Proposal of a New TNM Classification for Gastric Cancer: Focusing on pN3b and Cytology-Positive (CY1) Disease

  • Purpose: Gastric cancer with lymph node metastasis (LNM) more than 15 (N3b) was defined as stage IV until the 6th AJCC system. However, it has been reclassified as a localized disease (stage IIb or III) since the 7th system. The aim of this study is to demonstrate that the survival of N3b is comparable to cytology-only positive (CY1-only) stage IV and to propose a new TNM system interpreting N3b as an eligibility criterion for receiving more intensive chemotherapy regimens. Materials and Methods: 1,430 patients who underwent gastric cancer surgery at Seoul National University Hospital from 2007 to 2012 were retrospectively analyzed. The 5-year survival rate (5YSR) and 3-year recurrence-free survival (RFS) were evaluated according to the 7th and 8th systems, as well as a new categorization based on N-classification; N0-2 (LNM<7), N3a (LNM 7-15), or N3b (LNM>15). Results: The survival of N3b is comparable to that of CY1-only stage IV (log rank test, P=0.671) and is distinct from that of grossly stage IV (log rank test, P<0.001). The survival of the remaining stage IIIc (T4bN3a) was comparable to those of N3b and CY1-only stage IV. Most N3b patients had significantly shorter 3-year RFS and mean RFS than those with IIb-IIIc, as if N3b itself was a higher TNM stage. Conclusions: In terms of survival, T4bN3a, N3b, and CY1-only stage IV were unified as stage IVa, while grossly stage IV was defined as stage IVb. N3b can be regarded as an eligibility criterion for undergoing more intensive chemotherapy regimens.

Assessment of Dose Distributions According to Low Magnetic Field Effect for Prostate SABR

  • Background: Stereotactic ablative radiotherapy (SABR) plans in prostate cancer are compared and analyzed to investigate the low magnetic effect (0.35 T) on the dose distribution, with various dosimetric parameters according to low magnetic field. Materials and Methods: Twenty patients who received a 36.25 Gy in five fractions using the MR-IGRT system (ViewRay) were studied. For planning target volume (PTV), the point mean dose ($D_{mean}$), maximum dose ($D_{max}$), minimum dose ($D_{min}$) and volumes receiving 100% ($V_{100%}$), 95% ($V_{95%}$), and 90% ($V_{90%}$) of the total dose. For organs-at-risk (OARs), the differences compared using $D_{max}$, $V_{50%}$, $V_{80%}$, $V_{90%}$, and $V_{100%}$ of the rectum; $D_{max}$, $V_{50%}$, $V_{30Gy}$, $V_{100%}$ of the bladder; and $V_{30Gy}$ of both left and right femoral heads. For both the outer and inner shells near the skin, $D_{mean}$, $D_{min}$, and $D_{max}$ were compared. Results and Discussion: In PTV analysis, the maximum difference in volumes ($V_{100%}$, $V_{95%}$, and $V_{90%}$) according to low magnetic field was $0.54{\pm}0.63%$ in $V_{100%}$. For OAR, there was no significant difference of dose distribution on account of the low magnetic field. In results of the shells, although there were no noticeable differences in dose distribution, the average difference of dose distribution for the outer shell was $1.28{\pm}1.08Gy$ for $D_{max}$. Conclusion: In the PTV and OARs for prostate cancer, there are no statistically-significant differences between the plan calculated with and without a magnetic field. However, we confirm that the dose distribution significantly increases near the body shell when a magnetic field is applied.

  • The structure of universities needs to be adjusted and reformed to cope with the decrease in admission resources and the quality of education due to the low birth rate and aging population. Such a policy is receiving much attention. To analyze the relative efficiency of private universities in Korea from the perspective of resource and performance, this study evaluated the efficiency of private university operation by applying a DEA(Data Envelopment Analysis) technique. The DEA measurements were compared with the diagnosis results of the department of education (Government) in 2018. The input and output variables used in the research analysis were utilized by the university's notification materials (public disclosure information). An analysis of the operational efficiency showed that 48% (12 universities) of the 25 DMUs (Decision Making Unit) were efficient for DEA-BCC models and that some of the capacity-building universities were operating efficiently. In addition, the DEA analysis found ways to improve inefficient groups through DEA-Additive results. This paper can be meaningful because it confirmed the relative efficiency of private universities and suggested improvement directions through the DEA method, which is characterized by the simultaneous consideration of various input and output factors. This will help apply the limited resources related to the input and output elements of each university.

A study on the standard for determining airborne sound insulation performance of sound barrier panels (방음판의 음향투과손실 측정규격에 관한 연구)

  • Sound barrier walls are one of the most effective alternatives for reducing environmental noise on roads and railways in the city center. The insertion loss of the sound barrier against road traffic noise is the sum of the sound transmission loss, sound absorption loss, and sound energy reduction due to the diffraction attenuation of the sound barrier. The sound transmission loss of the sound barrier is one of the important factors that determine the insertion loss of the sound barrier and is a basic indicator that determines the performance of the sound barrier. Nevertheless, there is not a separate standard in Korea for measuring the acoustic transmission loss of sound barrier panels. There are only a few conditions in KS F 4770 series that stipulates on the general material of sound barrier panels. This thesis examines the necessity of the acoustic transmission loss measurement and evaluation standards of sound barrier walls, and seeks a measurement method in a free sound field (anechoic chamber) sound receiving room considering the characteristics of sound barrier walls installed in external spaces, unlike indoor building materials. In addition, a single number evaluation method using a reference spectrum was proposed so that the sound insulation effect according to various installation places such as roadside or railroad side can be easily displayed.

  • Ship and bridge structures are a type of long box-shaped structure, and resistance to vertical bending moment is a key factor in their structural design. In particular, because box girders are repeatedly exposed to irregular wave loads for a long time, the continuous collapse behavior of structural members must be accurately predicted. In this study, plastic collapse behavior, including buckling according to load changes of the box girder receiving pure bending moments, was analyzed using a numerical analysis method. The analysis targets were selected as three box girders used in the Gordo experiment. The cause of the difference was considered by comparing the results of the structural strength experiment with those of non-linear finite element analysis. This study proposed a combination of the entire and local sagging shape to reflect the effect of the initial sagging caused by welding heat that is inevitably used to manufacture carbon steel materials. The procedures reviewed in the study and the contents of the initial sagging configuration can be used as a good guide for analyzing the final strength of similar structures in the future.

A Study of the Measurement of Nonwoven Geotextile Deformation with Strain Gauges (스트레인 게이지를 이용한 부직포의 변형거동 계측에 관한 연구)

  • Because of the increasing use of clayey soil as the backfill in reinfurced soil structures and embankments, nonwoven geotextiles of drain capability have been receiving much attention. However, there are few studies on the deformation behavior analysis of nonwoven geotextiles in reinforced soil structures in the site because nonwoven geotextiles which have low tensile stiffness and higher deformability than geogrids and woven geotextiles, are difficult to measure their deformation by using strain gauges. In this study, it was suggested that a new and more convenient method could measure the deformation behaviour of nonwoven geotextile using a strain gauge and examine the availability of the method by conducting laboratory tests and applying to two geosynthetics reinforced soil (GRS) walls in the site. The result of wide-width tensile test conducted under confining pressure of 70 kPa shows that the local deformation of nonwoven geotextile to be measured with strain gauges has a similar pattern to the total deformation measured with LVDT. In the GRS walls, nonwoven geotextile shows a larger deformation range than the woven geotextile and geogrid. However, the deformation patterns of these three reinforcement materials are similar. The function of strain gauges attached to nonwoven geotextile in the walls works normally for 16 months. Therefore, the method proposed in this study for measuring nonwoven geotextile deformation using a strain gauge has proved useful.

Comparison of Laparoscopic and Open Gastrectomy for Patients With Gastric Cancer Treated With Neoadjuvant Chemotherapy: A Multicenter Retrospective Study Based on the Korean Gastric Cancer Association Nationwide Survey

  • Purpose: Despite scientific evidence regarding laparoscopic gastrectomy (LG) for advanced gastric cancer treatment, its application in patients receiving neoadjuvant chemotherapy remains uncertain. Materials and Methods: We used the 2019 Korean Gastric Cancer Association nationwide survey database to extract data from 489 patients with primary gastric cancer who received neoadjuvant chemotherapy. After propensity score matching analysis, we compared the surgical outcomes of 97 patients who underwent LG and 97 patients who underwent open gastrectomy (OG). We investigated the risk factors for postoperative complications using multivariate analysis. Results: The operative time was significantly shorter in the OG group. Patients in the LG group had significantly less blood loss than those in the OG group. Hospital stay and overall postoperative complications were similar between the two groups. The incidence of Clavien-Dindo grade ≥3 complications in the LG group was comparable with that in the OG group (1.03% vs. 4.12%, P=0.215). No statistically significant difference was observed in the number of harvested lymph nodes between the two groups (38.60 vs. 35.79, P=0.182). Multivariate analysis identified body mass index (odds ratio [OR], 1.824; 95% confidence interval [CI], 1.029-3.234; P=0.040) and extent of resection (OR, 3.154; 95% CI, 1.084-9.174; P=0.035) as independent risk factors for overall postoperative complications. Conclusions: Using a large nationwide multicenter survey database, we demonstrated that LG and OG had comparable short-term outcomes in patients with gastric cancer who received neoadjuvant chemotherapy.

The survey on foreign body ingestion and aspiration during the dental prosthetic treatment (치과 보철치료 과정에서 발생하는 이물질의 삼킴과 흡인에 대한 실태 조사)

  • Purpose: This study was to investigate the dentists' experience and awareness of foreign body ingestion and aspiration during dental prosthetic treatment. Materials and Methods: A survey of 157 dentists working at 108 dental institutions in Daejeon and Chungcheong provinces was carried out and frequency analysis was conducted. Chi-square test was conducted to find out the difference between the sociodemographic data and experience of foreign body ingestion and aspiration of patients and independent sample t-test was conducted to find out the difference in awareness depending on whether receiving related education. Results: The percentage of dentists experiencing dropping foreign body into patients' oral cavity was 99.4% and the percentage of dentists experiencing foreign body ingestion and aspiration of patients was 53.5%. There were more dentists who experienced foreign body ingestion and aspiration of patients in male than female, longer working career, and general practitioners than specialists (P < .05). 50.3% of the respondents received related education. When they receive related education, they had low degree of concern, high confidence in coping with situations, and low willingness to receive education in the future (P < .05). Conclusion: The percentage of foreign body dropping was 16% higher in prosthetic treatment than general treatment. The percentage of dentists with experience of foreign body ingestion and aspiration of patients was 53.5%. Among them, 92.9% experienced foreign body ingestion of patients and 7.1% of them experienced foreign body aspiration of patients.

  • The traditional economic order quantity (EOQ) model is analyzed under the basic assumption that the purchase price is paid immediately upon receiving the product. However, product suppliers may allow a certain period of deferral of payment for product purchase costs in order to differentiate themselves from competitors. From the distributor's point of view, such a credit transaction can temporarily divert product purchase costs, resulting in a reduction in inventory investment costs, and ultimately, a factor that lowers the selling price for the purpose of increasing end-customer demand can be. In addition, in that credit transactions are provided for the purpose of increasing the demand of suppliers as a means of differentiation from competitors, it is more general to be allowed flexibly according to the transaction volume. In this regard, assuming that the end customer's demand is represented by a linear decreasing function of the distributor's selling price, this study analyzes a model for determining the distributor's pricing and ordering policies under order-size-dependent delay in payments. For the analysis, we also assume that the inventory is depleted not only by customer's demand but also by decaying.

Measurement and Comparative Analysis of Propagation Characteristics in 3, 6, 10, and 17 GHz in Two Different Indoor Corridors (두 가지 서로 다른 실내 복도에서 3, 6, 10, 17 GHz의 전파 특성 측정 및 비교 분석)

  • Propagation characteristics in line-of-sight(LOS) paths in 3, 6, 10, and 17 GHz frequency bands were measured and analyzed in two different indoor corridors: second floors of Buildings D2 and E2. The measurement was designed to measure when the receiving antenna moved at 0.5 m intervals from 3 m to 30 m, while the transmission antenna was fixed. The analysis of the two indoor corridors was compared by applying basic transmission loss, root mean square (RMS) delay spread, and K-factor. For basic transmission loss, the loss coefficient of the floating intercept path loss model was higher in the indoor corridor of Building E2 than in that of Building D2. Similarly, the RMS delay spread in the time domain was greater in the indoor corridor of Building E2. However, the indoor corridor of Building D2 exhibited higher K-factor in the 3, 6, and 17 GHz bands with lower wave propagation in the 10 GHz band. Despite the 2 indoor corridors being identical, the propagation characteristics varied due to different internal structures and materials. The results provide measurement data for ITU-R Recommendations regarding various indoor environments.
