behavioral factors on the bone density of 125 middle aged women who visited Saint Bundo Hospital. 16.1% of subjects had osteoporosis and 40.3% had osteopenia according to the measurement of the bone density of lumbar spine. Mean age was 56.9 in osteoporosis group and 53.7 in osteopenia group. It was significantly different from the mean age of control group, 50.7. The mean bone density of the women who had menarche after 15 years old was significantly lower than that of the women who had menarche before 15. But the age of menopause, the total year of menstruation, irregularity of the menstrual cycle and percentage of subjects who had ovariectomy were not significantly different among osteoporosis, osteopenia and control group. The use of medication such as oral contraceptive, steroid, depressant, diuretic, and Ca supplement and the preference of salty food were not significantly different among three groups. The percentage of subjects who had rheumatism, gastric ulcer, and pain in neck or shoulder was higher in osteoporosis and osteopenia group than in control group. This study shows that the age and the age of menarche affect the bone density, and that behavioral factors were not sig nificantly different in osteoporosis and osteopenia group compared to the control group. Further researches are needed to find out the effective way to minimize the effect of age and other physiological conditions on the decrease of bone density.

An observational population based study on dysmenorrhea and its risk factors

  • Dysmenorrhea is the most common gynecological disorder in women of reproductive age. The prevalence of menstrual pain is relatively high and has been shown to be related to many factors like age, age at menarche, menstrual cycle regularity, parity, cigarette smoking, and dietary habits etc. This study was conducted to determine the possible link between dysmenorrhea and the factors related to it. We investigated the associated risk factors that could influence menstrual pain. This Cross Sectional Study was conducted at Dept. of OBG, NIUM, Bangalore. The study consisted of 230 women between the ages of 20 - 49 years. Included volunteers were newly married, nulliparious and parous women. The detailed reproductive history was recorded and was analyzed statistically. It was observed that there were only two factors that were influencing Dysmenorrhea namely the early age at menarche and increase parity. It was hypothesis that earlier start to reproductive life in some way decreases the sensitivity of the uterus to prostaglandin, whereas delayed menarche and postponing reproductive life increase sensitivity of the uterus to prostaglandins and cause severe menstrual pain. It is concluded that early age at menarche and high parity decrease menstrual pain.

초등학교 6학년 여학생 월경군과 비월경군의 비만도와 성지식 (Obesity Rate and Sexual Knowledge in Menarche and Non-Menarche among 6th Grade Elementary School Girls)

  • The purpose of this study was to investigate the level of obesity and sexual knowledge between age-matched 168 postmenarcheal and 168 premenatcheal girls among 6th grade elementary school girls in Busan. Anthropometric measurements were taken for height and body weight of subjects for obesity and a questionnaire for sexual knowledge representing 5 categories was developed by researcher. For the statistical analysis of the collected data, frequency, Chi-square, t-test, ANOVA, and Pearson correlation were performed by SPSS WIN Vesion 12.0 program. These measurements of menarcheal grils were significantly higher than those of non-menacheal girls in height, body weight, BMI and obesity level. The sexual knowledge was low, expecially they didn't know well about sexual disease and contraception. There was no significant difference between two groups, but there was significant difference among obesity level, obesity group had sexual knowledge than others. And there were significant positive relations between the menarche and the obesity. The most important findings in the study showed that is needed the systemic and concrete sexual education in home, school and community, and especially by the professionals in the sex education field.

부정교합자(不正咬合者)의 사춘기성장(思春期成長)과 성적(性的) 성숙간(成熟間)의 상호관계(相互關係)에 관(關)한 누년적(累年的) 연구(硏究) (A LONGITUDINAL STUDY ON THE INTERRELATION BETWEEN PUBERTAL GROWTH AND SEXUAL MATURITY IN MALOCCLUSION)

  • To study the sexual maturity at puberty and interrelation between pubertal growth peak and sexual maturity, 68 malocclusions (female 44, male 24) were longitudinally studied for 4 years and cross-sectional samples of 210 malocclusions were studied together. The pubertal growth peak was determined by stature increment and sexual maturity was studied using developmental stages of pubic hair and breast, menarche in female and those of pubic hair in male. The following results were obtained. 1. Mean age at menarche was 12.8 years and it was 11.8 months later than PHV. 2. At pubertal growth peak, developmental stage 2 (54.2%) and 3 (29.2%) of pubic hair were most in male, and stage 1 (72.7%) and 2 (20.5%) were most in female. 3. At pubertal growth peak, the developmental stage 2 (50%) of breast was most and the stage 3 (43.2%) was next. 4. In interrelation between skeletal growth and sexual maturity, PHV had most high correlation with menarche and stage 3 of breast, and menarche had most high correlation with stage 3 of breast. 5. Appearance of remarkable sexual maturity suggested that the PHV was passed already.

정상월경군과 희발월경군의 BMI, 양도락, 초경연령 비교를 통한 희발월경 환자의 특성 연구 (A Study on the Characters of Oligomenorrheic Patients by Through Comparison of BMI, Yangorak, Menarche of Normal Menstrual Cycle Group and Oligomenorrheic Group)

  • Objectives: Oligomenorrhea is clinically important because it can be progressed to amenorrhea. The purpose of this study is to analyse the characters of oligomenorrheic patients by comparison normal menstrual cycle group and oligomenorrheic group of BMI, Yangdorak, menarche. Methods: We studied 58 patients visiting $\bigcirc\;\bigcirc$ Medical Center from May 1st, 2010 to April 30th, 2011. 29 oligomenorrheic group and 29 normal menstrual cycle group were compared with BMI, Yangdorak, menarche. The SPSS 12.0 for windows was used to analyse the data. Results: Mean BMI of oligomenorrheic group was lower than normal menstrual cycle group and statistically significant. Mean H1, H2, H3, H4, H5, H6 of oligomenorrheic group was lower than normal menstrual cycle group and statistically significant. Mean menarche of oligomenorrheic group was lower than normal menstrual cycle group, but there was no statistical significance. Conclusions: The results suggest that diagnosis of oligomenorrheic patients by stress would be useful Yangdorak.

A study on the menstruation of Korean adolescent girls in Seoul

  • Purpose: Westernized eating habits have been associated with earlyage menstruation, which increases the incidence of dysmenorrhea and premenstrual syndrome among adolescent girls. We therefore surveyed changes in menarche timing and the general menstrual characteristics of adolescent girls in Seoul, Korea. Methods: We surveyed 538 teenage girls who visited our hospital between July and November 2007. Items explored included age at menarche, general menstrual characteristics, occurrence of premenstrual syndrome and treatment thereof, and an association between present dysmenorrhea and a family history of the condition. Results: Average age at menarche was 12.6 years, with 29% (n=156) subjects beginning menstruation at age 12 years. The prevalence of dysmenorrhea was 82% (n=435). The main symptoms were abdominal (53.2%) and lower back pain (34.2%), and 15.2% of girls who experienced such symptoms required medication. Present dysmenorrhea, and a family history thereof, were statistically correlated (P<0.05). In addition, 58.8% (n=316) of teenage girls had symptoms of premenstrual syndrome. The most frequent psychological symptoms were fatigue (36.4%) and nervousness (38.7%), whereas the most common physical symptom was menstrual cramps (46.5%). Most subjects (87.6%) tolerated the symptoms of premenstrual syndrome without medication; 11.4% took medicines including painkillers; but only 0.1% of subjects visited a doctor. Conclusion: The average age at menarche in Korean girls was 12.6 years, thus younger than in the past. Most teenage girls experienced dysmenorrhea and premenstrual syndrome, but few consulted a doctor. Organized treatment plans are required to manage menstrual problems in teenage girls.
