Behcet`s Syndrome with Aortic Aneurysm: A Case Report (Bechet`s 병과 합병된 상부대동맥류: 치험 1례 보고)

  • A 36 year old blindman, engineer was admitted with chief complaints of hemoptysis, recurrent sore throat, pyoderma in genital organ, uveitis and thrombophlebitis for 10 years. Above the chief complaints were remission or exacerbation during hospitalization. Physicalexamination showed that left radial, ulnar & brachial pulse was not palpable. No bruit or murmur was obtained over the mass. Neurologic examination revealed no significant finding.On admission, chest P-A showed hen egg sized round & oval compact hazy density on left upper lung field. Bronchogram revealed no pathological finding and Lt. tomogram showed well define large,ovoid mass density in the superior mediastinum. Fluoroscopy finding showed nonpulsatile on left upper lung field. Pre-op. aortography was not taken, under the impression of lung Ca. rule out .sortie aneurysm, exploratory operation was performed through the 2nd intercostal space, Lt. It was performed that the mass was ascending sortie aneurysm of saccular type. Direct aneurysmectomy with multiple figure of eight suture were done without any prosthetic graft. Post-op. control I.V.C graphy showed completely obstruction sign. Postopcontrol aortography revealed good surgical result. Final, histopathological answered non-specific sortie aneurysm, saccular type. Post-op. courses were uneventful except mild neurologic disturbance with subclavian steal syndrome and associated with both lower leg pitting edema due to inferior vena cava obstruction. After op, 3 month later, discharged to home, with big systemic problem. Behcet`s syndrome reviewed with related literatures. The coexistence of mouth and genital ulceration with hypopyon mentioned by hippocrates and described by various workers in the early part of this century was first defined as a syndrome by Behcet in 1937. In 1937 Behcet described a chronic relapsing triple symptom complex of oral ulceration, genital ulceration, and ocular inflammation. The place of the syndrome as part of a systemic disorder in now clearer, and the under lying pathology appears to be a vasculitis. The disease runs a- chronic course, blindness being the greatest disability and control nervous system involvement a cause of death. Thrombophlebitis is fairly frequent, france et al [1951] giving an incidence of 25% and Dowling [1961] 12%, superficial thrombophlebitis migrans and thrombosis of large veins, including venae cavae [Thomas, 1947: Boolukos 1960] are recorded. Little attention has been paid to arterial involvement. Mishima et al. [1961] described resection cf an aortic aneurysm in a 38 year old man with Behcet`s syndorme. Mounsey in a clinicopathological conference described a case [Brit, med. J., 1966] of ruptured aortic aneurysm in Bechcet`s syndrome treated by aorto-iliac graft. Also, Shikano and Oshima et al [1963] recorded two aneyrysm of smaller arteries. Unfrequently, aortic aneurysm was presumed to be secondary to osteomyelitis of the lumber spine, though the possible association between aortic aneurysm and Behcet`s syndrome was raised. A further case is reported here, in which ascending aortic aneurysm with Behcet`s Ds. appeared to form part of this generalized disease. This is a case report of surgical experience of Behcet`s Ds. with ascending aortic aneurysm which had nearly all the typical clinical features. Above mentioned and was reviewed with related literatures.

Reliability and Validity of the Behavioral Observation Method for Assessing Low Back Pain in Patients with Spinal Diseases (척추질환자의 요통사정을 위한 통증행위 관찰법의 신뢰도 및 타당도 검정)

  • The purpose of this study was to examine the reliability and validity of the observed behaviors of subjects who suffered from low back pain with spinal diseases, Thirty two low back pain patients admitted on the neurosurgical unit in an army hospital were compared with 30 normal controls belonged to an army unit, by means of matching the age, hight and weight. Observed pain behaviors were developed by the researcher on the bases of literature and patient observation. This tool consists of 18 behaviors seperated into two major groups : mutually exclusive and concomittent behaviors. The mutually exclusive behaviors included coding cathegories for 6. body motions assumed by the subjects during the observation session. These 6 standardized motions consisted of sitting from standing first, and serially tying down, reclining, sitting again, and then standing, 6 steps walking. Concomittent behaviors consisted of 12 observable patterns that can be observed systematically from the face, grimacing, bracing, rubbing, walking with arms fixed, support with hands on sitting or standing, guarded movement, limping, unbalaced weightbearing, stopped movement from tying position to sitting, sighing and graoning. Subjects were videotrecorded as they performed a 6-standardized sequence of motions, simultanously researcher measured the time spent performing each motion and step length. Patients were asked torate their subjective pain score on the 10 mm graphic rating scale ranging from 'no pain' to 'sever pain'. For scoring of the pain behaviors, two trained nursing officiers independently and simutanously viewwd each videorecording and checked subject 'pain behaviors at the observational item checklist. The result of the study are summarized as follows : 1. Reability of the observational tool was a=.845. 2. Spearman's rho and percentage agreement were p=.97 and 81.7 persent respectively, that indicate adequate interrater reability of this tool. 3. The sensitivity rate of the tool was .875 while specificity rate .866 for differentiating patient from the normal. 4. When difference in the objective pain indices between patient group and control were compared, there was significant difference of all indices, such as pain behavior(t=7.71, p=.0001). spent time performing motion(t=14.2, p=.0001), step length (t=-10.72, p=.0001). 5. There were differences in the objective indices the subjective pain subgroups (low, medium, high). Differences in the mean score of objective pain behavior (F=6.376. p=.005) and spent time for moyion(F=4.631, p=.018). But there were no significant differences in the step length among the subgroups(F=.667, p=.521). 6. Highly correlated pain behavior items wiyh subjective pain score were 'stopped movement from lying position to sitting', 'limping', 'support with hands on sitting or standing', 'bracing', 'guarding' and 'walking with arms fixed'. In summary, although some of rho behavior items such as sighing and groaning in this study could not be observed because of videotaped datd, the reliability and validity of the over all observation method were satifactory. Thus, the results of the present study demonstrate rye potetional utility of the tool in assessing objective pain complementing self-reported pain in low back pain patients.

Fruit Quality and Fruit Locule Air Hole of Kiwifruit (Actinidia deliciosa cv. Hayward) Affected by Early Defoliation (조기낙엽이 참다래 '헤이워드' 과실 바람들이와 품질에 미치는 영향)

  • BACKGROUND: The fruit quality and flowering characteristics of Kiwifruit (A. deliciosa cv. Hayward) in the following year is known to be affected by the extent and timing of defoliation of the current year. In korea, the production of kiwi, which is a perennial, straggling deciduous warm-temperate fruit, is often restricted by wind damage due to typhoons resulting to defoliation at the middle season of its growing period. In this paper, we report the effect of the different timing of defoliation and severities at the current season to the kiwifruit quality. METHODS AND RESULTS: Twenty seven-year-old 'Hayward' trees grown under polyethylene film rain-shelter were defoliated in different days from August to September at seven day-intervals. In each day, 0, 25, 50, 75 and 100% of leaves were removed from the trees. Fruits from each treatment were classified into four floating types (L: lying in bottom, S: standing on bottom, F: floating and SF: floating at the surface of water) by submerging them into tap water. Defoliation of kiwifruit trees in August and September caused air holes in locules of inner pericarp. Increased number of air hole in locules of a fruit was observed in floating types F and SF, and most of the air holes were located in stem end. The defoliation of trees in August significantly reduced the ratio of L-floating type fruits, which have the least number of locule air holes. The extent of defoliation also affected the distribution of the four types, the more leaves removed, the less L-floating type fruits harvested. The weight of fruits from trees defoliated in August was lower than that of fruits from September. Soluble solids content decreased as the number of locule air holes increased. Negative correlations were observed between the extent of defoliation and the weight and soluble solids content of fruits. CONCLUSION: Early defoliation effect on kiwifruit locule air hole occurrence and fruit quality were more severe in August than in September. And also if the defoliation severity is over 25%, severe fruit quality reduction expected to happen due to increase of fruit locule air hole in the inner pericarp.

Groundwater Flow Model for the Pollutant Transport in Subsurface Porous Media Theory and Modeling (지하다공질(地下多孔質) 매체(媒體)속에서의 오염물질이동(汚染物質移動) 해석(解析)을 위한 지하수(地下水)흐름 모형(模型))

  • This paper is on the modeling of two-dimensional groundwater flow, which is the first step of the development of Dynamic System Model for groundwater flow and pollutant transport in subsurface porous media. The particular features of the model are its versatility and flexibility to deal with as many real-world problems as possible. Points as well as distributed sources/sinks are included to represent recharges/pumping and rainfall infiltrations. All sources/sinks can be transient or steady state. Prescribed hydraulic head on the Dirichlet boundaries and fluxes on Neumann or Cauchy boundaries can be time-dependent or constant. Sources/sinks strength over each element and node, hydraulic head at each Dirichlet boundary node and flux at each boundary segment can vary independently of each other. Either completely confined or completely unconfined aquifers, or partially confined and partially unconfined aquifers can be dealt with effectively. Discretization of a compound region with very irregular curved boundaries is made easy by including both quadrilateral and triangular elements in the formulation. Large-field problems can be solved efficiently by including a pointwise iterative solution strategy as an optional alternative to the direct elimination solution methed for the matrix equation approximating the partial differential equation of groundwater flow. The model also includes transient flow through confining leaky aquifers lying above and/or below the aquifer of interest. The model is verified against three simple cases to which analytical solutions are available. The groundwater flow model shall be combined with the model of pollutant transport in subsurface porous media. Then the combined model, with the applications of the Eigenvalue technique and the Dynamic system theory, shall be improved to the Dynamic System Model which can simulate the real groundwater flow and the pollutant transport accurately and effectively for the analyses and predictions.

Immobilization Effect and Abdominal Pressure of Newly-Developed Lumbosacral Spinal Orthosis during Task Performance (새롭게 개발한 요천추 보조기의 과제 수행 중 척추의 고정효과와 복부압력)

  • Objective: We evaluated the efficacy of a newly-developed spinal orthoses (V-LSO) by comparing the stabilizing effect, abdominal pressure, and comfort of 3 different semirigid LSOs (classic LSO, V-LSO, and Cybertech®) during various body movements. Method: Thirty healthy volunteers (23~47 years, 24 males, 6 females) were selected. A dual inclinometer measured the range of motion (ROM) while the participants performed flexion/extension and lateral flexion of the lumbar spine with 3 LSOs. The LSO's pressure on the abdominal surface was measured using 9 pressure sensors while lying, sitting, standing, flexion/extension, lateral flexion, axial rotation, and lifting a box. Comfort and subjective immobilization were analyzed by a questionnaire. Results: V-LSO had a statistically significant effect on flexion over Cybertech®. No significant differences were noted during extension and lateral flexion between the 3 LSOs. The abdominal pressure showed no significant differences while supine. While sitting, standing, and lifting a box, the mean abdominal pressure for V-LSO were significantly higher than those for Cybertech®. During lumbar flexion, the mean abdominal pressures for classic LSO and V-LSO were significantly higher than that of Cybertech®. For extension, lateral flexion and axial rotation, the abdominal pressure for V-LSO was significantly higher than those of classic LSO and Cybertech®. In the subjective analysis, V-LSO and Cybertech® scored best for comfort. Conclusion: The V-LSO and Cybertech® were more comfortable than the classic LSO, and hence, may have improved compliance with decreased discomfort. V-LSO may be superior to the other LSOs in restricting lumbar movement and increasing intraabdominal pressure.

Cultivation of millet in Mahan-Baekje Villages: Archaeobotanical examination of Osan site, Jangheung, Korea (전남 마한백제계 취락의 서속(黍粟) 재배와 농경 체계: 장흥 봉림리 오산유적 자료를 중심으로)

  • Although foxtail and broomcorn millet have been cultivated since the Neolithic Age on the Korean Peninsula, there have been few studies on how the importance of millet cultivation changed over time. The discovery of millet has been rare in the Mahan-Baekje villages in the Jeonnam region, and archaeological discussions on ancient farming have centered around rice farming. However, a large amount of millet was found at the Osan site in Bongrim-ri, Jangheung, showing that the tradition of millet cultivation continued during the AD 3-4 centuries. In contrast to rice farming, which requires low-lying wetlands, hot and humid summers, and a large labor force, millet cultivation has few restrictions in terms of growing environment and labor mobilization. Regarding harvesting seasons, the crops at the Osan site can be divided into fall harvesting (rice, legumes, millet) and summer harvesting (wheat). Regarding field locations, the crops are divided into wetland crops (rice) and dryland crops (wheat, legumes, millet). The operation of the dual agricultural system contributed to subsistence resilience and agricultural production, making it possible to effectively manage social and climatic crises.

Distribution and Change of Radon Concentration of Groundwater in the Area of Yeonpung-myeon, Goesan-gun, Korea (괴산군 연풍면 일대 지하수 중 라돈 함량 분포와 변동)

  • To assess the distribution and temporal changes in radon concentration within a region in Yeonpung-myeon, Goesan-gun, known for elevated groundwater radon levels, we conducted a series of analyses, measuring radon concentration and DTW (Depth to water table) at 2-month intervals over 12 cycles. The investigation covered 10 groundwater wells and one stream within the designated area. The groundwater in the central part of the region exhibited high radon concentrations, ranging from 37.0 to 2,675.2 Bq/L. Conversely, the peripheral zones displayed comparatively lower radon concentrations, ranging from 10.6 to 37.9 Bq/L. This variation is attributed to the presence of granite porphyry that intruded into the Okcheon Formation, forming a fracture zone and contributing to elevated radon levels in the central part. In contrast, the peripheral locations, located within the Okcheon Formation and away from the granite porphyry intrusion, demonstrated lower radon concentrations. The observed significant fluctuation in radon concentration in the central area is associated with its low-lying topography. The pronounced seasonal changes in groundwater levels contribute to the migration of shallow, low-radon groundwater into areas with higher radon concentrations, explaining the observed variations in radon levels within the central part of the studied area.

Development of a Real-time Action Recognition-Based Child Behavior Analysis Service System (실시간 행동인식 기반 아동 행동분석 서비스 시스템 개발)

  • This paper describes the development of a system and algorithms for high-quality welfare services by recognizing behavior development indicators (activity, sociability, danger) in children aged 0 to 2 years old using action recognition technology. Action recognition targeted 11 behaviors from lying down in 0-year-olds to jumping in 2-year-olds, using data directly obtained from actual videos provided for research purposes by three nurseries in the Gwangju and Jeonnam regions. A dataset of 1,867 actions from 425 clip videos was built for these 11 behaviors, achieving an average recognition accuracy of 97.4%. Additionally, for real-world application, the Edge Video Analyzer (EVA), a behavior analysis device, was developed and implemented with a region-specific random frame selection-based PoseC3D algorithm, capable of recognizing actions in real-time for up to 30 people in four-channel videos. The developed system was installed in three nurseries, tested by ten childcare teachers over a month, and evaluated through surveys, resulting in a perceived accuracy of 91 points and a service satisfaction score of 94 points.

Topoclimatological interpretation of the daily air temperature minima at 17 locations crossing over Yangpyeong basin in 1986 spring (봄철 양평지역(楊平地域)의 지형(地形) 및 고도(高度)에 따른 일최저기온(日最低氣溫)의 분포(分布))

  • Frost damage which can reduce yields, impair fruit quality and cause loss of trees is closely related to the occurrence of daily minimum temperature. Horizontal distribution of air temperature minima can be characterized by conditions of radiational cooling and gravitational movement of cold air, which are influenced by the regional topographic features. Observations were made on the air temperature minima over Yangpyeong area, to delineate potential effects of topography on the temperature pattern during spring season. Two routes were selected for the observation. Liquid glass minimum thermometers were installed at 17 sites through the old peach orchards which had been closed due to the frequent freeze-frost hazards during the recent years. This route was 8.5km long and the highest point was 350m above mean sea level. The other route, which was 2.5km in distance, was run with a digital resistance thermometer during the hour just before sunrise. Observations were made both on a calm-clear day (April 30, 1986) and a windy-overcast day (May 1, 1986). The temperature on April 30 was in increasing trend with elevation but this was modified at near the riverside and the downtown area. An orchard lying on a hilltop showed the temperature $1^{\circ}C$ higher than near by lowland of which elevation was about 30m lower. The minimum temperature on the overcast day was little affected by terrestrial conditions but by the atmospheric lapse condition. The peach orchards severely damaged by cold air were found in the area where the lowest minimum temperature was observed. The results may be useful for selection of the proper orchard location to be developed in an area.

Characteristics of Bridal Palanquin Covers and Changes in Style from the late 19th Century to the early 20th Century (19세기 말~20세기 초 신부 가마덮개의 특성과 양식 변천)

  • In the late Joseon Dynasty, when the bride would ride a palanquin when she went to live with her in-laws, it was a custom to cover the palanquin with tiger skin to ward off misfortunes that may come her way. The higher classes used tiger skin or leopard skin for this purpose, but the common people had to substitute this expensive item with a tiger pattern painted on a blanket. Such blankets were called hotanja, hogu, hoguyok and the like. The term "hotanja" is a pure Korean word. It is not known when the cover for the bridal palanquin was first used, but it was popular from the end of the 19th century and then gradually disappeared. This is due to the introduction of new Western style weddings that eliminated the need for a bridal palanquin. The tiger print blanket was used not only to cover the bride's palanquin but also to cover a table or floor during the wedding ceremony. This study ran a material analysis on nine pieces of tiger print blankets. All of the blanket artifacts examined in this study had an outer cover and a lining made of fabric that used cotton thread for the warp and wool thread for the weft. Two kinds of wool were found in the weft thread in the outer covers: fat-tailed sheep hair from China and goat hair for carpets from the Hebei province, China. Records show that "blankets with painted tiger patterns" were imported from Russia, and the imported blankets were from Russia and China. The outer cover can be categorized into six types, and the lining into three types depending on the weave and direction of the thread twist. The hem facing can be divided into four types. The lining and outer cover use the full width of the fabric, which was woven in wide widths of 135 cm or wider. The tiger pattern on the blanket was made by stenciling. The stencil design of the body and tail of the tiger were placed on a red blanket to be painted in white, and then the background color of the tiger, which is yellow, would be painted over the white, and then black stripes would be added. The pattern of the tiger varies, which shows that the blankets were made by various craftspeople. The pattern of the tiger print blanket is usually of a tiger lying down, but there were tiger print blankets with a tiger standing up. The pattern of the tiger grew smaller over time, and flower patterns were added in the background. Decorative elements were gradually added to the tiger print blanket patterns, but its function as a palanquin cover became lost. By taking the features of tiger print blankets into consideration, it can be assumed that there are imported pieces among the remaining pieces, and were produced in various places because it was popular at that time.
