본 연구에서는 표준 게이트웨이 기반의 다중 생체계측 장비 및 CMCIS(Central Monitoring and Cardiology Information System) 기반의 통합 시스템을 개발하기 위한 타당성 분석을 위한 국내외 동향을 분석한 연구이다. 개발의 필요성을 타진하기 위하여 기술, 사회, 경제적 중요성과 국내외 관련기술의 현황을 분석하였다. 분석한 결과, 대부분 해외의 다국적 기업에 의존하는 실정이며 심혈관계 질환의 지속적 증가로 효율적인 의료 서비스를 위한 기술개발의 필요성이 높게 평가되었다. 또한 표준 게이트웨이 기술의 확보로 진단용 심전계 표준 게이트웨이를 적용함으로써 다양한 HIS 인터페이스가 가능하며, 편의성이 증대되어 활용성이 높아질 것으로 예상된다.

Filmless full-PACS in korea has rapidly been growing, since government had supported collaborative PACS project between industry and university hospital in late of 1995. At the same time, a small company had started PACS business, while the Korea PACS society was being formed. In the beginning, PACS societies had focused on developing peripheral solutions such as DICOM gateway for image acquisition, x-ray film digitizer, and viewing software for research or management of personal image data, while Samsung Medical Center had started installing an imported partial PACS system which had recently upgraded with a new system. In similar time frame, a few hospitals had started developing and installing domestic large scale full-PACS system. Several years later, many hospitals have installed full-PACS system with national policy of reimbursement for PACS exams in November 1999. It is believed that Korea is the first country that adopted PACS reimbursement for filmless full-PACS as a national policy. Both experiences of full-PACS installation and national policy generated tremendous intellectual and technological expertise about PACS at all levels, clinical, hospital management, education, and industrial sectors. There are currently three types of PACS system which includes domestic, imported, and hybrid PACS system with imported solution for core system and domestic solution for peripheral system. There are more than 20 domestic PACS companies and they have now enough experiences so that they are capable of installing a truly full-PACS system for large-scale teaching hospitals. PACS societies in Korea understand how to design, implement, install, manage, sustain, and provide good services for large-scale full-PACS. PACS society has also strength for the highest integration technology of the Hospital Information. However, further understanding and timely implementation of continuously evolving international standard and integrated healthcare enterprise concepts may be necessary for international leading of PACS technologies for the future.

Various standard terminologies in medical field are composed individually to different structure. Therefore, information on crosswalking between the terminologies is needed to combine and use the terminologies. Lots of mapping tools have been developed and used to create the information. However, since those tools deal with specific terminologies, the information is restrictly created. To overcome this problem, some tools have been developed, which perform mapping tasks by composing various terminologies. However, the tools also have difficulty of composing automatically the terminologies because the terminologies have different structures. Therefore, in this paper, we propose a method for composing and using the terminologies in the developed mapping system with keeping the original structure of the terminologies. In the proposed method, additional terminologies could be added on the mapping system and used by making metadata involving information on location and structure of the terminologies. And the mapping system could cope flexibly with the changes of the structure or context of the terminologies. Moreover, various types of mapping information could be defined and created in the system because mapping data are constructed as triplets in ontology. Therefore, the mapping data can be transformed and distributed in different formats such as OWL, RDF, and Excel. Finally, we confirmed the usefulness of the mapping system based on the proposed method through the experiments about creating mapping data.

The development of medical devices based on the theory of tranditional Korean medicine is increasing due to population aging and emerging demand for medical services. Various clinical trials are being conducted to ensure the clinical effectiveness of the developed medical devices, and the importance of systematic management of variously collected information in this process is increasing. In this paper, we designed and constructed the database that can systematically manage the medical information of the patient and the pulse measurement information in conducting clinical trials by using pulse analysis system which is a representative diagnostic method of traditional Korean medicine. The constructed database can be efficiently used for the verification of existing algorithms or developing new algorithms through the quality control of various information and measured data on clinical trials. It also has several advantages in the control of clinical data. The results of this study can contribute to the establishment of Korean medical device data standard for the construction of integrated database management system that can share information with different kind of pulse analysis system and other bio-signal measuring devices.

The development of the internet and information management has enabled new applications which include: Electronic medical record (EMR), intelligent transportation, environmental monitoring, etc. In this paper, we design and implement the Continuity Care Record(CCR) Data Stream management server that compiled with DSMS and DBMS in EMR system for processing, monitoring the incoming CCR data stream and storing the processed result with high-efficiency. The proposed system enables users not only to query stored CCR information from DBMS, but also enables to execute continue query for the real-time CCR Data Stream. By using of CCR Viewer Application users can view or update their personal health records even compare self health care records with standard health care records in order to monitor the healthy status, and the on line updating information would be minimized and medical error.

The definition and concept of disasters and their preparedness have been changing according to the modern situation. The basic change is that the concept of absolute standard and prevention of hardware damage in the past have been changing to the concept of relative standard and mitigation of direct damage to human. For achieving the purpose, advanced countries developed and used their own analysis method of hazard and vulnerability for disaster ; ASHE hazard and vulnerability evaluation method, hazard matrix method by CDC, FEMA model method and SMUG hazard priority method. Because each analysis method cannot evaluate the hazard and vulnerability for specific disaster, the advantages and disadvantages should be applied for specific situation of disaster in Korea and new analysis method should be extracted in the future.

Background : To evaluate the frequency and cause of return to the emergency department within 48 hours and to identify the nature of the problem. Methods : We reviewed the medical records of 76 patients who returned to the emregency department within 48 hours from September 1998 to February 1999. Results : Overall revisit rate within 48hours was 2.6%. Of 76 patients, 5(6.6%) had planned return, 64(84.2%) had unplanned return and 7(9.2%) had incomplete documentation. The causes of unplanned return were inadequate medical management (11.8%), discharge against medical advice (27.6%), return after scheduled ambulatory care (22.4%), and unavoidable revisit due to symptom aggrevation or development of new symptom (22.4%). Conclusion : The study provided a basic information for us to improve the quality of emergency care by reducing unnecessary return to the emergency department. It is necessary to monitor continuously the quality of emergency care and to develop the standard of emergency return rate.

The development of health information technology enables people to access, view and acquire personal health record. But still, there have been a number of obstacles such as the absence of the standard to realize the ideal Personal Health Record(PHR) system. In this study, we proposed the service model that serves periodic Health Record Summary which is made by a medical specialist to people who are in the busy lives. Healthcare data from EMR in a hospital including people generate themselves at home is sent to a physician to make a medical opinion, and then it is changed into Health Level 7 Continuity of Care Document(CCD) format for interoperability. After a physician writes his opinion about patient's health condition, it will send to people by email. People who receive the health record summary data by email can save them into a USB device to view own PHR and medical comments of a physician through a computer. It will help people managing their own health condition with an opinion of a medical specialist.

For competitiveness of national B2C (Business to Customer) service industry, improvement of process and analysis focused on customer and change of service system are needed. To achieve this goal, accurate feedbacks from customers play an important role; however, there is no quantitative and standard system in Korea. The medical tourism industry is taking center stage as a high value added industry among many other B2C service industries. In this paper, we suggest an evaluation model for customer satisfaction measurement about medical tourism industry and the application system on smartphone for that.

Malfunctions and failures linked to medical devices may result in significant damage for human being. Thus, in order to ensure that safety of medical device is achieved, it should be established and applied the international standard. It is required to integrate and customize activities at standards, owing to reference relationship between standards, especially, activities based safety analysis is too expensive. This paper proposes a integration process that integrate activities of development lifecycle and safety process. Additionally, we derived a guidance based on STPA for integration process. As a result, we can be performed systematically from early stage of the development and increased effectiveness of integration process by the guidance.
