Malignant Transformation of Fibrous Dysplasiainto Osteosarcoma - A Case Report - (골육종으로 악성 전환된 섬유성 골 이형성증 - 1예 보고 -)

  • Fibrous dysplasia is a developmental benign disorder in which normal bone is replaced by fibrous tissue. Malignant transformation of fibrous dysplasia is a rare complication and more commonly occur in the polyostotic form than monostotic form. We report a case in which osteosarcoma developed in area of fibrous dysplasia of proximal femur after treating with curettage, wedge osteotomy, internal fixation, and bone graft. A review of the literature is presented.

Diagnosis of Malignant Fibrous Histiocytoma(MFH) By Histologic Findings (악성 섬유성 조직구종의 조직학적 소견에 의한 진단)

  • Malignant fibrous histiocytoma was classified as fibrosarcoma, liposarcoma, rhabdomyosarcoma in times past and it was first introduced in 1963 to refer to a group of soft tissue tumors and reported in bone in 1972. It was postulated that the origin of tumor cell was derived from histiocytes that could assume the appearance and function of fibroblasts("facultative fibroblasts") at first. But, recently the immunohistochemical studies suggested origin from a primitive mesenchymal cell. The malignant fibrous histiocytoma were commonly misdiagnosed as other tumors such as myxoid liposarcoma, pleomorphic rhabdomyosarcoma, osteogenic sarcoma, leiomyosarcoma, malignant schwannoma, giant cell tumor, hemangiopericytoma, dermatofibrosarcoma protuberans due to the broad morphologic spectrum. The seventeen cases of the malignant fibrous histiocytomas diagnosed at Hanyang University Hospital since 1979 were reviwed, and the suggestions are as follows : 1. No fundamental histologic differences between the MFHs of soft tissues and bones. 2. The other tumors must be excluded in order to diagnose MFH due to wide histologic variety of MFH. 3. The review of the initial tissue slide was necessary for adequate diagnosis because the recurrent MFHs showed different histologic findings from original tumors.

Association of Cytotoxic T-lymphocyte Antigen-4 Polymorphisms with Malignant Bone Tumors Risk: A Meta-analysis

  • Background: Previous studies have assessed the association between the Cytotoxic T-lymphocyte Antigen-4(CTLA-4) polymorphism with the risk of malignant bone tumor, but the conclusions were inconsistent. We aimed to clarify association of cytotoxic T-lymphocyte antigen-4 polymorphisms with malignant bone tumors risk by performing a meta-analysis. Materials and Methods: The databases including PubMed, EMBase databases and the Cochrane Library were searched to identify the eligible studies prior to January 30 2016. Odds ratio (OR) with 95% confidence interval (95%CI) were used to estimate the strengths of the association between the CTLA-4 polymorphism and the malignant bone tumor risks. The meta-analysis was performed by STATA 12.0. Results: Four individual studies with a total of 1003 cases with malignant bone tumor and 1162 controls were included in our meta-analysis. The results of meta-analysis on those data demonstrated that CTLA-4 +49G>A polymorphism was associated with the risk of Ewing's sarcoma and osteosarcoma strongly (A vs. G: OR=1.36, 95%CI:1.20-1.54, p=0.000; AA+AG vs. GG: OR=1.35, 95%CI:1.14-1.61, p=0.001; AA vs. GG: OR=2.24, 95%CI:1.67-2.99, p=0.000; AA vs. AG+GG: OR=2.00, 95%CI:1.53-2.62, p=0.000), but CTLA-4 -318C/T polymorphism was not associated with the risk of malignant bone tumor (C vs. T: OR=0.76, 95%CI:0.76-1.08, p= 0.262; CC+CT vs. TT: OR=0.70, 95%CI:0.41-1.20, p= 0.198; CC vs. TT: OR=0.69, 95%CI:0.40-1.19, p= 0.183; CC vs. CT+TT: OR=0.92, 95%CI:0.75-1.13, p= 0.419). Subgroup analysis showed that there are significantly positive correlations between CTLA-4 +49G>A polymorphism and increased risks of malignant bone tumors in large size of sample (A vs. G: OR=1.347, 95%CI: 1.172,1.548, p=0.000; AA vs. GG: OR=2.228, 95%CI: 1.608,3.085, p=0.000), Ewing's Sarcoma or Osteosarcoma (A vs. G: OR=1.361, 95%CI: 1.201,1.540, p=0.000; AA vs. GG: OR=2.236, 95%CI: 1.674,2.986, p=0.000), and PCR-RFLP or Sequencing(A vs. G: OR=1.361, 95%CI: 1.201,1.540, p=0.000; AA vs. GG: OR=2.236, 95%CI: 1.674,2.986, p=0.000), but CTLA-4 -318C/T polymorphism was not associated with the risk of malignant bone tumors in diagnosis, genotype method, and sample size (all p>0.05). Conclusions: CTLA-4 +49A/G variant was associated with an increased risk of developing the malignant bone tumors, such as Ewing's sarcoma and osteosarcoma. However, it failed to show the association between CTLA-4 -318C/T polymorphism and the risk of malignant bone tumors. Future large-scale studies remain to be done to confirm our conclusions.

Vestibular Schwannoma Atypically Invading Temporal Bone

  • Vestibular schwannoma (VS) usually present the widening of internal auditory canal (IAC), and these bony changes are typically limited to IAC, not extend to temporal bone. Temporal bone invasion by VS is extremely rare. We report 51-year-old man who revealed temporal bone destruction beyond IAC by unilateral VS. The bony destruction extended anteriorly to the carotid canal and inferiorly to the jugular foramen. On histopathologic examination, the tumor showed typical benign schwannoma and did not show any unusual vascularity or malignant feature. Facial nerve was severely compressed and distorted by tumor, which unevenly eroded temporal bone in surgical field. Vestibular schwannoma with atypical invasion of temporal bone can be successfully treated with combined translabyrinthine and lateral suboccipiral approach without facial nerve dysfunction. Early detection and careful dissection of facial nerve with intraoperative monitoring should be considered during operation due to severe adhesion and distortion of facial nerve by tumor and eroded temporal bone.

Longitudinal Alterations of Microarchitecture and Mineralization Distribution on Trabecular Bone Due to Metastatic Bone Tumor (전이성 골암에 의한 해면골의 미세구조와 골화 분포 변화)

  • Purpose: The aim of present study is to detect longitudinal alterations of mechanical characteristic determined by bone quality (microarchitecture and degree of mineralization) on femur trabecular bone due to metastatic bone tumor Materials and Methods: Each 6 female SD rats (12 weeks old, approximate 250g) were allocated in SHAM and TUMOR Group. W256 (Walker carcinosarcoma 256 malignant breast cancer cell) was injected into the right femur (intraosseous injection) in TUMOR Group, whereas 0.9% NaCl (saline solution) was injected in SHAM Group. The right hind limbs of all rats were scanned by in-vivo micro-CT to acquire structural parameters, bone mineral density, X-ray attenuation and bone mineralization distribution at 0 week and 4 weeks after surgery. Results: BMD, BV/TV and Tb.N of trabecular bone in TUMOR group were markedly decreased (26%, 11% and 23%) while those in SHAM group were significantly increased (34%, 48% and 11%) (p<0.05). BS/BV, Tb.Sp and SMI in TUMOR group were significantly increased (-16%, 38% and 2%) compared with those in SHAM group (-33%, 12% and -16%) (p<0.05). Additionally, bone mineralization in TUMOR group significantly decreased while those in SHAM group was significantly increased (p<0.05). Conclusion: It is identified that how much bone microarchitecture and mineralization are diminished due to the metastatic bone tumor. The results may be helpful to prediction of fracture risk by metastatic bone tumor.

Analysis of Fracture Risk due to Alterations of Bone Quality by Metastatic Bone Tumor (전이성 골암으로 인한 골질 변화와 이로 인한 골절 위험성 분석)

  • While much has been learned about the mechanisms of metastatic spread of cancer to bone, there has been little headway in establishing guidelines for monitoring the alteration in bone quality and estimating fracture risk. The aims of this study are, therefore, 1) to evaluate bone quality induced by metastatic bone tumor by analyzing the characteristics on bone microarchitecture and degree of bone mineralization and 2) analyze fracture risk increased secondary to the bone quality changes by metastatic bone tumor through calculating mechanical rigidities based on in-vivo micro CT images. For this study, eighteen female SD rats (12 weeks old, approximate 250 g) were randomly allocated in Sham and Tumor groups. W256 (Walker carcinosarcoma 256 malignant breast cancer cell) was inoculated in the right femur (intraosseous injection) in Tumor group, while 0.9% NaCl (saline solution) was injected in Sham group. The right hind limbs of all rats were scanned by in-vivo micro-CT to acquire structural parameters and degree of bone mineralization at 0 week, 4 weeks, 8 weeks, and 12 weeks after surgery. At the same time, urine was collected by metabolic cages for a biochemical marker test in order to evaluate bone resorption. Then, bone metastasis had been directly identified by positron emission tomography. Finally, axial, bending and torsional rigidities had been calculated based on in-vivo micro CT images for predict fracture risk. The results of this study showed that metastatic bone tumor might induce significant decrease in bone quality and increase of fracture risk. This study may be helpful to monitoring a degree of bone metastasis and predicting fracture risk due to metastatic bone tumor. In addition, this noninvasive diagnostic methodology may be utilized for evaluating other bone metabolic diseases such as osteoporosis.

Metastatic Clear Cell Sarcoma of the Kidney in a Child's Hand Bone - A Case Report - (소아의 수부골에 전이된 신장의 투명세포육종 -1례 보고-)

  • Clear cell sarcoma of the kidney (CCSK) is a rare malignant tumor of the renal origin in childhood, distinguished from Wilms tumor by its pathologic and clinical features. Bone metastasis is one of the characteristic clinical features. The common site of metastasis of the clear cell Sarcoma of the kidney is axial skeleton including skull, spines, ribs and femur. A cases of clear cell Sarcoma of the kidney presented to us, which solely metastasized to the hand bones without metastasizing to any other tissues including axial skeleton. We report this case with review of literatures.

Malignant Proliferating Trichilemmal Tumor - A Case Report - (악성 증식성 모낭 종양 - 증례 보고 -)

  • Proliferating trichilemmal tumor(PTT) is a rare neoplasm derived from the outer root sheath of the hair follicle. Malignant transformation is rare, but abrupt enlargement of size, infiltrative growth, and non-scalp location are suggestive of malignant PTT. Histopathologic characteristics of malignant PTT are nuclear atypia, pleomorphism, and mitotic figures. Recurrence and metastasis have been documented in malignant PTT. Wide resection of the tumor with the normal tissue and accurate follow-up is the treatment of choice of the both malignant and benign PTT. We report a 51-year-old man with the mass on right 5th finger distal phalanx for 7 years. The tumor was excised with middle phalangiectomy, and based on the histopathologic findings of the tumor, this case was diagnosed as mailignant PTT. We cannot detect recurrence and metastasis because the patient was lost to follow-up.

Malignant Extrarenal Rhabdoid Tumor in Soft Tissue - A Case Report - (연부 조직에 발생한 악성 횡문근양 종양 - 증례 보고 -)

  • Malignant rhabdoid tumor is a highly aggressive tumor of children, that often arises in the kidney. Some rhabdoid tumors have been reported in various extra-renal location including the central nervous system, liver, skin, and soft tissues. In case of arising in soft tissues, it may be misdiagnosed as rhabdomyosarcoma. It is important to distinguish malignant rabdoid tumor from rhadomyosarcoma, because malignant rhabdoid tumor has more aggressive behavior and poorer survival rate. And this differential diagnosis can be performed by several immunohistochemistry. Here we report a case of malignant rhabdoid tumor that arose in lower abdominal wall with related articles.
