치과 의료기관 종사자의 방사선 방어에 대한 지식, 태도 및 행위 (An inquiry into dental personnel's Knowledge, attitude and behavior about the defense against dental radiation)

  • The purpose of this study was to understand the relationship among knowledge, attitude, and behavior of dental personnel about the defense against dental radiation according to general features, and help them recognize the importance of defense against dental radiation. A questionnaire survey of 187 dental personnel who were working in 121 medical institutions such as university hospitals, general hospitals, hospitals, clinics, and public health centers in Seoul area was conducted from December 2002 to January 2003. Study findings are as follows: 1. The average knowledge score for the defense of radiation was 9.51(full score=14): By the demographic features, the know level for the defense of radiation was appeared to be different with the occupational category, age, academic background, workplace, year of working, and whether having the education of radiation or not. By questionnaire items, the knowledge for the defense of radiation was high in general, but for the item. 'the average amount of being bombed with radiation according occupations cannot be over 20mSv in a year for prescribed 5 years'. it was 18.7%: and for the item. 'the longer the length between focus and film. the less the amount of skin exposure', it was 40.6%. showing less knowledge for these two items. 2. The average attitude score for the defense of radiation was 64.24 (full score=75): By the demographic features, the attitude score was higher with respondents who were 31-35 years old. 11-15 years of working. and having radiation education. The attitude score by questionnaire items was generally high for all the items. 3. The average behavior score for the defense of radiation was 45.43: (a) By the demographic features, the behavior score for the defense of radiation was appeared to be higher with respondents who were dentists for occupational category, the aged for age, holding higher diploma for academic background, working in the university hospitals for workplace, and having longer occupational career for the year of working. (b) The behavior score for the defense of radiation by questionnaire items was lower than that of the attitude score for all the items. For the item, 'the amount of X-ray radiation is adjusted according to such conditions as patients' age, radiating areas, and kind of films', the score was 4.03; and for the item, 'Must receive the education of safety management of radiation periodically', the score was 1.73 and it was the lowest one. 4. As for the correlation among the knowledge, attitude, and behavior for the defense of radiation, the higher the level of knowledge and attitude, the higher the level of behavior.

GC/MS에 의한 식품 포장재에 사용되는 접착제의 Toluene diisocyanate 분석 (Analysis of toluene diisocyanate of adhesives in food contact materials by GC/MS)

  • 식품 포장재에 사용되는 접착제의 원료로 사용되는 우레탄의 주원료인 톨루엔 디이소시아네이트(TDI)는 인체에 노출될 경우에 독성을 일으킬 수 있다. GC/MS를 사용하여 식품 포장재에 잔류하는 TDI를 분석하기 위한 시료 전처리 조건과 분석조건을 확립하였다. 포장재 중에 포함된 TDI는 n-헵탄, 4%아세트산 등과 같은 식품유사용매를 사용하여 추출된 후, 기체 크로마토그래프/질량분석기 (GC/MS)에 의해 분석되었다. 26개의 식품 포장재를 측정한 결과 10개의 시료에서 $0.51{\sim}60.88{\mu}g/ml$ 농도 범위에서 TDI가 검출되었다. 포장재의 절단된 면에 대한 추출 용매의 접촉을 피하고 포장재 면만이 접촉하도록 한 후 추출시간을 달리하면서 실험한 결과 2시간 추출 조건에서도 0.7% 수준으로 감소되어 실제 식품 조리시에 TDI가 다량 검출될 위험은 미미한 것으로 평가되었다. 본 연구에서 확립된 톨루엔 디이소시아네이트에 대한 분석법은 외국의 유해물질에 대한 관리기준을 만족시키는 정량한계를 확보한 신뢰성 있는 방법으로 추후 관련 연구를 위한 기초 분석기술로 활용도가 높을 것으로 예상된다.

산란선 제거를 위한 신개념 간접 평판형 검출기의 임상적용을 위한 최적 구조 : 입사 X선 각도에 따른 성능평가 (An Optimal Structure of a Novel Flat Panel Detector to Reduce Scatter Radiation for Clinical Usage: Performance Evaluation with Various Angle of Incident X-ray)

  • 진단용 X선 영상에서 산란선은 화질을 열화시키는 주요한 원인이다. X선 장치는 필름/스크린을 사용한 아날로그 시스템부터 Imaging plate (IP) 및 평판 검출기(Flat panel detector; FPD)를 사용한 디지털 시스템으로 바뀌어 가고 있다. 그러나 산란 X선 제거를 위한 Grid는 아날로그 시대에 사용됐던 구조부터 큰 변화가 없다. 본 논문에서는 선행연구에서 고안된 산란선 제거율을 향상시키기 위한 간접변환형 평판검출기의 새로운 구조를 다양한 입사 X선을 사용하는 임상현장에서의 활용 가능성을 검토했다. 일반적으로 FPD는 3개의 층으로 구성되어 있다. 신호를 검출하는 화소와 화소 사이에는 전압을 거는 voltage line이나 데이터를 전달하는 data line과 같은 X선 불감영역이 존재한다. 선행연구에서는 이 불감영역에 정확히 맞추어 방사선 불 투과성의 납을 그물 모양으로 substrate layer에 삽입함으로서 검출기 자체가 산란선 제거 효과가 있도록 설계하였다. 새로운 구조의 임상 유용성을 평가하기 위해, 삽입된 그물 모양의 납을 입사 X선에 대해 가로, 세로성분으로 나누어 각각의 성능을 확인하였으며, 동시에 납의 높이를 변화시켜 납 높이가 성능에 미치는 영향을 영상 대조도와 grid 노출 인자를 통해 검토했다. 검출기면에 대해 대각선으로 입사한 X선($0^{\circ}$, $15^{\circ}$, $30^{\circ}$)에 대해서, 입사 X선에 대해 평행한 가로성분이 세로 성분에 비해 높은 영상 대조도와 낮은 그리드 노출 인자를 나타냈으며, 세로성분의 납 높이가 높을수록 본 연구에서 고안한 검출기에 악영향을 미치는 것을 확인했다. 그러므로 본 연구에서 고안한 새로운 FPD 시스템은 FPD의 구조를 방사선검사 조건과 목적에 맞추어 최적화함으로써 임상 의료현장에서의 사용 가능성이 확인되었다.

사무직 근로자와 육체 노동자의 요통특성에 관한 비교 고찰 (Comparative Study of Low Back Pain between White Collar Workers and Blue Collar Workers)

  • This study has been attempted to be helpful for the back rehabilization of Korean workers by analyzing the general, occupational, social aspects of low back pain and to identify possible risk factors for back pain in White and Blue collar workers. The primary data were collected from 380 workers in Seoul city by means of a Questionnaire with random which was distributed from March 10 to 31, 1990. For the test of statistical significance, chi-square analysis was used to compare the back pain characteristics between above two groups. The results were as follows : 1. The incidence of low back pain in all walkers studied was $79.7\%$. The incidence of Blue collar with low back pain $(87.2\%)$ was higher than that of White collars $(75.0\%)$. 2. With regard to the relationship of back pain to the occupational characteristics, statistically significant differences were observed between workers with and without back pain concerning the job factors on work-time, job satisfaction, mental stress, chair fittness, work posture, work rotation, weight lifting, monotonous repetitive movements, vibration, and heavy noise (p<0.05). 3. With regard to the relationship of back pain to the social characteristics, there were no differences with respect to having car, personality types, drinking habits, and leisure-time activities. However, significant differences were showed between no pain and pain groups for the using bed, sleeping posture, traffic time amount, walking health state, smoking habits, and physical exercise (p<0.05). 4. The comparative analysis of back pain related to work factors showed highly significant differences with respect to mental stress, chair fittness, work posture, trunk rotation, weight lifting, monotonous repetitive work in White collar group (p<0.01) ; and job satisfaction, mental stress, trunk rotation, weight lifting, monotonus repetitive work, exposure to vibration and heavy noise in Blue collar group(p<0.01). 5. The comparative analysis of social factors in two groups showed differences with respect to the using bed, sleeping posture, walking amount, health state, physical exercise, smoking habits in White cellar group (p<0.05) ; and walking amount, traffic time amount, health state, smoking habits, physical exercise in Blue cellar group (p<0.05). 6. In regard to the general aspects of back pain between two groups, there were differences concerning etiolgy of back pain, counselling partners, treatment types, and sick-leaves(p<0.05), except pain duration, and awareness of back pain.

소아 두부 컴퓨터단층촬영검사에서 선량지표 분석을 통한 방사선 피폭선량 감소 방안에 대한 연구 (A Survey on the Radiation Exposure Doses Reduction Plan through Dose Index Analysis in the Pediatric Brain Computed Tomography)

  • 본 연구에서는 소아 두부 CT 검사에 있어서 피폭 방사선량을 경감할 수 있는 방법을 모색하고자 부산지역 병원들에서 시행된 년 간 소아 두부 CT 검사에 대한 CT 선량지표(CTDI)를 국내 진단참고준위와 비교하여 분석함으로써 제안점을 도출하고자 하였다. 결과적으로 병원별 분류에서는 병원, 종합병원, 상급종합병원 순으로 권고량이 초과하는 것으로 나타났으며 연령별 분류에서는 2-5세 이하, 1개월-1세 이하, 6-10세 이하 순으로 권고량이 초과하는 것으로 나타났다. 스캔방식별 분류에서는 Helical, Axial, Volume방식 순으로 권고량이 초과하고 있었으며 스캔범위를 진단참고준위에 맞게 재설정한 결과 상급종합병원에서 재설정으로 인한 감소율은 11.68%, 종합병원에서 15.79%, 병원에서 20.66%로 초과율이 높게 나타났다. 그리고 환자선량 권고량 가이드라인을 벗어나지 않는 환자의 평균 스캔범위의 분석결과 1개월-1세 이하에서 $103.2{\pm}11.8mm$, 2-5세 이하에서 $110.5{\pm}14.5mm$, 6-10세 이하에서는 $117.8{\pm}17.2mm$로 나타났다.

국내 거주 외국인의 침 치료 경험과 한의학에 대한 인식: 질적연구 (Perceptions and Experiences of Acupuncture among Expatriates Living in Korea: A Qualitative Study)

  • Objectives : In this study, we conducted a focus group interview among expatriates living in Korea, to investigate their experiences and perceptions of acupuncture and Korean Medicine in order to improve the usage of Korean Medicine in the medical tourism market. Methods : The inclusion criteria for the focus group interview was: (1) participants between the age of 20 to 75 years old; (2) those who had lived in Korea for more than 90 days, and (3) those who were able to express their thoughts freely in English. The participants were informed that the interviews will be recorded and transcribed. We ended recruitment once saturation of materials was met. Semi-structured interviews were conducted, and the data was analyzed after each interview. Results : The interviews were held between June to December 2016. Twenty-four participants were recruited but four dropped out and a total of twenty participants successfully finished the interviews. Six focus group interviews were conducted. Analysis revealed that Korean Medicine was relatively less known compared to Traditional Chinese Medicine or Complementary and Alternative Medicine. Participants automatically associated acupuncture with the management of pain or stress and replied that these were the areas that they thought acupuncture would be most effective. Positive experiences with acupuncture and with the acupuncture practitioners were factors that promoted the use of acupuncture whereas lack of awareness, accessibility and accreditation were factors that hindered its use. Other factors that hindered the use of acupuncture was needle phobia and the perception that acupuncture lacks scientific evidence. Conclusions : Awareness on Korean Medicine and acupuncture is low. Participants lack awareness on what diseases acupuncture can treat, the scientific evidence behind the mechanism of action, and the rigorous education system that Korean Medicine doctors must go through to get their license. Rigorous marketing should be encouraged, which includes greater exposure in the media, more honest reviews from patients, and promotion of the scientific evidence base.

해양성 Marinomonas sp. SH-2 균주가 생성하는 agarase의 분리 및 특성조사 (Characterization of Agarase Produced from the Isolated Marine Bacterium Marinomonas sp. SH-2)

  • 본 연구에서는 많은 생리활성 기능을 갖는 한천올리고당과 네오한천올리고당을 생산할 수 있는 agarase를 생성하는 신규 해양성 세균을 분리하고, 이 균주가 생성하는 한천분해효소의 특성을 조사하였다. 한천분해활성을 가진 신규의 SH-2 균주는 경상남도 남해군 연안에서 채취한 해수에서 분리하였으며, 16S rDNA 염기서열분석을 통해 Marinomonas 속 세균과 약 99% 유사하여 Marinomonas sp. SH-2로 명명하였다. Agarase는 Marinomonas sp. SH-2 균주의 배양액으로부터 추출하였으며, 한천분해활성을 측정한 결과, pH 6.0의 20 mM Tris-HCl buffer를 사용할 경우 30℃에서 최고 활성(170.2 units/l)이 나타났다. 하지만, 40℃ 이상의 온도에서 0.5시간 이상 처리할 경우 효소의 잔존활성이 40% 이하로 감소하는 것으로 보아 이 효소는 내열성을 가지지 않는다고 판단되었다. 효소의 가수분해산물을 TLC로 분석한 결과, Marinomonas sp. SH-2으로부터 생성되는 효소는 아가로스를 분해하여 neoagarohexaose와 neoagarotetraose를 생성하여 β-agarase로 확인되었다. 따라서 Marinomonas sp. SH-2와 이 균주의 한천분해효소는 식품, 화장품, 의약품 연구 등에 실용적으로 적용할 수 있을 것이다.

생활 습관, 주거지 주변 녹지 비율 및 지하수 내 라돈 농도 따른 실내 라돈 농도 영향 모델 (Affected Model of Indoor Radon Concentrations Based on Lifestyle, Greenery Ratio, and Radon Levels in Groundwater)

  • 라돈 및 그 자손은 폐암을 일으키는 환경적 위험인자로, 일상 활동 및 수면 등으로 많은 시간을 보내는 실내 라돈 농도 관리는 필수적이다. 이를 위해서는, 주거지를 둘러싼 개인적, 사회적, 환경적 요소에 대한 총체적 접근이 필요하다. 따라서 본 연구는 실내 라돈 농도에 영향을 미치는 다양한 인자를 찾아내고, 이를 활용한 포괄적 모델을 구축하고자 한다. 건축 자재 및 생활 양식을 포함한 주거 환경에 대한 자료를 얻기 위해 설문을 실시하였고, 의사결정트리 및 구조 방정식 모델링을 활용하였다. 그 결과 주거지 주변 녹지 비율, 불 투과성 층 비율, 주택과 지면의 맞닿은 상태, 매일 환기 습관, 난방 습관, 측정 장치 주위의 균열 및 침실여부는 실내 라돈 농도와 유의한 연관성을 보였다. 매일 환기 습관을 가질 경우 실내 라돈 농도가 $200Bq/m^3$ 이상인 비율이 11.6%로 줄었다. 한편 매일 환기습관이 없는 주거자의 주거지 주변 녹지 비율이 65% 이상이면 매일 환기 습관이 있는 주거자와 비교하여 15.3%의 비율이 증가하였다. 구축된 포괄적 모델의 실내 라돈 농도에 직접 영향을 미치는 인자는 주거지 주변 녹지 비율과 환기율이었다. 제시된 모델로 국내 라돈 농도에 대한 개인의 지리적 특성, 지하수 및 생활 양식 요소의 결합된 영향을 확인할 수 있었다.

Dietary Exposure to Transgenic Rice Expressing the Spider Silk Protein Fibroin Reduces Blood Glucose Levels in Diabetic Mice: The Potential Role of Insulin Receptor Substrate-1 Phosphorylation in Adipocytes

  • Type 2 diabetes mellitus (T2DM) is characterized by insulin resistance (IR). T2DM is correlated with obesity and most T2DM medications have been developed for enhancing insulin sensitivity. Silk protein fibroin (SPF) from spiders has been suggested as an attractive biomaterial for medical purposes. We generated transgenic rice (TR) expressing SPF and fed it to diabetic $BKS.Cg-m+/+Lepr^{db}$ mice to monitor the changes in blood glucose levels and adipose tissue proteins associated with energy metabolism and insulin signaling. In the present study, the adipocyte size in abdominal fat in TR-SPF-fed mice was remarkably smaller than that of the control. Whereas the adenosine monophosphate-activated protein kinase (AMPK)-activated protein kinase and insulin receptor substrate 1 (IRS1) protein levels were increased in abdominal adipose tissues after TR-SPF feeding, levels of six-transmembrane protein of prostate 2 (STAMP2) proteins decreased. Phosphorylation of AMPK at threonine 172 and IRS1 at serine 307 and tyrosine 632 were both increased in adipose tissues from TR-SPF-fed mice. Increased expression and phosphorylation of IRS1 at both serine 307 and tyrosine 632 in adipose tissues indicated that adipocytes obtained from abdominal fat in TR-SPF-fed mice were more susceptible to insulin signaling than that of the control. STAMP2 protein levels decreased in adipose tissues from TR-SPF-fed mice, indicating that STAMP2 proteins were reducing adipocytes that were undergoing lipolysis. Taken together, this study showed that TR-SPF was effective in reducing blood glucose levels in diabetic mice and that concurrent lipolysis in abdominal adipocytes was associated with alterations of AMPK, IRS1, and STAMP2. Increased IRS1 expression and its phosphorylation by TR-SFP were considered to be particularly important in the induction of lipolysis in adipocytes, as well as in reducing blood glucose levels in this animal model.

Enzyme-processed Korean Red Ginseng extracts protects against skin damage induced by UVB irradiation in hairless mice

  • UV irradiation is the main factor contributing to skin damages that are associated with an excessive production of matrix-degrading metalloproteinase (MMP)-1 and a deficient expression of collagens. To date, red ginseng has been revealed to possess many biomedical effects, such as anti-aging, anti-oxidation, and anti-inflammatory. In this study, we prepared the Korean Red Ginseng extracts treated with enzyme (KRGE) and investigated the effects of dietary KRGE on the formation of wrinkles generated by UVB irradiation in hairless mice. It was found that KRGE inhibited the UVB-induced formation of wrinkles, epidermal thickness, and skin dryness in hairless mice. Further results also showed that KRGE attenuated UVB-induced MMP-${\beta}$1 level, while accelerated procollagen type I, transforming growth factor-${\beta}$1 secretion. Interestingly, the expression of profilaggrin and filaggrin in both the epidermis and dermis were decreased due to UVB exposure and reversed by KRGE. The KRGE 0.06% was prior to KRGE 0.24%. In view of these results, which indicated that KRGE protected skin from UVB-induced photodamages, which may not only mediated by regulating of MMP-1 and procollagen type I, but also by increasing the production of profilaggrin and filaggrin. In conclusion, our results suggest that KRGE may be a promising agent for the treatment of skin photodamages. The challenge of KRGE will be expected as cosmeceuticals and nutraceuticals in order to intervene in aging-related degenerative skin changes.
