The Relations of Social Support to the Health Behaviors and Health Status in the Elderly (노인들의 사회적 지지와 건강행태 및 건강수준과의 관련성)

  • Objectives: This study intends to understand the difference within group of social support level and the effect of social support to health behaviors and health status of the elderly by selecting the old of local society as target. Methods: Data were obtained from self-administered questionnaire of 8,688 persons, older than 65 years, living in a community. We measured the sociodemographic characteristics, social support(family support, other support, quality of support), physical health state(subjective health status, number of chronic disease), physical function state(activities of daily living; ADL, instrumental activities of daily living; IADL), cognition state(mini-mental state examination-Korean; MMSE-K) and depression state(short form of geriatric depression scale; SGDS), health behaviors(smoking, drinking, exercise, eating habit). Univariate, multinominal logistic regression and covariance structure analysis were employed to analyze factors affecting on the social support of the elderly. Results: When considering the degree of social support by the sociodemographic characteristics of the older adults, the family support, other support and quality of support is better when the old is male, young, high education and self-reported living status is good and it has significance statistically. When considering the relation between social support and health status, the family support, other support and quality of support is better when the old's subjective and objective physical health status is good. The family support, other support and quality of support is better when the old's subjective health status is better. The other support and quality of support is better when the old's ADL(activities of daily living) and IADL(instrumental activities of daily living) are good. The family support, other support and quality of support is better when the old's cognitive function and depression state is better. When considering the relation between social support and health behaviors, in case of smoking and drinking, the quality of support, family support and other support is better when the old smokes and drinks rather than the old does not. In case of exercise and eating habit, the family support, other support and quality of support is better when the old exercises and eats regularly rather than the old does not. It has significance statistically. From the result of performing covariance structure analysis by structural equation modeling(SEM) with two endogenous variable(health behaviors and health status) and one exogenous variable(social support), factor loading of health status is 0.74 and factor loading of health behaviors is 0.05. The social support explains health status of 55.4% and health behaviors of 2.9%. Conclusions: This study has the meaning that it finds the difference of social support generating from inside of the group for the old residing in city and country and specifies the effect that the difference of social support influences to health status and health behaviors. From now on, in the development of health improvement strategy of the olds, it is necessary to approach from inclusive aspect while considering psychosocial factor such as social support and social economical factor as well as health status.

Normal Glenohumeral and Scapulothoracic Movement at the Coronal Plane (정상인의 관상면에서의 관절와상완운동 및 견갑흉곽운동)

  • We measured the glenohumeral and scapulothoracic movements during abduction of the arm in the coronal plane with radiologic analysis in the 30 shoulders of normal male adults who were without pain, limitation of motion, and history of trauma. In the resting position, the glenoid cavity of the scapula faced somewhat superiorly in over 80 percents of the individuals, the mean superior tilting was 5.7 degrees. The mean total scapulothoracic movement was 65.8 degrees and the mean total glenohumeral movement was 106.8 degrees during abduction of arm in the coronal plane. The mean ratio of the glenohumeral movement to the scapulothoracic movement was 1.6 and this GH/ST ratio was decreased toward the extreme abduction. When the arm was abducted, external rotation of the humeral head occurred and this external rotation was increased smoothly during 0 degree through 90 degrees, but steeply above 90 degrees. The acromiohumeral interval was 10.9 mm at the resting positon, and this interval decreased during the arm abduction. The superior migration of the humeral head was 3.1 mm while abducting the arm. Our measurement of the relationships of glenohumeral and scapulothoracic movements at the coronal plane would be useful in the understandings of the biomechanics of shoulder, but further study would be required for the analysis of the three dimensional relationship because of the limitation of our two dimensional analysis.

Relationship between Formants and Constriction Areas of Vocal Tract in 9 Korean Standard Vowels (우리말 모음의 발음시 음형대와 조음위치의 관계에 대한 연구)

  • The formants of the 9 Korean standard vowels(which used by the average people of Seoul, central-area of the Korean peninsula) were measured by analysis with the linear predictive coding(LPC) and fast Fourier transform(FFT). The author already had reported the constriction area for the Korean standard vowels, and with the existing data, the distance from glottis to the constriction area in the vocal tract of each vowel was newly measured with videovelopharyngograms and lateral Rontgenograms of the vocal tract. We correlated the formant frequencies with the distance from glottis to the constriction area of the vocal tract. Also we tried to correlate the formant frequencies with the position of tongue in the vocal tract which is divided into 2 categories : The position of tongue in oral cavity by the distance from imaginary palatal line to the highest point of tongue and the position in pharyngeal cavity by the distance from back of tongue to posterior pharyngeal wall. This study was performed with 10 adults(male : 5, female : 5) who spoke primary 9 Korean standard vowels. We had already reported that the Korean vowel [i], [e], $[{\varepsilon}]$ were articulated at hard palate level, [$\dot{+}$], [u] were at soft palate level, [$\wedge$] was at upper pharynx level and the [$\wedge$], [$\partial$], [a] in a previous article. Also we had noted that the significance of pharyngeal cavity in vowel articulation. From this study we have concluded that ; 1) The F$_1$ is related with the oral cavity articulated vowel [i, e, $\varepsilon$, $\dot{+}$, u]. 2) Within the oral cavity articulated vowel [i, e, $\varepsilon$, $\dot{+}$, u] and the upper pharynx articulated vowel [o], the F$_2$ is elevated when the diatance from glottis to the constriction area is longer. But within the lower pharynx articulated vowel [$\partial$, $\wedge$, a], the F$_2$ is elevated when the distance from glottis to the constriction area is shorter. 3) With the stronger tendency of back-vowel, the higher the elevation of the F$_1$ and F$_2$ frequencies. 4) The F$_3$ and F$_4$ showed no correaltion with the constriction area nor the position of tongue in the vocal tract 5) The parameter F$_2$- F$_1$, which is the difference between F$_2$ frequency and F$_1$ frequency showed an excellent indicator of differenciating the oral cavity articulated vowels from pharyngeal cavity articulated vowels. If the F$_2$-F$_1$ is less than about 600Hz which indicates the vowel is articulated in the pharyngeal cavity, and more than about 600Hz, which indicates that the vowel is articulated in the oral cavity.

Phonatory Caracteristics of Vwels and Resonant Consonants using the Electroglottography (전기성문파형검사를 이용한 모음과 공명 자음의 발성특성)

  • Background and Objectives : Vowels and resonant including nasals and liquid are produced with vocal folds vibration have been used for voice therapy of hyperadduction patients. This study was conducted to investigate phonatory characteristics of vowels and resonant consonants through the EGG measures from Lx. Speech studio (Laryngograph Ltd, UK). Materials and Method : 7 male adults produced sustained vowel /a/, /i/, /u/, nasals /m/, /n/, /${\eta}$/and liquid /I/ and read the sentences (1nasals-liquid sentence, 1 non-nasals-liquid sentence) and tongue-tip trill and humming. Fx(Hz), Ox(%) were obtained of vowels, nasals, liquid and each of the posterior vowel /a/ of /ma/, /na/, /la/, /ha/ with same F0(around F#165Hz) and amplitude (75${\pm}$5db). And also DFx(Hz), DQx(%), CFx(%) and CAx(%) were obtained from reading two kinds of sentences. Results : Qx(%) was the highest in /u/ of vowels, and nasal/n/ of the resonant consonants and nasals-liquid sentence was higher Qx than non-nasals-liquid sentence but significant differences were not found. Qx(%) of the posterior vowel /a/ of nasal consonants/n/ was higher than in the isolated vowel/a/ and other posterior vowel of resonant consonants and fricatives /h/. Regularity or periodicity and higher Qx were observed in the nasals-liquid sentence than non-nasals-liquid sentence in graphs of QxFx & CFx produced by Quantiative analysis. In the nasalance score, /u/vowel was significant higher among the vowels and /I/ liquid was significant lower among the resonant consonants and nasals-liquid sentence is higher than non-nasals -liquid sentence. CQ(%) was not significantly correlated with nasalance(%). Conclusion : These findings might signify resonant phonation was not correlated with nasalance.

Association of Anthropometric Indices with Prevalence of Hypertension in Korean Adult (일부 지역 주민에서 고혈압이환과 비만지표와의 관련성)

  • Objective: To examine the relationship between hypertension prevalence and the four commonest anthropometric measurements for obesity(body mass index(BMI), wasit-hip ratio(WHR), waist circumference(WC) and body fat in Korean adults. Methods: We studied the cross-sectional association of the anthropometric indices and blood pressure in 1,197 individuals(who were participants in the population-based cohort study). Hypertension was defined as blood pressure $\geq160/95\;mmHg$ or current use of antihypertensive medication. Informations on life-style factors were obtained from personal interview. Results: There were close associations between BMI, WHR and WC with blood pressure in both men and women. After age adjustment, BMI and WC showed significantly positive correlation with systolic and diastolic blood pressure levels in both men and women. Odds ratio(ORs) of being hypertensive were estimated comparing the highest to the lowest quantile, adjusting for age, smoking status, alcohol intake levels, education attainment. The simultaneously adjusted ORs of being hypertensive, comparing the highest vs the lowest categories, was for BMI 2.0(95% confidence interval(CI)=0.9-3.2) in men and 3.2 (95% CI=1.7-6.1) in women, for WC 2.1(95% CI=1.0-4.4) in men and 3.1 (95% CI=1.6-5.9) in women, for fat(%) 4.2(95% CI=1.9-9.5) in men and 2.1(95% CI=1.2-3.6) in women. Conclusion: In addition to measures of overall obesity(BMI) as well as central obesity(WHR, WC), body fat(%) was independently associated with prevalence of hypertension. Among obesity indices, body fat was the most predictor variable in hypertensive state in male and BMI was in female.

The Abnormal Rates of Blood Pressures and Blood Biochemical Properties with BMI in Health Checkup Examinees (건강검진 수검자들의 BMI에 따른 혈압, 혈액 및 혈액생화학적지표의 비정상치 분포)

  • The purpose of this study was to investigate the association of blood pressures, hematological and biochemical properties of blood according to the degree of obesity in health checkup examinees. Study subjects were 3,731 adults of 20 years and over (2,312 males, 1,419 females), who underwent health package check-up at the Korea Health Management Association from Jan. 2007 to Dec. 2009. As a result, the rates according to the degree of obesity of study subjects were 4.6% in low weight group, 44.3% in normal weight group, 25.0% in over weight group and 26.1% in obesity group. The over weight group were significantly higher in 40's age group than other groups, and the obesity group were significantly higher in 50's age group than other groups. The abnormal rates of SBP, DBP, TG, TC, HDL-C, LDL-C, ALT, AST in male were significantly higher than that of female. With respect to degree of BMI, the abnormal rates of SBP, DBP, TG, TC, HDL-C, LDL-C, ALT, AST, GGT, ALP were significantly increased with higher level of BMI. The age-adjusted odds ratio in the abnormal level of SBP, DBP, TG, TC, HDL-C, LDL-C, AST, ALT and GGT were significantly increased in overweight group and obese group than that of normal weight group.

Influence of Temporo-mandibular Joint Training Using Physical Therapy on the Vowel Acoustic Characteristics (TM Joint의 물리치료를 통한 훈련이 모음의 음향학적 특성에 미치는 영향)

  • This study was to examine the change of vowel acoustic characteristics of the temporomandibular joint disorder patients by maintaining normal vocalization pattern of the temporomandibular joint through increasing the range of motion, that was, the oral cavity sonorant cavity of the temporomandibular joint, related to vowel articulation through temporomandibular training using the physical therapy. The subjects of this study were 3 male adults in 20-30s that were diagnosed with temporomandibular joint disorder. As a result of conducting temporomandibular training program using the physical therapy, the $1^{st}$ Formant Frequency(F1), $2^{nd}$ Formant Frequency(F2), and Fundamental Frequency(F0) of the temporomandibular joint disorder patients were increased compared to before and this showed the change of the $1^{st}$ Formant Frequency(F1) related to the open mouth grade of a vowel, as well as the $2^{nd}$ Formant Frequency(F2), and Fundamental Frequency(F0) related to the front-back of a vowel which shows the relationship between the temporomandibular joint, vowels and voice calculation.

Survey on Consumer Perceptions of the Sensory Quality Attributes of Apple (사과의 품질결정을 위한 소비자 인식 조사)

  • Improving quality is a very important component of maintaining competitiveness of agricultural products. However, evaluation of 'high quality' indicates it is a very abstract concept and independent of some quality attributes, leading to differences in the perception of quality. Thus, there is a pressing need to objectively define 'high quality' and to develop basic technologies for its measurement, for application in the production, storage and distribution of competitive agricultural products. To objectively quantify apple quality, a survey was conducted on consumer preferences and awareness of quality attributes including color, taste, flavor and shape. The survey questionnaire targeted male and female adults (463 persons) ranging in age from 20 to 59 years. The questionnaire was based on purchases made at a wholesale market (50.1%) or a traditional market (18.8%). The majority of purchases were as small packets (62.0%) or as individual pieces (20.5%). Apples of moderate size (fist size, 60.5%) were preferred over small (4.3%) or large (32.6%) apples. The questionnaire provided consumer data on external quality attributes including color, shape and variety. Taste attributes were evaluated in relation to the balance between sour and sweet taste, and flavors peculiar to apples.

Association of among alcohol consumption and blood vitamin D and lead concentrations: Based on 2010-2012 Korea national health and nutrition examination survey (알코올 섭취와 혈중 비타민 D 그리고 납 농도 사이의 관련성: 2010-2012년 국민건강영양조사 자료를 이용하여)

  • This paper investigates the association of among alcohol consumption and blood vitamin D and lead concentrations. Subjects to 4,074 with adults over the age of 20 in the 5th Korea National Health and Nutrition Examination Survey, correlation analysis is used to validate the relations between an average daily alcohol consumption and blood vitamin D and lead concentration, and to study the effects of blood vitamin D to the lead concentration is carried out regression analysis. The correlation analysis shows that alcohol consumption has the correlation with blood vitamin D and lead concentration(p<0.01). As the results of regression analysis, age(p<0.01), education(p<0.01), occupation(p<0.05), alcohol consumption(p<0.01), vitamin D(p<0.05) appear significant influence on lead concentration in male. Alcohol consumption increases not only lead concentration but also vitamin D. Blood vitamin D also contributes to the rise in lead concentration, but in the reverse the metabolic activity of blood vitamin D by influence of lead receives a negative effect in our body.

Thyroid Gland Evaluation of Adults in Their 20s by means of a Screening Ultrasound Test and a Self-Test (스크리닝 초음파검사와 자가진단에 의한 20대 성인의 갑상선 평가)

  • Korea Central Cancer Registry reported in 2011 that the prevalence rate of thyroid cancer was about 19.6% (the highest) in both men and women. The ratio of thyroid cancer, among cancers that women suffer from, was about 31.1%, the highest among female cancers and about five times more than men. Regarding the incidence of thyroid cancer by age group, the crude rate was highest in women and men aged 15~34. From these figures it can be inferred that there is a greater relationship between thyroid cancer and hormones compared with other cancers. Therefore, young women aged 20~25, even if they are in full health, should undergo a medical examination. This study is aimed at examining changes in thyroid gland detected through self evaluation tests and the analysis of ultrasound screening images in persons aged 20~25. According to the study, 213 persons (about 46.6%) out of 457 people who took part in the study had abnormal echo patterns. With regard to women's abnormal patterns, about 73.4% of them were found to have calcified cystic, 11.8% diffuse-type low-echo and 7.3% cystic echo. With regard to male participants, about 61.1% were found to have calcified cystic, followed by 19.4% with the size of the isthmus increased and 13.9% with diffuse-type low-echo. According to the outcomes of an analysis regarding self-testing for hyperthyroidism, the average points of participants who were found to have abnormal echo patterns in ultrasound tests were 6.85 and the figure was 5.88 in persons with normal patterns. The figure was about 15% higher in abnormal respondents.
