요통에 대한 협진 레지스트리 임상연구: 예비 임상연구 프로토콜 (Clinical Research of Korean Medicine and Western Medicine Collaboration Registry for Low Back Pain: A Pilot Study Protocol)

  • Objectives Collaboration medicine means cooperate with western medicine and traditional korean medicine to treat the one disease. In Korea, Interest in collaboration medicine is increasing, But the number of studies is scare. Therefore we will conduct collaboration medicine study for the low back pain. Methods This study composes prospective cohort registry study. If the patients who need collaboration medicine by doctor come, we will ask regist this study. And patient select collaboration treatment group and single treatment group. Total 120 patients will recruit from collaboration pilot project hospitals. Each group patient will observed 4 weeks. Telephone research will conducted after 1 month from the last follow up. During the treatment, patients are treated usual treatment type of each medicine. Primary outcome is NRS and secondary outcomes are EQ-5D and ODI. We will analyze difference of 1 week and 4 week outcome result. Conclusions This study is the first large sample size study effect of collaboration medicine in Korea for low back pain. We check present collaboration system and improve collaboration system. Aim of this study is to find the effectiveness collaboration medicine for low back pain in the real condition. And we expect this pilot study will provide the clinical collaboration information and basis.

다빈도 협진 질환의 후향적 진료기록 분석 연구 : 예비연구 (Retrospective Medical Record Analysis on Frequent Disease of Collaboration: A Pilot Study)

  • Objectives: This pilot study aimed to confirm the possibility of applying our design to the main study, a retrospective medical record analysis of the diseases which have most frequently been treated with collaborations of Korean and Western medicine, and to identify what corrections and statistical models are needed to conduct the main study. Methods: Data were collected from a case report form developed for patients who received treatment in the medical institutions. Appropriate statistical techniques, like Propensity Score (PS) and Generalized Estimation Equation (GEE) models, were used to compare the indicators of collaboration and non-collaboration groups for patients in comparable diseases. Results: Using PS matching for each M and S disease group, the indicators were compared by balancing the collaboration and non-collaboration group, and the GEE models compared indicators between groups in each disease over follow-up. Through this process we identified two limitations, insufficient samples and a large deviation of the follow-up period. Conclusion: This pilot study confirmed that the study design and case report form are applicable. The main study will be conducted by collecting sufficient samples and reflecting deviation of follow-up period.

Multimedia Presentation Authoring and Virtual Collaboration in Medicine

  • Web-based virtual collaboration is increasingly gaining popularity in almost every area in our society due to the fact that it can bridge the gap imposed by time and geographical constraints. However, in medical field, such collaboration has been less popular than other fields. Some of the reasons were timeliness, security, and preciseness of the information they are dealing with. In this paper, we are proposing a web-based distributed medical collaboration system called Virtual Collaboration System for Medicine (VCSM) for medical doctors that meet the needs. The proposed system consists of two parts - multimedia presentation and recordable virtual collaboration. The former supports synchronized multimedia presentation using Synchronous Multimedia Integration Language (SMIL.) It allows synchronization of the contents of a PowerPoint presentation file and a video file. The presentation may be provided to the participants before the discussion begins. Next, in the virtual collaboration stage, participants can use text along with associated symbols during the discussion over the presented medical images. The symbols such as arrows or polygons can be set or removed dynamically to represent areas of interest in digital images using so called layered architecture that separates image layer from annotation layer. XML files are used to record participants' opinions along with the symbols over some particular images

신체활동 증진을 위한 부문간 협력 (Inter-sectoral Collaboration for Physical Activity Promotion)

  • Objectives: This article intends to explore the major documents about inter-sectoral collaboration for physical activity promotion based on new paradigm. Methods: In addition to the documents after 1990s collected through several physical activity related projects of author, additional snowballing exploration has been done. Results: Globally extensive participation of stake-holders and partnership developed were strategically recommended and implementation strategy and good example were provided. In Europe individual, micro-environmental and macro-environmental specific role of major sectors were provided. Nationally series of Australian documents officialized inter-sectoral linkages from early times and Slovenian program showed examples of inter-sectoral evaluation and health indicators. U.S. national physical activity plan showed specific example of 6 guiding principles and 5 overarching strategies among 8 sectors for good inter-sectoral collaboration. In city level, healthy city approach activated. Conclusions: Various specific good examples of intersectoral physical activity collaboration could searched globally, regionally and nationally for application in Korea.

한의학과 서양의학의 협진 프로세스를 통한 협진 현황 보고 (A Report on the Status of Cooperation through the Korean Medicine and Western Medicine Collaboration Process)

  • Objectives The purpose of this study is to investigate present status of cooperation through Korean medicine and Western medicine collaboration process. Methods We have established Korean medicine and Western medicine collaboration process to examine the status of cooperation. The medical records were investigated retrospectively, the general characteristics of gender, age of cooperative patients, classification of disease, frequency of medical treatment and type of insurance were analyzed. Results The Korean medicine and Western medicine collaboration process conducted in 4 stages, the convenience of patient movements is considered. A total of 245 people received cooperative medical treatment, 156 were out-patient department (OPD) patients and 89 were hospitalized patients, both group have more women than men. When classified as disease, OPD patients were most diagnosed with Sprain and strain of lumbar spine, while hospitalized patients were most diagnosed with lumbar and other intervertebral disc disorders with radiculopathy. A total of 72.7% patients were received cooperative medical treatment at once. In a survey of cooperative satisfaction, 68.5% of all medical staff responded positively to the treatment effect, and 68.6% said the need for cooperation was necessary. Conclusions As a result of this study, we were able to see the present status of cooperation, and through this, we found an improvement in the continuation of the cooperation. Based on the present study, It is hoped that a cooperative process will emerge that can improve the problems shown in this study.

지역사회 기관 간 금연사업 네트워크 모델: 블록모델링을 중심으로 (Interorganizational Networks for Smoking Prevention and Cessation: A Blockmodeling Approach)

  • Purpose: This study examined characteristics and patterns of interorganizational networks for smoking prevention and cessation in Korea. Methods: We surveyed two community health centers, ninety-five hospitals or clinics, ninety- two pharmacies, and sixty-five health welfare organizations in two districts of Seoul in 2020. Data on the organizations' characteristics of smoking cessation and interorganizational activities for information sharing, client referral, and program collaboration were collected and analyzed using network statistics and blockmodeling. Results: Network size was in the order of information sharing, client referral, and program collaboration networks. Network patterns for interorganizational activities on information sharing, client referral, and program collaboration among four organizations were similar between the two districts. Community health centers provided information and received clients from a majority of the organizations. Their interactions were not unidirectional but mutual with other organizations. Pharmacies were involved in information sharing with health welfare organizations and client referrals to hospitals or clinics. Health welfare organizations were primarily connected with the community health centers for client referrals and program collaboration. Conclusion: A community health center is the lead agency in interorganizational activities for smoking prevention and cessation. However, hospitals or clinics, pharmacies, and health welfare organizations also participate in interorganizational networks for smoking prevention and cessation with diverse roles. This study would be evidence for developing future interorganizational networks for smoking prevention and cessation.

The Anti-nociceptive and Anti-inflammatory Effect of Achyranthes Japonica Nakai

  • Objective : Achyranthes japonica Nakai (AJ) has been classified as a herb that activates blood flow and clears the stagnated blood. In this study, we evaluated its anti-nociceptive and anti-inflammatory activity in animals to clarify the effect of AJ on pain or inflammation. Methods : ICR mice and Sprague-Dawley rats were pretreated with an ethanolic extract of AJ with two dosages of 200 mg/kg (p.o.) and 400 mg/kg (p.o.). Nociceptive responses of acute pain were determined by hotplate and tail-flick tests. The effects of AJ on inflammation were evaluated by flexion/extention test and mechanical hyperalgesia test in models induced by both carrageenan and Complete Freund's Adjuvant (CFA). Results : AJ showed significant analgesic effects in both hotplate and tail-flick tests at the dose of 400 mg/kg. It also produced a significant inhibition of carrageenan-induced paw edema and CFA induced arthritis in rats at the dose of 400 mg/kg. Conclusion : We have demonstrated the analgesic and anti-inflammatory properties of an 80% ethanolic extract of AJ in animals. This suggests the application of AJ in relief of pain or inflammatory disease.

A Clinical Study of Collaboration between Western and Korean Medicine for the Treatment of Peripheral Facial Palsy in a Korean Medicine Hospital

  • Background: The purpose of this study was to investigate the characteristics and direction of treatment for peripheral facial palsy (PFP) based on medical collaboration between Western and Korean medicine departments. Methods: There were 195 outpatients with PFP identified retrospectively by examining electronic medical records. These patients were treated with Korean and Western medicine from January 1st, 2018 to December 31st, 2018 at the Kyung Hee University hospital. Records were analyzed according to the patients' demographic characteristics and clinical features of the collaborative combined treatment. Results: According to the collaborative treatment pathway, the number of patients consulted from Western medicine departments was more than consulted from Korean department for the first time. The time taken by the Western medicine departments to consult with the patients at the Korean center for the first time was 14.9 days from the onset of symptoms. Acupuncture was the most frequently used Korean medicine treatment. The total treatment period for Korean medicine sessions and intervals were 91.9 days, 23 times and 3.6 days, respectively. When the Korean medicine center consulted with Western medicine departments, the time taken until the first consultation was 8.5 days from the onset of symptoms. Medication was the most used treatment, prescribed after 3.1 days. The most frequently used clinical test was an electromyogram, and this was performed after 20.5 days. The total treatment period, sessions and intervals were 21.2 days, 2.8 times and 5 days, respectively. Conclusion: The clinical status of collaborative treatment for PFP was determined including the timing and interval of consultation according to treatments.

동서 협진 비만클리닉을 이용한 비만환자 37례에 관한 임상 관찰 (A Clinical Study on the Effect of 37 Obesity Patients through Obesity Clinic in Ease-West Collaboration Center)

  • Objectives : The purpose of this study is to evaluate the effect the of treatment in obesity clinic in east-west collaboration center, furthermore to search for how to join and improve east medicine and west medicine more closely. Methods : 37 patients were classified into four groups by Body Mass Index(BMI); normal, overweight, 1st obesity and 2nd obesity groups. Weight, BMI, body fat rate and abdominal fat rate were compared in each group by paired t-test. Results & Conclusions : In distribution of sex, male were 10(27%), female were 27(73%). Mean age was $34{\pm}13$ and the patients in twenties un the most. Among four groups, the number of End obesity groups was the most. Most patients had a preference for western medication and ear acupuncture therapy. In general we could find the correlation between the effect of obesity treatment and the degree of obesity. The more the obesity degree was, the more weight loss was.
