Micelle-Enhanced Ultrafiltration (MEUF) is a membrane separation processes that improving ultrafiltration process with the formation of micelles of the surface active agents. Surface active agents are widely used to improve membrane processes due to the ability to trap organic compounds and metals in the treatment of industrial waste water. In this study, surface active agents are used to improve micelle-enhanced ultrafiltration (MEUF) to reduce chemical oxygen demand (COD), total dissolved solid (TDS), turbidity and clogging the membrane in dairy wastewater treatment. Three important operational factors (anionic surface active agent concentration, pressure and pH) and these interactions were investigated by using response surface methodology (RSM) and Box-Behnken design. Results show that due to the concentration polarization layer and increase the number of Micelles; the anionic surface active agent concentration has a negative effect on the flux and has a positive effect on the elimination of contamination indices. pH, and the pressure have the greatest effect on flux. On the other hand, it could be stated that these percentages of separation are in the percentages range of Nano-filtration (NF). While MEUF process has higher flux than NF process. The results have been achieved at lower pressure while NF process needs high pressure, thus making MEUF is the replacement for the NF process.
Continuous renal replacement therapy (CRRT) has become the preferred dialysis modality to support critically ill children with acute kidney injury. As CRRT technology and clinical practice advances, experiences using CRRT on small infants and neonates have increased. In neonates with hyperammonemia or acute kidney injury during extracorporeal membrane oxygenation (ECMO) therapy, CRRT can be a safe and effective technique. However, there are many limitations of CRRT in neonates, including vascular access, bleeding complications, and lack of neonatespecific devices. This review discusses the basic principles of CRRT and the special considerations when using this technique in neonates and infants.
Fracture of prosthetic valve leaflets in the absence of traumatic injury is very rare. Leaflet fracture can cause acute pulmonary edema and cardiogenic shock and is potentially life-threatening, requiring emergency surgery. Thus, a leaflet fracture must be diagnosed quickly and accurately. We present the case of a 46-year-old man with CarboMedics prosthetic aortic and mitral valve replacements implanted 24 years previously. The patient presented at our emergency department with abrupt dyspnea and fever. We diagnosed severe mitral valve regurgitation with anterior leaflet fracture. The patient underwent venoarterial extracorporeal membrane oxygenation and delayed mitral valve replacement. The foreign body was removed step by step because the diagnosis was missed. Two pieces of broken leaflets were found in the left common iliac artery and left external iliac artery. The patient was treated successfully and remains asymptomatic 1 year following surgery.

Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with cardiorespiratory failure. The serum albumin level is an important prognostic marker in critically ill patients. We evaluated the efficacy of using pre-ECMO serum albumin levels to predict 30-day mortality in patients with cardiogenic shock (CS) who underwent venoarterial (VA) ECMO. Methods: We reviewed the medical records of 114 adult patients who underwent VA-ECMO between March 2021 and September 2022. The patients were divided into survivors and non-survivors. Clinical data before and during ECMO were compared. Results: Patients' mean age was 67.8±13.6 years, and 36 (31.6%) were female. The proportion of survival to discharge was 48.6% (n=56). Cox regression analysis showed that the pre-ECMO albumin level independently predicted 30-day mortality (hazard ratio, 0.25; 95% confidence interval [CI], 0.11-0.59; p=0.002). The area under the receiver operating characteristic curve of albumin levels (pre-ECMO) was 0.73 (standard error [SE], 0.05; 95% CI, 0.63-0.81; p<0.001; cut-off value=3.4 g/dL). Kaplan-Meier survival analysis showed that the cumulative 30-day mortality was significantly higher in patients with a pre-ECMO albumin level ≤3.4 g/dL than in those with a level >3.4 g/dL (68.9% vs. 23.8%, p<0.001). As the adjusted amount of albumin infused increased, the possibility of 30-day mortality also increased (coefficient=0.140; SE, 0.037; p<0.001). Conclusion: Hypoalbuminemia during ECMO was associated with higher mortality, even with higher amounts of albumin replacement, in patients with CS who underwent VA-ECMO. Further studies are needed to predict the timing of albumin replacement during ECMO.

An automatic method is developed for the determination of SOx in atmosphere. The method involves SOx sampling in diffusion scrubber followed by ion chromatographic analysis. Filtered air is withdrawn at 1.8.ell./min through a diffusion scrubber of which inner tube is made of PTFE(Gore-tex) membrane tubing. 1mM $H_{2}$$O_{2}$ is used as absorbing solution so that SOx is oxidized to S $O_{4}$$^{2-}$. The scrubbered solution is automatically injected into ion chromatograhpy eith conductivity detection for sulphate determination. Replacement of commonly used polyproplene membrane with PTFE gives several merits such as easy preparation of diffusion scrubber, better collection efficiency. No measurable memory effect is experienced, and this isin contrast to previous work for ammonia. Detection limit of this method defined by three times standard deviation is 0.56ppbv. The precision is 0.4% RSD at SOx concentration of 7.3ppbv Results for Seoulatmosphere ate presented.

Surface of hydrophobic polyethylene membrane was modified to become hydrophilic by ion beam irradiation. Submerged membrane filtration reactors contained pristine membrane or surface-modified membrane and the influent to reactors was pickled radish wastewater. The objectives of this study was to investigate the variation of flux and pressure and the characteristics of pollutant removal such as organics, suspended solids and nutrients with time. The result of experiments using intermittent pristine membrane showed the occurrence of severe fouling by increasing permeate pressure rapidly in case of pickled radish wastewater but in synthetic wastewater, this phenomenon was not occurred. In experiments of variation flux after chemical cleaning and water cleaning in pristine membrane, chemical cleaning must be necessary for renewals of pollutant membrane. Performance of intermittent operation is higher than that of continuous operation. Reaching fouling time in the case of surface-modified membrane is 6 times as long as pristine membrane. According this reason, replacement expense of surface-modified membrane could be 1/6 of that of pristine membrane. Effluent from this process was relatively good water quality and performance in the removal efficiency of SS, nitrogen and phosphorus was particularly higher.

Cytochrome c (cyt c) binds to acidic membranes at low ionic strength. Replacement of Lys-72 or Lys-87 by Glu reduced the binding affinity of cyt c toward large unilamellar vesicles (LUV) in liquid crystalline phase. The differences were smaller for LUV in gel phase. In order to investigate the effects of the double replacement of $\small{L}$-Pro, $\small{D}$-Pro, $\small{D}$-Leu or Nleu (the peptoid residue for Leu) in the hydrophobic face (positions 9 and 13) of amphipathic ${\alpha}$-helical non-cell-selective antimicrobial peptide $L_8K_9W_1$ on the structure, cell selectivity and mechanism of action, we synthesized a series of $L_8K_9W_1$ analogs with double replacement of $\small{L}$-Pro, $\small{D}$-Pro, $\small{D}$-Leu or Nleu in the hydrophobic face of $L_8K_9W_1$. In this study, we have confirmed that the double replacement of $\small{L}$-Pro, $\small{D}$-Pro, or Nleu in the hydrophobic face of $L_8K_9W_1$ let to a great increase in the selectivity toward bacterial cells and a complete destruction of ${\alpha}$-helical structure. Interestingly, $L_8K_9W_1$-$\small{L}$-Pro, $L_8K_9W_1$-$\small{D}$-Pro and $L_8K_9W_1$-Nleu preferentially interacted with negatively charged phospholipids, but unlike $L_8K_9W_1$ and $L_8K_9W_1$-$\small{D}$-Leu, they did not disrupt the integrity of lipid bilayers and depolarize the bacterial cytoplasmic membrane. These results suggested that the mode of action of $L_8K_9W_1$-$\small{L}$-Pro, $L_8K_9W_1$-$\small{D}$-Pro and $L_8K_9W_1$-Nleu involves the intracellular target other than the bacterial membrane. In particular, $L_8K_9W_1$-$\small{L}$-Pro, $L_8K_9W_1$-$\small{D}$-Pro and $L_8K_9W_1$-Nleu had powerful antimicrobial activity (MIC range, 1 to $4{\mu}M$) against methicillin-resistant Staphylococcus aureus (MRSA) and multidrug-resistant Pseudomonas aeruginosa (MDRPA). Taken together, our results suggested that $L_8K_9W_1$-$\small{L}$-Pro, $L_8K_9W_1$-$\small{D}$-Pro and $L_8K_9W_1$-Nleu with great cell selectivity may be promising candidates for novel therapeutic agents, complementing conventional antibiotic therapies to combat pathogenic microorganisms.
For 26 months since August 1991, 10 consecutive patients with congenital left ventricular outflow tract obstruction underwent corrective surgery in Pusan Paik Hospital. Their ages ranged from 2 to 18 years. There were 6 male and 4 female patients. According to stenotic site, obstruction were classified into supravalvular [n = 5], subvalvular [n = 4], valvular stenosis [n = 1]. We have performed patch enlargement of ascending aorta [n = 2], supravalvular membrane resection and patch enlargement of ascending aorta [n = 3], subvalvular membrane resection [n = 2], subvalvular membrane resection and left ventricular myectomy [n = 2] and aortic annuloplasty with Dacron patch and aortic valve replacement [n = 1]. Preoperative mean value of systolic pressure gradient were 85.0 $\pm$29.2mmHg[supravalvular], 70.0mmHg[valvular], 72.5 $\pm$ 22.5mmHg[subvalvular], and 78.5 $\pm$ 24.3mmHg[total]. Postoperative mean value of systolic pressure gradient were 31.0 $\pm$ 8.9mmHg[supravalvular], 0mmHg[valvular], 15.0 $\pm$ 10.8mmHg[subvalvular], and 21.5 $\pm$ 13.9mmHg[total]. Postoperative systolic pressure gradient was decreased significantly[p = 0.001]. Postoperative course and short-term follow up results were good except one case of transient heart failure.

Hranfa's marl, a local natural mineral, is selected for the decontamination by adsorption of aqueous effluents in textile industry. Its physicochemical characterization is first performed. It is composed mainly of Calcite, Quartz, Ankerite and Muscovite. Its specific surface area is 40 ㎡ g-1. Its adsorption performance is then tested in batch conditions using an industrial organic dye, Bemacid Red E-TL, as a model pollutant. The measured adsorption capacity of Hranfa's marl is 16 mg g-1 which is comparable to that of other types of natural adsorbents. A hybrid process is tested coupling adsorption of the dye on marl in suspension and microfiltration. An adsorption reactor is inserted into the circulation loop of a microfiltration pilot using ceramic membranes. This makes possible a continuous extraction of the treated water provided that a periodic replacement of the saturated adsorbent is done. The breakthrough curve obtained by analyzing the dye concentration in the permeate is close to the ideal one considering that no dye will cross the membrane as long as the adsorbent load is not saturated. These first experimental data provide proof of concept for such a hybrid process.
