프로세스 중심의 진료의사결정 지원 시스템 구축 (Development of process-centric clinical decision support system)

  • In order to provide appropriate decision supports in medical domain, it is required that clinical knowledge should be implemented in a computable form and integrated with hospital information systems. Healthcare organizations are increasingly adopting tools that provide decision support functions to improve patient outcomes and reduce medical errors. This paper proposes a process centric clinical decision support system based on medical knowledge. The proposed system consists of three major parts - CPG (Clinical Practice Guideline) repository, service pool, and decision support module. The decision support module interprets knowledge base generated by the CPG and service part and then generates a personalized and patient centered clinical process satisfying specific requirements of an individual patient during the entire treatment in hospitals. The proposed system helps health professionals to select appropriate clinical procedures according to the circumstances of each patient resulting in improving the quality of care and reducing medical errors.

A Preliminary Study on Clinical Decision Support System based on Classification Learning of Electronic Medical Records

  • We employed a hierarchical document classification method to classify a massive collection of electronic medical records(EMR) written in both Korean and English. Our experimental system has been learned from 5,000 records of EMR text data and predicted a newly given set of EMR text data over 68% correctly. We expect the accuracy rate can be improved greatly provided a dictionary of medical terms or a suitable medical thesaurus. The classification system might play a key role in some clinical decision support systems and various interpretation systems for clinical data.

  • Customer Support Knowledge of Customer Support Organization is one of the important assets of enterprises and "Customer Support Knowledge Management" is also the critical aspect of Business Knowledge Management; however, the attributes of Customer Support Knowledge are complicated, diverse, renewed rapidly and difficult to be managed. Thus, in order to design a successful Customer Support Knowledge Management System, apart from the consideration of "human" and "information technology" aspects, the concerns of attributes and Customer Support Knowledge and industry characteristics should be involved for meeting the requirements of Customer Support Organization and allowing the organization to acquire the competitive advantage of "Differentiation Service". This research used the "Customer Support Knowledge Management System" in a high-tech industry as an example and treated the end users of medical instruments in different types of hospitals in Taiwan which have received the support service of our company in recent six months as the population. The end users were mostly the nursing executives or ultrasonic wave technical personnel in intensive care unit and they had similar educational background and incomes and adopted the medical instruments such as physical supervision system, ultrasonic wave system, heart start or ECG machine produced by our company; the research method was to randomly treat the investigation results of the telephone customers' satisfaction from respective 30 end users in the population three months before and after this system execution as the samples and use hypotheses to validate if the end users' customer satisfaction significantly improved in terms of "Remote Support," "On-site Support," "Service Turn Around time," "Technical Competence" and "Manner" in order to understand the influence and managerial significance of execution of "Customer Support Knowledge Management System" on Customer Support Organization.

  • 최근 의료IT 분야 솔루션들이 분산 환경 기반으로 제공되고 있는 추세이다. 국내에서도 분산 환경에서 의료정보를 공유할 수 있는 임상의사결정지원시스템 개발의 필요성이 인식되어 연구되고 있다. 기존 임상의사결정지원시스템은 병원 내의 자체적인 의료정보만을 사용하여 구축되고 있다. 이로 인해 기존의 시스템은 의사결정지원의 효율성 및 정확성 측면에서 좋은 결과를 얻기 어렵다. 이러한 한계점을 해결하기 위해 이 논문에서는 의료분야의 공통 데이터 모델을 기반으로 하는 임상의사결정지원시스템 모델을 설계하고 구축방안을 제시한다. 제안 모델의 적용 과정을 설명하기 위해 대장암 진단을 위한 임상의사결정지원시스템의 개발 시나리오를 기술한다. 또한 성공적인 임상의사결정지원시스템 개발을 위한 필수 요구사항을 제시한다. 이를 통해 여러 병원에서 공통으로 사용이 가능하고 시스템의 효율성과 정확성을 높일 수 있는 임상의사결정지원시스템 개발이 가능할 것으로 기대한다.

  • By means of the model competition, this research analyzed the factor of patient management, the factor of policy support, and the factor of medical treatment system. Concretely, the factor of policy support forms a positive effects on the factor of medical treatment system. Practically, well-established healthcare policy provide and facilitate the effective medical treatment system. of the hospital. And, in the effective medical treatment system, hospitals try to develop the patient management of the chronic disease. From the empirical research, this paper concluded that the factor of medical treatment system. mediated by the factor of policy support. Also, the factor of medical treatment system promotes the development of patient management in the chronic disease.

  • This article examines the features of improving the public management of the market of medical and social services using information technology to improve the efficiency of public activity in modern conditions. The interpretation of the essence of the categories "information technologies" and "information support" is analyzed. The importance of the organization of information support of public authorities in the insurance of the state apparatus of information management for the adoption and solution of state issues of the market of medical and social services has been studied. Issues and system of information support of public management of the market of medical and social services are considered. The stages of the ICT implementation process in the market of medical and social services are described. The tools of state regulation in the market of medical and social services and the introduction of targeted programs for the development of the social sphere and health care are highlighted. Recommendations for improvement are formulated public management of the market of medical services and social way introduction of ICT tools, which provide for the implementation of a set of measures aimed at intellectualizing the entire system public data management spheres. The necessity of development of innovative tools of public management of the market of medical and social services of Ukraine for the purpose of working out of strategies of increase of quality of medical and social services in the XXI century is substantiated. A number of benefits from the implementation of an effective process of application of information support of public management of the market of medical and social services to ensure the efficiency of public authorities of Ukraine. It is proposed to create a "Department of information support of self-government" and electronic health care system (e-Health) to improve the management decision-making process, which will provide an opportunity to accumulate, process, analyze, and as a result, get the expected effect in the form of balanced management decisions by public authorities of Ukraine.

의료과오를 줄이기 위한 효율적인 병원서비스운영에 대한 실증적 연구 (An Empirical Study on the Efficient Hospital Service Operation Management for the Reduction of Medical Errors)

  • This paper presents a research model, which identifies a relationship between medical error reduction, efficiency of organizational systems, and employee satisfaction with organizational support. The proposed model was tested through hypotheses, based on data collected from 210 respondents from the medical staff of large -sized (i.e., more than 500 beds) residential hospitals in cities of South Korea. The results of the study showed that medical error reduction is associated with corrective system and employee satisfaction with organizational support. Therefore, it is very important that organizations improve their employees' satisfaction by providing sufficient support (e.g., information support and sharing, work guide book, etc.) for their work. In addition, in organizational systems, the corrective system has positive relationship with medical error reduction. In terms of corrective procedures, leaders or managers can make improvements by providing and supporting a friendly work environment where errors may be reported without blame and discussed in order to be corrected.

웹기반 진단 보조 시스템의 진단 일치도 연구 (A Study for Diagnostic Agreement between Web-based Diagnosis Support System and Korean Medical Doctors' Diagnosis)

  • Objectives: This study aims to evaluate the clinical validity of the system by conducting a clinical study to assess the diagnostic agreement between the system and Korean medical doctors. Methods: This study was conducted from September 7, 2023, to December 7, 2023, across five Korean medicine institutions, involving 100 adult participants aged 20-64 who consented to participate. Participants first entered their symptoms into a web-based program, which utilized an AI-based algorithm to diagnose 36 types of pattern differentiation. Subsequently, Korean medical doctors conducted face-to-face diagnoses using the same 36 types. The diagnostic agreement between the system and the doctors' diagnoses was analyzed using descriptive statistical analysis, and the results were expressed as a percentage agreement. Results: Analysis of the diagnostic data from 100 participants revealed that the web-based diagnosis support system identified an average of 7.76±0.79 patterns per patient, while Korean medical doctors identified an average of 7.99±0.10 patterns per patient. The diagnostic agreement between the system and the doctors showed an average of 7.08±1.08 patterns per patient, with an overall diagnostic agreement rate of 88.57±13.31%. Conclusion: This study developed a web-based diagnosis support system for traditional Korean medicine and evaluated its clinical validity by assessing diagnostic agreement. Comparing the diagnoses of the system with those of Korean medical doctors for 100 patients, the system showed an approximately 89% agreement rate with the clinical diagnoses. The system holds potential for aiding Korean medical doctors in pattern differentiation diagnosis in clinical practice.

  • 최근 바이오헬스산업의 중요성이 증가함에 따라 정부지원의 혁신성이 요구되고 있으나 지원틀의 한계가 있어 사업화를 위한 지원이 미비하여 보완적 지원체계가 시급한 실정이다. 이에 따라 본 연구는 국내의 기존 지원 및 평가가 따르는 TRL 개념을 고찰하고 이를 중심으로 현재 국내 적용 지원체계 실태와 해외의 지원사례 등을 검토하여, 정부가 펼치고 있는 현 TRL 기반의 지원정책이 오랜 기간과 투자가 많이 소요되는 바이오헬스 분야 사업화를 위해서는 한계를 가지고 있음을 지적하였다. 이를 바탕으로 후기 연구개발단계 문제점 해소를 위한 개선점과 바이오산업의 특성을 반영한 사업화 중심의 새로운 지원체계 개념을 상정하고 연구개발사업에 맞게 제시될 수 있는 정책방향과 연결지었다. 본 연구는 바이오헬스분야 사업화를 위해 연구와 산업의 거리를 좁히는 가이드라인 역할이 될 것임에 의의가 있다 할 것이다.

신장암 표준임상빅데이터 구축 및 머신러닝 기반 치료결정지원시스템 개발 (Constructing a Standard Clinical Big Database for Kidney Cancer and Development of Machine Learning Based Treatment Decision Support Systems)

  • Since renal cell carcinoma(RCC) has various examination and treatment methods according to clinical stage and histopathological characteristics, it is required to determine accurate and efficient treatment methods in the clinical field. However, the process of collecting and processing RCC medical data is difficult and complex, so there is currently no AI-based clinical decision support system for RCC treatments worldwide. In this study, we propose a clinical decision support system that helps clinicians decide on a precision treatment to each patient. RCC standard big database is built by collecting structured and unstructured data from the standard common data model and electronic medical information system. Based on this, various machine learning classification algorithms are applied to support a better clinical decision making.
