  • This study was performed to measure the mandibular movement and the changes of masicatory and sternocleidomastoid muscle activity reflected by occlusal disturbance during habitual chewing. For this study, 18 subjects(14 males and 4 females with an average age of 24.0) were selected. The impression of each subject were taken for measuring intermolar distance on lower dentition. The activities of masticatory and sternocleidomastoid muscle and the mandibular movement were recorded and analyzed during habitual chewing by means of E.M.G.(electromyograph), E.G.N.(electrognathograph), rotate program in BioPak analyzing system(BioResearch Inc.). 전산화단층촬영법을 이용한 하악 전돌증 환자의 하악지 시상 골절단술후 하악과두 위치변화 분석 (EVALUATION OF CONDYLAR POSITION USING COMPUTED TOMOGRAPH FOLLOWING BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY)

  • 본교실에서 하악지 시상 골절단술을 시행한 하악전돌증 환자 20명(남자 9명, 여자 11명)을 대상으로 술전, 술직후, 장기관찰기간동안의 전산화단층촬영법을 이용한 하악과두의 위치변화와 술전, 술직후, 악간고정제거 24시간후 및 장기관찰기간동안의 측모두부방사선사진에서의 재발과의 상관관계를 연구한 결과 다음과 같은 결과를 얻었다. 1. 두부 축방향 전산화단층사진에서의 과두간거리(MM')는 $84.45{\pm}4.01mm$ 였으며, 장축각은 우측 11.89%5.1 $9^{\circ}$좌측 $11.65{\pm}2.09^{\circ}$로 좌우 비슷하였으며, 기준선(AA')에서 과두의 외측점은 12mm, 내측점은 7mm 정도 전방에 위치하였다. 관상면 전산화단층사진에서의 과두간 거리(mm')는 $84.43{\pm}3.96mm$ 였으며, 사축각은 우측 $78.12{\pm}3.43^{\circ}$ 좌측 $78.09{\pm}6.12^{\circ}$로 좌우 비슷하였다 . 2. 술전후 과두위치 변화(T2C-T1C)는 통계적인 유의성은 없었으나(p>0.05), 다소 증가하는 경향을 보였으며, 장기관찰에 따른 회귀성향(TLC-T2C)에서는 LMD, LLD(p<0.05), RLD, RMD(p<0.01), mm'(P<0.001)는 모두 감소하였다. 3. 측면두부방사선사진상에서 술직후와 악간고정제거 24시간후(T3-T2)에서 하악의 초기재발은 통계적인 의의가 없었으며(p>0.05) 하악전치의 경우만 평균 0.33mm 전방이동하였다(p<0.05). NN'L1, NN'Pog, NN'Gn, NN'Me, over-jet에서 통계적인 유의성이 있었으며(p<0.05), NN'Ll 1.2%, NN'B 5.0%, NN'Pog 2.0%, NN'Gn 9.1%, NN'Me 10.3%의 총재발량을 보였다. 4. 하악골의 술전, 술후 변화량(T2-T1)이 총재발량에 미치는 영향에 대한 회귀분석에서 후퇴량이 많을수록 하악골의 총재발량이 많은 것으로 나타났다(p<0.05). 5. 하악골 변화량(T2-T1)과 하악과두 변화량(T2C-T1C, TLC-T2C), 하악과두 변화량(T2C-T1C, TLC-T2C)과 총재발량(TL-T2), 술전 하악과두 형태(T1C)와 하악과두 변화량(T2C-T1C, TLC-T2C), 그리고 술전 하악골 외형(T1)과 하악과두 변화량(T2C-T1C, TLC-T2C)에 대하여 단순 및 복잡회귀분석에서 통계적인 유의성은 없었다(p>0.05). 6. 술전 하악과두 형태(T1C)가 총재발량(TL-T2)에 미치는 영향에 대한 복잡회귀분석결과 우측과두에서는 과두간 거리가 멀고 장축각이 적고 사축각이 클수록 하악골의 수평적인 재발(NN'Ll, NN'8, NN'Pog, NN'Gn, NN'Me)이 많은 것으로 나타났으며(p<0.05), 좌측과두에서는 NN'Ll, NN'Me에서 같은 결과를 보였다. 7, 술전 하악과두 형태(T1C)가 술전 하악골외형(T1)에 미치는 영향에 대한 복잡회귀분석결과 우측과두에서는 과두간 거리가 멀고 장축각이 적고 사축각이 클수록 하악골의 수직고경이 큰 것으로 나타났고(p<0.05), 좌측과두에서는 과두간 거리가 멀고 장축각이 적고 사축각이 클수록 수직고경이 크며, 전돌된 양상으로 나타났다(p<0.05) . 8. 술전 하악골 외형(T1)이 총 재발량(TL-T2)에 미치는 영향에 대하여 단순회귀분석을 시행한 결과 NN'L1, NN'B, NN'Gn, NN'Me, over-jet 둥의 계측점에서 하악골이 전돌된 양상을 보일수록 NN'B에서의 총재발량은 많은 것으로 나타났다(p<0.05). 또 수평피개량(over-jet)이 클수록 NN'B, NN'Pog, NN'Gn, NN'Me에서의 총재발량이 많은 것으로 나타났다(p<0.05). 따라서 과두보존술을 이용하여 과두를 안정화시키고 난후 악골 형태를 재구성하였을 때 이로 인하여 생기는 하악과두 이동은 미미하여 술후 재발에 크게 영향을 미칠 정도는 아니라고 사료된다.

측두하악장애 환자에서 하악과두의 퇴행성 변화의 발생 양상에 대한 연구 (The Occurrence of Degenerative Change in the Mandibular Condyles of Korean Patients with Temporomandibular Disorders)

  • 골관절염은 관절의 연조직, 관절연골 및 연골하 골조직의 합성과 분해 사이의 불균형으로 인한 조직의 변성을 야기하는 관절의 질환이다. 일반적으로 골관절염은 연령에 따라 급격히 증가하고, 여성에서 호발하는 특징을 보이는 것으로 알려져 있지만, 측두하악관절에서의 골관절염의 유병률에 관한 연구는 측두하악관절의 독특한 특성에도 불구하고 정확한 연구가 부족한 실정이다. 본 연구에서는 측두하악관절에서 발생하는 골관절염의 실제 성별, 연령별 분포 및 임상적 특징을 알아봄으로 써 병인의 이해에 필요한 기초 자료를 제시하고자 한다. 본 연구에서는 2007년 1월부터 12월까지 1년 동안 서울대학교 치과병원에 측두하악장애를 주소로 처음 내원한 4327명의 환자를 대상으로 하였다. 측두하악장애의 진단을 위해 촬영된 orthopantomogram, TMJ tomogram, transcranial radiograph를 이용하여 측두하악관절의 퇴행성 변화를 조사하였으며, 성별, 연령, 골변화 양상에 따른 분포를 분석하여 다음과 같은 결과를 얻었다. 1. 측두하악장애 증상을 호소하는 환자 4327명 중 883명 (20.4%)에서 퇴행성 변화가 관찰되었고, 남성에서는 177명 (12.6%), 여성에서는 706명 (24.1%)에서 관찰되어 여자에서 호발하는 것으로 관찰되었으며, 이러한 여성 우위의 호발 양상은 모든 연령대에서 관찰되었다. 2. 측두하악장애 증상을 호소하는 환자 중 하악과두의 퇴행성 변화를 보이는 환자의 비율은 연령대가 증가함에 따라 완만하게 증가하였고, 연령별로 구분하여 볼 때에는 여성에서는 연령대별 발생비율의 증가가 통계적으로 유의하였으나 (P < 0.001), 남성에서는 통계적으로 유의하지 않았다. 3. 퇴행성 변화 환자군에서 나타나는 퇴행성 변화의 유형 중, 발생빈도가 가장 높은 형태는 sclerosis였고 (84.6%), 발생 환자 의 평균연령이 가장 높은 형태는 osteophyte였다 (44.8세). 4. 퇴행성 변화의 좌, 우 발생 분포를 보면 좌측에서 호발하였고, 남성 내에서는 좌측, 여성 내에서는 양측에서 모두 발생한 비율이 가장 높았으나, 전체적인 좌, 우 발생 양상은 남녀간에 통계적으로 유의한 차이를 보이지 않았다. 결론적으로, 측두하악장애 환자에서 나타나는 측두하악관절의 퇴행성 변화는 여성에서 호발하고 연령이 증가함에 따라 완만한 증가를 보이며 젊은 연령층의 환자에서도 높은 비율로 발생함을 알 수 있다. 측두하악장애가 젊은 연령층에서 호발한다는 이전의 보고들을 토대로 볼 때 측두하악관절의 퇴행성 변화는 타관절에서 발생하는 퇴행성 변화와는 다른 역학적 특징을 가지고 있을 가능성이 크며, 이러한 사실은 측두하악관절에서 발생하는 퇴행성 변화의 병인을 이해하고 그 치료법을 개발하는데 있어서 매우 중요한 의미를 가진다.

Novel condylar repositioning method for 3D-printed models

  • Background: Along with the advances in technology of three-dimensional (3D) printer, it became a possible to make more precise patient-specific 3D model in the various fields including oral and maxillofacial surgery. When creating 3D models of the mandible and maxilla, it is easier to make a single unit with a fused temporomandibular joint, though this results in poor operability of the model. However, while models created with a separate mandible and maxilla have operability, it can be difficult to fully restore the position of the condylar after simulation. The purpose of this study is to introduce and asses the novel condylar repositioning method in 3D model preoperational simulation. Methods: Our novel condylar repositioning method is simple to apply two irregularities in 3D models. Three oral surgeons measured and evaluated one linear distance and two angles in 3D models. Results: This study included two patients who underwent sagittal split ramus osteotomy (SSRO) and two benign tumor patients who underwent segmental mandibulectomy and immediate reconstruction. For each SSRO case, the mandibular condyles were designed to be convex and the glenoid cavities were designed to be concave. For the benign tumor cases, the margins on the resection side, including the joint portions, were designed to be convex, and the resection margin was designed to be concave. The distance from the mandibular ramus to the tip of the maxillary canine, the angle created by joining the inferior edge of the orbit to the tip of the maxillary canine and the ramus, the angle created by the lines from the base of the mentum to the endpoint of the condyle, and the angle between the most lateral point of the condyle and the most medial point of the condyle were measured before and after simulations. Near-complete matches were observed for all items measured before and after model simulations of surgery in all jaw deformity and reconstruction cases. Conclusions: We demonstrated that 3D models manufactured using our method can be applied to simulations and fully restore the position of the condyle without the need for special devices.

  • This study was intended to evaluate condyle position and the relationship of condyle position change and post surgical relapse following the sagittal split ramus osteotomy for mandible setback in 25 patients by paired t-test and multiple regression analysis. We used oblique transcranial and cephalometric radiographs taken before operation, immediate after operation, and at least 6 months post operatively. 1. In oblique transcranial view, posterior joint space was decreased immediate after operation and increased 6 months after operation. To compare the measurement before and 6 months after operation, there was no statistically significant change in over all joint spaces(P>0.05). 2. The joint spaces changed under the 0.2mm were 30%, 0.2mm to 1.0mm were 60.7%, above 1.0mm were 9.3%. This result reveals that condyle position was relatively reproduced to pre-operative state. 3. Statistically, the amount of mandible set back didn't influence the post operative relapse(P>0.05). 4. Statistically, the amount of mandible set back didn't influence the condylar displacement(P>0.05), and the amount of joint space change didn't influence the post operative relapse.(P>0.05) The changes in joint space is in the standard tracing error or within the adaptive capacity of the individual, it was too small to influence the stability of surgery.

파노라마방사선사진에 있어서 이차상에 관한 연구 (A STUDY ON SECONDARY IMAGES IN PANORAMIC RADIOGRAPH)

  • This study was performed to observe the secondary images and to analyse the relationships between the primary and secondary images in panoramic radiograph. Using the Moritta's Panex-EC panoramic x-ray machine and the human dry skull, the author analysed 17 radiographs which were selected from 65 radiographs of the dry skull that attached the radiopaque materials, and the attached regions of the radiopaque materials were the normal anatomical structures which were important and selected as a regions for the evaluation of the secondary images effectively. The results were as follows; 1. The cervical vertebrae showed three images. The midline image was the most distorted and less clear, and bilateral images were slightly superimposed over the posterior border of the mandibular ramus. 2. In mandible, the secondary image of the posterior border of the ramus was superimposed on the opposite ramus region, and this image was elongated from the anterior border of the ramus to the lateral side of the posterior border of the ramus. The secondary image of the condyle was observed on the upper area of the coronoid process, the sigmoid notch and the condyle in opposite side. 3. In maxilla, the posterior region of the hard palate showed the secondary image on the lower part of the nasal cavity and the medial wall of the maxillary sinus. 4. The primary images of the occipital condyle and the mastoid process appeared on the same region, and only the secondary image of the occipital condyle was observed symmetrically on the opposite side with similar shape to the primary one. 5. In the cranial base, the anatomical structures of the midsagittal portions like a inferior border of the frontal sinus, sella turcica, inferior borderr of the sphenoid sinus and inferior border of the posterior part of the occipital bone showed the simillar shape between the primary and secondary images symmetrically. 6. The petrous portion of the temporal bone showed the secondary image on the lateral side of the sella-turcica, and the secondary images of the posterior border of lesser wing, superior border of greater wing of the sphenoid bone and posterior border were observed on the anterior-superior and inferior region of the sella-turcica.

Lateral arthroplasty along with buccal fat pad inter-positioning in the management of Sawhney type III temporomandibular joint ankylosis

  • Objectives: The objective of this study was to highlight the role of lateral arthroplasty along with interposition of the buccal fat pad (BFP) in the management of Sawhney type III temporomandibular joint (TMJ) ankylosis. Materials and Methods: Ten patients with TMJ ankylosis (7 unilateral and 3 bilateral, total of 13 joints) were treated with lateral arthroplasty and BFP interposition. The bony bridge of the ankylotic mass on the lateral aspect was resected, leaving a distance of 1.5 to 2.0 cm from the base of the skull to the neck of the condyle. The condyle was left intact. Coronoidectomy was performed on the ipsilateral side via the same approach in all cases. The inter-incisal opening was measured at that time, and if it was less than 35 mm, contralateral coronoidectomy was performed by using the intra-oral approach. After satisfactory inter-incisal mouth opening (${\geq}35mm$) was achieved, the TMJ surgical site was revisited, and BFP was retrieved and used to cover the lateral aspect of the medially placed condyle. Results: With lateral arthroplasty, the medially displaced condyle can be left in-situ to maintain the mandibular ramal height and function and to act as a growth center in children. Interposition of the BFP prevents reformation of the lateral bony bridge that was removed. Conclusion: Lateral arthroplasty along with interpositioning of the BFP is a novel technique for managing Sawhney type III ankylosis that achieves management goals while avoiding complex and advanced reconstructive surgical procedures.

가령에 따른 백서 악관절 원판의 형태 및 미세구조적 변화 (CHANGES IN THE SHAPE AND ULTRASTRUCTURE OF THE ARTICULAR DISC OF THE RAT MANDIBULAR JOINT WITH AGING)

  • The purpose of this study was to investigate changes in the shape and ultrastructure of the articular disc of the rat mandibular joint with aging. Mechanical stress applied to the articular disc changes during neonatal, suckling, juvenile, adult and senile stages. Mandibular joints of 6 groups of rats(1-, 7-, 17-, 27-, 55-day and over-1-year groups) were removed en bloc and processed for light and electro microscopic study. The changes in the shape of articular disc were examined by light microscope in each group. Structural and ultrastructural changes in the articular disc were examined by light and electron microscope in each group. The results were as follows : In the 1-day and 7-day groups, the articular disc was long and slender in shape and the articular disc was not fitted with the shape of the mandibular fossa and condyle. However' after that time, the anterior and posterior portions of the articular disc were more bulged and the middle portion was shorter and biconcave. Thus the articular disc was well fitted with the shape of the mandibular fossa and condyle. The cell density decreased with aging. In the l -day and 7-day groups, the Golgi apparatus, rough endoplasmic reticulum and free ribosome, which are involved in the synthesis of intracellular and extracellular matrix, were developed. In the 17-day, 27-day and 55-day groups, not only the cell organelles involved in the synthesis of the intracellular and extracellular matrix but also the cell organelles involved in the remodeling of the extracellular matrix(i.e., finger-like cell process, lysosome and mitochondria)were well developed. With advancing age, intracytoplasmic microfilaments were more accumulated and condroid cells increased. In the over-1-year group, the majority of cells of the articular disc were chondroid cells. The majority of cytoplasmic compartment were filled with intracytoplasmic microfilaments and cell organelles were not developed. Therefore, metabolic activities of the cell was markedly reduced and cells contained structures enduring mechanical stress, and cells which were in the process of degeneration were observed occasionally.

Open versus closed treatment for extracapsular fracture of the mandibular condyle

  • Objectives: Selection of treatment methods for mandibular condylar fractures remains controversial. In this study, we investigated treatment methods for condylar fractures to determine the indications for open or closed reduction. Patients and Methods: Patients >12 years of age treated for mandibular condylar fractures with a follow-up period of ≥3 months were included in this study. The medical records of enrolled patients were reviewed for sex, age, fracture site, treatment method (open or closed reconstruction), postoperative intermaxillary fixation period, operation time, and complications. Radiological analysis of fracture fragment displacement and changes in ramal height difference was performed using computed tomography and panoramic radiography. Results: A total of 198 patients was investigated, 48.0% (n=95) of whom underwent closed reduction and 52.0% (n=103) underwent open reduction. There was no significant correlation between reduction method and patient sex, age, or follow-up period. No statistically significant difference between the incidence of complications and treatment method was observed. None of the patients underwent open reduction of condylar head fracture. Binary logistic regression analysis showed that open reduction was significantly more frequent in patients with subcondylar fracture compared to in those with a fracture in the condylar head area. There was no statistically significant correlation between the groups and fracture fragment displacement. However, there was a significant difference between the treatment groups in amount of change in ramal height difference between the fractured and the non-fractured sides during treatment. Conclusion: No significant clinical differences were found between the open and closed reduction methods in patients with mandibular condylar fractures. According to fracture site, closed reduction was preferred for condyle head fractures. There was no significant relationship between fracture fragment displacement and treatment method.

Kirschner wire를 사용한 과두하 골절의 구강내 접근법 (INTRAORAL OPEN REDUCTION OF MANDIBULAR SUBCONDYLAR FRACTURES USING KIRSCHNER WIRE)

  • The treatment of mandibular subcondylar fractures is a matter of controversy. The majority of mandibular subcondylar fracture are treated by closed reduction, but the displaced or dislocated mandibular subcondylar fractures may be treated by open reduction. The characteristics of open reduction are the anatomical reduction, the functional restoration, the rapid function, the maintenance of vertical ramus dimension, the better appearance and the less resultant TMJ problem etc. When an open reduction is considered, the wire, miniplate, lag screw and Kirschner wire are available with internal fixation. Of these, Kirschner wire is a simple method relatively and correct positioning of the wire achieves rigid fixation. But many open reduction methods for mandibular subcondylar fractures require extraoral approach. The extraoral approach has some problems, the facial scar and the risk of facial nerve injury. On the other hand, the intraoral approach eliminates the potency of the facial scar and the facial nerve injury, but is difficult to access the operation site. Since the intraoral approach was first described by Silverman (1925), the intraoral approach to the mandibular condyle has been developed with modifications. The purpose of this article is to describe the intraoral technique with the Kirschner wire on mandibular subcondylar fractures. Conclusion : The intraoral reduction with Kirschner wire on mandubular subcondylar fractures avoids the facial scar and facial nerve injury and is simple method to the extraoral approach. And it has minimal morbidity and better esthetics.
