경추 신경근병증의 통증, 관절가동범위, 경부장애에 대한 도수치료의 효과 (The Effects of Manual Therapy on Pain, ROM and Disability of Cervical Radiculopathy)

  • Background: Cervical radiculopathy is the result of cervical nerve root pathology that may lead to chronic pain and disability. Although manual therapy interventions including cervical traction and neural mobilization have been advocated to decrease pain and disability caused by cervical radiculopathy, their analgesic effect has been questioned due to the low quality of research evidence. The purpose of this paper is to present the effect of manual therapy on pain, ROM, disability in a patient experiencing cervical radiculopathy. Methods: In this study, 30 participants who met the diagnostic criteria for cervical radiculopathy were randomized into two groups: group I (nerve mobilization group) & group II (Mckenzie exercise group). The experimental group was performed manual cervical traction and neural mobilization technique. The control group was performed manual cervical traction and cervical retraction, extension exercise. Assessments were performed to 30 participants before and after 4 weeks therapy. The components of assessments were pain intensity (PI), cervical rotation ROM (CR) and neck disability index (NDI). Results: After 4 weeks therapy, PI, CR and NDI were significantly reduced in both groups (p<.01). The PI, CR and NDI were no significantly reduced between group I and group II (p>.01). Conclusions: Manual therapy could reduce the symptoms of cervical radiculotpathy.

Effects of Manual Therapy on Upper Extremity Pain after Breast Cancer Surgery: A Systematic Review and Meta-analysis

  • Objective: The most common type of cancer in women is breast cancer, and pain in the upper extremity and trunk is a discomfort experienced by more than half. Based on the evidence that manual therapy is effective for pain control in postoperative rehabilitation, this study aims to analyze the effects of manual therapy on upper extremity pain and function in patients after breast cancer surgery. Design: A systematic review and meta-analysis. Methods: We searched MEDLINE, Embase, PEDro, and CINAHL databases until August 2021. We included randomized controlled trial evaluating pain and function in patients after breast cancer surgery. Qualitative analysis was performed using Cochrane's risk of bias tool, and quantitative analysis was performed using RevMan 5.4 to analyze post-intervention outcomes. Results: Four randomized controlled trials were selected to evaluate the effects of upper extremity pain and function in 133 patients who underwent manual therapy after breast cancer surgery. In the results of qualitative and quantitative analysis, the experimental group treated with manual therapy showed a significant improvement in pain compared to the control group (-0.62; 95% confidence interval (CI) -0.97 to -0.27). However, there was no significant improvement in upper extremity function (-0.09; 95% CI -0.43 to 0.25). Conclusions: Current evidence shows that manual therapy is effective for pain control in patients who complain of upper extremity pain after breast cancer surgery.

  • PURPOSE; The main purpose of this article is to document correctly of the orthopedic manual therapy's history in Korea. Therefore this study aimed not only to know the root of the historical Korean OMT, but also to develope of the Korean manual therapy advanced in future. RESULT; The results of this study were as following; 1. The first introduction for OMT in Korea is the '1st International Orthopedic Physical Therapy Seminars' at Seoul National University Hospital by Prof. Russell M. Woodman etc(Quinnipiac college, USA), 1991. 1.16-1.21. 2. The first OMT study group in Korea is to establish the 'Korean Study Group of OMT at Hallym University Kangdon Sacred Heart Hospital, 1992.2.22. 3. The first OMT journal in Korea is the 'Jr. of KAOMT(1:1)', 1995.10.10.

두통을 동반한 근막성 턱관절 장애 환자의 관리를 위한 정형도수치료기법과 가정 자가-치료적 운동의 적용: 사례연구 (Use of Orthopedic Manual Physical Therapy and Home Self-Therapeutic Exercise to Manage Myofascial Temporomandibular Disorder Accompanied by Headache: Case Study)

  • Purpose: The current case study focuses on identifying the effects of manual therapy and home self-therapeutic exercise including on mouth opening and pain relief in patients with continuous neck pain with myofascial temporomandibular disorders (TMDs) accompanied by headache induced by masticatory myalgia Subjects: The study participant was a 27-year-old woman who was treated a year ago for pain related to TMDs accompanied by a headache. Methods: Manual therapy of the cervical spine with upper cervical spine posterior-to-anterior mobilization (C1~C2), upper cervical spine flexion mobilization (C0~C2), upper cervical spine lateral flexion mobilization (C0~C1), upper cervical spine thrust manual therapy (C1~C2) and manual therapy of the temporomandibular joint and muscles with transverse medial accessory temporomandibular joint mobilization, manual therapies for the temporal, the masseter, and medial pterygoid muscles were performed twice a week for about 30 minutes for 4 weeks. This protocol included 3 sessions in total. The home self-therapeutic exercise was to be performed two to three times a day. Results: The values more improved MMO increased to 41.4 mm, left masseter muscle PPT to 2.9 kgf/cm2, right masseter muscle PPT to 3.1 kgf/cm2, KHIT-6 to 46 points, neck pain intensity (by NRS) to 2 points, headache frequency to per weeks, cervical kyphotic angle to -8.06%, and GCPS to grade 1 (low-intensity pain without pain-related disorder). Conclusion: Manual therapy and home self-therapeutic exercise can be helpful for mouth opening and pain relief in patients with myofascial TMDs accompanied by secondary headaches induced by masticatory myalgia.

The Effects of Group Exercise, Manual Therapy and Home Exercise on Pain, Range of Motion and Function in Patient with Adhesive Capsulitis

  • Purpose: The purpose of this study was to determine the effects of group exercise, manual therapy, and a home exercise program on VAS (visual analogue scale), ROM (range of motion) of shoulder, and functional outcome in adhesive capsulitis. Methods: The study was conducted with 45 patients. All subjects were randomly assigned to 3 groups: group exercise (n=15), manual therapy (n=15), home exercise (n=15). The subjects performed an intervention program 3 times a week for 4 weeks a total of 12 times. Outcome measurements were VAS, ROM, and Constant-Murley score outcome measures. Measurements were taken four times, at baseline, 4 weeks later, 8 weeks later, and 12 weeks later. Results: VAS, ROM of the shoulder, and functional outcomes showed improvement in all groups at the final follow-up (p<0.05). VAS and ROM showed greater improvement with group exercise and manual therapy than home exercise, however, there were no significant differences between the two groups (p>0.05). Functional outcomes showed the greatest improvement in group exercise at the final follow-up. This improvement was greater than with manual therapy or home exercise (p<0.05). Conclusion: Group exercise and manual therapy were more effective for improving pain, ROM, and function than home exercise in adhesive capsulitis. In particular, group exercise is more effective in functional recovery than other physiotherapy interventions.

요추부 전방전위증 환자의 도수치료 효과 연구 (Effects of Manual Therapy Approach for Lumbar Spondylolisthesis)

  • Purpose : The purpose of this study is to find out the evidence concerning the effectiveness of manual therapy intervention in the treatment of low back pain related to spondylolisthesis Methods : 12men with lumbar pain from L5~S1 spondylolisthesis of 2 or 3 grade were treated during 12-week period. The manual therapy applied twice per week. The results of back and radiating pain were measured by VAS, and abdominal muscle strength was measured by Power track II. The measurement of degree of slip in spondylolisthesis was confirmed by the lateral view of X-ray and took a measurements of the anterior displacement of a vertebral body in relation to the vertebral below. Results : 1. There was significant decrease in the back and radiating pain.(p<0.05) 2. There was significant increase in the abdominal muscle strength.(p<0.05). 3. There was significant decrease in the degree of the anterior displacement. Conclusion: The intervention of manual therapy for lumbar spondylolisthesis is effective in back pain, radiating pain, abdominal muscle strength and degree of the anterior displacement. So, nonsurgical treatment should be attempted before surgical treatment.

급성 요추염좌에 대한 도수치료와 치료적 운동이 미치는 효과 : 단일사례연구 (The Effects of Manual Therapy and Therapeutic Exercise in Patient with Acute Lumbar Sprain: Case Study)

  • Background: The purpose of this study was to effect of the manual therapy and pelvic floor muscle exercise interventions in patient with acute lumbar sprain. Based on this, proceed to present an effective physical treatments. Methods: Subject was 55 year old female patient with acute lumbar sprain. Subject was damaged, wash in the morning. Subject was hoping to return to work and Activity daily living (ADL) without pain. I proceed in order screening, evaluation, diagnosis, prognosis, treatment planning and intervention, re-screening. To solving problem, was conducted in parallel to a manual therapy and therapeutic exercise. Results: To investigate changes in body functions and activities was measured, Visual analog scale (VAS) and Oswestry disability index (ODI), changes in posture, maintain in posture, 10m walk test. The improved results were compared before and three weeks after mediation interventions. Conclusions: Frequency manual therapy and pelvic floor muscle exercise is thought to be an efficient way of patients with acute lumbar sprain.

원발성 고혈압에 대한 추나요법의 효과 : 체계적 문헌 고찰 (Chuna Manual Therapy for Essential Hypertension : A Systematic Review)

  • Objectives : To determine the evidence of effectiveness and safety of Chuna manual therapy(CMT) for essential hypertension Methods : We searched 11 electronic databases(Pubmed, Web of Science, EMBASE, Cochrane Library, CAJ, KISTI, KISS, NDSL, KMBASE, RISS, DBpia) and related 2 journals up to April 2017. We included randomized controlled trials(RCTs) of testing Chuna manual therapy for hypertension patients. Results : Twenty one RCTs were eligible in our inclusion criteria. The meta-analysis of eighteen studies showed positive results for the using Chuna manual therapy for essential hypertension. Conclusions : There is favorable evidence of Chuna manual therapy for treating essential hypertension with meta-analysis. However, our systematic review has limited evidence to support Chuna manual therapy for essential hypertension because of low quality of original articles and further well-designed RCTs should be encouraged.

  • Objectives : The purpose of this study is to evaluate the Korean medical treatment for chronic tension-type headache, especially chuna manual therapy on cervical vertebrae. Methods : One patient was treated with acupuncture, herbal medication and chuna manual therapy on cervical vertebrae. To evaluate the pain of head and neck, visual analog scale(VAS) was measured. Results : After treatment for 1 week, the pain of head and neck was declined from VAS 10 to VAS 3. Conclusions : Korean medical treatment including acupuncture, herbal medication and cervical chuna manual therapy is effective for chronic tension-type headache. But further studies are required to prove the effectiveness of chuna manual therapy on cevical vertebrae.

경추성 현훈에 대한 추나요법의 치료 효과 : 체계적 문헌고찰 (Chuna Manual Therapy for Cervicogenic Dizziness : A Systematic Review)

  • Objectives : To evaluate the evidence for effectiveness of Chuna manual therapy, for cervicogenic dizziness Methods : We conducted a search across 6 electronic databases to find all randomized controlled clinical trials(RCTs) that discuss Chuna manual therapy till September 2018. Results : 7 RCTs were selected for our inclusion criteria. The systematic review showed positive results for using Chuna manual therapy, for treating cervicogenic dizziness. Conclusions : Our systematic review found that effectiveness Chuna manual therapy for cervicogenic dizziness. However, our systematic review had several limited evidence. We recommend an additional research to test the effectiveness of Chuna manual therapy.
