The pathogenesis of inflammatory bowel diseases is not very well understood; it is currently thought to be caused by the interaction between genetic factors, environmental factors, intestinal microbes, and immune factors. Biological agents such as anti-tumor necrosis factor (anti-TNF) are widely being used as therapeutic agents. Infliximab, a chimeric monoclonal IgG1 antibody against tumor necrosis factor, has been demonstrated to have an effect in the induction and maintenance of remission in Crohn's disease in children. The effects of biological agents, typified by anti-TNFs, in inflammatory bowel disease in children; the recent concern on the administration of biological agents in combination with immunomodulators; and 'Top-down' therapy are some of the topics covered in this review.
The effects of halothane and pentobarbital sodium anaesthesia in implantation and maintenance of pregnancy were compared in the rabbit ovariectomized the day after insemination and given steroid replacement therapy. The result has shown that halothane anaesthesia did not favour the overall process of implantation or foetal development, compared with pentobarbital sodium.
Objective : The purpose of this study was to look at a systematic review on the effects of occupational reminiscence therapy in Dementia offered National Long term care Insurance. Through this comprehensive study, we have to compare the studies. Methods : We systematically examined papers published in journal from 2009 to 2013, using KERIS. Main words to examine are Dementia, Long term care service, Day care center, reminiscence therapy, occupational centered, etc. Results : 6 studies were selected, All of them were occupational reminiscence therapy. Reminiscence therapy can be devided into communication centered reminiscence therapy and occupational reminiscence therapy. The results demonstrated that the intervention significantly affected the maintenance cognitive skills and reminiscence skills, decreased depression, behavioral changes, improvement of communication and interaction skills and quality of life etc. of elderly people with dementia (p<.05). Conclusions : If occupational therapists can obtain knowledge and make a program for occupational reminiscence therapy, research in this field will be further developed. In the future study, the use of occupational reminiscence therapy applied to a variety of interventions and majority of patients is needed on occupational therapy.
The passive scattering system nozzle of the proton therapy accelerator was simulated to evaluate the neutrons generated by each component in each nozzle by energy. The Monte Carlo N-Particle code was used to implement spread out Bragg peak with proton energy 220 MeV, reach 20 cm, and 6 cm length used in the treatment environment. Among the proton accelerator components, neutrons were the highest in scatterers, and the neutron flux decreased as it moved away from the central flux of the proton. This study can be used as a basic data for the evaluation of the radiation necessary for the maintenance and dismantling of proton accelerators.
Gradual attrition of telomere to a critical short length elicits successive cellular response of cellular senescence and crisis. Cancer cells evade this process by maintaining functional telomeres via one of two known mechanisms of telomere maintenance. The first and most frequent mechanism involves reactivation of enzyme activity of telomerase, a ribonucleoprotein complex mainly via transcriptional up-regulation of TERT, a catalytic subunit of telomerase complex. The second mechanism utilizes telomerase-independent way termed ALT (for Alternative Lengthening of Telomere), which possibly involves recombination pathways. Thus master key for cellular immortalization is supposed to possess adequate telomere reserves. Indeed, telomerase can alone induce the immortalization under culture on feeder cell layers without generally known inactivation mechanism of tumor suppressor genes. Including this phenomena, this review will focus on telomerase and telomere-associated proteins, thereby implication of these proteins for cellular immortalization processes.
Amidarone is one of the most commonly prescribed anti-arrythmic agents for almost all arrythmias, whether atrial or ventricular in origin. There are several side effects associated with amiodarone therapy. These include corneal deposits, abnormal liver function tests, hyper and hypothyroidism, bluish discolorations of the skin, bone marrow suppression, coagulopathies, peripheral neuropathies, and pulmonary toxicity. Amiodarone-induced pulmonary toxicity(APT), which was first described in 1980, is potentially serious side effects that are believed to develop in 5% of patients. Doctors often assume that APT occurs only when high amiodarone doses are used for a long time, but in practice a low maintenance dose of amiodarone may also be toxic. In this report, a case of amiodarone-induced pulmonary toxicity after a long course of a low dose therapy for refractory supraventricular arrythmia is described.
A 7-year-old, spayed female, Yorkshire terrier dog with polyuria/polydipsia (PU/PD), interdigital edema, pruritus and abdominal enlargement was referred to the Veterinary Medical Teaching Hospital of the present university. Pituitary-dependent hyperadrenocortisim (PDH) was diagnosed by clinical signs, physical examination, laboratory examination (complete blood count, serum chemistry, urinalysis, ACTH stimulation test and high dose dexamethasone suppression test) and diagnostic imaging (radiography, ultrasonography and computed tomography). Clinical signs were improved after trilostane treatment, and maintenance therapy with trilostane still continued after successful induction therapy. Trilostane can be used as an alternative to mitotane therapy in hyperadrenocortisim in dogs.

This report details a case of 8-year-old girl showing failure of odontogenesis after chemo-radiation therapy for the rhabdomyosarcoma at the age of 4. The observed results were as follows : 1. Past history revealed that she had received for a total radiation dose of 4430cGy, 29 fractions in 6 weeks and chemotherapy with vincristine, actinomycin D and cytoxan, followed as maintenance phase for 2 years. 2. The patient was symptom -free and appointed for the treatment of multiple dental caries. 3. Oral examination showed hypoplastic enamel on whole erupted permanent teeth and showed retarded eruption. 4. Conventional radiograms showed failure of root development including abrupt cessation of root formation and root agenesis, and microdontia, missing teeth, irregular enamel, dislocation of the impacted teeth. Additional finding showed good healing bone pattern on the left mandibular ramus and angle area. 5. Cephalometric analysis revealed failure of bite raising due to incomplete eruption of all the first molars and made it possible to suspect entrapped mandibular growth and then Class II tendency growth. 6. There was correlation between the time of chemo-radiation therapy and the damage of the teeth.
Pulmonary surfactant is a lipoprotein complex composed primarily of phospholipid and lung specific apoproteins that reduces surface tension in the alveolus and maintains alveolar stability at low lung volume. Adult respiratory distress syndrome still carries a very high morbidity and mortality. The surfactant system is vital to the maintenance of proper lung function, any type of surfactant deficiency, whether primary or secondary, will contribute significantly to the development of pulmonary pathophysiology. Various mechanisms in adult respiratory distress syndrome may be responsible for such alterations in the surfactant system. Surfactant replacement is now an established treatment for neonatal respiratory distress syndrome, reducing both incidence of complications and mortality. With the current knowledge of surfactant physiology and the pathophysiology of the adult respiratory distress syndrome exogenous surfactant treatment or stimulation of endogenous surfactant synthesis and secretion will prove to be beneficial in preventing and treating the adult respiratory distress syndrome. The study of clinical surfactant therapy for adult respiratory distress syndrome is just beginnig and this can be viewed as an area with exciting potential. As soon as surfactant preparations become more widely available trials should begin to define the role of surfactant treatment in the adult respiratory distress syndrome as an adjunct to available treatment techniques.
Psychsocial factors appear to play an important role in the maintenance and development of chronic disability from low back pain. Fear of pain may be more disabling than the pain itself in patients with nonacute low back pain. The purpose of this study was to identify the contribution of gender, age, depression and pain-related fear to pain intensity and disability in nonacute low back pain patients. This was a cross-sectional survey study of eighty four patients who had low back pain for at least 4 weeks. More than moderate correlations were found between pain intensity, disability, fear-avoidance beliefs and depression. Regression analyses revealed that disability ratings and fear-avoidance beliefs for work activities significantly contributed to the prediction of pain intensity, even when controlling for age, gender and pain duration. Also, fear-avoidance beliefs for physical activity, pain intensity, age and depression, significantly contributed to the prediction of disability, even when controlling for gender and pain duration. These findings suggest that disability scores and fear-avoidance beliefs for work activities are important determinants of pain intensity. They also suggest that fear-avoidance beliefs for physical activity, pain intensity, age and depression are important determinants of disability.
